Orbital apex syndrome after tooth extraction in an immunocompromised patient

A 60 year-old man presented with acute on set of left eye proptosis and ptosis. It was associated with poor vision, eye pain and restriction of eye movement of the same eye after 5 hours post left upper molar tooth extraction. The visual acuity in the left eye was hand movement. There was severe ptosis and proptosis of the left eye. The conjunctiva was chemotic with quite anterior chamber. The pupil was mid dilated and sluggished to light. The ocular movement was restricted in all directions. Fundoscopy of the left eye revealed features of central retinal artery occlusion with hyperaemic disc and subretinal exudates at posterior pole. The right eye appeared normal. Urgent MRI brain and orbit revealed severe left paranasal sinusitis with anterior displacement of the left globe and presence orbital abscess. Patient was managed with Otorhinolaryngology and Neurosurgery teams. He underwent emergency transnasal drainage of abscess. Histopathological examination of unhealthy sinus mucosa showed evidence of fungal infection. However, the culture and sensitivity result was inconclusive. Patient was treated with amphotericin B, ceftriaxone, amoxicillin clavulanate and metronidazole. Patient was detected to have high blood sugar level and was managed accordingly. The proptosis improved with treatment. However, his vision, ptosis and ophthalmoplegia remained static. Assessing the immunocompromised status is important for the management of patient presented as acute orbital apex syndrome to avoid fatal outcome.

目的:探讨联合两个经保存的同源角巩膜修补片紧急修复伴有晶状体受挤压和玻璃体暴露的角膜穿孔,在成形性穿透性角膜移植术后的应用。方法:一位49岁的患者患广泛性真菌性角膜溃疡并发后弹力层膨出和微小穿孔。患者在接受了暨成形性穿透性角膜移植术后暴发了自发性大规模角膜穿孔伴晶状体驱逐和玻璃体暴露。鉴于覆盖破损处的迫切需要,供体角膜又无法立即获得,采用联合两个经保存的角巩膜修补片的双移植片。结果:手术成功不仅恢复了眼球的完整性,也免于眼内容物剜除。在接下来的几周内,移植片开始愈合,除了补片的巩膜部分,其它部位有良好的再上皮化,没有感染的迹象,并且能够保持手动视力。鉴于上皮缺损,患者配戴了绷带式角膜接触镜,同时被建议重复穿透性角膜移植术。结论:在缺乏角膜移植片的情况下,可采用一个或多个经保存的同源角巩膜修补片覆盖大范围的角膜缺损。

Lithospheric thinning and ignition of a Cordilleran magmatic flare-up:Geochemical and O-Hf isotopic constraints from Cretaceous plutons in southern Korea

Northeast Asian continental margins contain the products of magma emplacement driven by prolonged subduction of the(paleo-)Pacific *** observed in many Cordilleran arcs,magmatic evolution in this area was punctuated by high-volume pulses amid background *** present study investigates the early evolution of the Cretaceous magmatic flare-up using new and published geochronological,geochemical,and O-Hf isotope data from plutonic rocks in the southern Korean *** a long(~50 m.y.)magmatic hiatus and the development of the Honam Shear Zone through flat-slab subduction,the Cretaceous flare-up began with the intrusion of monzonites,granodiorites,and granites in the inboard Gyeonggi Massif and the intervening Okcheon *** to Jurassic granitoids formed during the former flare-up,Albian(~111 Ma)monzonites found in the Eopyeong area of the Okcheon Belt have distinctly higher zirconε_(Hf)(t)(-7.5±1.3)andδ^(18)O(7.78‰±0.25‰)values and lower wholerock La/Yb and Sr/Y *** voluminous coeval granodiorite and granite plutons in the Gyeonggi Massif are further reduced in Sr/Y and to a lesser extent,in La/Yb,and have higher zirconε_(Hf)(t)values(-13 to-19)than the Precambrian basement(ca.-30).These chemical and isotopic features indicate that Early Cretaceous lithospheric thinning,most likely resulting from delamination of tectonically and magmatically overthickened lithospheric keel that was metasomatized during prior subduction episodes,and consequent asthenospheric upwelling played vital roles in igniting the magmatic *** O-Hf isotopic ranges of synmagmatic zircons from the Albian plutons and their Paleoproterozoic and Jurassic inheritance attest to the involvement of lithospheric mantle and crustal basement in magma generation during this decratonization *** magmatism then migrated trenchward and culminated in the Late Cretaceous,yielding widespread granitoid rocks emplaced at shallow crustal *** early Late Cretaceous(94-8

