High-definition colonoscopy with i-Scan:Better diagnosis for small polyps and flat adenomas

AIM： To investigate if high-definition （HD） colonoscope with i-Scan gave a higher detection rate of mucosal le- sions vs standard white-light instruments. METHODS： Data were collected from the computer- ized database of the endoscopy unit of our tertiary referral center. We retrospectively analyzed 1101 con- secutive colonoscopies that were performed over 1 year with standard white-light （n = 849） or HD＋ with i-Scan （n = 252） instruments by four endoscopists, in an outpatient setting. Colonoscopy records included patients＇ main details and family history for colorectal cancer, indication for colonoscopy （screening, diagnos- tic or surveillance）, type of instrument used （standard white-light or HD＋ plus i-Scan）, name of endoscopist and bowel preparation. Records for each procedure included whether the cecum was reached or not and the reason for failure, complications during or imme- diately after the procedure, and number, size, location and characteristics of the lesions. Polyps or protruding lesions were defined as sessile or pedunculated, and nonprotruding lesions were defined according to Paris classification. For each lesion, histological diagnosis was recorded. RESULTS： Eight hundred and forty-nine colonosco- pies were carried with the standard white-light video colonoscope and 252 with the HD＋ plus i-Scan video colonoscope, The four endoscopists did 264, 300, 276 and 261 procedures, respectively; 21.6%, 24.0%, 21.7% and 24.1% of them with the HD＋ plus i-Scan technique. There were no significant differences be- tween the four endoscopists in either the number of procedures done or the proportions of each imaging technique used. Both techniques detected one or more mucosal lesions in 522/1101 procedures （47.4%）. The overall number of lesions recognized was 1266; 645 in the right colon and 621 in the left. Nanomaterials with unique characteristics exhibit favorable therapeutic and diagnostic properties,implying their enormous potential as biomedical candidates. C60 has been used in gene- and drug-delivery, as imaging agents, and as photosensitizers in cancer therapy. In this study, the influences of a cationic functionalized fullerene on cellular behavior of human colorectal cancer cell line(HT-29) were investigated. Results indicated that HT-29 treated with the studied compound showed a lower sensitivity but a significant impairment in migration and invasion by interfering with the activities of matrix metalloproteinases(MMP-2 and9). The presence of fullerene also altered the capacity of adhesion-related proteins to perform their activity,thereby inducing dramatically adverse effects on the cell physiological functions such as cell adhesion. Thus,our study suggests that this compound is a new potential anti-metastatic effector and a therapeutic component for malignant colorectal cancer.

Ultrahigh-pressure（UHP） metamorphic rocks are distinctive products of crustal deep subduction,and are mainly exposed in continental subduction-collision terranes. UHP slices of continental crust are usually involved in multistage exhumation and partial melting, which has obvious influence on the rheological features of the rocks, and thus significantly affect the dynamic behavior of subducted slices. Moreover,partial melting of UHP rocks have significant influence on element mobility and related isotope behavior within continental subduction zones, which is in turn crucial to chemical differentiation of the continental crust and to crust-mantle *** melting can occur before, during or after the peak metamorphism of UHP rocks. Post-peak decompression melting has been better constrained by remelting experiments; however, because of multiple stages of decompression, retrogression and deformation, evidence of former melts in UHP rocks is often erased. Field evidence is among the most reliable criteria to infer partial melting. Glass and nanogranitoid inclusions are generally considered conclusive petrographic evidence. The residual assemblages after melt extraction are also significant to indicate partial melting in some cases. Besides field and petrographic evidence, bulk-rock and zircon trace-element geochemical features are also effective tools for recognizing partial melting of UHP rocks. Phase equilibrium modeling is an important petrological tool that is becoming more and more popular in P-T estimation of the evolution of metamorphic rocks; by taking into account the activity model of silicate melt, it can predict when partial melting occurred if the P-T path of a given rock is *** silicate melt is commonly leucogranitic and peraluminous in composition with high SiO_2,low MgO, FeO, MnO, TiO_2 and CaO, and variable K_2 O and Na_2 O contents. The study investigated the effects of choline combined with Bangerter filter in the treatment of amblyopia. All amblyopic subjects used a Bangerter filter on the corrective spectacle lens（1 d over the left eye, 1 d over the right eye）. Choline was then administered orally to 39 patients once daily, five days per week for the entire study period. Subjects treated with the Bangerter filter showed a mean visual acuity of 0.27 logM AR; at 12 mo of treatment, the mean visual acuity reached 0.09 log MAR. Patients treated with the Bangerter filter and citicoline showed a mean visual acuity of 0.35 log MAR; at 12 mo of treatment, the mean visual acuity reached 0.01 logMAR. No significant changes in the angle of deviation were observed in both groups. Subjects in both forms of amblyopia therapies demonstrated an increase in visual acuity. However, these effects were markedly enhanced when coupled with the administration of choline. Findings suggest that the effects are particularly relevant in the more severe amblyopic cases.

Although gastroenteropancreatic neuroendocrine neoplasms(GEP-NENs)have always been considered rare tumors,their incidence has risen over the past few *** represent a highly heterogeneous group of neoplasms with several prognostic factors,including disease stage,proliferative index(Ki67),and tumor *** of these neoplasms express somatostatin receptors on the cell surface,a feature that has important implications in terms of prognosis,diagnosis,and *** International Guidelines propose algorithms aimed at guiding therapeutic strategies,GEP-NEN patients are still very different from one another,and the need for personalized treatment continues to *** surgery is always the best option when feasible;however,up to 80%of cases are metastatic upon *** medical treatments,as GEP-NENs are characterized by relatively long overall survival,multiple therapy lines are adopted during the lifetime of these patients,but the optimum sequence to be followed has never been clearly ***,although new molecular markers aimed at predicting the response to therapy,as well as prognostic scores,are currently being studied,their application is still far from being part of daily clinical *** they represent a complex disease,with therapeutic protocols that are not completely standardized,GEP-NENs require a multidisciplinary *** review will provide an overview of the available therapeutic options for GEP-NENs and attempts to clarify the possible approaches for the management of these patients and to discuss future perspectives in this field.
