Physical rehabilitation for lung transplant candidates and recipients: An evidence-informed clinical approach

Physical rehabilitation of lung transplant candidates and recipients plays an important in optimizing physical function prior to transplant and facilitating recovery of function post-transplant. As medical and surgical interventions in lung transplantation have evolved over time, there has been a demographic shift of individuals undergoing lung transplantation including older individuals, those with multiple co-morbidites, and candidates with respiratory failure requiring bridging to transplantation. These changes have an impact on the rehabilitation needs of lung transplant candidates and recipients. This review provides a practical approach to rehabilitation based on research and clinical practice at our transplant centre. It focuses on functional assessment and exercise prescription during an uncomplicated and complicated clinical course in the pre-transplant, early and late post-transplant periods. The target audience includes clinicians involved in pre- and posttransplant patient care and rehabilitation researchers.

Selective Cytotoxic Effect of ZnO Nanoparticles on Glioma Cells

In this study we examined the cytotoxic effect of ZnO nanoparticles on various human cancer and normal *** found that the ZnO nanoparticles exerted a cytotoxic effect on the human glioma cell lines A172,U87,LNZ308,LN18,and LN229,whereas no cytotoxic effect was observed on normal human ***,the ZnO nanoparticles induced cell death in breast and prostate cancer cell lines while no major effect was observed in the respective normal breast and prostate cell *** the fl uorescent dye 2,7-dichlorofl uorescein diacetate,we found that treatment of the glioma cells with ZnO nanoparticles induced a large increase in the generation of reactive oxygen species(ROS)and treatment of the cells with N-acetyl cysteine decreased the cytotoxic effect of the ZnO *** contrast,a smaller effect on ROS generation was observed in the normal *** results suggest that ZnO nanoparticles may be employed as a selective cytotoxic agent for the eradication of cancer cells.

Background:We investigate whether the success of contrarian investment strategies can be attributed to differences in the relative illiquidity of stocks categorized as value investments versus those categorized as glamour ***:Following Lakonishok et al.(J Financ 49:1541–1578,1994),we assess the illiquidity characteristics of portfolios that underlie contrarian investment strategies that are based on the level of stock’s book to ***:We find strong evidence that those portfolios characterized as value investments are associated with dramatically greater levels of illiquidity than glamour *** further demonstrate that strategies based on the illiquidity in the year prior to portfolio formation result in return characteristic of ostensibly contrarian ***:These results suggest that the higher returns associated with contrarian investment strategies are the result of the higher illiquidity associated with value portfolios and represent compensation that the investor receives for accepting *** also suggest that researchers should be cautious before attributing apparent anomalies to behavior-driven expectational errors rather than to other attributes unrelated to behavior,such as illiquidity.

New mechanisms of multidrug resistance: an introduction to the Cancer Drug Resistance special collection

Cancer Drug Resistance publishes contributions to understanding the biology and consequences of mechanisms that interfere with successful treatment of cancer. Since virtually all patients who die of metastatic cancer have multidrug-resistant tumors, improved treatment will require an understanding of the mechanisms of resistance to design therapies that circumvent these mechanisms, exploit these mechanisms, or inactivate these multidrug resistance mechanisms. One example of a resistance mechanism is the expression of ATP-binding cassette efflux pumps, but unfortunately, inhibition of these transporters has not proved to be the solution to overcome multidrug resistance in cancer. Other mechanisms that confer multidrug resistance, and the confluence of multiple different mechanisms (multifactorial multidrug resistance) have been identified, and it is the goal of this Special Collection to expand this catalog of potential multidrug resistance mechanisms, to explore novel ways to overcome resistance, and to present thoughtful reviews on the problem of multidrug resistance in cancer.

How have breeders adapted rice flowering to the growing region?

