Outcome of In-Hospital Cardiac Arrest in Adult General Wards

Objective: Few studies have focused on factors influencing outcomes of patients with in-hospital cardiac arrest (IHCA) in general wards. The goal of this study was to report the outcomes of adult patients with IHCA in the general wards and identified the prognostic factors. Methods: Adult patients with IHCA having received cardiopulmonary resuscitation in general wards from January 2008 to December 2011 were retrospectively reviewed from our registry system. The primary outcome was survival to hospital discharge, while the secondary outcome was sustained return of spontaneous circulation (ROSC). Results: A total of 544 general ward patients were analyzed for event variables and resuscitation results. The rate of establishing a ROSC was 40.1% and the rate of survival to discharge was 5.1%. Ventricular tachycardia/ventricular fibrillation (VT/VF) was the initial rhythm in 3.9% of patients. Pre-arrest factors including a high Charlson comorbidity index (CCI) ≥ 9 (OR 0.251, 95% CI 0.098 - 0.646), cardiac comorbidity (OR 0.612, 95% CI 0.401 - 0.933), and arrest time on the midnight shift (OR 0.403, 95% CI 0.252 - 0.642) were independently associated with a low possibility of ROSC. The initial VT/VF presenting rhythms (OR 0.135, 95% CI 0.030 - 0.601) were independently associated with a high survival rate, whereas patients with deteriorated disease course were independently associated with a decreased hospital survival (OR 3.902, 95% CI 1.619 - 9.403). Conclusions: We demonstrated that pre-arrest factors can predict patient outcome after IHCA in general wards, including the association of a CCI ≥ 9 and cardiac comorbidity with poor ROSC, and deteriorated disease course as an independent predictor of a low survival rate.

Therapeutic plasma exchange in patients with hyperlipidemic pancreatitis

AIM: To clarify the effectiveness of plasma exchange by comparing the mortality and morbidity before and after the intervention of plasma ***: Plasma exchange has been available as an optional therapy for hyperlipidemic pancreatitis since August 1999 in our hospital. The patients were assorted into 2 groups (group I: before August 1999 and group II: after August1999). Group I consisted of 34 patients (before the availability of plasma exchange). Group II consisted of 60 patients (after the availability of plasma exchange).Twenty patients in group II received plasma exchange after giving their consent. The mortality and morbidity were compared between group I and group II. Furthermore,the patients with severe hyperlipidemic pancreatitis (Ranson's score≥3) were analyzed separately. The mortality and morbidity were also compared between those receiving plasma exchange (group A) and those who did not receive plasma exchange (group B).RESULTS: There was no statistical difference in the mortality, systemic and local complications between group I and group II. When the patients with severe hyperlipidemic pancreatitis were analyzed separately, there was no statistical difference between group A and group ***: Plasma exchange can not ameliorate the overall mortality or morbidity of hyperlipidemic *** time of plasma exchange might be the critical point. If patients with hyperlipidemic pancreatitis can receive plasma exchange as soon as possible, better result may be *** study with more cases is needed to clarify the role of plasma exchange in the treatment of hyperlipidemic pancreatitis.

Association of Fusobacterium nucleatum infection with colorectal cancer in Chinese patients

AIM: To investigate Fusobacterium nucleatum (F. nucleatum) abundance in colorectal cancer (CRC) tissues and its association with CRC invasiveness in Chinese ***: The resected cancer and adjacent normal tissues (10 cm beyond cancer margins) from 101 consecutive patients with CRC were collected. Fluorescent quantitative polymerase chain reaction (FQ-PCR) was applied to detect F. nucleatum in CRC and normal tissues. The difference of F. nucleatum abundance between cancer and normal tissues and the relationship of F. nucleatum abundance with clinical variables were evaluated. Fluorescence in situ hybridization (FISH) analysis was performed on 22 CRC tissues with the highest F. nucleatum abundance by FQ-PCR testing to confirm FQ-PCR ***: The median abundance of F. nucleatum in CRC tissues [0.242 (0.178-0.276)] was significantly higher than that in normal controls [0.050 (0.023-0.067)] (P -ΔCT was significantly greater in tumor samples [0.242 (0.178, 0.276)] than in normal controls [0.050 (0.023, 0.067)] (P 0.05). FISH analysis also found more F. nucleatum in CRC than in normal tissues (median number 6, 25th 3, 75th 10 vs 2, 25th 1, 75th 5) (P < 0.01).CONCLUSION: F. nucleatum was enriched in CRC tissues and associated with CRC development and metastasis.

Listening Comprehension Ability and Language Attitudes： American vs. British English

American English is predominantly prescribed as the primary English learning model at every educational level in Taiwan and limited attention is paid to other accents of English. It is therefore the intention of this paper to investigate whether Taiwan Residents EFL learners have difficulties understanding British English after years of instruction in American English, and to identify differences between learners＇ attitudes towards these two varieties of English. EFL learners from two universities were randomly assigned to American and British English groups and required to listen to short texts recorded with either American or British accent. An independent simple t-test showed that no significant differences were found in listening comprehension scores, but responses on questionnaires revealed significant differences in attitudes towards American and British English. The influence of long-term instruction in American English was evidenced. Based on these findings, suggestions are provided for English language teaching in Taiwan and implications are drawn for the concept of English as an International Language （ELL）, that in order to pursue successful international communication with people of various native languages, EFL learners need to experience various accents of English.

