This study aimed to report the outcomes of active surveillance(AS)in the management of low-risk prostate cancer(PCa).It recruited87 men who were prospectively followed up according to the Prostate Cancer Research International Active Surveillance(PRIAS)protocol with local adaptation at SH Ho Urology Centre,Prince of Wales Hospital,Hong Kong,*** investigated the predictorsof disease progression and found that baseline prostate-specific antigen density(PSAD)and the presence of the highest ProstateImaging-Reporting and Data System(PI-RADS)score 5 lesion on magnetic resonance imaging(MRI)are significantly correlatedwith disease ***,men with PSAD>0.2 ng ml^(−2)or PI-RADS 4 or 5 lesions had significantly worse upgradingfree survival compared to those with PSAD≤0.2 ng ml−2 and PI-RADS 2 or 3 *** study concludes that AS is a safe andeffective management strategy for selected patients to defer radical treatment and that most disease progression can be detectedafter the first repeated *** combination of PSAD>0.2 ng ml^(−2)and PI-RADS 4 or 5 lesions may serve as a useful predictorof early disease progression and provide a guide to optimize follow-up protocols for men in different risk groups.

We aim to evaluate prostate health index as an additional risk-stratification tool in patients with Prostate Imaging Reporting and Data System score 3 lesions on multiparametric magnetic resonance *** with biochemical or clinical suspicion of having prostate cancer who underwent multiparametric magnetic resonance imaging in two tertiary centers(Queen Mary Hospital and Princess Margaret Hospital,Hong Kong,china)between January 2017 and June 2022 were ***-magnetic resonance imaging fusion biopsies were performed after prostate health index *** who only had Prostate Imaging Reporting and Data System score 3 lesions were further stratified into four prostate health index risk groups and the cancer detection rates were *** of the 747 patients,47.3%had Prostate Imaging Reporting and Data System score 3 lesions *** detection rate of clinically significant prostate cancer in this group was 15.0%.The cancer detection rates of clinically significant prostate cancer had statistically significant differences:5.3%in prostate health index<25.0,7.4%in prostate health index 25.0-34.9,17.9%in prostate health index 35.0-54.9,and 52.6%in prostate health index≥55.0(P<0.01).Among the patients,26.9%could have avoided a biopsy with a prostate health index<25.0,at the expense of a 5.3%risk of missing clinically significant prostate *** health index could be used as an additional risk stratification tool for patients with Prostate Imaging Reporting and Data System score 3 *** could be avoided in patients with low prostate health index,with a small risk of missing clinically significant prostate cancer.

A biosynthetic gene cluster for the bioactive fungal sesterterpenoids variecolin(1)and variecolactone(2)was identified in Aspergillus aculeatus ATCC *** production of 1 and 2 was achieved in Aspergillus oryzae by expressing the sesterterpene synthase VrcA and the cytochrome P450 ***,the replacement of VrcB with homologous P450s from other fungal terpenoid pathways yielded three new variecolin analogues(5-7).Analysis of the compounds’anticancer activity in vitro and in vivo revealed that although 5 and 1 had comparable activities,5 was associated with significantly reduced toxic side effects in cancer-bearing mice,indicating its potentially broader therapeutic *** study describes the first tests of variecolin and its analogues in animals and demonstrates the utility of synthetic biology for creating molecules with improved biological activities.

In this paper,two lifting mechanism models with opposing placements,which use the same hydraulic hoist model and have the same angle of 50°,have been *** mechanical and hydraulic simulation models are established using MATLAB Simscape to analyze their kinetics and dynamics in the lifting and holding *** simulation findings are compared to the analytical calculation results in the steady state,and both methods show good *** the early lifting stage,Model 1 produces greater force and discharges goods in the container faster than Model ***,Model 2 reaches a higher force and ejects goods from the container cleaner than its counterpart at the end lifting *** established simulation models can consider the effects of dynamic loads due to inertial moments and forces generated during the system *** is crucial in studying,designing,and optimizing the structure of hydraulic-mechanical systems.

