Background: In addition to the control of gene transla- tion intra-cellularly, microRNAs (miRNAs) have been found to exist extracellularly. However, extracellular miRNA identified in previous studies were largely confined to microvesicles. It remains uncertain whether free extracellular miRNA exists. Methods: We quantify a panel of miRNAs (miRNA 200 family, miR-205, and miR-192) in spent culture medium of renal tubular epithelial cells, as well as serum and urine from patients with systemic lupus erythematosus and healthy controls. Microvesicle bound and free miRNA were separated by ultracentrifugation. Results: In spent cell culture medium, we found substantial amount of miR-200a, miR-200b, miR-200c, miR-141, miR-429, miR-205, and miR-192 in microvesicles, as well as the presence of miR-200a, miR-200b, miR-429, miR-205 and miR-192, but not miR-200c or miR-141 as non-vesicle bound free form. In healthy individuals, we found substantial amount of miR-200a, miR-200b, miR-200c, miR-141 and miR-192 in microvesicles from serum and urine, while only miR-141 exists as free form in both serum and urine. There was no significant difference in serum or urinary free miR-141 levels between SLE patients and healthy controls. In SLE patients, urinary free miR-141 level was signifiantly higher than serum. Neither serum nor urinary free miR-141 levels correlated with lupus disease activity. Conclusion: We found miRNA is present extracellularly, both within microvesicles and as free form, in spent cell culture medium, serum and urine. The biological role of extracellular miRNA requires further study.

Background Podocyte injury probably plays important roles in the pathogenesis of IgA nephropathy (IgAN). We studied intra-renal and urinary messenger RNA (mRNA) expression of podocyte-associated molecules in patients with IgAN. Methods We studied 43 consecutive patients with biopsy-proven IgAN. Intra-renal and urinary expression of mRNAs was determined and compared to that of 20 patients with nephrectomy for kidney cancer and 12 normal subjects. Results Intra-renal mRNA expression levels of nephrin, podocin and synaptopodin were significantly lower in patients with IgAN than that of controls. In contrast, their urinary mRNA expression levels were similar. Intra-renal gene expression of nephrin inversely correlated with proteinuria (r = –0.620, P <0.001), GFR (r = 0.538, P <0.001), and the degree of tubulointerstitial scarring (r = –0.423, P = 0.013). After followed for an average of 33.4 ? 12.6 months, intra-renal nephrin expression significantly correlated with the rate of GFR decline (r = 0.324, P = 0.041). Conclusions Intra-renal mRNA expression of podocyte associated molecules was down-regulated in patients with IgAN, and the degree of down- regulation of nephrin correlated with disease severity and the rate of progression. Our result supports the hypothesis that podocyte injury is an important component in the pathophysiology of IgAN.

Background The role of neutrophil gelatinase associated lipocalin (NGAL) and Toll-like receptor 9 (TLR9) in the pathogenesis of lupus nephritis remain elusive. Methods We quantified the glomerular and tubulointerstitial mRNA expression of NGAL and TLR9 in 42 patients with lupus nephritis (LN group) and 10 controls. Results As compared to controls, LN group had higher glomerular expression of TLR9, and higher tubulointerstitial expression of NGAL and TLR9. Tubulointerstitial NGAL expression significantly correlated with proteinuria (r = 0.492;p = 0.003), renal function (r = -0.386;p = 0.022) and histological chronicity index (r = 0.540;p = 0.004). Proteinuria had significant correlation with glomerular (r = 0.554;p = 0.001) and tubulointerstitial (r = 0.379;p = 0.043) TLR9 expression. Furthermore, there was a significant difference in tubulointerstitial expression of NGAL between treatment response groups. Conclusion There is an increase in intra-renal mRNA expression of NGAL and TLR9 in LN. Although tubulointerstitial expression of NGAL does not correlate with systemic disease activity, it correlates with proteinuria, renal function, and therapeutic response. The role of NGAL in the pathogensis in LN, as well as its application as biomarker for lupus nephritis, requires further study.

Memory is a ubiquitous characteristic of complex systems,and critical phenomena are one of the most intriguing phenomena in ***,we propose an Ising model with memory,develop a corresponding theory of critical phenomena with memory for complex systems,and discover a series of surprising novel *** show that a naive theory of a usual Hamiltonian with a direct inclusion of a power-law decaying long-range temporal interaction violates radically a hyperscaling law for all spatial dimensions even at and below the upper critical *** entails both indispensable consideration of the Hamiltonian for dynamics,rather than the usual practice of just focusing on the corresponding dynamic Lagrangian alone,and transformations that result in a correct theory in which space and time are inextricably interwoven,leading to an effective spatial dimension that repairs the hyperscaling *** theory gives rise to a set of novel mean-field critical exponents,which are different from the usual Landau ones,as well as new universality *** exponents are verified by numerical simulations of the Ising model with memory in two and three spatial dimensions.