Endoscopic fibrin sealant closure of duodenal perforation after endoscopic retrograde cholangiopancreatography

Traditionally, perivaterian duodenal perforation can be managed conservatively or surgically. If a large volume of leakage results in fluid collection in the retroperitoneum, surgery may be necessary. Our case met the surgical indication for perivaterian duodenal perforation after endoscopic retrograde cholangiopancreatography(ERCP) with sphincterotomy and endoscopic papillary balloon dilatation. The patient developed a retroperitoneal abscess after the procedures, and a perivaterian perforation was suggested on computed tomography(CT). CT-guided abscess drainage was performed immediately. We unsuccessfully attempted to close the perforation with hemoclips initially. Subsequently, we used fibrin sealant(Tisseel) injection to occlude the perforation. Fibrin sealant injections have been previously used during endoscopy for wound closure and fistula repair. Based on our report, fibrin sealant injection can be considered as an alternative method for the treatment of ERCPrelated type Ⅱ perforations.

研究背景:牙科治疗椅供水管路(Dental Unit Waterlines)中的微生物污染最近几年广为被医疗各界关注,主要讨论管道的供水一旦被污染,可疑致病菌就能够通过不同方式进入患者口腔,或化成水气雾散发在科诊室的空气中,对医务人员和患者都是一种潜在的危险。在之前采用常规培养的方法及16s高通量测序研究结果表明,牙科治疗椅水系统细菌污染主要由放线菌、葡萄球菌和大肠杆菌等等引起。根据目前临床的标准,牙科治疗的供应用水不完全符合微生物学要求的微生物参数,由于牙椅使用的消毒模式和每天频率和年限也不同,其被污染的可能性和程度也不一样,使用的水管道中的细菌污染及主要的可疑致病菌除了目前研究常见的几种之外还有哪些是危险致病菌,是本课题研究的目的。综上所述,牙科治疗椅供水系统中的细菌污染,已经是医源性感染重要危险因素,对牙科用水的消毒和微生物监控也成为目前院感工作者关心的问题之一。目的:本研究的目的是调查某大型口腔医院牙科治疗椅中高速手机及其接头的供水系统管道遭到口腔细菌污染的情况,借此通过高速手机水路调查了解管道细菌污染的现状,探究进一步改进消毒手段的必要性及相关策略。方法:对经过常规消毒的牙科治疗椅进行采样。随机抽取医院20台牙科治疗椅的水样(从高速手机取样),设为系统水样(20例),同时取得实验室蒸馏水样(1例)作为阴性对照。封口后存放在-4℃环境下待测,随机抽取样本5例做16s高通量测序后拟订可疑微生物,采用定量PCR检测供水系统内的伯克氏菌和变异蛋白菌分布,为院感控制提供实验数据和依据。结果:通过一般培养的方法,结果表明牙科治疗椅的医生用高速手机中均验出以10秒组培养结果菌数最多,检测结果符合国家标准要求。16sDNA的筛选结果表明葡萄球菌、链球菌、放线菌,放线菌居多。而在定量PCR的实验中显示伯克氏菌及变异蛋白菌在检出的微生物中占据重要位置。结论:在这次分子生物检验结果中可以确认,Burkholderia和Proteobacteria两种细菌确实存在在牙科综合治疗椅里,其在水道中的含量可能与使用频率相关。如何防止细菌污染和主要种的细菌病防治需要进一步的研究和探讨.总之,它已被证明严重的牙科水系统污染,不仅仅是钻消毒问题的一部分,而是最重要的医院内感染。水的使用和预防和控制口腔消毒是当务之急。在PCR研究中洁牙机的含量相对都是高于其他水样本的。而与蒸馏水的阴性对照相比两个菌种都是很明显的增高。综上所述伯克氏菌及变异蛋白菌都是高含量存在于用水量多的牙科综合治疗椅中的(p<0.05)。

Comparison of Vs and SPT Soil Liquefaction Assessments of NCEER: Including Hypothesis Testing

Soil liquefaction is one of the complex research topics in geotechnical engineering and engineering geology. Especially after the 1964 Niigata earthquake (Japan) induced many soil liquefaction incidents, a variety of soil liquefaction studies were conducted and reported, including the liquefaction potential assessment methods utilizing the shear wave velocity (Vs) or SPT-N profiles (SPT: standard penetration test). This study used the Vs and SPT methods recommended by the National Center for Earthquake Engineering Research (NCEER) to examine which is more conservative according to the assessment results on 41 liquefiable soil layers at sites in two major cities in Taiwan. Statistical hypothesis testing was used to make the analysis more quantitative and objective. Based on three sets of hypothesis tests, it shows that the hypothesis—the SPT method is more conservative than the Vs method—was not rejected on a 5% level of significance.
