Laparoscopic retroperitoneal lymph node dissection versus open retroperitoneal lymph node dissection for testicular cancer:A comparison of clinical and perioperative outcomes

Objective:This study was performed to evaluate the clinical and perioperative outcomes of laparoscopic retroperitoneal lymph node dissection(L-RPLND)and open retroperitoneal lymph node dissection(O-RPLND)performed by one surgeon at a single ***:We evaluated 30 patients with stage IIA germ cell tumors who underwent retroperitoneal lymph node dissection(15 underwent L-RPLND and 15 underwent O-RPLND)at our institution between April 1,2010 and March 31,*** clinical parameters were compared between patients who underwent L-RPLND using the retroperitoneal approach and those who underwent O-RPLND using the transperitoneal *** were no significant differences in the background characteristics of the two groups except for the median follow-up duration(46 months for L-RPLND and 71 months for O-RPLND,p=0.02).Results:L-RPLND was associated with a shorter mean operative time(mean 222 min for L-RPLND vs.453 min for O-RPLND,p<0.001).There was significantly less blood loss during surgery in the L-RPLND group compared to the O-RPLND group(mean 165 mL for L-RPLND vs.403 mL for O-RPLND,p<0.001).Parameters related to postoperative recovery were significantly better for the L-RPLND group than for the O-RPLND *** were no differences in the histopathological characteristics between the two *** patients in either group exhibited disease ***:Patients who underwent L-RPLND had more rapid recovery,and shorter hospital stay compared to those who underwent O-RPLND;complications were comparable between the two groups.L-RPLND is an efficient procedure with the benefits of minimally invasive surgery.

Reconstruction of the Lacustrine Delta and Lake Level Change Analyzing Subsurface Geology and Geomorphology: Changes That Occurred during the Holocene in the Oguraike Reclaimed Land Area, Southern Kyoto, Japan

A paleo-lacustrine delta in Kyoto, Japan was reconstructed on the basis of subsurface geological and geomorphological analysis, and paleo-lake level changes were estimated from the structure of the delta. These analyses of the study region, i.e., the Oguraike reclaimed land area provided evidence that Lake Ogura existed until about 60 years ago in southern Kyoto, Japan. The Uji river delta was provided influents to this lake until ca. 400 years ago, as is indicated by an upward-coarsening delta succession of about 2 - 4 m thickness. The lake level could also have changed in the past as a result of a change in altitude of the delta-front (foreset) and delta-plain boundary, which probably reflects the lake surface elevation. About 400 years ago, the Paleo-Uji River was separated from Ogura Lake because a levee was constructed along the river for building a castle and for constructing a waterway for transportation. As a result of this construction, the lake level that was more than 13.0 m in elevation was reduced by 1.5 m. In a more ancient times, the lake level experienced two stages—one in which the elevation was more than 13.5 m, and one in which the elevation was reduced to less than 10 m. These changes in the lake level are represented by a flat surface with four steps and small cliff of height ca. 0.5 - 2 m (relative elevation) separating them, recognized at the southern lakeshore. The observation of strata along with the archaeological survey in the north of Ogura Lake reveals that the lake level was decreased ca. 800 - 680 years ago. The lake level was at its highest during two periods, the first from before the 8th century to the end of the 8th century and the second from the 14th century to 400 years ago.

Ⅰ. GEOLOGYSamples studied were collected by LUO Xiang-jun from Kangsulake, Yutiantown. Geology of this area has been given by LIU Jia-qi. Two lava layers can be recognized. The upper layer is 1.1 Ma BP and the lower is 1.4 Ma BP (LUO Xiang-jun, personal communication, 1987). A K-Ar age of 1.24 Ma was obtained by the authors (1988), which is in agreement with the result reported by LIU Jia-qi.

