Prostate health index can stratify patients with Prostate Imaging Reporting and Data System score 3 lesions on magnetic resonance imaging to reduce prostate biopsies

We aim to evaluate prostate health index as an additional risk-stratification tool in patients with Prostate Imaging Reporting and Data System score 3 lesions on multiparametric magnetic resonance imaging.Men with biochemical or clinical suspicion of having prostate cancer who underwent multiparametric magnetic resonance imaging in two tertiary centers(Queen Mary Hospital and Princess Margaret Hospital,Hong Kong,China)between January 2017 and June 2022 were included.Ultrasound-magnetic resonance imaging fusion biopsies were performed after prostate health index testing.Those who only had Prostate Imaging Reporting and Data System score 3 lesions were further stratified into four prostate health index risk groups and the cancer detection rates were analyzed.Out of the 747 patients,47.3%had Prostate Imaging Reporting and Data System score 3 lesions only.The detection rate of clinically significant prostate cancer in this group was 15.0%.The cancer detection rates of clinically significant prostate cancer had statistically significant differences:5.3%in prostate health index<25.0,7.4%in prostate health index 25.0-34.9,17.9%in prostate health index 35.0-54.9,and 52.6%in prostate health index≥55.0(P<0.01).Among the patients,26.9%could have avoided a biopsy with a prostate health index<25.0,at the expense of a 5.3%risk of missing clinically significant prostate cancer.Prostate health index could be used as an additional risk stratification tool for patients with Prostate Imaging Reporting and Data System score 3 lesions.Biopsies could be avoided in patients with low prostate health index,with a small risk of missing clinically significant prostate cancer.

任务型教学法是20世纪80年代以来在对外汉语教学中逐渐形成和应用的一种新的教学方法。此后这种教学方法逐渐发展并在对外汉语教学领域得到广泛应用。由于该教学方法具有许多优点,最突出的优点是促进学生学习的创造力和主动性,提高学生的语言能力。因此,近年来世界上有很多语言学家对该教学方法从不同角度进行研究。对越南的汉语教学界来说,任务型教学法因为诸多客观条件的限制,并未在汉语教育领域中得到广泛应用,尤其是线上教学形式。本论文在前人语言专家成果的理论基础上,继续对基于任务型教学法应用在越南初级汉语口语线上课堂进行深入研究。本论文分为6章:其中第一章、第二章主要论述基于任务型教学法的理论基础,设计原则以及相关文献综述。第三章的内容是从第二章的理论基础进行教学设计,本论文的研究对象是信锦越南厂责任有限公司的越南人员,使用的教材是速通汉语教程。第四章根据第三章的教学设计进行两轮实施教学,收集学生的反馈。第五章根据教师和学生对初级汉语口语线上课堂设计任务的反馈,进行对任务型初级汉语口语教学的反思。最后,第六章针对任务型教学法在越南汉语初级口语线上课堂的设计任务和编写任务型教学法的口语教材提出建议。本研究的研究方法是行动研究,通过两轮行动研究,实施教学,提出问题,从自己实施教学中的观察和经验提出建议。本研究的教学设计结果包括:速通汉语教程第一册第八课《你的生日是几月几号?》,第十课《你家有几口人?》;第二册第一课《你在听什么?》,第二课《你平时几点起床?》基于任务型教学法设计的教案。通过这篇论文,基于任务型教学法在越南初级汉语口语线上课堂教案设计和两轮实施教学创建一个在信锦越南厂责任有限公司应用的汉语教学模式,任务的设计会考虑到学习者的学习特点。希望未来能够推广在越南河南工业区所有的中国台湾公司应用。

Endometriosis is a common chronic gynecological disease with endometrial cell implantation outside the uterus.Angiogenesis is a major pathophysiology in endometriosis.Our previous studies have demonstrated that the prodrug of epigallocatechin gallate(ProEGCG)exhibits superior anti-endometriotic and anti-angiogenic effects compared to epigallocatechin gallate(EGCG).However,their direct binding targets and underlying mechanisms for the differential effects remain unknown.In this study,we demonstrated that oral ProEGCG can be effective in preventing and treating endometriosis.Additionally,1D and 2D Proteome Integral Solubility Alteration assay-based chemical proteomics identified metadherin(MTDH)and PX domain containing serine/threonine kinase-like(PXK)as novel binding targets of EGCG and ProEGCG,respectively.Computational simulation and BioLayer interferometry were used to confirm their binding affinity.Our results showed that MTDH-EGCG inhibited protein kinase B(Akt)-mediated angiogenesis,while PXK-ProEGCG inhibited epidermal growth factor(EGF)-mediated angiogenesis via the EGF/hypoxia-inducible factor(HIF-1a)/vascular endothelial growth factor(VEGF)pathway.In vitro and in vivo knockdown assays and microvascular network imaging further confirmed the involvement of these signaling pathways.Moreover,our study demonstrated that ProEGCG has superior therapeutic effects than EGCG by targeting distinct signal transduction pathways and may act as a novel antiangiogenic therapy for endometriosis.

