High sensitivity C-reactive protein and cardfiac resynchronization therapy in patients with advanced heart failure

Background The data on the prognostic values of high sensitivity C-reactive protein （hsCRP） levels in patients with advanced symp-tomatic heart failure （HF） receiving cardiac resynchronization therapy （CRT） are scarce. The aim of present study was to investigate the association of serum hsCRP levels with left ventricle reverse remodeling after six months of CRT as well as long-term outcome. Methods A total of 232 CRT patients were included. The assessment of hsCRP values, clinical status and echocardiographic data were performed at baseline and after six months of CRT. Long-term follow-up included all-cause mortality and hospitalizations for HF. Results During the mean follow-up periods of 31.3 ± 31.5 months, elevated hsCRP （〉3 mg/L） prior to CRT was associated with a significant 2.39-fold increase （P=0.006） in the risk of death or HF hospitalizations. At 6-month follow-up, patients who responded to CRT showed significant reductions or maintained low in hsCRP levels （–0.5 ± 4.1 mg/L reduction） compared with non-responders （1.7 ± 6.1 mg/L increase, P=0.018）. Com-pared with patients in whom 6-month hsCRP levels were reduced or remained low, patients in whom 6-month hsCRP levels were increased or maintained high experienced a significantly higher risk of subsequent death or HF hospitalizations （Log-rank P〈0.001）. The echocardio-graphic improvement was also better among patients in whom 6-month hsCRP levels were reduced or remained low compared to those in whom 6-month hsCRP levels were raised or maintained high. Conclusions Our findings demonstrated that measurement of baseline and follow-up hsCRP levels may be useful as prognostic markers for timely potential risk stratification and subsequent appropriate treatment strategies in patients with advanced HF undergoing CRT.

The Large sky Area Multi-Object Fiber Spectroscopic Telescope（LAMOST） general survey is a spectroscopic survey that will eventually cover approximately half of the celestial sphere and collect 10 million spectra of stars, galaxies and QSOs. Objects in both the pilot survey and the first year regular survey are included in the LAMOST DR1. The pilot survey started in October 2011 and ended in June 2012, and the data have been released to the public as the LAMOST Pilot Data Release in August 2012. The regular survey started in September 2012, and completed its first year of operation in June 2013. The LAMOST DR1 includes a total of 1202 plates containing 2 955 336 spectra, of which 1 790 879 spectra have observed signalto-noise ratio（SNR） ≥ 10. All data with SNR ≥ 2 are formally released as LAMOST DR1 under the LAMOST data policy. This data release contains a total of 2 204 696 spectra, of which 1 944 329 are stellar spectra, 12 082 are galaxy spectra and 5017 are quasars. The DR1 not only includes spectra, but also three stellar catalogs with measured parameters： late A,FGK-type stars with high quality spectra（1 061 918 entries）, A-type stars（100 073 entries）, and M-type stars（121 522 entries）. This paper introduces the survey design, the observational and instrumental limitations, data reduction and analysis, and some caveats. A description of the FITS structure of spectral files and parameter catalogs is also provided.

Mammalian target of rapamycin(m TOR) inhibitor as an attractive drug target with promising antitumor effects has been widely investigated. High quality clinical trial has been conducted in liver transplant(LT) recipients in Western countries. However, the pertinent studies in Eastern world are paucity. Therefore, we designed a clinical trial to test whether sirolimus can improve recurrence-free survival(RFS) in hepatocellular carcinoma(HCC) patients beyond the Milan criteria after LT. This is an open-labeled, single-arm, prospective, multicenter, and real-world study aiming to evaluate the clinical outcomes of early switch to sirolimus-based regimens in HCC patients after LT. Patients with a histologically proven HCC and beyond the Milan criteria will be enrolled. The initial immunosuppressant regimens are center-specifc for the frst 4-6 weeks. The following regimens integrated sirolimus into the regimens as a combination therapy with reduced calcineurin inhibitors based on the condition of patients and centers. The study is planned for 4 years in total with a 2-year enrollment period and a 2-year follow-up. We predict that sirolimus conversion regimen will provide survival benefts for patients particular in the key indicator RFS as well as better quality of life. If the trial is conducted successfully, we will have a continued monitoring over a longer follow-up time to estimate indicator of overall survival. We hope that the outcome will provide better evidence for clinical decision-making and revising treatment guidelines based on chinese population data.

