Haemorrhagic Lupus: A Diagnosis Emergency, Report about 6 Cases

Introduction: Haemorrhagic manifestations rarely represent the mode of onset of lupus and are a therapeutical and diagnosis emergency. We report 6 cases of haemorrhagic manifestations disclosing lupus. Patients and methods: It’s about a retrospective, descriptive and multicentric study ranged from 1st January 2014 to 5th January 2019. Were included in the study all cases of lupic disease revealed by hemorrhagic disorders, recorded in 3 Internal Medicine Departments in Dakar: DalalJamm, Dantec, Pikine. The diagnosis of lupus was made on the basis of the 1997 modified classification criteria of ACR for SLE. Results: We gathered 6 cases of women affected by lupic disease. The mean age was 32 years old with extremes ranged from 17 to 56 years. According to haemorrhagic disorders, the diagnosis of lupus was automatically made in 5 of the cases and it took 6 months to confirm the last one. Haemorrhagic features were ruled by purpura (3/6 cases) and nose-bleeding associated with gumbleeding (2/6 cases), epistaxis and melena (1/6 case), hypermenorrhea (1/6 case), cerebral haematoma (1/6case). Cutaneous (5/6), articular (4/6), renal (2/6) and serous (2/6) disorders specially represented the other manifestations. Biological findings showed cytopenia with 3 cases of anaemia, 1 case of leucopenia and 6 cases of thrombopenia. The average of the platelet count was at 20,000 cells/mm3 with extremes ranged from 1000 to 35,000 cells/mm3. Thrombocytopenia was related to autoimmune thrombocytopenic purpura in 3 cases, to Evans’ syndrome in 2 cases, to macrophage activation syndrome in 1 case. Immunological investigations found the presence of antinuclear antibodies (1/6), anti-dsDNA (2/6) and anti-Sm (4/6). All patients received a bolus of Solumedrol followed by an administration of Prednisone at a dosage of 1.5 mg/kg (3 cases) and 1 mg/kg to the other patients. Influence of Fresh Water on Microbubble Generation in an Airlift Column Applied to Aquaculture: Extraction Capacity

Bubble flows consist a liquid phase and a gaseous phase dispersed as bubbles. They occur in nature and in many industrial applications, such as oil transportation in pipelines and steam generators for power generation. Due to large difference in density between gas and liquid, the flottability force causes bubbles to rise, which in turn can generate overall motion and agitation in liquid. This use of gravity as a flow driver, which is specific to disperse phase systems, is used in process engineering (bubble columns and gasosiphon) to sparingly promote mixing and exchange between gas and liquid. In many applications, bubbles are used to agitate a liquid in order to promote mixing and transfers. This work is devoted to study of hydrodynamics of a bubble column. Experimentally, we have determined properties fluctuations of velocities inside the aquarium of rising homogeneous bubbles for different bubble sizes and vacuum rates. The interfacial area between gas and liquid phase is a crucial factor for mass transfer in bubble columns. The molecular exchange between a given volume of gas and water can be enhanced by formation of smaller bubbles, leading to a larger gas-liquid interface. This work presents the various physical phenomena that apply to bubbles, as well as associated dimensionless numbers. A state art of Micro-Bubble Generators (MBG) is then presented, presenting systems using various phenomena such as cavitation, electrolysis, or shear.

Screening for Peripheral Artery Disease (PAD) in Hypertensive Black Patients: Ankle-Brachial Index in a Senegalese Primary Care Setting

Background: Peripheral artery disease (PAD) poses a significant health concern, particularly in Sub-Saharan Africa, where its prevalence is increasing. [1] Despite its significance, there is limited understanding of PAD among hypertensive patients in this region, highlighting a critical gap in knowledge. This study aimed to investigate the prevalence of PAD and associated factors in black hypertensive patients. Methods: A descriptive and analytical cross-sectional study was conducted over two years at a primary care center in Senegal using their ankle-brachial index (ABI) database. Data collection was strictly retrospective, and sociodemographic characteristics and clinical parameters were retrieved from the local patient database. We included all hypertensive patients who had benefited from an ABI. Statistical analysis was performed using the SPSS 18.0 software program. Results: Among the 220 hypertensive patients enrolled, PAD prevalence was 35%. Significant associations were observed between PAD occurrence and older age (>75 years, p = 0.008) and triple therapy (p = 0.015). Multivariate analysis confirmed age >75 years as a strong predictor of PAD in hypertensive patients (p = 0.01, OR = 4.6). Furthermore, PAD prevalence increased with the severity of hypertension (p = 0.03), emphasizing the need for targeted screening strategies in this population. Conclusion: Despite its limits, this study underscores the urgent need for improved access to healthcare services and tailored screening programs. The findings highlight the growing burden of PAD in Sub-Saharan Africa and the essential role of early detection and intervention, particularly in high-risk populations such as hypertensive individuals. Collaborative efforts involving healthcare providers, policymakers, and community stakeholders are crucial to implement effective interventions and reduce the impact of PAD on population health outcomes.

