Factors associated with increased incidence of severe toxicities following yttrium-90 resin microspheres in the treatment of hepatic malignancies

AIM: To further define variables associated with increased incidences of severe toxicities following administration of yttrium-90 (90Y) ***: Fifty-eight patients undergoing 79 treatments were retrospectively assessed for development of clinical and laboratory toxicity incidence following 90Y administration. Severe toxicity events were defined using Common Terminology Criteria for Adverse Events version 4.03 and defined as grade ≥ 3. Univariate logistic regression analyses were used to evaluate the effect of different factors on the incidence of severe toxicity events. Multicollinearity was assessed for all factors with P linearity were included in a multivariate logistic regression model for each measurement of severe ***: Severe (grade ≥ 3) toxicities occurred following 21.5% of the 79 treatments included in our analysis. The most common severe laboratory toxicities were severe alkaline phosphatase (17.7%), albumin (12.7%), and total bilirubin (10.1%) toxicities. Decreased pre-treatment albumin (OR = 26.2, P = 0.010) and increased pre-treatment international normalized ratio (INR) (OR = 17.7, P = 0.048) were associated with development of severe hepatic toxicity. Increased pre-treatment aspartate aminotransferase (AST; OR = 7.4, P = 0.025) and decreased pre-treatment hemoglobin (OR = 12.5, P = 0.025) were associated with severe albumin toxicity. Increasing pre-treatment model for end-stage liver disease (MELD) score (OR = 1.8, P = 0.033) was associated with severe total bilirubin toxicity. Colorectal adenocarcinoma histology was associated with severe alkaline phosphatase toxicity (OR = 5.4, P = 0.043).CONCLUSION: Clinicians should carefully consider pre-treatment albumin, INR, AST, hemoglobin, MELD, and colorectal histology when choosing appropriate candidates for 90Y microsphere therapy.

At present, there is little commercial sale of biochar, since farmers find they can not gain a return on their investment in this amendment in the first few years after its application, because of the high cost associated with large application rates. To overcome this constraint, development of artificially aged enriched biochar-mineral complexes(BMCs), having a higher mineral content, surface functionality, exchangeable cations, high concentration of magnetic iron(Fe) nanoparticles, and higher water-extractable organic compounds has been undertaken by a combined team of researchers and a commercial company. Two biochars produced under different pyrolysis conditions were activated with a phosphoric acid treatment. A mixture of clay, chicken litter, and minerals were added to the biochar, and then this composite was torrefied at either 180 or 220?C. In this study a pot experiment was carried out in glasshouse conditions to determine the effects of four different BMCs, with different formulations applied at rates of 100 and 200 kg ha-1, on the mycorrhizal colonisation, wheat growth and nutrient uptake, and soil quality improvement. It was found that the phosphorus(P) and nitrogen uptake in wheat shoots were significantly greater for a low application rate of BMCs(100 kg ha-1). The present formulation of BMC was effective in enhancing growth of wheat at low application rate(100 kg ha-1). The increase in growth appeared due to an increase in P uptake in the plants that could be partly attributed to an increase in mycorrhizal colonisation and partly due to the properties of the BMC.

Active vaccination to prevent de novo hepatitis B virus infection in liver transplantation

The shortage of organ donors mandates the use of liver allograft from anti-HBc(+) donors, especially in areas highly endemic for hepatitis B virus(HBV) infection. Theincidence of de novo hepatitis B infection(DNH) is over 30%-70% among recipients of hepatitis B core antibody(HBcA b)(+) grafts without any prophylaxis after liver transplantation(LT). Systematic reviews showed that prophylactic therapy [lamivudine and/or hepatitits B immunoglobulin(HBIG)] dramatically reduces the probability of DNH. However, there are limited studies regarding the effects of active immunization to prevent DNH, and the role of active vaccination is not welldefined. This review focuses on the feasibility and efficacy of pre- and post-LT HBV vaccination to prevent DNH in HBsA g(-) recipient using HBcA b(+) grafts. The presence of HBs Ab in combination with lamivudine or HBIG results in lower incidence of DNH and may reduce the requirement of HBIG. There was a trend towards decreasing incidence of DNH with higher titers of HBs Ab. High titers of HBs Ab(> 1000 IU/L) achieved after repeated vaccination could eliminate the necessity for additional antiviral prophylaxis in pediatric recipients. In summary, active vaccination with adequate HBsA b titer is a feasible, cost-effective strategy to prevent DNH in recipients of HBc Ab(+) grafts. HBV vaccination is advised for candidates on waiting list and for recipients after withdrawal of steroids and onset of low dose immunosuppression after transplantation.

