Sitagliptin in patients with non-alcoholic steatohepatitis: A randomized, placebo-controlled trial

AIMTo evaluate the effect of sitagliptin vs placebo on histologic and non-histologic parameters of non-alcoholic steatohepatitis (NASH).METHOdSTwelve patients with biopsy-proven NASH were randomized to sitagliptin (100 mg daily) (n = 6) or placebo (n = 6) for 24 wk. The primary outcome was improvement in liver fibrosis after 24 wk. Secondary outcomes included evaluation of changes in NAFLd activity score (NAS), individual components of NAS (hepatocyte ballooning, lobular inflammation, and steatosis), glycemic control and insulin resistance [including measurements of glycated hemoglobin (HbA1C) and adipocytokines], lipid profile including free fatty acids, adipose distribution measured using magnetic resonance imaging (MRI), and thrombosis markers (platelet aggregation and plasminogen activator inhibitor 1 levels). We also sought to determine the correlation between changes in hepatic fat fraction (%) [as measured using the Iterative decomposition of water and fat with Echo Asymmetry and Least-squares estimation (IdEAL) MRI technique] and changes in hepatic steatosis on liver *** was not significantly better than placebo at reducing liver fibrosis score as measured on liver biopsy (mean difference between sitagliptin and placebo arms, 0.40, P = 0.82). There were no significant improvements evident with the use of sitagliptin vs placebo for the secondary histologic outcomes of NAS total score as well as for the individual components of NAS. Compared to baseline, those patients who received sitagliptin demonstrated improved HbA1C (6.7% ± 0.4% vs 7.9% ± 1.0%, P = 0.02), and trended towards improved adiponectin levels (4.7 ± 3.5 μg/mL vs 3.9 ± 2.7 μg/mL, P = 0.06) and triglyceride levels (1.26 ± 0.43 mmol/L vs 2.80 ± 1.64 mmol/L, P = 0.08). However, when compared with placebo, sitagliptin did not cause a statistically significant improvement in HbA1C (mean difference, -0.7%, P = 0.19) n

Taking the full advantage of the conformal growth characterizing atomic layer deposition(ALd),the possibility to grow Co thin films,with thickness from several tens down to few nanometers on top of a granular topological insulator(TI)Sb2Tes film,exhibiting a quite high surface roughness(2-5 nm),was *** study the Co growth on the Sb2Tes substrate,we performed simultaneous Co depositions also on sputtered Pt substrates for *** conducted a thorough chemical-structural characterization of the Co/Sb2Tes and Co/Pt heterostructures,confirming for both cases,not only an excellent conformality,but also the structural continuity of the Co layers.X-ray diffraction(XRd)and high-resolution transmission electron microscope(HRTEM)analyses evidenced that Co on Sb2Te3 grows preferentially oriented along the[0oe]direction,following the underlying rhombohedric ***,Co crystallizes in a cubic phase oriented along the[111]direction when deposited on *** work shows that,in case of deposition of crystalline materials,the ALd surface selectivity and conformality can be extended to the definition of local epitaxy,where in-plane ordering of the crystal structure and mosaicity of the developed crystallized grains are dictated by the underlying ***,a highly sharp and chemically-pure Co/Sb2Tes interface was evidenced,which is promising for the application of this growth process for spintronics.

Using an array of coupled microwave resonators arranged in a deformed honeycomb lattice,we experimentally observe the formation of pseudo-Landau levels in the whole crossover from vanishing to large pseudomagnetic field *** result is achieved by utilising an adaptable setup in a geometry that is compatible with the pseudo-Landau levels at all field *** adopted approach enables us to observe the fully formed flat-band pseudo-Landau levels spectrally as sharp peaks in the photonic density of states and image the associated wavefunctions spatially,where we provide clear evidence for a characteristic nodal structure reflecting the previously elusive supersymmetry in the underlying low-energy *** particular,we resolve the full sublattice polarisation of the anomalous 0th pseudo-Landau level,which reveals a deep connection to zigzag edge states in the unstrained case.

Gaining a full understanding of the mechanisms of action of natural products as therapeutic agents includes observing the effects of natural products on cellular morphology,because abnormal cellular morphology is an important aspect of cellular transformations that occur as part of disease *** this study a set of natural products was examined in search of small molecules that influence the cylindrical morphology of fission yeast Schizosaccharomyces *** flow cytometry of large populations of *** exposed to natural products captured cell images and revealed changes in mean length and aspect ratio of *** natural products were found to alter ***’s morphology relative to control,in terms of elongating cells,shrinking them,or making them more *** results may facilitate future investigations into methods by which cells establish and maintain specific shapes.