This paper proposes a micro-LED backlight module with a distributed Bragg reflector(DBR) structure to achieve excellent micro-LED backlight module quality and uses deep reinforcement learning(DRL) architecture for optical design. In the DRL architecture, to solve the computing environment problems of the two extreme structures of micro-scale and macro-scale, this paper proposes an environment control agent and virtual-realistic workflow to ensure that the design environment parameters are highly correlated with experimental results. This paper successfully designed a micro-LED backlight module with a DBR structure by the abovementioned methods.The micro-LED backlight module with a DBR structure improves the uniformity performance by 32% compared with the micro-LED backlight module without DBR, and the design calculation time required by the DRL method is only 17.9% of the traditional optical simulation.

Background:Previously published meta-epidemiological studies focused on Western medicine have identified some trial characteristics that impact the treatment effect of randomized controlled trials(RCTs).Nevertheless,it remains unclear if similar associations exist in RCTs on Chinese herbal medicine(CHM).Further,Chinese medicine-related characteristics have not been explored yet.Objective:To investigate trial characteristics related to treatment effect estimates on CHM RCTs.Search strategy:This meta-epidemiological study searched 5 databases for systematic reviews on CHM treatment published between January 2011 and July 2021.Inclusion criteria:An eligible systematic review should only include RCTs of CHM and conduct at least one meta-analysis.Data extraction and analysis:Two reviewers independently conducted data extraction on general characteristics of systematic reviews,meta-analyses and included RCTs.They also assessed the risk of bias of RCTs using the Cochrane risk of bias tool.A two-step approach was used for data analyses.The ratio of odds ratios(ROR) and difference in standardized mean differences (dSMD) with 95%confidence interval (CI) were applied to present the difference in effect estimates for binary and continuous outcomes,respectively.Results:Ninety-one systematic reviews,comprising 1338 RCTs were identified.For binary outcomes,RCTs incorporated with syndrome differentiation (ROR:1.23;95%CI:[1.07,1.39]),adopting Chinese medicine formula (ROR:1.19;95%CI:[1.03,1.34]),with low risk of bias on incomplete outcome data (ROR:1.29;95%CI:[1.06,1.52]) and selective outcome reporting (ROR:1.12;95%CI:[1.01,1.24]),as well as a trial size≥100 (ROR:1.23;95%CI:[1.04,1.42]) preferred to show larger effect estimates.As for continuous outcomes,RCTs with Chinese medicine diagnostic criteria (dSMD:0.23;95%CI:[0.06,0.41]),judged as high/unclear risk of bias on allocation concealment (dSMD:-0.70;95%CI:[-0.99,-0.42]),with low risk of bias on incomplete outcome data (dSMD:0.30;95%CI:[0.18,0.43]),conducted at a single center

Comparing the effects of dual-task training and single-task training on walking, balance and cognitive functions in individuals with Parkinson’s disease: A systematic review

Background: This systematic review aimed to examine whether dual-task (DT) training was superior to single-task (ST) training in improving DT walking, balance and cognitive functions for individuals with Parkinson’s disease (PD).Methods: Literature search was performed in the following electronic databases: PubMed, the Cochrane Library, Web of Science, and Metstr covering inception to May 10, 2023. And in order to facilitate comparison across trials, we calculated the effect size (Hedges’ g) of gait, balance, cognitive, and other parameters under both ST and DT conditions, using the mean change score and standard deviation (SD) of change score of the experimental and control groups. Randomized controlled trials that examined the effects of DT motor and cognitive training in individuals with Parkinson’s disease were included for this systematic review.Results: A total of 214 participants recruited from six articles (actually five trials) were involved in this review. In terms of walking ability, only double support time and stride time variability showed significant between-group difference (Hedges’ g = 0.34, 0.18, respectively). Compared to ST training group, DT training group had a more improvement effect in laboratory balance measurement (Hedges’ g = 0.18, 1.25), but no significant improvement in clinical balance measurement. No significant between-group differences were observed, thus its training effect on cognitive function was inconclusive.Conclusions: The DT training failed to achieve promising results better than ST training in improving DT walking and balance functions for individuals with PD. Any firm conclusion cannot be drawn at present, due to the limited number of eligible publications. Larger sample size and high-quality studies are needed to investigate the effectiveness of DT training in individuals with PD.

