Hepatocellular carcinoma(HCC)is rarely associated with autoimmune paraneoplastic *** report a case of anti-transcriptional intermediary factor-1 gamma(TIF1-γ)-positive dermatomyositis(DM)as clinical presentation of HCC recurrence in a 72-year-old male patient admitted to our hospital due to fatigue,myalgia,and typical skin *** medical history was notable for hepatitis C-related cirrhosis,successful treatment with direct-acting antiviral agents,and previously efficacious treatment of *** testing showed significant rhabdomyolysis with antiTIF1-γantibodies at high titer,and DM was *** a careful diagnostic workup,HCC recurrence was *** first-line corticosteroid treatment,azathioprine and intravenous immunoglobulin treatments were administered;unfortunately,he mounted only partial *** to the compromised performance status,no HCC treatment was feasible,and,according to international guidelines,he received only best supportive ***,we discuss the diagnostic,prognostic,and pathogenic roles of anti-TIF1-γantibodies associated with paraneoplastic DM and the scant literature data on its occurrence in HCC *** the TIF1 gene family’s established role in oncogenesis,we also review the role of TIF1-γas a tumor-related neoantigen,leading to the development of clinically overt antiTIF1-γantibodies-positive DM.

Emerging molecular targets and therapy for cholangiocarcinoma

Cholangiocarcinoma(CCA) is a rare cancer arising from the biliary tree with a poor prognosis and limited therapeutic options. Recent large scale molecular characterisation studies have identified recurrent genetic alterations in CCA which may be amenable to therapeutic targeting. In this review we explore the genomic landscape of CCA and examine results from trials of molecularly targeted agents and immunotherapy in this disease. Challenges in CCA diagnosis, treatment and trial design are discussed and we reflect on future directions which may lead to improved outcomes for CCA patients.

Simultaneous removal of critical rare earth elements and chalcogen oxyanions by anaerobic granular sludge

Conventional mining of economically and strategically important critical rare earth elements(REEs)(such as neodymium,lanthanum and dysprosium),and chalcogens(such as selenium and tellurium)are associated with a huge economic and environmental ***,the need to recover REEs as well as chalcogens from different waste streams including wastewaters is becoming *** assays on synthetic chalcogen/REE-laden wastewater showed that the presence of REEs significantly improved the tellurite removal rate(>80%)and enhanced selenate removal by 66%±10%.Three 3.9 L continuous upflow anaerobic granular sludge bed(UASB)reactors were operated at a hydraulic retention time of 24 h and 30℃.Selenate reduction was achieved with a removal efficiency of~98% with an influent p H of 4.0 for more than 28 *** effect of REEs on tellurite removal in the UASB bioreactor could not be clearly established since a soluble tellurium removal efficiency of more than 98%was observed already before the addition of REEs at elevated tellurite *** complete REE removal in both batch assays and UASB reactors at higher pH(7.0±0.5)was attributed to precipitation,whereas chalcogen oxyanions removal was due to microbial ***,at acidic p H,biosorption was responsible for REE's removal,and the Se-enriched sludge exhibited a superior REE's removal efficiency than the non-enriched and Te-enriched sludge.

Endoscopic ultrasonography for evaluating patients with recurrent pancreatitis

Acute recurrent pancreatitis (ARP) is still a complex diagnostic and therapeutic challenge in clinical practice. In up to 30% of cases of ARP, it is not possible to establish the etiology of the disease. In the other 70%, many factors play an etiological role in ARP: microlithiasis, sphincter of Oddi dysfunction (SOD), pancreas divisum, hereditary pancreatitis, cystic fibrosis, a choledochocele, annular pancreas, an anomalous pancreatobiliary junction, pancreatic tumors or chronic pancreatitis are diagnosed. EUS should be useful in ARP as it is sensitive for diagnosing bile duct stones, gallbladder sludge, pancreatic lesions, ductal abnormalities and chronic pancreatitis. Endoscopic ultrasound (EUS) appears to be diagnostic in the majority of patients with previously unexplained pancreatitis, and offers an alternative to endoscopic retrograde cholangiopancreatography (ERCP) as the initial diagnostic test in patients with ARP.