Cause of idiopathic sudden sensorineural hearing loss:The stress response theory

The stress response theory is a relatively new concept about the cause of idiopathic sudden sensorineural hearing loss(ISHL). A number of possible etiologies have been proposed in the literature, as discussed in this paper, but each proposed etiology has been both supported and refuted in the literature. However, the stress response theory can integrate hypotheses that have been advocated so far. The word "stress" refers to a constellation of physical and psychological stimuli including systemic viral and bacterial illness, systemic inflammatory disorders, and physical, mental or metabolic stress. Numerous studies have demonstrated adverse effects of systemic stress on health. Stress causes changes in the immune system and cytokine network through activation of the hypothalamus-pituitary-adrenal axis and the sympathetic nervous system. Several types of catecholamine and cytokine receptors are in the cochlea cells other than capillary cells, and then they can respond to systemic stressors. However, there are few studies examining how systemic stress is associated with cochlear dysfunction. The stress response theory addresses this question. In the theory, a variety of stressors and risk factors contribute to the onset of ISHL in varying degrees. The lateral wall of the cochlea has very unique responses to systemic stressors. It plays a critical role in causing ISHL. Systemic stressors converge at the lateral wall and trigger pathological activation of nuclear factor κ-light-chain-enhancer of activated B cells, a transcriptional factor known as a stress sensor. This activation enhances local expression of genes associated with immune and inflammatory system, resulting in cochlear dysfunction. We review the original stress response theory advocated by Adams et al and the integrative stress response theory that integrates our knowledge about the etiologies of ISHL so far.

The acoustic properties of supersonic jet noise from a convergent-divergent nozzle with a baffle have been studied experimentally over the range of nozzle pressure ratios from 2.0 to 8.0. Acoustic measurements were conducted in a carefully designed anechoic room providing a free-field environment. A new approach for screech noise suppression by a cross-wire is proposed. Schlieren photographs were taken to visualize the shock wave patterns in the supersonic jet with and without the cross-wire. The effects of the baffle and the cross-wire on acoustic properties are discussed. It is shown that the baffle has little effect on the screech frequency for the underexpanded supersonic jet without the cross-wire. Also, the cross-wire introduced in supersonic jets is found to lead to a significant reduction in overall sound pressure level.

Quantitative versus Qualitative Geospatial Data in Spatial Modelling and Decision Making

In general, geospatial data can be divided into two formats, raster and vector formats. A raster consists of a matrix of cells where each cell contains a value representing quantitative information, such as temperature, vegetation intensity, land use/cover, elevation, etc. A vector data consists of points, lines and polygons representing location or distance or area of landscape features in graphical forms. Many raster data are derived from remote sensing techniques using sophisticated sensors by quantitative approach and many vector data are generated from GIS processes by qualitative approach. Among them, land use/cover data is frequently used in many GIS analyses and spatial modeling processes. However, proper use of quantitative and qualitative geospatial data is important in spatial modeling and decision making. In this article, we discuss common geospatial data formats, their origins and proper use in spatial modelling and decision making processes.

Evaluation of the relationship between hepatocellular carcinoma location and transarterial chemoembolization efficacy

AIM To evaluate the relationship between the location of hepatocellular carcinoma(HCC) and the efficacy of transarterial chemoembolization(TACE).METHODS We evaluated 115 patients(127 nodules), excluding recurrent nodules, treated with TACE between January 2011 and June 2014. TACE efficacy was evaluated according to m RECIST. The HCC location coefficient was calculated as the distance from the central portal portion to the HCC center(mm)/liver diameter(mm) on multiplanar reconstruction images rendered(MPR) to visualize bifurcation of the right and left branches of the portal vein and HCC center. The HCC location coefficient was compared between complete response(CR) and non-CR groups in Child-Pugh grade A and B *** The median location coefficient of HCC among all nodules, the right lobe, and the medial segment was significantly higher in the CR group than in the non-CR group in the Child-Pugh grade A patients(0.82 vs 0.62, P 0.05). On the other hand, in the Child-Pugh grade B patients, the HCC median location coefficient in each lobe and segment was not significantly different between in the CR and in the non-CR *** Improved TACE efficacy may be obtained for HCC in the peripheral zone of the right lobe and the medial segment in Child-Pugh grade A patients.

