Biomechanical comparison of locking plate and crossing metallic and absorbable screws fixations for intra-articular calcaneal fractures

The locking plate and percutaneous crossing metallic screws and crossing absorbable screws have been used clinically to treat intra-articular calcaneal fractures, but little is known about the biomechanical differences between them. This study compared the biomechanical stability of calcaneal fractures fixed using a locking plate and crossing screws. Three-dimensional finite-element models of intact and fractured calcanei were developed based on the CT images of a cadaveric sample. Surgeries were simulated on models of Sanders type III calcaneal fractures to produce accurate postoperative models fixed by the three implants. A vertical force was applied to the superior surface of the subtalar joint to simulate the stance phase of a walking gait. This model was validated by an in vitro experiment using the same calcaneal sample. The intact calcaneus showed greater stiffness than the fixation models. Of the three fixations, the locking plate produced the greatest stiffness and the highest von Mises stress peak. The micromotion of the fracture fixated with the locking plate was similar to that of the fracture fixated with the metallic screws but smaller than that fixated with the absorbable screws. Fixation with both plate and crossing screws can be used to treat intra-articular calcaneal fractures. In general, fixation with crossing metallic screws is preferable because it provides sufficient stability with less stress shielding.

Summer Monsoon Indices on June Rainfall in Hong Kong and Its Vicinity

Climatologically, June is usually the wettest month in Hong Kong. With significant interannual variation of the summer monsoon, the rainfall variability in June is also large. As Hong Kong is in close proximity to the peripheries of different monsoon regions, the variability of June rainfall largely depends on the relative strength of various monsoon systems. In the present study, a new index comparing the relative condition of the western North Pacific summer monsoon and the South china Sea summer monsoon is developed based on the difference between the respective monsoon indices WNPMI （western North Pacific summer monsoon index） and UMI （unified monsoon index）. It is shown that June rainfall in Hong Kong and its vicinity is better correlated with this new index than either WNPMI or UMI alone. Based on the signs of the new index in conjunction with those of WNPMI and UMI, a framework to stratify the monsoon conditions into different configurations together with a simple scheme to summarize the associated rainfall responses is formulated. This study highlights how the rainfall variability on a local or regional scale can be quantified by the broad-scale conditions of different monsoon systems.

APACHE system is better than Ranson system in the prediction of severity of acute pancreatitis

BACKGROUND: It has been suggested that addition of obesity score to the APACHE-Ⅱ system can lead to more accurate prediction of severity of acute pancreatitis. However there is scanty information on the usefulness of the combined APACHE-O scoring system in Asian patients. This study aimed to compare the accuracy of Ranson, APACHE-Ⅱ and APACHE-O systems in assessing severity of acute pancreatitis in a local chinese population. METHODS: One hundred and one consecutive patients with acute pancreatitis were prospectively studied. Body mass index (BMI) was measured on admission. Ranson score, APACHE-Ⅱ and APACHE-O scores were recorded on admission and at 48 hours. By adopting the cut-off levels and definitions advocated in the Atlanta consensus for severe disease, the diagnostic accuracy of the three scoring systems was compared by the area under the curve (AUC) under the receiver operator characteristic curve. RESULTS: Of the 101 patients, 12 (11.9%) patients suffered from severe pancreatitis. Obesity was uncommon and only two patients (2.0%) had BMI >30. Eighty-two (81.2%) patients were normal weight (BMI≤25) whereas 17 (16.8%) were overweight ( BMI 25-30 ). Overweight or obesity (BMI >25) was not associated with severe pancreatitis (P= 0.40). The AUC for admission scores of Ranson, APACHE-Ⅱ, and APACHE-O systems was 0. 549, 0. 904 and 0. 904, respectively. The AUC for 48-hour scores of Ranson, APACHE-Ⅱ and APACHE-O systems was 0.808, 0.955 and 0.951, respectively. CONCLUSIONS: The APACHE-Ⅱ scoring system is more accurate than the Ranson scoring system of the prediction of severity in acute pancreatitis. Addition of obesity score does not significantly improve the predictive accuracy of the APACHE-Ⅱ system in our local population with a low prevalence of obesity.

A 43-year-old chinese patient with a history of psoriasis developed fulminant ulcerative colitis after immunosuppressive therapy for steroid-resistant minimal change disease was stopped. Minimal change disease in association with inflammatory bowel disease is a rare condition. We here report a case showing an association between ulcerative colitis, minimal change disease, and psoriasis. The possible pathological link between 3 diseases is discussed.

This study identifies the atmospheric circulation features that are favorable for the occurrence of low-level turbulence at Hong Kong International Airport [below 1600 feet （around 500 m）]. By using LIDAR data at the airport, turbulence and nonturbulence cases are selected. It is found that the occurrence of turbulence is significantly related to the strength of the southerly wind at 850 hPa over the South china coast. On the other hand, the east-west wihd at this height demonstrates a weak relation to the occurrence. This suggests that turbulence is generated by flow passing Lantau Island from the south. The southerly wind also transports moisture from the South china Sea to Hong Kong, reducing local stability. This is favorable for the development of strong turbulence. It is also noted that the strong southerly wind during the occurrence of low-level turbulence is contributed by an anomalous zonal gradient of geopotential in the lower troposphere over the South china Sea. This gradient is caused by the combination of variations at different timescales. These are the passage of synoptic extratropical cyclones and anticyclones and the intraseasonal variation in the western North Pacific subtropical high. The seasonal variation in geopotential east of the Tibetan Plateau leads to a seasonal change in meridional wind, by which the frequency of low-level turbulence is maximized in spring and minimized in autumn.

Cathelicidins, are host defense peptides synthesized and stored in circulating leukocytes and numerous types of epithelial tissues in particular the gastrointestinal (GI) tract and skin. They have been known for their antimicrobial activities against a variety of microbes. Recently it was discovered that they have other significant biological functions and produce appealing pharmacological actions against inflammation and cancer in the GI tract through defined mechanisms. Experimental evidence shows that these actions could be tissue and disease specific and concentration dependent. This article reviews some of the physiological functions of cathelicidins and also their therapeutic potential in the treatment of inflammation and cancer and also the delivery system for this peptide as targeted therapy for various disorders in the GI tract both in animals and humans.

A 10-year-old chinese boy who had a history of congenital thrombocytopathy presented with severe iron deficiency anemia secondary to chronic gastric inflammation and duodenal ulcerations. Subtle oculocutaneous albinism led to the finding of diminished dense bodies in the platelets under electron microscopy, hence the diagnosis of Hermansky-Pudlak syndrome (HPS). Biopsies from the stomach and duodenum revealed a lymphocytic infiltration in the submucosa, but H pylori infection was absent. The gastroduodenitis responded to the treatment with omeprazole while iron deficiency anemia was corrected by oral iron therapy. HPS is a rare cause of congenital bleeding disorder with multisystemic manifestations. Upper gastrointestinal involvement is rare and should be distinguished from a mere manifestation of the bleeding diathesis.
