Introduction Pelvic floor disorders(PFDs)pose substantial physical and psychological burdens for a growing number of *** the ubiquity of these conditions and known patient reluctance to seek care,primary care providers(PCPs)have a unique opportunity to increase treatment and provide appropriate referrals for these *** An online survey was administered to PCPs to assess provider practices,knowledge,comfort managing and ease of referral for *** regression was used to assess the association between demographic/practice characteristics of PCPs and two primary outcomes of interest:discomfort with management and difficulty with referral of *** Of the 153 respondents to the survey,more felt comfortable managing stress urinary incontinence(SUI)and overactive bladder(OAB),compared with pelvic organ prolapse(POP)and faecal incontinence(FI)and were less likely to refer patients with urinary *** providers elicited symptoms for POP and FI as compared with SUI and *** variables that were significantly associated with discomfort with management varied by PFD,but tended to correlate with less exposure to PFDs(eg,those with fewer years of practice,and internal medicine and family physicians as compared with geriatricians);whereas the factors that were significantly associated with difficulty in referral,again varied by PFD,but were related to practice characteristics(eg,specialist network,type of practice,practice setting and quantity of patients).Conclusion These findings highlight the need to increase PCPs awareness of PFDs and develop effective standardised screening protocols,as well as collaboration with pelvic floor specialists to improve screening,treatment and referral for patients with PFDs.

BACKGROUND Sleep deprivation can lead to increased body weight and blood pressure(BP),but the latent effects of partial sleep deprivation related to required night sentry duties within a short-term period on cardiometabolic characteristic changes in military personnel are *** To investigate the association between night sentry duty frequency in the past 3 months and cardiometabolic characteristics in armed forces *** A total of 867 armed forces personnel who were aged 18-39 years and did not take any antihypertensive medications in Taiwan in 2020 were *** frequency of night sentry duty was self-reported via a questionnaire(average number of night sentry shifts per month for the past 3 months).Hemodynamic status was assessed via the resting BP and pulse rate(PR).Cardiometabolic risk factors were defined according to the International Diabetes Federation *** linear regression analyses of the associations between night sentry duties and PR,BP,and other metabolic syndrome(MetS)marker levels were performed,with adjustments for age,sex,substance use,body mass index and aerobic *** logistic regression analysis was carried out to determine the associations between night sentry duties and the prevalence of each MetS feature.

Gender differences in patients undergoing coronary stenting in current stent era

Background Prior studies have demonstrated worse results of women in both hospital and short-term outcomes post-percutaneous coronary intervention. However, with advanced devices like drug-eluting stents （DESs） available,there are no consistent data revealing gender impact in outcome. This study examined whether gender affected hospital outcome and showed one-year single-center patient results of coronary *** The study group included 969 consecutive patients （250 women and 719 men） undergoing coronary stenting for stable or unstable angina. Clinical events were assessed for at least 1 year *** Compared to men, women were older, presented more often with diabetes, hypertension, dyslipidemia, and lower creatinine clearance rate （Ccr）; they had less percutaneous transluminal coronary angioplasty （PTCA） history,smaller vessel size, and shorter lesions. The hospital major adverse cardiovascular event （MACE） rate was 2.8% of women and 0.97% of men （P=0.037）. The one-year MACE rate was 10.0% of women and 10.4% of men （P=0.874）. After adjusting other covariates, women still had significantly higher hospital MACE rates （P=0.034） and odds ratios （0.18;95% confidence interval： 0.036-0.874）. In women （n=250）, there was no statistically significant difference in hospital or one-year MACE between bare metal stent （BMS） and DES groups. Meanwhile, in men （n=719）, DES had a significant one-year improvement of MACE compared to BMS （P=0.004）. The female hospital MACE rate was five times greater than male results. However, there were similar one-year outcomes between women and men. DES currently have an advantage in long-term *** Currently, with the use of BMS and DES, adverse hospital post-procedure cardiovascular event rate has occurred more often in women than in men. However, the MACE rate differences between women and men resolved with one year follow-up.

Proactive infection control measures to prevent nosocomial transmission of carbapenem-resistant Enterobacteriaceae in a non-endemic area