Treatment and prognosis of hepatic epithelioid hemangioendothelioma based on SEER data analysis from 1973 to 2014

Background: Hepatic epithelioid hemangioendothelioma (HEH) is a rare tumor of vascular origin with an unknown etiology, a low incidence, and a variable natural course. We evaluated the management and prognosis of HEH from the Surveillance, Epidemiology and End Results (SEER) program and changes in treatment modalities of HEH over 30 years. Methods: From 1973 to 2014 in the SEER database, we selected patients diagnosed with HEH. We analyzed the clinical characteristics, patterns of management, and clinical outcomes of patients with HEH. Results: We identi ed 79 patients with HEH (median age: 54.0 years;male to female ratio: 1:2.6). The initial extent of disease was local in 22 (27.8%) patients, regional metastasis in 22 (27.8%), distant metas-tasis in 31 (39.2%) and unknown in 4 (5.1%). The median size of primary tumor was 3.85 cm (interquartile range, 2.50 7.93 cm). Among 74 patients with available management data, the most common manage-ment was no treatment (29/74, 39.2%), followed by chemotherapy only (22/74, 29.7%), liver resection-based (13/74, 17.6%), and transplantation-based therapy (6/74, 8.1%). The 5-year cancer-speci c survival rate was 57.8%. Patients who underwent surgical treatment had signi cantly higher survival than those who underwent non-surgical treatment (5-year survival;88% vs. 49%, P=0.019). Multivariate analysis revealed that surgical therapy was the only independent prognostic factor for survival (hazard ratio: 0.20, P=0.040). Conclusions: Resection or liver transplantation is worth considering for treatment of patients with HEH.

In the field of electrolysis of water,the design and synthesis of catalysts over a wide pH range have attracted extensive *** this paper,Co and N are co-introduced into the structural unit of tungsten disulfide(WS_(2)),and the hydrogen evolution reaction(HER)performances of different WS_(2)-based catalysts are theoretically predicted and systematically studied by density functional theory(DFT)*** the guidance of DFT calculations,an evaporation-pyrolysis strategy is applied to prepare Co and N co-doped WS_(2)(Co,N-WS_(2))flower-like nanosheets,which exhibits excellent HER performance over a wide pH *** addition,the DFT calculations show that the active sites in Co,N-WS_(2) have a good ability of hydrogen adsorption after the introduction of Co and N,suggesting that such a co-doping system will be an ideal catalyst for oxidative dehydrogenation(ODH).The following experiment results indeed evidence that the Co,N-WS_(2) catalyst displays a high activity in the ODH of 1,2,3,4-tetrahydroquinoline(4H-quinoline)and its ***,this work provides a good example for the rational design and accurate preparation of functional catalysts,which enables it possible to develop other efficient catalysts with multiple functions.

Objective:Benefits of adjuvant treatment in pT1 N1 gastric cancer(GC)remain ***,an effective biomarker for early GC is the need of the *** prognostic and predictive roles of single patient classifier(SPC)were validated in stageⅡ/Ⅲ*** this study,we aimed to elucidate the role of SPC as a biomarker for pT1 N1 ***:The present retrospective biomarker study(NCT03485105)enrolled patients treated for pT1 N1 GC between 1996 and 2012 from two large hospitals(the Y cohort and S cohort).For SPC,mRNA expression of four classifier genes(GZMB,WARS,SFRP4 and CDX1)were evaluated by real-time reverse transcription-polymerase chain reaction *** SPC was revised targeting pT1 stages and the prognosis was stratified as high-and lowrisk group by the expression of SFRP4,a representative epithelial-mesenchymal transition ***:SPC was evaluated in 875 patients(n=391 and 484 in the Y and S cohorts,respectively).Among 864 patients whose SPC result was available,41(4.7%)patients experience GC *** to revised SPC,254(29.4%)patients were classified as high risk[123(31.5%)and 131(27.1%)in the Y and S cohorts,respectively].The high risk was related to frequent recurrence in both Y and S cohort(log-rank P=0.023,P<0.001,respectively),while there was no difference by GZMB and WARS *** analyses of the overall-cohort confirmed the high risk of revised SPC as a significant prognostic factor[hazard ratio(HR):4.402(2.293-8.449),P<0.001]of GC.A significant difference was not detected by SPC in the prognosis of patients in the presence and absence of adjuvant treatment(log-rank P=0.670).Conclusions:The present study revealed the revised SPC as a prognostic biomarker of pT1 N1 GC and suggested the use of the revised SPC for early-stage GC as like stageⅡ/Ⅲ.