Study and Optimization of a Photovoltaic Mill System Functioning on the Course of the Sun

This paper dealt with the optimization of the performance of a photovoltaic mill system operating on the sun race. Depending on the characteristics of the powered load which is a DC motor driving a grain mill and on the weather conditions (temperature and illumination), we noted a very big difference between the potential maximum power and that actually transferred to the load. In order to improve the overall efficiency of the system, we use an adaptation circuit consisting of a boost converter controlled by a numerical MPPT (Maximum Power Point Tracking) command. With the Perturb & Observe (P & O) algorithm, the MPPT control measure the photocurrent, the photo tension and the power released by the photovoltaic generator. From this result, the MPPT control adjusts the duty cyclic of the converter to bring the system to the optimum operating point. Hence, using MATLAB/Simulink software, we did the modeling and the simulation of the system which is composed by a PV generator, a boost converter, a Pulse Width Modulation and a DC motor.

Seasonal Behavior of Aerosol Vertical Concentration in Dakar and Role Played by the Sea-Breeze

The Westward transport of mineral dust from the North Africa continent to Atlantic Ocean can produce poor air quality, low visibilities, and negatively impacting respiratory and cardiac health due to the optical and physical properties of aerosols. The dynamical impact of the sea-breeze on the dust vertical distribution in West Africa remains unknown. To investigate this issue, we have used in-situ measurements from lidar. We have focused on the attenuated backscatter of aerosols to study the effect of the local circulation on the vertical profile of mineral dust at land-sea transition. The results highlight a strong diurnal cycle of mineral dust associated with the nocturnal low-level jet (NLLJ). The jet is located between 500 m and 1000 m and crucially affected by the dynamic of the sea-breeze circulation.

Assessment of Climate Change Impact on Water Resources in the Upper Senegal Basin (West Africa)

This study assesses the potential impacts of climate change on water resources and the effect of statistical bias correction on the projected climate change signal in hydrological variables over the Upper Senegal Basin (West Africa). Original and bias corrected climate data from the regional climate model REMO were used as input for the Max Planck Institute for Meteorology-Hydrology Model (MPI-HM) to simulate river discharge, runoff, soil moisture and evapotranspiration. The results during the historical period (1971-2000) show that using the bias corrected input yields a better representation of the mean river flow regimes and the 10th and 90th percentiles of river flow at the outlet of the Upper Senegal Basin (USB). The Nash-Sutcliffe efficiency coefficient is 0.92 using the bias corrected input, which demonstrates the ability of the model in simulating river flow. The percent bias of 3.88% indicates a slight overestimation of the river flow by the model using the corrected input. The evaluation demonstrates the ability of the bias correction and its necessity for the simulation of historical river regimes. As for the potential changes of hydrological variables by the end of 21st century (2071-2100), a general decrease of river discharge, runoff, actual evapotranspiration, soil moisture is found under two Representative Concentration Pathways (RCP4.5 and RCP8.5) in all simulations. The decrease is higher under RCP8.5 with uncorrected data in the northern basin. However, there are some localized increases in some parts of the basin (e.g. Guinean Highlands). The projected climate change signal of these above variables has the same spatial pattern and tendency for the uncorrected and bias corrected data although the magnitude of the corrected signal is somewhat lower than that uncorrected. Improving the Stability of the Inflatable Soil of Diamniadio by Using Typha Australis in Order to Value It in Partitioning Wall