Background Identification of hospitalized carbapenem-resistant Enterobacteriaceae （CRE）-positive patient is important in preventing nosocomial *** objective of this study was to illustrate the implementation of proactive infection control measures in preventing nosocomial transmission of CRE in a healthcare region of over 3200 beds in Hong Kong between October 1,2010 and December 31,*** The program included active surveillance culture in patients with history of medical tourism with hospitalization and surgical operation outside Hong Kong within 12 months before admission,and ＂added test＂ as an opportunistic CRE screening in all fecal specimens submitted to the *** investigation and contact tracing were conducted for CRE-positive *** quantitative culture was performed on CRE-positive patients and the duration of fecal carriage of CRE was *** During the study period,a total of 6533 patients were screened for CRE,of which 76 patients were positive （10 from active surveillance culture,65 from ＂added test＂,and 1 secondary case from contact tracing of 223 patients with no nosocomial outbreak）,resulting in an overall rate of CRE fecal carriage of 1.2％.The median time of fecal carriage of CRE was 43 days （range,13-119 days）.Beta-lactam-beta-lactamase-inhibitors,cephalosporins,and fluoroquinolones were associated significantly with high fecal bacterial load when used 90 days before CRE detection,while use of cephalosporins,carbapenems,and fiuoroquinolones after CRE detection are significantly associated with longer duration of *** duration of fecal carriage of CRE also correlates significantly with the initial fecal bacterial load （Pearson correlation：0.53; P=0.02）.Conclusion Proactive infection control measures by enhanced surveillance program identify CRE-positive patients and data obtained are useful for the planning of and resource allocation for CRE control.

目的探讨冠状动脉内血管性血友病因子裂解蛋白酶(ADAMTS13)活性和全球急性冠状动脉事件注册(grace)评分对ST段抬高型心肌梗死(STEMI)行急诊经皮冠脉介入术(PCI)的病人住院期间主要心血管不良事件(MACE)的预测价值。方法选取2019年10月至2021年6月在潍坊医学院附属医院住院的162例STEMI接受急诊PCI治疗的病人,根据住院期间是否发生MACE,分为MACE组(38例)和无MACE组(124例)。记录两组病人的重要临床特征、grace评分。采用荧光共振能量转换法(FRETS-VWF73)分别测定两组病人急诊PCI术前外周血和冠脉内的ADAMTS13活性。四格表χ~2检验及独立样本t检验比较两组病人的重要临床特征、grace评分。Pearson相关性分析探讨ADAMTS13活性与grace评分的相关性。多因素logistic回归分析MACE的独立危险因素。应用受试者操作特征曲线(ROC曲线)分析冠脉内ADAMTS13活性、grace评分以及联合诊断对MACE的预测效能。结果两组比较,MACE组病人糖尿病史、吸烟史更多,年龄较大、外周血血小板计数更低、凝血酶原时间(PT)更长、N端脑钠肽前体(NT-proBNP)和肌钙蛋白I(cTnI)更高(Pgrace评分更高[(187.26±35.50)分比(153.87±32.95)分,Pgrace评分中等强度负相关(r=-0.40,Pgrace评分无明显相关性(r=-0.08,P=0.457)。多因素logistic回归分析,糖尿病史和grace评分为MACE的独立危险因素[OR=2.513,95%CI:(2.045,3.381),P=0.007;OR=1.089,95%CI:(1.033,1.147),P=0.001],术前冠脉内ADAMTS13活性是MACE的保护性因素[OR=0.568,95%CI:(0.429,0.753),P=0.001]。术前冠脉内ADAMTS13活性、grace评分诊断MACE的AUC及其95%CI分别为0.833(0.735,0.932)和0.746(0.624,0.867),截断值分别为50.95%和158.50分,联合诊断的AUC为0.893(0.824,0.962)。结论 grace评分增高、术前冠脉内ADAMTS13活性降低均是接受急诊PCI治疗的STEMI病人住院期间MACE的独立危险因素,二者联合诊断的预测价值更高。

目的研究能够预测grace评分为高风险的急性冠脉综合征患者远期发生射血分数减低型心力衰竭风险的生物标记物。方法收集2015年至2018年我院收治的CRACE风险分层为高风险的患者231例。所有患者治疗出院后1个月再次检测血清sST2和Galectin-3水平。计算治疗出院后1月内sST2和Galectin-3的下降百分比。随访12个月,终点事件定义为12个月内发生的射血分数减低型心力衰竭(heart failure with reduced ejection fraction,HFrEF)。受试者工作曲线评估血清sST2和Galectin-3对于既往NSTE-ACS患者发生HFrEF的预测价值,同时根据约登指数和最佳截断值对进行患者分组。chi-Square检验分析不同分组患者1年内发生HFrEF的发生风险,Logistic多因素回归分析明确1年内NSTE-ACS患者发生HFrEF的独立危险因素。结果所有GARCE评分高风险的NST-ACS患者出院后1年内HFrEF发生率为34.2%。sST2和Galectin-3下降百分比的受试者工作曲线下面积(Area under curve,AUC)分别为0.86和0.69。最佳截断值分别为43%和54%。chi-Square分析显示相对于sST2下降百分比超过43%的患者其1年内HFrEF发生率相对于对照组显著降低(=22.50,Pgrace高风险的NSTE-ACS患者出院后1个月的sST下降程度是其1年内发生HFrEF的独立危险因素,出院后1个月内下降比例超过54%的患者具有更低的发生HFrEF的风险。
