Externally-induced meditative states： an exploratory fMRI study of architects＇ responses to contemplative architecture

Built environments can induce contemplative states, but direct evidence for their impact on the brain is lacking. This exploratory work investigated brain correlates of internal states elicited by architecture designed for contemplative experience. Functional MRI and self-reports of 12 architects were assessed to study their responses to photographs of ordinary and contemplative architectures. Images of contemplative buildings：（1） induced attentive, receptive, and absorbing experiences and diminished internal dialogue; （2） involved decreased engagement of prefrontal cortex; and （3） activated the occipital lobe, precentra[ gyrus, and inferior parietal lobule. They suggest that viewing buildings designed for contemplation may evoke experiential and brain signatures that consistently differ from those induced by buildings that serve everyday functions. The depth of such externally induced states was inversely correlated with the engagement of the Default Mode Network. Our study points toward a novel avenue for investigating how contemplation can be cultivated in the human brain/mind.

This paper is concerned with Bernstein polynomials on k-simploids by which we mean a crossproduct of k lower dimensional simplices. Specifically, we show that if the Bernstein polynomials ofa given function f on a k-simploid form a decreasing sequence then f+l, where l is any correspondingtensor product of affine functions. achieves its maximum on the boundary of the k-simploid. Thisextends recent results in [1] for bivariate Bernstein polynomials on triangles. Unlike the approachused in [1] our approach is based on semigroup techniques and the maximum principle for secondorder elliptic operators. Furthermore, we derive analogous results for cube spline surfaces.

Systemic evidence of acute seizureassociated elevation in serum neuronal injury biomarker in patients with temporal lobe epilepsy

Background:Patients with drug refractory temporal lobe epilepsy frequently accumulate cognitive impairment over time,suggesting loss of neurons induced by *** objective was to determine whether there is a temporal association between changes in serum levels of neural injury markers and electroencephalographic(EEG)evidence of seizures and interictal ***:We measured serum levels of neuron-specific enolase(NSE),a neuronal injury marker,relative to levels of S100β,a marker of glial injury,at 6 AM,9 AM,noon,3 PM,and 6 PM over the course of several days in 7 epilepsy patients undergoing continuous video-EEG monitoring and in 4 healthy ***:All epilepsy patients exhibited significant deviations in NSE levels through time,and 4 of the epilepsy patients exhibited large sample entropy values and large signal variation metrics for NSE relative to S100β.Controls did not exhibit such *** analysis revealed that NSE levels were significantly elevated after *** was also a highly significant relationship between increased EEG spike frequency and an increase in serum NSE levels measured 24 h ***:The detection of large but transient post-ictal increases in NSE suggests that even self-limited seizures may cause an injury to neurons that underlies cognitive decline in some *** this study used a small patient population,the pilot findings suggest that post-ictal assessment of serum NSE may serve as a biomarker for measuring the efficacy of future acute neuroprotective strategies in epilepsy patients.

Impact of gender and race on expectations and outcomes in total knee arthroplasty

Unplanned hospitalizations for metastatic cancers:The changing patterns of inpatient palliative care,discharge to hospice care,and in-hospital mortality in the United States

Objective:To describe the rates and temporal trends of inpatient end-of-life care among patients hospitalized with metastatic cancer in the United ***:We used data from the Nationwide Inpatient Sample to conduct a cross-sectional analysis of unplanned inpatient hospitalizations of patients aged 18 years or older with metastatic cancer from 2002 to *** logistic regression was used to assess patient-and hospital-level predictors of discharge to hospice care,palliative care,and in-hospital *** trends in outcomes were characterized with use of joinpoint ***:There were an estimated 350,241 unplanned hospitalizations per year of patients with a diagnosis of metastatic *** their inpatient stay,5.8%of patients received palliative care,and among those discharged alive,12.2%were referred to hospice *** rate of inpatient palliative care increased from 2.3%to 13.6%,the rate of discharge to hospice care increased from 4.1%to 15.6%,and the in-hospital mortality rate decreased from more than 14.0%to 9.8%.These patterns were consistent across cancer subtypes,and were most pronounced among patients with extreme risk of ***:Despite increases in the provision of comfort-oriented care to patients with meta-static cancer,few receive such *** recommend screening protocols in hospitals to identify patients who are good candidates for palliative care consultation and hospice referral.

Comparison of the extrusion rate of Crawford tubes

· AIM: To compare the outcomes of dacryocystorhinostomy(DCR) using traditional Crawford tubes(TCT) and Crawford tubes with suture(CTS) in the lumen.· METHODS: Retrospective case series consisting of patients who underwent DCR between 2008 and 2013.·RESUlTS: A total of 61 DCRs were performed on 50 patients. Patients who underwent DCR using CTS had higher rates of prolapse compared to the TCT group(50% vs 9.4%; P =0.003). Stent removal occurred earlier in patients who received CTS(3.3mo vs 5.1mo; P =0.004). Success rates were equivalent between the two groups(75% vs 81.1%; P =0.684).·CONClUSION: CTS in the lumen increases the risk of prolapse, prompting earlier tube removal in patients following DCR for nasolacrimal duct obstruction(NlDO).Earlier removal of tubes does not appear to significantly decrease success rates.

A novel class of segmented copolymers, dimethyldiphenylpolysiloxane/poly (oxytetramethylene)-polyurea (PSPEU), was synthesized from α , ω-bis (γ-aminopropyl) dimethyldiphenylpolysiloxane (APMPS), which was prepared by means of basic ring-opening copolymerization of octamethylcyclotetrasiloxane, hexaphenylcyclotrisiloxane and 1, 3-bis (γ-aminopropyl) tetramethyl disiloxane. The relationships between the diphenylsiloxy contents and the properties of APMPS, including refractive index, glass transition temperature, solubility parameter as well as thermal stability, were investigated; meanwhile, the thermal stability, dynamic mechanical properties, mechanical properiesas well as the antithrombogenicity in vitro of the PSPEU were also revealed.

Poly （ethyleneglycol monomethylether） methacrylate （PEGMM）was synthesized by means of the reaction of methacrylyl chloride with sodium monomethylpolyethyleneglycoxide and was characterized by FTIR,;H-NMR,and ultraviolet spectrometries. A series of poly （vinyl alcohol）-graft-PEGMM （PVA-g-PEGMM ）and methyl methacrylate-PEGMM copolymer （PMMA-PEGMM） were prepared and tested for antithrombogenicity in vitro. The results indicate that the antithrombogenicity of the copolymers basically increases with the increasing of the DP of polyoxyethylene （POE） chain and tends to a plateau after the DP around 114,i.e. the long chain structure of POE is favourable to the antithrombogenicityof its copolymers ;moreover, the extent of the improvement ofantithrombogenicity also relates to the PEGMM content of the copolymers and the kind of the matrix that the POE chains are located on. These results are consistent with the anticipation of the hypothesis of maintaining proteins normal conformations for blood compatible bioraaterials.
