Prediction of mechanical ventilation outcome by early abdominal-visceral-blood-flow-and-function score in critically ill patients after cardiopulmonary bypass in the ICU: A prospective observational study

Background:Abdominal organs are important organs that sense and respond to ischemia and hypoxia,but there are few evaluation *** use ultrasonography to evaluate abdominal organ function and blood flow in patients with mechanical ventilation(MV)after cardiopulmonary bypass and to obtain a semiquantitative score for abdominal organ function and blood ***:Patients with cardiopulmonary bypass in the Critical Care Department of Peking Union Medical College Hospital in China from March to July 2021 were enrolled in this prospective observational *** correlation of the abdominal-visceral-blood-flow-and-function score(AVBFS)with the duration of MV,number of days spent in the intensive care unit(ICU),acute physiology and chronic health evaluation II(APACHE-II),sequential organ failure assessment(SOFA),lactate,epinephrine,and norepinephrine use was analyzed,and the results were used to assess the predictive value of the receiver operating characteristic curve(ROC)regression analysis score for the duration of ***:Of the 92 patients who underwent cardiopulmonary bypass,41 were finally *** AVBFS were significantly correlated with the duration of MV,number of days spent in the ICU,APACHE-II score,SOFA score,and norepinephrine use *** AVBFS in a group of patients using ventilators≥36 h were significantly higher than those obtained for a group of patients using ventilators<36 h(P<0.05).The evaluation results for the AVBFS at 0-12 h after ICU admission were as follows:area under the ROC curve(AUC)=0.876(95%confidence interval[CI]:0.767 to 0.984),cut-off value=2.5,specificity=0.842,and sensitivity=***:Abdominal visceral organ function and blood perfusion can be used to evaluate gastrointestinal *** is related to early and late extubation after cardiac surgery.

20世纪30年代,茅盾关注苏联高尔基的传记电影,体现出其身为作家对人物形象建构问题的内在志趣。20世纪40年代,左翼电影批评工作者采用阶级分析理论,赞扬苏联电影,否定美国好莱坞电影。茅盾更进一步发掘了苏联电影人物形象内蕴的“伟大”“英雄气概”的美学特质,以及主题的“严肃”,并用传统儒家文化的“忠臣孝子”话语,阐释苏联电影人物形象。20世纪50年代,中国观众对苏联译制片里人物形象的理解存在困难,茅盾给出了解决路径。

陆侃如、冯沅君写作《中国诗史》,深受民国学术风气熏染。其书在文献考据层面,认夏代以前为不可知,全盘接受了胡适一派的疑古思潮。在诗史架构层面,则颇自出手眼:把“诗”的范围扩充至多数韵文,论证各时代主流体裁如何环环衍生,使得叙述更加系统化;以白话、文言迭相起伏贯穿全史,但不与体裁更替强行牵合,使得叙述更加弹性化。这两层面表现,均属新文化运动促成的文史研究转型之产物。前者当时已遭遇挑战,后者却代表着新文化立场介入文学史书写的另一方案,值得长久与之对话。

钟泰深研《荀子》,对其义理作了重构。这主要表现为两点:一是揭示荀子虽主张性恶,却仍不废性善,对于荀学所言由精神虚静而获取的认知能力,反有所轻忽;二是揭示荀子所称“先王”“后王”,辞异义同。具体所指,他先以西周当之,后扩展至三代。这些说法,同荀子原意多有参差。究其成因,或立足于钟泰自身程朱理学的立场,或服从于构造先秦诸子谱系的要求,或有取于稍前章太炎的议论。钟泰经由重释荀子,将理学式的“心统性情”说上溯至先秦;同时,荀学注重制度建设,钟泰则赋予制度以历时变化的动力,并加入反对世袭等现代观念。两面合观,反本而开新,可谓一种创造性阐释。

