Exercise-based real-time telerehabilitation for older patients recently discharged after transcatheter aortic valve implantation: An extended feasibility study

OBJECTIVES To assess the extended feasibility of a telerehabilitation program and its effects on physical performance in older adults who have recently undergone transcatheter aortic valve implantation(TAVI).METHODS In this single-center feasibility study, patients underwent an eight-week telerehabilitation program, involving webbased home exercise training twice weekly, an activity tracker, access to an informative website, and one online session with a nurse, starting one-week postoperative. Data collection was performed before surgery and three months postoperative. The feasibility of the intervention was based on recruitment and adherence to the program. As a secondary outcome, we evaluated the change in six-minute walk distance from before surgery to three months *** Forty-one patients scheduled for TAVI were assessed for eligibility;15 patients(37%) were enrolled. Of these, eight were excluded after surgery due to tiredness(n = 2), non-cardiac related hospital readmission(n = 2), fluctuating health(n = 1),death during hospital stay(n = 1), and reduced cognition(n = 2). Seven patients completed the eight-week web-based intervention and were evaluated three months postoperative. Their median(IQR) age was 83 [81, 87] years, and the sample comprised three men and four women. Their walked distance improved from median(IQR) 262 [199, 463] before surgery, to 381 [267, 521]meters three months postoperative. No adverse events were *** Web-based telerehabilitation, including supervised exercise training, in older adults who have recently undergone TAVI was feasible for a small number of patients who completed the eight-week intervention. This was reflected in an improvement in their walked distance three months after the surgery. However, the low recruitment and retention rates do question the overall feasibility of this intervention in a frail, older population of post-TAVI patients.

Background: Cervical cancer (CC) is one of the most frequent cancers and the leading cause of death from gynecological cancer in Low and middle income countries, Cameroon inclusive. Surgery is the primary treatment modality when the disease is diagnosed at early stage. Radical hysterectomy in cervical cancer has not been evaluated in recent years in Cameroon. The purpose of this study is thus to evaluate the epidemiological and clinical features and short term outcomes of patients who underwent surgery. Patients and methods: This retrospective study was conducted at the Douala Gynaeco-obstetric and Pediatric Hospital and the Douala General Hospital. Cervical cancer patients who underwent Radical hysterectomy between January 2015 and December 2020 were included. A pre-established data collection tool was used to record socio-demographic, clinical and outcomes information from patients’ files;additional outcome information was obtained from phone calls. Descriptive analysis was done using the SPSS version 26. Bivariate analysis was used to determine associations between disease and patients characteristics and occurrence of adverse postoperative outcome. P value of 0.05 was considered. Results: Sixty one patients were enrolled. Their ages ranged from 33 to 74 years with a mean age of 51.95 ± 10.29 years. Over 85% of women were married, 65.57% were unemployed and 86.88% were multiparous. Only 28% had never done cervical cancer screening. Most patients had stage IB1 to IB2 stage disease (57.1%). Less than 9% underwent radical hysterectomy and 8 of those (13.11%) suffered intraoperative complications. Twenty-five patients (40.98%) presented immediate and short term complications. There was no significant association between the disease or patients’ characteristics and adverse outcomes. Introduction: Anthropometry applied to newborns is a reliable indicator of the quality of fetal growth. The latter is influenced by genetic, racial and nutritional factors varying from one population to another, explaining why a standard cannot be applied to all populations. Research question: should the Caucasian frame of reference be dogmatically applied in our African context? Multicenter studies are therefore necessary;hence the interest of this work, the main objective of which was to describe the anthropometric profile of full-term newborns in the city of Douala. Methodology: We carried out a cross-sectional study with an analytical aim and prospective data collection in the maternity wards of the Douala General Hospital, Laquintinie Hospital, District hospitals of Deido, Nylon and Bonassama over a period of 4 months (January to April 2020). We were interested in any newborn, born alive, vaginally or by cesarean section, seen in the first 24 hours from a full-term single-fetal pregnancy whose mother had given consent. We excluded newborns whose term was unclear and those with congenital malformations or signs of embryo-foetopathy. Data collection was done using structured and pre-tested survey sheets. The study variables were obstetric and anthropometric. Statistical analyzes were carried out with CS Pro 7.3 and SPSS version 25.0 software. The Student, Chi-square and Fischer tests were used to compare the means of the variables, the percentages with a significance threshold P value Results: During the study period, 305 full-term newborns were included, divided into 172 boys and 133 girls. The average anthropometric parameters of the full-term newborn in the city of Douala were: average weight: 3305 grams, average height: 49.8 centimeters, average head circumference: 34.6 centimeters, average upper arm circumference: 11.3 centimeters, circumference average thoracic: 32.8 centimeters. This paper investigates the importance of trust in international institutions for the development of tax morale by focussing on interactions between trust in the national government and trust in the European Union(EU)or trust in the United Nations(UN).Using large-scale survey data from European countries,we provide evidence that all three trust variables are significantly related to the individual level of tax ***,the results regarding trust in the EU and the UN are very similar,while trust in the national government appears to be the main factor in driving tax ***,depending on the national context,trust in the national government interacts differently with trust in different international institutions with respect to the shaping of tax morale.

