Acute kidney injury following spinal instrumentation surgery in children

AIM To determine acute kidney in jury(AKI) incidence and potential risk factors of AKI in children undergoing spinal instrumentation *** AKI incidence in children undergoing spinal instrumentation surgery at British Columbia Children's Hospital between January 2006 and December 2008 was determined by the Acute Kidney Injury Networ classification using serum creatinine and urine output criteria. During this specific time period, all patients following spinal surgery were monitored in the pediatric intensive care unit and had an indwelling Foley catheter permitting hourly urine output recording. Cases of AKI were identified from our database. From the remaining cohort, we selected group-matched controls that did not satisfy criteria for AKI. The controls were matched for sex, age and underlying diagnosis(idiopathic vs nonidiopathic scoliosis).RESULTS Thirty five of 208 patients met criteria for AKI with an incidence of 17%(95%CI: 12%-23%). Of all children who developed AKI, 17(49%) developed mild AKI(AKI Stage 1), 17(49%) developed moderate AKI(Stage 2) and 1 patient(3%) met criteria for severe AKI(Stage 3). An inverse relationship was observed with AKI incidence and the amount of fluids received intra-operatively. Patients who developed AKI were more frequently exposed to nephrotoxins(non steroidal anti inflammatory drugs or aminoglycosides) than control patients during their peri-operative course(60% vs 22%, P < 0.001).CONCLUSION We observed a high incidence of AKI following spinal instrumentation surgery in children that is potentially related to the frequent use of nephrotoxins and the amount of fluid administered peri-operatively.

This study explores the spatial structure and transport characteristics of eddies in the Arabian Sea(AS)using Argo profiles and satellite *** majority of eddies occur in the northern AS,especially along its northeastern *** contrast,the western AS had a relatively higher eddy kinetic energy compared to the eastern ***,the strongest energetic eddies were present in the Somali Current *** composite results revealed the evident thermohaline anomalies caused by cyclonic eddies(CEs)and anticyclonic eddies(AEs)in the upper 300m *** anomalous temperature structure within CEs and AEs showed a dominant dipole structure in the near-surface layer and a monopole structure below,with maximum temperature anomalies of approximately−0.8℃and+1.0℃located at depths of 100–150m,*** composited salinity structures for CEs and AEs exhibited monopole vertical structures and sandwich-like *** AEs,large positive salinity anomalies occurred at subsurface layers of 60–180 m with a peak value of about 0.07,and weak negative values were observed above 60m and below 180 m.A similar vertical structure but with an opposite sign operates for *** composited CE and AE caused an equatorward salt flux with values of−8.1×10^(4)and−2.2×10^(4)kg s^(−1),*** caused an equatorward heat flux of−7.7×10^(11)W,and AEs induced a poleward flux of 1.5×1011 W.

Damping characteristics of friction damped braced frame and its effectiveness in the mega-sub controlled structure system

Based on energy dissipation and structural control principle, a new structural configuration, called the megasub controlled structure （MSCS） with friction damped braces （FDBs）, is first presented. Meanwhile, to calculate the damping coefficient in the slipping state a new analytical method is proposed. The damping characteristics of one-storey friction damped braced frame （FDBF） are investigated, and the influence of the structural parameters on the energy dissipation and the practical engineering design are discussed. The nonlinear dynamic equations and the analytical model of the MSCS with FDBs are established. Three building structures with different structural configurations, which were designed with reference to the conventional mega-sub structures such as used in Tokyo City Hall, are comparatively investigated. The results illustrate that the structure presented in the paper has excellent dynamic properties and satisfactory control effectiveness.

