Pyogenic liver abscess caused by Klebsiella pneumoniae:analysis of the clinical characteristics and outcomes of 84 patients

Background The increased incidence of pyogenic liver abscess caused by Klebsiella pneumoniae （K.pneumoniae） was reported in the recent literature. This study was conducted retrospectively to investigate the clinical characteristics and outcomes of these patients. Methods Microbiological and medical databases of a medical center were searched from January 2000 to June 2003. Eighty-four patients with liver abscess caused by K. pneumoniae were analyzed. Results In the 84 patients, 52 men and 32 women aged （58.2±13.3） years on average, 64.4% had concomitant diabetes mellitus and 23.8% had biliary disease. The most common clinical symptoms were fever （98.8%）, chills （69.0%） and abdominal pain （58.3%）. 85.7% of the 84 patients received catheter drainage for the abscess. The length of hospital stay was （17.4 ±8.7） days. The mortality rate was 7.1%. Older age and presence of biliary disease were associated with mortality. Conclusions The low mortality of our patients was probably related to the high proportion of patients who received catheter drainage. Older age and presence of biliary disease were associated with the mortality.

BACKGROUND: Simulation-based medical education has been growing rapidly and becomes one of the most popular teaching methods for improving patient safety and patient care. The Simulation Subcommittee of the Hong Kong College of Emergency Medicine organized an educational program emphasizing the team training, clinical decision-making and communication skills. This study aimed to evaluate the attitude of the participants toward a new training program and the change in the knowledge on clinical performance in emergency physicians and nurses after attending the educational program.METHODS: A course evaluation form was filled in by the participants at the end of the workshop. An assessment of 20 multiple-choice questions with 5 options was administered to the participants before and after the 2-day simulation-based training workshop.RESULTS: A total of 72 doctors and nurses working in the Accident and Emergency Department were enrolled. The average pretest and posttest scores were 12 and 14.3 respectively. The percentage improvement in the mean score of the pretest and posttest was 11.5%. The chi-square test showed signifi cant improvement in the pretest and posttest score grading(P=0.00). Paired t-test revealed signifi cant difference between the mean scores of the pretest and posttest(P=0.00).CONCLUSIONS: Participants had positive attitude toward this new training program. Significant improvement of the knowledge on clinical performance in healthcare professionals in the Accident and Emergency Department was observed after the participation in this simulation-based educational program.

许多非感染性眼炎性疾病是根据病史和临床表现来分类的。近年来,它们的发病机理已不那么模糊了,看来,自身免疫性病变起着重要作用。为评价各种葡萄膜炎免疫活性细胞的特性和局部定位,已用针对各种炎症细胞的特殊膜抗原的单克隆抗体来进行免疫组织学研究。事实证明了炎症病灶占优势的细胞主要是 T-淋巴细胞,早期为辅助性/诱导性 T 细胞,而后期为抑制性/细胞毒性 T 细胞则相对增多。这一事实得到了实验性自身免疫性葡萄膜炎的相似发现的支持(和阐明),提示了免疫病理病变起始时 T-辅助性/诱导性细胞起主要作用,而抑制性/细胞毒性细胞则减弱—调节眼炎症活动。基于诊断的目的,我们在正常眼组织上滴几滴取自各种类型葡萄膜炎病人的血清进行染色;这样血清作为初次抗体用来检测针对正常组织里的光感受器和M(?)ller 细胞的自身免疫抗体。结果反映继发的或病理的现象与原发的致病因子稍有(或没有)直接关系。这一技术也利于诊断和鉴别某些不确定的葡萄膜炎。

Risk of cardiovascular thrombotic events after surgical castration versus gonadotropin-releasing hormone agonists in chinese men with prostate cancer