Medical treatment of unresectable hepatocellular carcinoma: Going beyond sorafenib

Even though Sorafenib has radically changed the natural history of those hepatocellular carcinoma patients who are not amenable for curative treatments, further therapeutic improvements are badly needed. As it was for Sorafenib, our increasingly refined understanding of the complex mechanisms underlying HCC carcinogenesis are the starting point for the future development of such treatments. Presently, a number of molecularly targeted agents are in different stages of development for this once orphan cancer. Indeed, several pathways are presently being explored to identify potentially active drugs, including epidermal growth factor receptor, vascular endothelial growth factor/vascular endothelial growth factor receptors, mammalian target of rapamycin, phosphatidyl-inositol-3-kinase/Akt, insulin growth factor, Aurora kinase, Wnt/β-catenin, retinoic acid receptor and hepatocyte growth factor/C-Met. This review is aimed at addressing the results obtained so far with these newer drugs, also considering the challenges we shall face in the near future, including the issue of response evaluation and identification of predictive/ prognostic biomarkers.

Nanotechnological approaches for counteracting multidrug resistance in cancer

Every year,cancer accounts for a vast portion of deaths *** clinical protocols are based on chemotherapy,which,however,is not tumor-selective and produces a series of unbearable side effects in healthy *** a consequence,multidrug resistance(MDR)can arise causing metastatic progression and disease *** therapy has demonstrated limited responses in the treatment of MDR,mainly due to the different pharmacokinetic properties of administered drugs and to tumor heterogeneity,challenges that still need to be solved in a significant percentage of cancer *** this perspective,we briefly discuss the most relevant MDR mechanisms leading to therapy failure and we report the most advanced strategies adopted in the nanomedicine field for the design and evaluation of ad hoc *** present some emerging classes of nanocarriers developed to reverse MDR and discuss recent progress evidencing their limits and promises.

Endometritis associated with Enterococcus casseliflavus in a mare:A case report

Infectious endometritis is one of the main causes of subfertility/infertility in the mare. In this report, we present the first case of endometritis in mare associated with a strain of Enterococcus casseliflavus, an unusual gram-positive bacterium which can also be a zoonotic agent. Furthermore, the isolated strain showed a worrying multidrug-resistant profile. The accurate finding of a successful antimicrobial treatment and consequently,the pregnancy diagnosis indicate the importance to isolate, identify and define the antibiotic resistance profile of bacteria associated with endometritis.

Fungal cell-wall-degrading enzymes have great utility in the agricultural and food *** cell-wall-degrading enzymes are known to have functions that can help defend against pathogenic *** existing methods used to discover these enzymes are not well adapted to fungi culture and morphology,which prevents the proper evaluation of these *** report the first droplet-based microfluidic method capable of long-term incubation and low-voltage conditions to sort filamentous fungi inside nanoliter-sized *** new method was characterized and validated in solid-phase media based on colloidal chitin such that the incubation of single spores in droplets was possible over multiple days(2–4 days)and could be sorted without droplet *** long-term culture,we examined the activity of cell-wall-degrading enzymes produced by fungi during solid-state droplet fermentation using three highly sensitive fluorescein-based *** also used the low-voltage droplet sorter to select clones with highly active cell-wall-degrading enzymes,such as chitinases,β-glucanases,and β-Nacetylgalactosaminidases,from a filamentous fungi droplet library that had been incubated for>4 *** new system is portable,affordable for any laboratory,and user-friendly compared to classical droplet-based microfluidic *** propose that this system will be useful for the growing number of scientists interested in fungal microbiology who are seeking high-throughput methods to incubate and sort a large library of fungal cells.