Risk of ischaemic and haemorrhagic stroke in Chinese undergoing percutaneous coronary intervention treated with potent P2Y12 inhibitor versus clopidogrel

Background Stroke after acute coronary syndrome(ACS)can be devastating.It is uncertain whether the risks of ischaemic stroke or intracranial haemorrhage(ICH)are associated with different choices of P2Y12 inhibitors(potent P2Y12 inhibitors such as ticagrelor and prasugrel vs clopidogrel).Even though East Asians are known to have different thrombotic and haemorrhagic profiles from Caucasians,data on Chinese patients are sparse.Method This was a retrospective cohort study conducting in Chinese patients with ACS who underwent first-ever percutaneous coronary intervention from 14 hospitals in Hong Kong between 2010 and 2017.The primary efficacy endpoint was ischaemic stroke.The secondary efficacy endpoint was a composite outcome of thrombotic events including all-cause mortality,non-fatal myocardial infarction and ischaemic stroke.The primary safety endpoint was ICH.The secondary safety endpoint was a composite of major bleeding events.Results After adjustment of baseline characteristics by 1:1 propensity score matching,a total of 6220 patients(3110 on each group)were analysed.Compared with clopidogrel,potent P2Y12 inhibitors were associated with a lower risk of ischaemic stroke(HR 0.57;95%CI 0.37 to 0.87;p=0.008)and a lower risk of thrombotic events(HR 0.77;95%CI 0.66 to 0.90;p=0.001).Potent P2Y12 inhibitor was associated with similar risk of ICH(HR 0.65;95%CI 0.34 to 1.25,p=0.20)and major bleeding(HR 0.83;95%CI 0.68 to 1.01,p=0.069).Conclusions Potent P2Y12 inhibitors were associated with a lower adjusted risk of ischaemic stroke and thrombotic events,compared with clopidogrel.The risks of ICH and major bleeding were similar.

Federated Feature Concatenate Method for Heterogeneous Computing in Federated Learning

Federated learning is an emerging machine learning techniquethat enables clients to collaboratively train a deep learning model withoutuploading raw data to the aggregation server. Each client may be equippedwith different computing resources for model training. The client equippedwith a lower computing capability requires more time for model training,resulting in a prolonged training time in federated learning. Moreover, it mayfail to train the entire model because of the out-of-memory issue. This studyaims to tackle these problems and propose the federated feature concatenate(FedFC) method for federated learning considering heterogeneous clients.FedFC leverages the model splitting and feature concatenate for offloadinga portion of the training loads from clients to the aggregation server. Eachclient in FedFC can collaboratively train a model with different cutting layers.Therefore, the specific features learned in the deeper layer of the serversidemodel are more identical for the data class classification. Accordingly,FedFC can reduce the computation loading for the resource-constrainedclient and accelerate the convergence time. The performance effectiveness isverified by considering different dataset scenarios, such as data and classimbalance for the participant clients in the experiments. The performanceimpacts of different cutting layers are evaluated during the model training.The experimental results show that the co-adapted features have a criticalimpact on the adequate classification of the deep learning model. Overall,FedFC not only shortens the convergence time, but also improves the bestaccuracy by up to 5.9% and 14.5% when compared to conventional federatedlearning and splitfed, respectively. In conclusion, the proposed approach isfeasible and effective for heterogeneous clients in federated learning.

The objective of this study was to determine the degree of citizen participation in urban planning processes in the municipality of Comala,Colima,Mexico to have a broader vision of the citizens and the environment in which they live.An instrument was designed specifically to perform this study and the instrument was validated by calculating Cronbach’s Alpha.The results showed that citizens were highly involved in issues concerning their urban environment,and the main problems of the public spaces were also identified.