We investigated the cardiovascular thrombotic risk after surgical castration （SC） versus genadotmpin-releasing hormone agonists （GnRHa） in chinese men with prostate cancer. All chinese prostate cancer patients who were treated with SC or GnRHa from year 2000 to 2009 were reviewed and compared. The primary outcome was any new-onset of cardiovascular thrombotic events after SC or GnRHa, which was defined as any event of acute myocardial infarction or ischemic stroke. The risk of new-onset cardiovascular thrombotic event was compared between the SC group and the GnRHa group using Kaplan-Meier method. Multivariate Cox regression analysis was performed to adjust for other potential confounding factors. A total of 684 chinese patients was included in our study, including 387 patients in the SC group and 297 patients in the GnRHa group. The mean age in the SC group （75.3 ~ 7.5 years） was significantly higher than the GnRHa group （71.8 ~ 8.3 years）（P 〈 0.001）. There was increased risk of new cardiovascular thrombotic events in the SC group when compared to the GnRHa group upon Kaplan-Meier analysis （P= 0.014）. Upon multivariate Cox regression analysis, age （hazard ratio [HR] 1.072, 95% confidence interval [CI] 1.04-1.11, P 〈 0.001）, hyperlipidemia （HR 2.455, 95% Cl 1.53-3.93, P 〈 0.001）, and SC （HR 1.648, 95% CI 1.05-2.59, P = 0.031） were significant risk factors of cardiovascular thrombotic events. In conclusion, SC was associated with increased risk of cardiovascular thrombotic events when compared to GnRHa. This is an important aspect to consider while deciding on the method of androgen deprivation therapy, especially in elderly men with known history of hyperlipidemia.

Including the middle hepatic vein in the right lobe liver graft has the advantage of providing direct venous drainage of the right anterior segment. To allow unimpeded passage of blood flow, we previously designed venoplasty of the middle and right hepatic veins. We found that venoplasty is also feasible when the inferior right hepatic vein is near to the right hepatic vein, or when multiple segment 8 hepatic vein orifices are exposed adjacent to the middle hepatic vein at the graft transection surface. By joining the hepatic vein orifices into a single opening, the anastomosis into the inferior vena cava is much facilitated. The technique is simple, yet versatile , and able to cope with variation of the configurations of the hepatic vein.

Background Data of classical inborn errors of metabolism （IEM） of amino acids, organic acids and fatty acid oxidation are largely lacking in Hong Kong, where mass spectrometry-based expanded newborn screening for IEM has not been initiated. The current study aimed to evaluate the approximate incidence, spectrum and other characteristics of classical IEM in Hong Kong, which would be important in developing an expanded newborn screening program for the local area.

For the non-stop demands for a better and smarter society, the number of electronic devices keeps increasing exponentially;and the computation power, communication data rate, smart sensing capability and intelligence are always not enough. ABSTRACT: Fulminant hepatic failure is a medical emer-gency. When this condition declared itself irreversible, atimely liver transplantation is the only effective treatment.A 34-year-old chinese with fulminant hepatic failure wasevaluated as a potential liver transplantation candidate. Onthe erect chest radiograph, chilaiditi' s sign has developedover a very short period of a week due to rapid shrinkage ofthe liver. Awareness of chilaiditi' s sign facilitated distin-guishing the condition of free gas under the diaphragm dueto bowel perforation and subphrenic abscess by gas formingmicro-organisms. Rapidity of onset of this sign parallels thedeterioration of liver function and reflects the urgency ofcondition.

Quercetin （QE） and resveratrol （RSV） are powerful antioxidants with the potential to protect the testes against ischemia/reperfusion （I/R） injury. We compared their effects in testicular torsion/detorsion （T/D） in adult rats. Twenty-four male Wistar rats were divided into four groups： sham （group A）, T/D （group B）, T/D treated with QE （group C）, and T/D treated with RSV （group D）. QE （20 mg kg-1） and RSV （20 mg kg-1） were injected intra-peritoneally at 60 min of torsion. After 90 rain of surgically induced torsion, the testicular cord was restored to its anatomical position. Twenty-four hour after torsion, blood and tissue samples were obtained for further examination. Testicular tissue malondialdehyde （MDA） and nitric oxide （NO） levels and serum total oxidant status （TOS） were higher in group B than in group A （P 〈 0.05）. Group A had higher serum total antioxidant status （TAS） than group B. （P 〈 0.05） QE and RSV significantly lowered MDA, NO, and TOS levels and TAS consumption （P 〈 0.05）. QE reduced the MDA and TOS levels more than RSV （P〈 0.05）, but their effects on NO reduction and TAS consumption were similar （P〉 0.05）. Group A had normal testicular architecture （grade 1）. Groups C （mean grade 2.60） and D （mean grade 3.00） had lower testicular injury grades than group B （mean grade 3.45）（P 〈 0.05）. Group C had lower testicular injury grade than group D （P 〈 0.05）. Treatment with QE and RSV protects against I/R injury after testicular T/D. QE may exhibit better function than RSV at the doses tested in this study.