Impact of guideline adherence on the prognosis of Barcelona clinic liver cancer stage B hepatocellular carcinoma

Effect of robot-assisted gait training on the biomechanical properties of burn scars:a single-blind,randomized controlled trial

Background:Robot-assisted gait training(RAGT)is more effective in the range of motion(ROM)and isometric strength in patients with burns than conventional ***,concerns have been raised about whether RAGT might negatively affect the scars of patients with ***,we investigated the effects of RAGT-induced mechanical load on the biomechanical properties of burn ***:This was a single-blind,randomized clinical trial conducted on inpatients admitted to the Department of Rehabilitation Medicine between September 2020 and August *** was conducted for 30 min per day,five days a week for 12 weeks and the control group received conventional gait training for 12 *** pre-training ROM of lower extremity joints was evaluated and the levels of melanin,erythema,trans-epidermal water loss,scar distensibility and elasticity were assessed before training and at 4 and 12 weeks after ***,19 patients in the gait assistance robot(GAR)group and 20 patients in the control group completed the 12-week trial and all ***:There were no significant differences in the epidemiologic characteristics,pre-training ROM of joints and pre-training biomechanical properties of the burn scar between the groups(p>0.05 for all).None of the patients experienced skin abrasion around the burn scar where the fastening belts were applied or musculoskeletal or cardiovascular adverse events during the *** thickness significantly increased in both groups(p=0.037 and p=0.019)and scar distensibility significantly decreased in the control group(p=0.011)during the *** was significantly decreased in the GAR group during the training(p=0.038).The GAR and control groups showed significant difference in the change in the values of hysteresis between pre-training and 12 weeks after training(p=0.441 and p=0.049).Conclusions:RAGT significantly decreased hysteresis in hypertrophic burn scars and did not cause a significant decrease

On-table hepatopancreatobiliary surgical consults for difficult cholecystectomies:A 7-year audit

Background: Cholecystectomy is considered a general surgical operation. However, general surgeons are not trained to manage severe complications such as bile duct injury(BDI) and should refer to hepatopancreatobiliary(HPB) surgeons when difficulty arises. This study aimed to investigate the outcomes of patients who had on-table HPB consults during cholecystectomy. Methods: This is an audit of 50 patients who required on-table HPB consult during cholecystectomy from 2011 to 2017. Consultations were classified as “proactive” and “reactive”, where consults were made before or after surgical incision, respectively. Patient demographics and perioperative details were collected. Results: The median age of the patients was 62.5 years [interquartile range(IQR) 50.8–71.3 years]. Eight(16%) patients had underlying HPB co-morbidity. Gallbladder wall was thickened in all patients(median 5 mm, IQR 4–7 mm), and common bile duct was of normal caliber in all patients(median 5 mm, IQR 4–6 mm). Median length of operation and length of stay were 165 min(IQR 124–209 min) and five days(IQR 3–7 days), respectively. Subtotal cholecystectomy was performed in 18(36%) patients. Forty-eight patients were initially managed by laparoscopic approach, 15(31%) required open conversion;majority(9/15, 60%) were initiated before on-table consult. Majority of referrals(98%) were reactive. Common reasons for referral included unclear anatomy or anatomical variations(30%), presence of dense adhesions and/or contracted gallbladder(18%) and impacted stones in Hartmann’s pouch(16%). Three(6%) patients were referred for BDI(2 Strasberg D and 1 Strasberg E1), and two(4%) were referred for torrential bleeding from arterial injury(1 cystic artery and 1 right hepatic artery). Any morbidity and 30-day readmission were 22% and 6%, respectively. There was no 90-day mortality. Conclusions: Calling for help in BDI is obligatory, but in other instances is a personal choice.