Effects of Thermal Activation on the Electrocatalysis of the Ru(0001) Surface

Variable temperature in situ FTIR spectroscopy has been used as the primary tool to investigate the effects of temperature(10 to 50 ℃) on formaldehyde dissociative adsorption and electro oxidation on the Ru(0001) electrode in perchloric acid solution, and the results were interpreted in terms of the surface chemistry of the Ru(0001) electrode and compared to those obtained during our previous studies on the adsorption of CO under the same conditions. It was found that formaldehyde did undergo dissociative adsorption, even at -200 mV vs . Ag/AgCl, to form linear(CO L) and 3 fold hollow(CO H) binding CO adsorbates. In contrast to the adsorption of CO, it was found that increasing the temperature to 50 ℃ markedly increased the amount of CO adsorbates formed on the Ru(0001) surface from the adsorption of formaldehyde. On increasing the potential, the electro oxidation of the CO adsorbates to CO 2 took place via reaction with the active (1×1) O oxide. A significant increase in the surface reactivity was observed on the RuO 2(100) phase formed at higher potentials. Formic acid was detected as a partial oxidation product during formaldehyde electro oxidation. The data obtained at 50 ℃ are markedly different from those collected at 10 and 25 ℃ in terms of the amount of both CO 2 and formic acid formed and the adsorbed CO L and CO H species observed. These results were rationalized by the thermal effects on both the loosening of the CO adlayer and the activation of surface oxide on increasing the temperature.

Extended central hepatectomy with preservation of segment 6 for patients with centrally located hepatocellular carcinoma

BACKGROUND： In order to preserve functional liver parenchyma, extended central hepatectomy （segments 4, 5, 7 and 8 resection） was proposed for the management of centrally located hepatocellular carcinoma invading the right and middle hepatic veins, reconstructing segment 6 outflow in the absence of the thick inferior right hepatic vein. The present study was to describe our surgical techniques of extended central ***： Between 2008 and 2012, 5 patients with centrally located hepatocellular carcinoma invading or in the vicinity of the right and middle hepatic veins underwent extended central hepatectomy. The thick inferior right hepatic vein was preserved during dissection. Gore-Tex graft was used for segment 6 outflow reconstruction in the absence of the thick inferior right hepatic ***： The mean future remnant liver volume for segments 2 and 3 was 28% versus 45% on segment 6 preservation. The mean tumor diameter was 7.4 cm. The thick inferior right hepatic vein was found in 1 patient. Outflow reconstruction from segment 6 was performed in 4 patients. Postoperative complications included bile leakage （1 patient）, pleural effusion （2） and liver failure （1）. The rate of graft patency was 75%. There was no perioperative ***： Extended central hepatectomy is a safe alternative for extended hepatic resection in selected patients attempting to preserve the functional liver parenchyma.

Usefulness of fecal lactoferrin in predicting and monitoring the clinical severity of infectious diarrhea

AIM:To explore the value of fecal lactoferrin in predicting and monitoring the clinical severity of infectious ***:Patients with acute infectious diarrhea ranging from 3 mo to 10 years in age were enrolled,and one to three stool samples from each subject were *** parameters,including white blood cells /differential count,C-reactive protein,fecal mucus,fecal pus cells,duration of fever,vomiting,diarrhea and severity(indicated by Clark and Vesikari scores),were recorded and *** lactoferrin was determined by enzyme-linked immunosorbent assay and compared in different pathogen and disease *** estimating equations(GEE) were also used for ***:Data included 226 evaluations for 117 individuals across three different time *** lactoferrin was higher in patients with Salmonella(11.17 μg/g ± 2.73 μg/g) or Campylobacter(10.32 μg/g ± 2.94 μg/g) infections and lower in patients with rotavirus(2.82 μg/g ± 1.27 μg/g) or norovirus(3.16 μg/g ± 1.18 μg/g) *** of fecal lactoferrin were significantly elevated in patients with severe(11.32 μg/g ± 3.29 μg/g) or moderate(3.77 μg/g ± 2.08 μg/g) disease activity compared with subjects with mild(1.51 μg/g ± 1.36 μg/g) disease activity(P < 0.05).GEE analysis suggests that this marker could be used to monitor the severity and course of gastrointestinal infections and may provide information for disease ***:Fecal lactoferrin increased during bacterial infection and with greater disease severity and may be a good marker for predicting and monitoring intestinal inflammation in children with infectious diarrhea.

Ultra-low power IGZO optoelectronic synaptic transistors for neuromorphic computing

Inspired by biological visual systems, optoelectronic synapses with image perception, memory retention, and preprocessing capabilities offer a promising pathway for developing high-performance artificial perceptual vision computing systems. Among these, oxide-based optoelectronic synaptic transistors are wellknown for their enduring photoconductive properties and ease of integration, which hold substantial potential in this regard. In this study, we utilized indium gallium zinc oxide as a semiconductor layer and high-k ZrAlOxas a gate dielectric layer to engineer low-power high-performance synaptic transistors with photonic *** biological synaptic functions, including excitatory postsynaptic currents, paired-pulse facilitation,and the transition from short-term to long-term plasticity, were replicated via optical pulse *** simulation was sustained even at an operating voltage as low as 0.0001 V, exhibiting a conspicuous photonic synaptic response with energy consumption as low as 0.0845 fJ per synaptic event. Furthermore,an optoelectronic synaptic device was employed to model “learn-forget-relearn” behavior similar to that exhibited by the human brain, as well as Morse code encoding. Finally, a 3 × 3 device array was constructed to demonstrate its advantages in image recognition and storage. This study provides an effective strategy for developing readily integrable, ultralow-power optoelectronic synapses with substantial potential in the domains of morphological visual systems, biomimetic robotics, and artificial intelligence.