Background A multi-center large scale study is needed to confirm the efficacy and safety of domestic peritoneal dialysis （PD） solutions. Some researchers believe that 6 lid is enough for adequate dialysis, but there is no multi-center prospective study on chinese population to confirm this. In this study, we evaluated the efficacy and safety of domestic PD solution （Changfu） and its difference between 6 L and 8 L dosage. Methods Adult PD patients who had taken PD therapy for at least one month were selected and divided into four groups according to two dialysis solution brands and two dialysis dosages, i.e., 6 L dose with Changfu dialysis solution, 6 L dose with Baxter dialysis solution, 8 L dose with Changfu dialysis solution, and 8 L dose with Baxter dialysis solution. After 48 weeks, the changes of primary and secondary efficacy indices were compared between different types and different dosages. We also analyzed the changes of safety indices. Results Changes of Kt/V from baseline to 48 weeks between Changfu and Baxter showed no statistical differences; so did those of creatinine clearance rate （Ccr）. Normalized protein catabolic rate （nPCR） from baseline to 48 weeks between Changfu and Baxter showed no statistical differences; so did those of net ultrafiltration volume （nUF） and estimated glomerular filtration rate （eGFR）. Changes of nPCR from baseline to 48 weeks between 6 L and 8 L showed no statistical differences; so did those of nUF and eGFR. The decline of Kt/V from baseline to 48 weeks in 6 L group was more than that in 8 L group. Change of Ccr was similar. During the 48-week period, the mean Kt/V was above 1.7/w, and mean Ccr was above 50 L.1.73 m-2.w1. More adverse events were found in Changfu group before Changfu Corporation commenced technology optimization, and the statistical differences disappeared after that. 目的:探讨心脏再同步治疗(CRT)对复极离散度和室性心律失常的影响。方法:入选86例CRT植入患者,根据术后6个月左心室射血分数(LVEF)绝对值是否较基线水平提高≥10%,分为反应组(n=43)和无反应组(n=43)。比较两组患者CRT植入后不同时期QRS间期、校正的QT间期(QTc间期)、T波峰末间期(Tp Te间期)等复极离散度指标变化及室性心律失常事件情况。结果:反应组患者术后1年QRS间期和Tp Te间期较术前和术后0.05)。在术后1年中,反应组室性早搏(PVCs)和室性早搏连发(PVC runs)的发生次数,明显少于无反应组[lg PVCs:(1.78±0.77)vs(2.73±0.61);lg PVC runs:(0.64±0.48)vs(1.98±0.72),P<0.05]。分别对CRT术后lg(PVCs)和lg(PVC runs)的多重线性回归分析发现,术后<24 h的Tp Te间期是lg(PVCs)和lg(PVC runs)的独立预测因素(B=0.143,比值比=1.154,P=0.001;B=0.122,比值比=1.047,P=0.001)。结论:CRT心室逆重构可以减小复极离散度,降低患者室性心律失常事件发生风险,改善预后。术后当天的Tp Te间期时限对CRT术后室性心律失常事件有预测意义。

目的:建立首批贝那鲁肽国家标准品。方法:采用质谱法、N-末端氨基酸序列测定、氨基酸比值法以及质量肽图法进行结构确证;采用HPLC法测定纯度;采用质量平衡法对原料药A(大规格样品)进行含量赋值,然后以原料药A为对照品,采用HPLC外标法对原料药B(待标品)的含量进行标定;采用体外细胞培养法对原料药B(待标品)进行效价测定。并对原料药B(待标品)稳定性及均一性进行了考察。结果:确证了贝那鲁肽的结构,并确定了首批贝那鲁肽国家标准品的含量为0.932 mg·支^(-1),生物效价为1.0×10^(4)U·支^(-1)。原料药B(待标品)分别在4℃及25℃,RH 80%条件下放置10 d,纯度无明显下降,光照10 d纯度明显下降。15支原料药B(待标品)含量测定结果的RSD为0.86%,均一性良好。结论:首批贝那鲁肽国家标准品可作为贝那鲁肽及相关制剂HPLC法鉴别、含量测定及生物活性测定用标准品。

The Jiangmen Underground Neutrino Observatory(JUNO)features a 20 kt multi-purpose underground liquid scintillator sphere as its main *** of JUNO's features make it an excellent location for^8B solar neutrino measurements,such as its low-energy threshold,high energy resolution compared with water Cherenkov detectors,and much larger target mass compared with previous liquid scintillator *** this paper,we present a comprehensive assessment of JUNO's potential for detecting^8B solar neutrinos via the neutrino-electron elastic scattering process.A reduced 2 MeV threshold for the recoil electron energy is found to be achievable,assuming that the intrinsic radioactive background^(238)U and^(232)Th in the liquid scintillator can be controlled to 10^(-17)g/*** ten years of data acquisition,approximately 60,000 signal and 30,000 background events are *** large sample will enable an examination of the distortion of the recoil electron spectrum that is dominated by the neutrino flavor transformation in the dense solar matter,which will shed new light on the inconsistency between the measured electron spectra and the predictions of the standard three-flavor neutrino oscillation *** m^(2)_(21)=4.8times10^(-5);(7.5times10^(-5))eV^(2),JUNO can provide evidence of neutrino oscillation in the Earth at approximately the 3sigma(2sigma)level by measuring the non-zero signal rate variation with respect to the solar zenith ***,JUNO can simultaneously measureDelta m^2_(21)using^8B solar neutrinos to a precision of 20% or better,depending on the central value,and to sub-percent precision using reactor antineutrinos.A comparison of these two measurements from the same detector will help understand the current mild inconsistency between the value of Delta m^2_(21)reported by solar neutrino experiments and the KamLAND experiment.

JUNO is a multi-purpose neutrino observatory under construction in the south of *** publication presents new sensitivity estimates for the measurement of the △m_(31)^(2),△m_(21)^(2),sin^(2)θ_(12),and sin^(2)θ_(13) oscillation parameters using reactor antineutrinos,which is one of the primary physics goals of the *** sensitivities are obtained using the best knowledge available to date on the location and overburden of the experimental site,the nuclear reactors in the surrounding area and beyond,the detector response uncertainties,and the reactor antineutrino spectral shape constraints expected from the TAO satellite *** is found that the △m_(21)^(2) and sin^(2)θ_(12) oscillation parameters will be determined to 0.5%precision or better in six years of data *** the same period,the △m_(31)^(2) parameter will be determined to about 0.2%precision for each mass ordering *** new precision represents approximately an order of magnitude improvement over existing constraints for these three parameters.