目的:探究中国西部5城市中老年人血清25羟基维生素D[25-hydroxyvitamin D,25(OH)D]与握力的相关性。方法:基于2023年2—7月在中国西部5个城市开展的横断面调查数据,通过问卷收集中老年人的相关人口学特征,通过体格检查记录握力,采集空腹血液样本分离血清并使用高效液相色谱串联质谱法测定血清25(OH)D浓度,使用卡方检验进行组间比较,使用Logistic回归分析西部5城市中老年人血清25(OH)D与握力的相关性。结果:我国西部5城市中老年人维生素D的缺乏率和不足率分别为52.9%和34.5%[维生素D缺乏定义为血清25(OH)D总浓度<20μg/L,维生素D不足定义为血清25(OH)D总浓度为20~30μg/L],高龄、女性、采样季节在冬季的中老年人血清25(OH)D水平更低(P<0.05)。25.3%的中老年人发生了握力减退,65~80岁25(OH)D缺乏的老年人握力减退的发生率高于25(OH)D不足的老年人(45.0%vs.32.6%)和25(OH)D充足的老年人(45.0%vs.20.6%);25(OH)D缺乏的75~80岁老年人握力减退的发生率最高(62.1%),25(OH)D不足组次之(11.1%,P<0.05)。与25(OH)D充足的中老年人相比,25(OH)D缺乏的中老年人发生握力减退的风险增加1.4倍(OR=2.403,95%CI:1.202~4.804,P=0.013),未发现25(OH)D不足与中老年人握力状况存在显著相关关系;血清总25(OH)D每增加5μg/L,其发生握力减退的风险降低13.1%(OR=0.869,95%CI:0.768~0.982,P=0.025);血清25(OH)D 2每增加5μg/L,其发生握力减退的风险降低24.1%(OR=0.759,95%CI:0.582~0.990,P=0.042);未发现血清25(OH)D 3含量与握力减退风险存在显著相关关系。血清25(OH)D水平每增加一个等级(缺乏、不足和充足),中老年人发生握力减退的风险降低25.2%(OR=0.748,95%CI:0.598~0.936,P=0.011)。在65~80岁和65~69岁的两组调查对象中,血清25(OH)D水平每增加一个等级,发生握力减退的风险降低40.0%,而在75~80岁的调查对象中,25(OH)D水平每增加一个等级,发生握力减退的风险降低80.0%。结论:中国西部5城市中老年人血清总25(OH)D和25(OH)D 2含量与握力状况存在一定关联。

Background Low-back pain(LBP)in nurses is a major health concern that affects their quality of life and ability to work,with consequences for their economic *** This study evaluates the effect of low-level laser acupuncture combined with auricular acupressure(LAA)on pain intensity,pain interference and quality of life in nurses with ***,setting,participants and interventions This randomized controlled trial recruited a convenience sample of hospital-based nurses from one teaching hospital in Taiwan,*** were randomly assigned to the LAA group(n=38)receiving low-level laser acupuncture and auricular acupressure for 4 weeks,and the control group(n=38)receiving only sham laser acupuncture treatment without laser energy *** outcome measures Data were collected for the primary pain outcome using the Short Form of the Brief Pain Inventory,while the secondary outcome,quality of life,was evaluated using the Roland-Morris Disability *** primary and secondary outcomes were scored before the intervention,and after 2-week and 4-week *** rate of LBP recurrence was evaluated at the 4th week and 8th week after the end of *** After controlling for prior pain,the result of linear mixed model analysis showed trends in significant between-group differences in the level of current pain occurring in week 4(P<0.001),worst pain in week 2(P<0.001)and week 4(P<0.001),least pain in week 2(P=0.032)and week 4(P<0.001),pain interference in week 2(P=0.009)and week 4(P<0.001),and in the life dysfunction in week 2(P<0.001)and week 4(P<0.001).Recurrence rates of LBP at the 4th and 8th weeks after the end of intervention were 0%and 36.89%in the LAA group,and 69.44%and 36.11%in the control *** This study shows that 4-week LAA intervention reduced pain intensity and pain interference,and improved quality of life for hospital-based nurses with *** effects were maintained continuously for at least 4

Structural Changes of a Tropical Cyclone during Landfall: β-plane Simulations

This study investigated the effects of landfall on the structure of a tropical cyclone （TC）. Numerical simulations were performed using the Weather Research and Forecasting Model on a β-plane. Two landfall experiments, one with an east–west and another with a north–south oriented coastline, were performed. Similar to previous studies on an f-plane, large-scale flows in the low-to-middle troposphere were modified due to friction. A pair of counter rotating gyres was found, which was shown to be consistent with the slight deffection of the TC relative to the control experiment without land. Compared to previous f-plane simulations, because of the inherent asymmetries due to the β-gyres, the large-scale pattern of flows and convergences/divergences related to friction were found to depend on coastline orientations. On the other hand, regardless of the coastline orientation, convergences were found to be stronger to the left for both cases near landfall, as in previous f-plane simulations. Such a convergence pattern subsequently induced a change in convection and rainfall at the eyewall.

目的:探究中国10城市6~36月龄婴幼儿膳食多样性与自报患病情况的相关性。方法:基于2019—2020年在中国10城市开展的横断面调查的数据,通过问卷收集婴幼儿的相关人口学特征、自报患病情况及膳食多样性情况,使用卡方检验、Logistic回归、负二项回归模型分析婴幼儿膳食多样性与自报患病发生率及患病次数之间的相关性。结果:我国城市婴幼儿最低膳食多样性满足率为74.6%,婴幼儿年龄越大、家庭人均月收入越高、父亲教育程度越高其最低膳食多样性满足率更高(P<0.05)。满足最低膳食多样性的婴幼儿其呼吸系统疾病(IRR=0.76,95%CI:0.58~0.98,P=0.035)以及腹泻(IRR=0.65,95%CI:0.48~0.88,P=0.006)发生的次数更低。暂未发现婴幼儿膳食多样性与各自报疾病的发生率之间存在显著相关关系。结论:中国城市6~36月龄婴幼儿膳食多样性与呼吸系统疾病以及腹泻存在一定关联。