BACKGROUND:This study aimed to evaluate the discriminatory performance of 11 vital sign-based early warning scores(EWSs)and three shock indices in early sepsis prediction in the emergency department(ED).METHODS:We performed a retrospective study on consecutive adult patients with an infection over 3 months in a public ED in Hong *** primary outcome was sepsis(Sepsis-3 definition)within 48 h of ED *** c-statistics and the DeLong test,we compared 11 EWSs,including the National Early Warning Score 2(NEWS2),Modified Early Warning Score,and Worthing Physiological Scoring System(WPS),etc.,and three shock indices(the shock index[SI],modified shock index[MSI],and diastolic shock index[DSI]),with Systemic Inflammatory Response Syndrome(SIRS)and quick Sequential Organ Failure Assessment(qSOFA)in predicting the primary outcome,intensive care unit admission,and mortality at different time ***:We analyzed 601 patients,of whom 166(27.6%)developed ***2 had the highest point estimate(area under the receiver operating characteristic curve[AUROC]0.75,95%CI 0.70-0.79)and was significantly better than SIRS,qSOFA,other EWSs and shock indices,except WPS,at predicting the primary ***,the pooled sensitivity and specificity of NEWS2≥5 for the prediction of sepsis were 0.45(95%CI 0.37-0.52)and 0.88(95%CI 0.85-0.91),*** discriminatory performance of all EWSs and shock indices declined when used to predict mortality at a more remote time ***:NEWS2 compared favorably with other EWSs and shock indices in early sepsis prediction but its low sensitivity at the usual cut-off point requires further modification for sepsis screening.

This article addresses the finite-time boundedness(FTB)problem for nonlinear descriptor ***,the nonlinear descriptor system is represented by the Takagi-Sugeno(T-S)model,where fuzzy representation is assumed to be appearing not only in both the state and input matrices but also in the derivative *** using a descriptor redundancy approach,the fuzzy representation in the derivative matrix is reformulated into a linear ***,we introduce a fuzzy sliding mode control(FSMC)law,which ensures the finite-time boundedness(FTB)of closed-loop fuzzy control systems over the reaching phase and sliding motion ***,by further employing the descriptor redundancy representation,the sufficient condition for designing FSMC law,which ensures the FTB of the closed-loop control systems over the entire finite-time interval,is derived in terms of linear matrix inequalities(LMIs).Finally,a simulation study with control of a photovoltaic(PV)nonlinear system is given to show the effectiveness of the proposed method.

Compensated methods for networked control system with packet drops based on compressed sensing

Due to unreliable and bandwidth-limited characteristics of communication link in networked control systems,the realtime compensated methods for single-output systems and multioutput systems are proposed in this paper based on the compressed sensing(CS)theory and sliding window technique,by which the estimates of dropping data packets in the feedback channel are obtained and the performance degradation induced by packet drops is ***,in order to reduce the cumulative error caused by the algorithm,the compensated estimates for single-output systems are corrected via the regularization term;considering the process of single-packet transmission,a new sequential CS framework of sensor data streams is introduced to effectively compensate the dropping packet on single-channel of multi-output systems;in presence of the medium access constraints on multi-channel,the communication sequence for scheduling is coupled to the algorithm and the estimates of the multiple sensors for multi-output systems are obtained via the regularization *** results illustrate that the proposed methods perform well and receive satisfactory performance.

Fungus Aspergillus sp., which was isolated from soft coral Sarcophyton tortuosum, was cultured in the GPY medium containing glucose 10 g/L, peptone 5 g/L, yeast extract 2 g/L, sea water 1 L, at pH=7.5. Four com pounds, 3,6-diisobutyl-2（1H）-pyrazinone（1）, 3-isobutyl-6-（1-hydroxy-2-methylpropyl）-2（1H）-pyrazinone（2）, 3-methoxy-4-methyl-2,4-dien-pentanoic acid（3） and penicillic acid（4） were obtained from the AcOEt extract of the culture broth. Their structures were elucidated mainly based on the NMR, HR-EI-MS and X-ray single crystal diffraction experimental data. Compound 3 is a new compound. Compound 1 was previously proposed to be the tautomer of flavacol（3,6-diisobutylpyrazin-2-ol, 5）. However, the evaluation of the NMR and X-ray single crystal diffraction experimental data permitted us to propose that compound 1 existed as amide form instead oftautomers. Compound 1 is stable both in solution and crystal due to strong intermolecular hydrogen bonds. Incidentally supplying anthracenone to the GPY medium with a dose of 100 mg/L, the yield of penicillic acid（4） in the culture broth of the strain Aspergillus sp. was increased dramatically from 6 mg/L to 57 mg/L.
