Chemotherapy-induced neurotoxicity in the treatment of gynecological cancers:State of art and an innovative approach for prevention

Chemotherapy-induced peripheral neuropathy(CIPN)is a common side effect that occurs in 20%of ovarian cancer patients treated with the combination of carboplatin/paclitaxel(CP).This toxicity is directly correlated with the dose of paclitaxel administered.Several studies have investigated whether different formulations of taxane can induce this side effect at a lower rate,but,unfortunately,no significant improvement was obtained.CIPN can be disabling in the daily lives of patients and can cause dose reduction or early termination of the treatment.Neuropathy can last for months and even years after its onset.Moreover,patients responsive to CP treatment are candidates for a reintroduction of the same drugs when disease relapse occurs,and residual neuropathy can affect the continuation of treatment.There are no approved drugs that mitigate or prevent the onset of CIPN.In this review,we summarize the evidence regarding the incidence of CIPN with different taxane formulations,regimen schedules and prevention systems.In particular,the Hilotherm®Chemo care device is a regional cooling system that lowers the temperature of the hands and feet to reduce the flow of chemotherapy into the capillaries.We used hilotherapy during chemotherapy infusion to prevent the onset of CIPN.Updated data from 44 ovarian cancer patients treated with 6 cycle of CP show that hilotherapy was well tolerated;only two patients(4.5%)stopped hilotherapy because of cold intolerance,and only one patient(2.2%)experienced grade≥2 CIPN.

Thyroid carcinoma showing thymus-like differentiation: Case presentation of a young man

Ectopic thymic tissue can be present in the thyroid gland and a carcinoma showing thymus-like differentiation(CASTLE) may arise from such tissue. We are reported the case of a 26-year-old man with CASTLE, with cervical subcutaneous nodules relapse, who showed a good response to treatment with surgery, chemotherapy and radiotherapy. The problematic aspect of this case was the diagnosis; only on review were we able to make a final diagnosis. CASTLE is a very rare neoplasm. It is important to differentiate this cancer from others tumors such as primary or metastatic squamous cell carcinoma of the head and neck or squamous cell thyroid carcinoma, because the therapy and prognosis are different.Diagnosis is complicated and requires careful histological analysis(CD5- and P63-positive with presence of Hassall's corpuscles); unfortunately there is no gold standard treatment so, in this case, we administered a sandwich of chemotherapy and radiotherapy.

Characterization of machine tools and measurement system for micromilling

Technological progress has led to increased demand for small components with tiny features, which cannot be achieved through conventional machining. Industrial application of processes based on microcutting is limited by some issues concerning the geometrical scale. The process performance is significantly affected by milling machine, tool holder, tool, workpiece material microstructure, workpiece fixtures, and process parameters. At present, an ultimate micromachining assessment procedure is not available. This study aims to propose and conduct an experiment on a testing procedure for micromilling. The set up to be implemented and the output to be considered are defined and described. Three major stages are identified: estimation of the effective bandwidth of the load cell–tool holder system, the milling machine natural frequency measurement, and micromilling test execution. The entire procedure is performed, and its robustness is demonstrated.

Association analyses of single nucleotide polymorphisms in the <i>leptin</i>and <i>leptin receptor</i>genes on milk and morphological traits in Holstein cows

To date, only a few SNP in the leptin receptor gene have been detected and analyzed while the leptin gene has been extensively characterized and several polymorphisms were detected and associated to functional and productive traits in cattle. In this work, for the first time, the bovine leptin receptor gene was fully characterized by sequencing the whole coding region and part of the 5’ flanking region. A group of 95 Holstein cows was genotyped in order to search for SNP at both the LEPR and LEP genes, and to associate them with milk and morphological traits. Nine novel SNP in the leptin receptor gene and one novel SNP in the leptin gene were detected. Four SNP of the leptin receptor gene and one SNP of leptin gene showed a significant effect on one or more analyzed traits, and, in all cases, the greatest effect was observed on fat content. This study provided knowledge of the existence of further polymorphisms in the leptin receptor and leptin genes that have an influence on some important economic traits.

Background: The main dermatology textbooks describe only in passing pruritus in psoriasis and rarely mention other symptoms. A quantification of the presence of symptoms is not available for clinical subgroups of psoriasis. Objectives: T o investigate the prevalence of symptoms experienced by patients with different clinical types of psoriasis. Methods: The study was carried out in patients hosp italized for psoriasis between February 2000 and February 2002 at the inpatient wards of the Istituto Dermopatico dell’Immacolata, Rome, Italy. Symptoms were evaluated using the symptoms scale of Skindex- 29. Clinical severity was assess ed by the dermatologists using the Psoriasis Area and Severity Index (PASI), and by the patients completing the self-administered PASI. Psychiatric morbidity was evaluated using the 12- item General Health Questionnaire. Results: In tota l, 936 eligible patients were analysed. The proportions of patients experiencing symptoms often or always in the 4 weeks before hospitalization were: 63.8% it ching, 59.7% irritation, 46.1% burning/stinging, 39% sensitivity, 26% pa in (from 10% in guttate psoriasis to 50% in arthropathic)- , 25.4% bleedi ng (17% pustular, 19% localized plaque, 36% palmoplantar), and 23.9% bot hered by water (from 8.5% in the guttate form to 68% in palmoplantar). The p revalence of all symptoms was significantly higher in women and tended to increa se with clinical severity. Conclusions: Our study provides evidence of the high frequency of a number of symptoms in different subgroups of psoriasis patients d etermined by their sociodemographic characteristics, clinical type and disease s everity. Symptoms represent a serious disabling factor in patients affected by p soriasis, including those with low levels of psychological distress. Dermatologi sts should include symptoms in the evaluation of disease severity both in clinic al practice and in clinical trials.

Despite being common, depression often goes undetected in patients with skin diseases. Our aim was to examine and compare the performance of three depression screeners. We studied dermatological inpatients aged 18- 65 years. They completed the questionnaires Primary Care Screener for Affective Disorders (PC- SAD), Patient Health Questionnaire (PHQ) and General Health Questionnaire (GHQ- 12) and were administered a standardized psychiatric interview (SCID- 1) by a mental health professional, who was unaware of the questionnaire answers. The analysis was performed on 141 patients with complete data (79% of all eligible patients, 89% of all patients who agreed to participate). The prevalence of the main forms of depression, major depressive disorder and dysthymic disorder, was 8.4% and 6.3% , respectively. For major depressive disorder, the sensitivity and specificity of the questionnaires were as follows: PC- SAD, 73% and 88% ; PHQ, 55% and 91% ; GHQ- 12, 73% and 78% . For dysthymic disorder, the sensitivity and specificity were as follows: PC- SAD, 56% and 95% ; PHQ, 44% and 90% ; GHQ- 12, 56% and 76% . The small sample size suggests caution in drawing conclusions about the relative merits of these screeners. Although both the GHQ and the PHQ are short and easily hand scored, the first is a generic screener for psychiatric morbidity that is not specific for depression, while the second displayed modest sensitivity. The PC- SAD, with short average administration time, acceptable sensitivity and high specificity, might be particularly useful in settings where the technology for computer automated scoring is available. Although screening programmes might be useful, they should be supplemented by quality improvement programmes and by the development of consultation- liaison services.