Painful stimuli elicit first-line reflexive defensive reactions and,in many cases,also evoke second-line recuperative behaviors,the latter of which reflects the sensing of tissue damage and the alleviation of *** lateral parabrachial nucleus(lPBN),composed of external-(elPBN),dorsal-(dlPBN),and central/superior-subnuclei(jointly referred to as slPBN),receives sensory inputs from spinal projection neurons and plays important roles in processing affective information from external threats and body integrity ***,the organizational rules of lPBN neurons that provoke diverse behaviors in response to different painful stimuli from cutaneous and deep tissues remain *** this study,we used region-specific neuronal depletion or silencing approaches combined with a battery of behavioral assays to show that slPBN neurons expressing substance P receptor(NK1R)(lPBNNK1R)are crucial for driving pain-associated self-care behaviors evoked by sustained noxious thermal and mechanical stimuli applied to skin or bone/muscle,while elPBN neurons are dispensable for driving such ***,lPBNNK1R neurons are specifically required for forming sustained somatic pain-induced negative teaching signals and aversive memory but are not necessary for fear-learning or escape behaviors elicited by external ***,both lPBNNK1R and elPBN neurons contribute to chemical irritant-induced nocifensive *** results reveal the functional organization of parabrachial substrates that drive distinct behavioral outcomes in response to sustained pain versus external danger under physiological conditions.

Constraints on real scalar inflation from preheating using LATTICEEASY

In this paper,we undertake a detailed study of real scalar inflation using LATTICEEASY simulations to investigate preheating ***,the scalar inflation potential with non-minimal coupling can be approximated using a quartic *** observe that the evolutionary behavior of this potential remains unaffected by the coupling ***,the theoretical predictions for the scalar spectral index(n_(s))and tensor-toscalar power ratio(r)are independent of this ***,the coefficients of this model are not constrained by Planck ***,the properties of preheating after inflation provide a viable approach to examining these *** LATTICEEASY simulations,we trace the evolution of particle number density,scale factor,and energy density during the preheating ***,we derive the parameters,such as the energy ratio(γ)and the e-folding number of preheating(N_(pre)),which facilitate further predictions of n s and *** successfully validate real scalar inflation model using preheating in LATTICEEASY simulations based on the analytical relationship between preheating and inflation models.

Influence of blood glucose fluctuations on chemotherapy efficacy and safety in type 2 diabetes mellitus patients complicated with lung carcinoma

BACKGROUND Patients with type 2 diabetes mellitus(T2DM)have large fluctuations in blood glucose(BG),abnormal metabolic function and low immunity to varying degrees,which increases the risk of malignant tumor diseases and affects the efficacy of tumor *** hyperglycemia may have important therapeutic implications for cancer *** To clarify the influence of BG fluctuations on chemotherapy efficacy and safety in T2DM patients complicated with lung carcinoma(LC).METHODS The clinical data of 60 T2DM+LC patients who presented to the First Affiliated Hospital of Ningbo University between January 2019 and January 2021 were retrospectively *** patients underwent chemotherapy and were grouped as a control group(CG;normal BG fluctuation with a mean fluctuation<3.9 mmol/L)and an observation group(OG;high BG fluctuation with a mean fluctuation≥3.9 mmol/L)based on their BG fluctuations,with 30 cases *** indices,tumor markers,serum inflammatory cytokines and adverse reactions were comparatively *** correlation analysis was performed to analyze the correlation between BG fluctuations and tumor *** The fasting blood glucose and 2-hour postprandial blood glucose levels in the OG were notably elevated compared with those in the CG,together with markedly higher mean amplitude of glycemic excursions(MAGE),mean of daily differences,largest amplitude of glycemic excursions and standard deviation of blood glucose(P<0.05).In addition,the OG exhibited evidently higher levels of carbohydrate antigen 19-9,carbohydrate antigen 125,carcinoembryonic antigen,neuron-specific enolase,cytokeratin 19,tumor necrosis factor-α,interleukin-6,and highsensitivity C-reactive protein than the CG(P<0.05).Pearson analysis revealed a positive association of MAGE with serum tumor *** incidence of adverse reactions was significantly higher in the OG than in the CG(P<0.05).CONCLUSION The greater the BG fluctuation in LC patients after

在碳中和目标的驱动下,炼化企业必须寻找新的路径,大力开发低碳、零碳的炼化新技术。生物基燃料性能与石油基燃料相近,但其全生命周期碳排放大幅降低,以生物质为原料生产替代燃料的技术,正成为炼油厂实现低碳、零碳发展的重要手段之一。本文综述了由生物质原料制碳氢类燃料(b-Fuels)相关技术的研究进展和发展趋势,主要包括无甘油生成的酯交换技术,控制产物选择性的酯交换技术,生物质油热裂解、催化裂解、加氢裂化技术,以及生物质与石油馏分共炼技术等,同时对未来炼化企业实现碳中和的技术发展路径进行了探讨,以期为炼厂转型发展提供思路借鉴。
