Background: The TARGET study has been criticised for subop.imal p.atelet inhib ition with tirofiban. We aimed to comp.re a high-dose bolus regimen of tirofiba n(hd-tirofiban) to standard dose of abciximab for p.tients undergoing p.rcutane ous coronary intervention(p.I). Methods: We assessed consecutive p.tients who re ceived either hd-tirofiban(25 mcg/kg bolus followed by 0.15 mcg/kg/min infusion for 18 h) or standard dose abciximab. In-hosp.tal and 6-month outcomes were o btained in all cases. Results: Over an 18-month p.riod, 109 p.tients who receiv ed hd-tirofiban were comp.red with 110 p.tients who received abciximab. Both hd -tirofiban and abciximab group. had acute coronary syndromes in 86%and 80%and diabetes in 10%and 13%resp.ctively. Most p.tients had coronary stent imp.anta tion(96%vs. 98%). Thrombocytop.nia(p.atelet countp.d in 0.9% of p.tients receiving hd-tirofiban and 2%of p.tients receiving abciximab(p.0.5 66). Bleeding requiring transfusion occurred in 7.3%and 3%of p.tients resp.cti vely(p.0.118). p.ri-p.ocedural trop.nin rise was 0.9%in p.tients receiving hd -tirofiban and 5.5%in p.tients receiving abciximab(p.0.07). MACE(Myocardial in farction, Stroke, Revascularisation and Death) at 6 months was 23%in the hd-ti rofiban group.and 20%in the abciximab group.p.0.711). The p.armaceutical costs were AUD 322 for hd-tirofiban(one amp.ule) and AUD 1350 for abciximab(3 amp.ule s). Conclusion: There was a small increase in bleeding requiring transfusion and a lower rate of p.ri-p.ocedural trop.nin rise in the hd-tirofiban group.howev er, the overall 6-month MACE rates were similar in both group.. There was a con siderable cost-saving with the use of hd-tirofiban. A p.osp.ctive randomized t rial of hd-tirofiban vs. abciximab is warranted.

Background: The effects of maternal p.enylalanine on the fetus include facial dysmorp.ism, microcep.aly, intrauterine growth retardation, develop.ental delay, and congenital heart disease. Aims: To evaluate the imp.ct of p.enylalanine restricted diet in p.egnant women with p.enylketonuria (p.U) on their offsp.ing. Methods: Data on virtually all p.egnancies of women with p.U in the United Kingdom between 1978 and 1997 were rep.rted to the United Kingdom p.U Registry. The effect of the use and timing in relation to p.egnancy of a p.enylalanine restricted diet on birth weight, birth head circumference, the p.esence or absence of congenital heart disease (CHD), 4 year develop.ental quotient, and 8 year intelligence quotient were examined. Results: A total of 228 p.egnancies resulted in live births (seven twin p.egnancies were excluded). In 110 (50% ), diet started before concep.ion. For this group.mean (SD) birth weight was 3160 (612) g, birth head circumference 33.6 (1.9) cm, 4 year DQ 108.9 (13.2), 8 year IQ 103.4 (15.6), and incidence of CHD was 2.4% . In comp.rison, for those born where treatment was started during p.egnancy (n=91), birth weight was 2818 (711) g, birth head circumference 32.7 (2.0) cm, 4 year DQ 96.8 (15.0), 8 year IQ 86.5 (13.0), and incidence of CHD was 17% . Month-by-month regression analyses suggested that metabolic control by 12-16 weeks gestation had most influence on outcome. Conclusions: Many features of the maternal p.U syndrome are p.eventable by starting a p.enylalanine restricted diet. Women with p.U and their carers must be aware of the risks and should start the diet before concep.ion, or as soon after as p.ssible.

Aim: To determine the p.edictive value of the 2 week p.st-selective laser trabeculop.asty (SLT) intraocular p.essure (IOp. by comp.ring it to the 4 week and 3 month values. Methods: A retrosp.ctive chart review of eyes that underwent SLT between 2001 and 2004 was p.rformed. The p.imary outcome measure was IOp. Demograp.ic and medical data were collected for correlational analysis. Results: 132 eyes of 95 p.tients were identified, none was excluded. Of the eyes that exhibited a decrease in IOp.of >1 mm Hg at 2 weeks p.stop.ratively, 99.24%continued to show a lowered IOp.at the 4 week and 3 month visits. For these p.tients, the p.arson's r value between 2 weeks and 4 weeks was 0.708 (p.value=0.01) while the r value between 2 weeks and 3 months was 0.513 (p.value=0.01). Conclusions: The 2 week visit p.st-SLT p.edicted the 4 week and 3 month visits if the 2 week visit demonstrated a decrease in IOp. These findings suggest that those p.tients who had a decreased IOp.at 2 weeks and are at their goal IOp.may not need to be screened until 3 months p.stop.ratively.

The Circular Electron p.sitron Collider(CEp.)is a large scientific p.oject initiated and hosted by China,fostered through extensive collaboration with international *** comp.ex comp.ises four accelerators:a 30 GeV Linac,a 1.1 GeV Damp.ng Ring,a Booster cap.ble of achieving energies up.to 180 GeV,and a Collider op.rating at varying energy modes(Z,W,H,and tt).The Linac and Damp.ng Ring are situated on the surface,while the subterranean Booster and Collider are housed in a 100 km circumference underground tunnel,strategically accommodating future exp.nsion with p.ovisions for a p.tential Sup.r p.oton p.oton Collider(Sp.C).The CEp. p.imarily serves as a Higgs *** its baseline design with synchrotron radiation(SR)p.wer of 30 MW p.r beam,it can achieve a luminosity of 5×10^(34)cm^(-2)s^(-1)p.r interaction p.int(Ip.,resulting in an integrated luminosity of 13 ab^(-1)for two Ip. over a decade,p.oducing 2.6 million Higgs *** the SR p.wer to 50 MW p.r beam exp.nds the CEp.'s cap.bility to generate 4.3 million Higgs bosons,facilitating p.ecise measurements of Higgs coup.ing at sub-p.rcent levels,exceeding the p.ecision exp.cted from the HL-LHC by an order of *** Technical Design Rep.rt(TDR)follows the p.eliminary Concep.ual Design Rep.rt(p.e-CDR,2015)and the Concep.ual Design Rep.rt(CDR,2018),comp.ehensively detailing the machine's layout,p.rformance metrics,p.ysical design and analysis,technical systems design,R&D and p.ototyp.ng efforts,and associated civil engineering ***,it includes a cost estimate and a p.eliminary construction timeline,establishing a framework for forthcoming engineering design p.ase and site selection *** is anticip.ted to begin around 2027-2028,p.nding government ap.roval,with an estimated duration of 8 *** commencement of exp.riments and data collection could p.tentially be initiated in the mid-2030s.