Flowering time is a crucial rice trait that influences its adaptation to various environments, cropping schedules, and agronomic characteristics. Rice breeders have exploited spontaneous mutations in heading date genes to regulate the flowering time. In the present study, we investigated how breeders in Fukui Prefecture regulated days to heading while developing promising rice varieties. Genome-wide association studies (GWAS) identified Hd1, Hd16, and Hd18 as the major genes controlling days to heading in the population. However, we suspected that this highly bred population might exhibit genomic stratification, which could lead to spurious or false correlations in the GWAS. Thus, we also conducted correlation and partial correlation analyses, which uncovered another key heading date gene, Hd17, that GWAS failed to detect because of its linkage disequilibrium with the major effect gene Hd16. Examination of haplotype frequencies across different breeding periods revealed that the early-heading Hd16 (Hd16(E)) and late-heading Hd17 (Hd17(L)) were increasingly co-selected in the Hd1 functional population. Varieties carrying this Hd16(E)/Hd17(L) combination exhibited days to heading in the range of 70-80, which corresponds to the peak temperature and sunshine period and is also optimal for grain quality and yield components in the Fukui environment. The present study highlights that it is imperative to remain vigilant for Type I (false positives) and Type II (false negatives) errors when performing GWAS on highly bred populations and to implement appropriate countermeasures by accounting for gene-by-gene interactions established through the breeding process. We also discuss the effectiveness of Hd16(E), which is not used outside Japan for subtle days to heading control but is widely used in Japan at certain latitudes.

Laparoscopic surgery in pregnancy

Each year, roughly 2% of pregnant women will undergo non-obstetrical abdominal surgery. Appendicitis, symptomatic cholelithiasis and adnexal masses are some of the common diagnoses encountered. Pregnancy poses challenges in the diagnosis and surgical management of these conditions for several reasons. Since the 1990's, laparoscopic surgery has gained popularity and in the past few years has become the standard of care for pregnant women with surgical pathologies. The advantages of laparoscopic surgery include shorter hospital stay, lower rates of wound infection, and decreased time to bowel function. This brief review discusses key points in laparoscopic surgery during pregnancy and highlights studies comparing laparoscopic and open approaches in common surgical conditions during pregnancy.

Estimation of successful capping with complete aspiration of bladder via nephroureterostomy tube

BACKGROUND Ureteral stent and nephroureterostomy tube(NUT)are treatments of ureteral *** stent provides better quality of *** of NUT is desired whenever *** To assess outcomes of capping trial among cancer patients with complete aspiration of retained contrast from bladder via *** Our Institutional Review Board approved retrospective review of all NUT placement,NUT exchange and conversion of nephrostomy catheter into NUT performed during June 2013 to June 2015(n=578).Cases were excluded due to lack of imaging of bladder(n=37),incomplete aspiration of bladder(n=324),no attempt at capping NUT(n=166),and patients with confounding factors interfering with results of capping trial including non-compliant bladder,bladder outlet obstruction and catheter malposition(n=14).Study group consisted of 37 procedures in 34 patients(male 19,female 15,age 2-83 years,average 58,median 61)most with cancer(prostate 8,endometrial 5,bladder 4,colorectal 4,breast 2,gastric 2,neuroblastoma 2,cervical 1,ovarian 1,renal 1,sarcoma 1,urothelial 1 and testicular 1)and one with Crohn’s *** records were reviewed to assess outcomes of capping *** 95%confidence intervals(95%CI)were *** Among patients with complete aspiration of retained contrast,30(81%,95%CI:0.65-0.92)catheters were successfully capped(range 12-94 d,average 40,median 24.5)until planned conversion to internal stent(23),routine exchange(5),removal(1)or death unrelated to catheter(1).Seven capping trials(19%,95%CI:0.08-0.35)were unsuccessful(range 2-22 d,average 12,median 10)due to leakage(3),elevated creatinine(2),fever/hematuria(1)and nausea/vomiting(1).CONCLUSION Capping trial success among patients with complete aspiration of retained contrast/urine from bladder via NUT appears high.