Atm deficient zebrafish model reveals conservation of the tumour suppressor function and a role in fertility

Biallelic loss-of-function variants in ATM (Ataxia Telangiectasia Mutated) cause Ataxia Telangiectasia (AT), a rare disorder associated with cerebellar degeneration and ataxia, cancer predisposition, infertility, growth retardation, etc. ATM is a phosphoinositide 3-kinase-related kinase (PIKK) with a role in DNA repair and maintenance of genome stability. Studying a multisystem genetic disease like AT requires animal models to ascertain its pathogenesis at the level of tissues, organs and the organism. Due to its small size, cheap maintenance, large progeny, rapid development and initial transparency, zebrafish (Danio rerio) is an increasingly popular vertebrate model organism, suitable for genetic modifications and large-scale in vivo therapeutic screens as embryos are chemically permeable to small compounds. Currently, no zebrafish model for AT exists.1 We generated atm knock-outs through CRIPSR-Cas9 mutagenesis. We show that atm conserved its function as a tumour suppressor gene and is involved in gametogenesis and fertility. Therefore, this mutant is of great value for further studies investigating the role of atm in reproduction and tumorigenesis.

Antigen specificity and receptor diversity are hallmarks of B and T cells. Only 1%-5% of all T cells bear a γδ T-cell receptor （TCR）, and these cells have features of both innate and adaptive cells. While the antigen presentation process was long ago elucidated for αβ T cells, it has remained enigmatic for γδ T cells. In a recent study by Vavassori and co-workers,1 this major gap in know-ledge was addressed and dosed.

【目的】通过调研“订阅-开放”(S2O)模式的源起与发展,并对比分析讨论该模式相比于其他出版模式的优缺点,为中国传统订阅期刊在开放获取的浪潮中进一步转型发展提供参考与启示。【方法】采用文献调研、对比分析和案例分析法,分析S2O模式的发展现状及与其他开放获取模式相比存在的利弊,并以EDP Sciences出版社的实践为例,提出传统订阅期刊采取S2O模式向开放获取转型的可行性。【结果】结合EDP Sciences出版社期刊使用S2O模式转型的实践经验,发现S2O模式对于传统订阅期刊转型较为有利。既给予期刊更多的灵活度与自由度,也不需要过度依赖外部资金的支持,同时符合开放科学的精神,为更多的科研人员,尤其是为资金欠缺地区的科研人员带来支持。【结论】在开放科学、开放获取的发展潮流中,一些中国传统订阅期刊也在寻求转型与过渡,除了主要的开放获取模式之外,S2O模式也是可行的选择。积极采用新兴开放获取模式有利于中国期刊进一步走向国际,在全球开放科学的交流中获得更多的主动权。

Testicular sperm extraction(TESE)outcomes in the context of malignant disease:a systematic review

Advances in the oncology field have led to improved survival ***,quality of life after remission is anticipated,which includes the possibility to conceive *** cancer treatments are potentially gonadotoxic,fertility preservation must be *** fertility preservation is mainly based on ejaculated sperm *** this is not possible,testicular sperm extraction(TESE)may be *** identify situations in which TESE has been beneficial,a systematic review was *** search was carried out on the PubMed,Scopus,Google Scholar,and CISMeF databases from 1 January 2000 to 19 March *** Reporting Items for Systematic Reviews and Meta-Analyses(PRISMA)recommendations were followed in selecting items of ***-four articles were included in the systematic review,including 15 articles on oncological testicular sperm extraction(oncoTESE),18 articles on postgonadotoxic treatment TESE and 1 article on both oncoTESE and postgonadotoxic treatment *** sperm freezing was possible for 42.9%to 57.7%of patients before gonadotoxic treatment and for 32.4%to 75.5%of patients after gonadotoxic treatment,depending on the type of malignant *** no formal conclusion could be drawn about the chances to obtain sperm in specific situations,our results suggest that TESE can be proposed before and after gonadotoxic *** treatment,TESE is more often proposed for men with testicular cancer presenting with azoospermia since TESE can be performed simultaneously with tumor removal or *** chemotherapy,TESE may be planned if the patient presents with persistent azoospermia.

Adjuvant-attenuated symptom severity of influenza infections in vaccinated children

Background:Young children are at high risk for developing complications of influenza,as well as severe clinical presentation of *** provides direct protection and reduces symptom severity in breakthrough *** assessed whether adjuvanted trivalent seasonal influenza vaccine is associated with symptom severity in children who developed laboratory-confirmed influenza,as compared to children who received quadrivalent inactivated ***:A cluster randomized controlled trial of influenza vaccines in Canadian Hutterite colonies was conducted from the 2016-2017 to the 2018-2019 influenza *** were vaccinated with either quadrivalent inactivated influenza vaccine(QIV),or the MF59 adjuvanted trivalent influenza vaccine(aTIV).We assessed children who developed PCR-confirmed influenza infection for symptom severity outcomes using multivariable generalized negative binomial ***:Among vaccinated children,49 infections were observed across 1779 *** formulation(aTIV vs QIV)was not significantly associated with composite symptom outcomes,including total number of symptoms or total duration of symptom presentation(p>0.05 for all outcomes).Receipt of aTIV vaccination was significantly associated with attenuation of fever,with an estimated 74%reduction in fever *** influenza A type infections,adjuvanted vaccination was significantly associated with reduced systemic symptoms(incidence rate ratios:0.16,95%confidence intervals:0.03,0.64,p=0.01).No associations were observed between vaccine formulation and symptom severity in influenza B ***:In vaccinated children who develop an influenza infection,vaccine formulation was associated with attenuated fever severity,leading to reduced systemic *** influenza A infections,adjuvanted vaccination was significantly associated with reduced systemic symptoms.