Study on semi-active control of mega-sub controlled structure by MR damper subject to random wind loads

The recently proposed mega-sub controlled structure （MSCS）, a new type of structure associated with the design and construction of super-tall buildings, has attracted the attention of designers for use in enhancing the control effectiveness in mega-frame buildings. In this paper, a dynamic equation and method to assemble parameter matrixes for a mega-sub controlled structure under random wind loads is presented. Semi-active control using magnetorheological dampers for the MSCS under random wind loads is investigated, and is compared with a corresponding system without dampers. A parametric study of the relative stiffness ratio and relative mass ratio between the mega-frame and the substructures, as well as the additional column stiffness ratio that influences the response control effectiveness of the MSCS, is discussed. The studies reveal, for the first time, that different control mechanisms exist. The results indicate that the proposed structure employing semi-active control can offer an effective control mechanism. Guidelines for selecting parameters are provided based on the analytical study.

Assessment of Cystatin C-Based GFR Estimating Equations as a Novel Reliable Biomarker for Renal Pathology Diagnosis in Patients with Mild to Severe Tubular Affection

Background and Objective: Serum creatinine, a commonly used biomarker in determining glomerular filtration rate (GFR) and chronic kidney disease (CKD) stage, is highly variable biologically and does not rise until > 50% of renal function (RF) impairment occurs. Also, its production is not constant & is affected by many factors as muscle mass, age, inflammation. On the other hand, Cystatin C shows more stable production making it more suitable for assessment of kidney function. Also, It has been shown that the progression of CKD to renal failure, even in glomerular diseases, correlated better with the degree of tubular damage and interstitial fibrosis. So, our aim was to investigate the relation between kidney function assessed by different cystatin (Cys-C)-based estimated glomerular filtration rate (eGFR) in comparison to the gold standard Iohexol (Ioh) based measured (m)GFR in relation to the pathological degree of tubular damage in renal biopsy. To our knowledge, this is the first study that evaluates the relation of (Cys-C)-based eGFR to tubulointerstitial fibrosis in renal biopsy. Methods: This cross-sectional study was performed on 20 CKD cases who attended the Nephrology Department at Ain Shams University, where a renal biopsy was obtained, and individuals were allocated into two groups: group A (GA) with mild tubular affection (TA) and group B (GB) with moderate to severe TA. All participants were referred for measure-ment of GFR using Iohexol (Ioh) together with serum Cys-C level and eGFR was calculated using different Cys-C-based GFR estimating equations, which were further compared using Multivariate Linear Regression and Bland-Altman analyses. Results: Our results revealed a substantial statistical difference among the two studied groups regarding Hb, s creatinine, urea. GB had significantly lower levels for both eGFR and mGFR (82, 93, 115, or 115) ml/min/1.73m2, Vs. GA (200, 123, 162 or 124) ml/min/1.73m2, according to GFR_iohexol,

Harvesting of forest products and implications for Afrotemperate bird communities in a montane forest of the Eastern Cape, South Africa

Background: Harvesting of forest products is a widespread driver of disturbance in developing nations, where policies are increasingly aimed at managing natural forests for sustainable use. There is thus need for research aimed at understanding the impact of resource use on forest habitats and concomitant effects on *** forests in the Eastern Cape, South Africa are harvested informally for poles and medicinal bark and occur along elevational gradients of 800–1600 m above sea level. Patterns of spatial diversity and human disturbance are expected to be affected by elevation. Furthermore, species’ responses to disturbance are expected to vary depending on their level of habitat specialisation. Understanding harvest impacts on forest biodiversity thus requires disentangling the separate effects of elevation and disturbance, and considering forest-specialist and forest-generalist species separately. This study comprises two components. First, harvest activities, resultant harvestmediated habitat heterogeneity, and avifaunal species richness, composition and beta-diversity were compared across two elevational zones in a harvested forest. Second, the role of harvest-mediated habitat heterogeneity in driving patterns of avifaunal diversity were assessed, while controlling for elevation, and considering forest-specialist and forest-generalist species ***: Harvest rates were higher, and activities more varied in the lower elevation zone, with significant impacts of harvesting on habitat features resulting in higher harvest-mediated habitat heterogeneity at lower ***-mediated increases in habitat heterogeneity positively affected forest-generalist species richness, while forest-specialist richness was negatively affected. While species composition of both groups differed across elevational zones, variation in harvest-mediated habitat heterogeneity did not fully account for this, suggesting that factors other than disturbance