Introduction of Music Therapy Incorporated into Cognitive Remediation: A New Approach to Cognitive Dysfunction in Psychiatric Disorders and a Preliminary Report on Its Effects in Schizophrenia

Aim: Cognitive dysfunction and negative symptoms interfere social participation in patients with schizophrenia. Cognitive remediation has proven effective for cognitive deficits. While there are few effective treatments for the negative symptoms of schizophrenia, music therapy is expected to ameliorate negative symptoms. With the goal of improving both cognitive dysfunction and negative symptoms, we originally developed a program of Music Therapy incorporated into Cognitive Remediation (MTCR). We introduce the MTCR program and our preliminary results of conducting MTCR in patients with schizophrenia. Methods: The program was based on the NEAR (neuropsychological educational approach to cognitive remediation) program. The MTCR program uses instrumental performances and ensemble vocal performances to train cognitive functions, which consists of 30 sessions in total, with each session being 60 minutes long (45 minutes of music, 15 minutes of verbal session). The participants attended sessions semiweekly in groups of fewer than 12 members. We measured the participants’ symptoms twice, before the intervention and after the final session. For both measurements, we used the BACS and BPRS. Thirty-six individuals (26 men, 10 women;average age of 42.4 years) with schizophrenia participated in this study. They participated in an average of 28.2 sessions. Results: BACS total scores (p Conclusions: The present study demonstrated that MTCR program can improve both cognitive dysfunction and negative symptoms of patients with schizophrenia, which implies that it can also potentially facilitate the social participation of such patients.

Oxidation of Solid Phase and Ionic Strength Effect to the Cesium Adsorption on Pumice Tuff

In-situ oxidation of solid phase was considered to investigate adsorption behavior under different geochemical parameters like pH, initial concentration and ionic strength. Pumice tuff, a potential host rock for low and intermediate radioactive wastes, has been affected by the redox zone. The characterization of the fresh and oxidized tuff was performed by X-ray diffractometer, scanning electron microscope and mercury intrusion porosimetry. In order to compare the difference of distribution coefficient (Kd) in fresh and oxidized pumice tuffs, a batch adsorption study was carried out at the range of pH (4 - 12), ionic strength (0.003, 0.1, 1.0 and 3.0 mol/dm3) and initial cesium concentration (10ǆ, 10Ǉ, 10ǈ and 10ǉ mol/dm3). Based on experimental Kd values, ionic strength was found to be the most influential factor, whereas the effects of pH, initial Cs concentration and weathering condition of pumice tuff were negligible. The recalculated Kd values suggest that the existing surface complexation model is applicable to explain the sorption coefficients through the wide range of solution conditions.

Simultaneous Occurrence of Myofascial Pain Referred to Tooth and Endodontic Lesions in Patient with Toothache: A Case Report

Toothache is almost always caused by odontogenic toothache, but diagnosis is more difficult in the case of nonodontogenic toothache. We report a case of simultaneous occurrence of odontogenic and nonodontogenic toothache. This manuscript presents a case report for a 35-year-old woman who visited our Orofacial and Head Pain Clinic with the chief complaint of continuous dull pain in left maxillary molar teeth region. It was concluded to be a case of simultaneous odontogenic toothache and nonodontogenic toothache. It was successfully treated by an endodontist and an orofacial pain specialist. The endodontist performed root canal treatment against odontogenic toothache caused by apical periodontitis using a dental operating microscope. The presence of a trigger point (TP) resulting in tooth pain was inferred. A trigger point injection (TPI) was administered by orofacial pain specialist, and toothache relief was confirmed. Myofascial pain was diagnosed definitively. After confirming that the toothache had resolved at multiple TPIs, a crown prosthesis was placed. Following the application of crown prosthesis, we were concerned but did not find recurrence of toothache from myofascial pain due to increased occlusal force. This case suggests that there is no single cause of chronic pain and that multiple causes must be considered for diagnosis, suggesting the need for treatment by multiple specialists.