The construction industry in Senegal is experiencing an upsurge in the development of residential housing units and administrative buildings within the new Diamniadio municipality, an area located just about 35 km from the capital city of Dakar. Laboratory oedometer or expansive-index tests, however, show poor density and compromised shear strength in the soil samples within the area -posing serious construction problems due to significant volume changes (swellings) that occur when subjected to moisture, thus, bringing into question the structural performance integrity of the soil, and the financial implications of substitute soil types used to compensate for its poor tensile strength. By way, the companies collect the soil of this location (Diamniadio) and throw it to make pile or micro-pile (pious) for their structures. This article demonstrates how we can value the soil of Diamniadio and the Typha Australis in residential building with a reasonable cost. Typha Australis is a plant generally luxuriant within the northern and central belts of Senegal but also known for its negative ecological impacts on the agricultural production of rice. The valorisation of the soil of Diamniadio will pass by the ability to increase its stability or to decrease its shrinkage/swelling rate. When we achieve it, the soil associate with Typha Australis will be used for small bricks which can be useful for partition wall and so the soil will not be ever thrown as a non-useful material. Tests results of soil specimens extracted from the site show a shrinkage rate of 29.19% with estimated cracking depths of 3.5 cm. But by adding gradually Typha Australis, the shrinkage rate will pass from 29.19% to 5.13% with the material treats with 10% by mass of Typha Australis. Moreover, the crackings in the cylindrical specimen disappear. That shows the increase of the stability;thus the composition will be used for building bricks. Approach Thermal Habitat Assessment in N’Djamena in Chad

For the earth material construction is the most used one in Chad, the object of this work is the assessment of the thermal quality of earthen adobe mixed with straw. Different simulation software analyzes the CoDyBa which is conducted to determine the behavior of this material compared to living comfort thanks to the Fanger model, which is a method of approach to thermal comfort standpoint. The study focused on two configurations on the formation of the walls of a room. The first of these configurations led to the definition of a first cell constructed earthen adobe named Batter roofing sheet aluminum and a second cell built in blocks named Batbet which had the same geometric characteristics (thickness, dimensions and side openings) and covered in the same manner as the ground cell. The different comfort indices PMV and PPD values of these two configurations of habitat were identified and were used to determine their thermal comfort rating.

Radiotherapy of Esophageal Carcinomas at the Dantec University Hospital of Dakar from 2010 to 2015

We performed a descriptive retrospective study of radiotherapy of esophageal carcinomas at the Dantec University Hospital in Dakar from 2010 to 2015. The aim was to describe the type of patient, and to evaluate the indication and the results of the irradiation. We consulted 117 cases and selected 26 cases. The average age of patients is 46.1 years (Extremes 17 - 83 years and peaks 30-39 and 50 - 59 years). The sex ratio is 1.67. Smoking was reported in 38.4% of cases and alcoholism in 15.3%. Dysphagia is the predominant sign (100% of patients). The general condition is classified WHO 1 for 42.3% of cases. The tumor was localized in 61.5% in the lower third and is stenosing in 84.6% of our patients. Squamous cell carcinoma is the most common type (88.5% of cases). The size of the tumor, specified in 50% of cases, varies from 2.6 to 11.2 cm. Computed tomography is performed in more than 50% of cases. Endoscopic ultrasonography is not performed and surgical exploration has contributed to the staging of 7.7% of irradiated patients. The tumor is localized in 47.8% of cases;at an advanced stage in 52% and is metastatic in 4.3%. We note a transhiatal excision and a gastrostomy in 15% and 69% of the cases and no surgery of excision in healthy margin. Chemotherapy or neoadjuvant radiotherapy is not performed. The radiotherapy was: exclusive (11%), adjuvant after surgery in margin R2 (15%), palliative in 3 × 5 Gy per 3 week (4%) and concomitant with chemotherapy cisplatyl 5FU (70%). The spread ranges from 2 to 6 weeks. The partial and complete response rate is 38.4% and 11.5%. The stabilization is 30.7% and the progression is 19.4%. Grade I-2 toxicity was noted in 46.1% (asthenia, radiodermatitis, cough) and grade 3 radiodermite in 4%. The average follow-up time is 198.19 days with extremes of 2 days and 32 months.

Thermophysical Characterization of Typha’s Concrete for Its Integration into Construction

Energy consumption in the building sector is constantly increasing and represents more than 44% in the residential and tertiary sectors in many countries [1]. Thus, the building represents a real possibility of energy saving and is the subject of several studies particularly in actual context of experimentation with materials based on plant fibers (hemp, flax, millet wastage, etc.). These biobased materials such as typha have shown real interest in the buildings construction due to their light and porous nature. It’s in this context that we were interested in typha australis mixed with classic aggregates (cement, sand, gravel 3/8) to make typha’s concrete. On this concrete, we carried out experimental measurements in order to better understand its contributions in the building. The interesting results obtained show that typha australis, which is an invasive and harmful grass, can be valued and integrated among local building materials in the form of typha’s concrete in order to improve the energy efficiency of buildings.