Feasibility of pancreatectomy following high-dose proton therapy for unresectable pancreatic cancer

AIM To review surgical outcomes for patients undergoing pancreatectomy after proton therapy with concomitant capecitabine for initially unresectable pancreatic *** From April 2010 to September 2013,15 patients with initially unresectable pancreatic cancer were treated withproton therapy with concomitant capecitabine at 1000 mg orally twice daily. All patients received 59.40 Gy(RBE) to the gross disease and 1 patient received 50.40 Gy(RBE) to high-risk nodal targets. There were no treatment interruptions and no chemotherapy dose reductions. Six patients achieved a radiographic response sufficient to justify surgical exploration,of whom 1 was identified as having intraperitoneal dissemination at the time of surgery and the planned pancreatectomy was aborted. Five patients underwent resection. Procedures included:Laparoscopic standard pancreaticoduodenectomy(n = 3),open pyloris-sparing pancreaticoduodenectomy(n = 1),and open distal pancreatectomy with irreversible electroporation(IRE) of a pancreatic head mass(n = 1). RESULTS The median patient age was 60 years(range,51-67). The median duration of surgery was 419 min(range,290-484),with a median estimated blood loss of 850 cm^3(range,300-2000),median ICU stay of 1 d(range,0-2),and median hospital stay of 10 d(range,5-14). Three patients were re-admitted to a hospital within 30 d after discharge for wound infection(n = 1),delayed gastric emptying(n = 1),and ischemic gastritis(n = 1). Two patients underwent R0 resections and demonstrated minimal residual disease in the final pathology specimen. One patient,after negative pancreatic head biopsies,underwent IRE followed by distal pancreatectomy with no tumor seen in the specimen. Two patients underwent R2 resections. Only 1 patient demonstrated ultimate local progression at the primary site. Median survival for the 5 resected patients was 24 mo(range,10-30).CONCLUSION Pancreatic resection for patients with initially unresectable cancers is feasible after

Located between Shanghai and Nanjing the heart of China＇s prosperous Jiangnan on the shores of Lake Tai, Wuxi lies in region. Traveling in the region today, it can be difficult to discern where the ＂city＂ ends and the ＂countryside＂ begins since the entire region seems urbanized. Toby Lincoln＇s valuable research on Wuxi reveals that the process of urbanizing Wuxi county began in the early twentieth century and has continued with only relatively minor disruptions ever since. Urbanization in this study refers to the growth of cities, but it also entails administrative, economic, physical, sociocultural, and political transformations that normally accompany urban growth. Eschewing the more amorphous concept of modernity, Lincoln convincingly argues that using urbanization as a lens allows us to see more clearly the impact of global industrial capitalism, which has affected cities and their surrounding hinterlands. Rather than creating an urban-rural gap, as many scholars have claimed, an urban-rural continuum persisted; but through urbanization, society was reoriented towards the city. In short, economic, administrative, and discursive transformations that took place in larger contexts of global, national, and regional change were all mediated through processes of urban development, as the countryside increasingly was affected by and emulated the city.

The 3d Water Chemistry Atlas is an intuitive,open source,Web-based system that enables the three-dimensional(3d)sub-surface visualization of ground water monitoring data,overlaid on the local geological model(formation and aquifer strata).This paper firstly describes the results of evaluating existing virtual globe technologies,which led to the decision to use the Cesium open source WebGL Virtual Globe and Map Engine as the underlying *** it describes the backend database and search,filtering,browse and analysis tools that were developed to enable users to interactively explore the groundwater monitoring data and interpret it spatially and temporally relative to the local geological formations and aquifers via the Cesium *** result is an integrated 3d visualization system that enables environmental managers and regulators to assess groundwater conditions,identify inconsistencies in the data,manage impacts and risks and make more informed decisions about coal seam gas extraction,waste water extraction,and water reuse.

Using an Electronic Nose to Rapidly Assess Grandlure Content in Boll Weevil Pheromone Lures

Samples of pheromone lures used in boll weevil, Anthonomus grandis （Boheman）, eradication programs are routinely analyzed by Gas Chromatography （GC） to ensure lures are adequately dosed with grandlure, the synthetic aggregation phero- mone produced by male weevils. However, preparation of GC samples is tedious, time consuming, and requires a moderate level of experience. We examined the use of a commercially-available electronic nose （e-nose） for rapidly assessing the grandlure contents of lures. The e-nose was trained to recognize headspace collections of grandlure emitted from new lures and after lures were aged under field conditions for 4 d, 7 d, 10 d, and 14 d. Based on cross-validation of the training set, the e-nose was 82% accurate in discriminating among the different age classes of lures. Upon sampling headspace collections of pheromone from a different set of field-aged lures, the e-nose was 〈50% accurate in discriminating 4 d, 7 d, and 10 d aged lures from the other age classes of lures. However, the e-nose identified new and 14 d aged lure samples with 100% accuracy. In light of these findings, e-nose technology shows considerable promise as an alternative approach for rapidly assessing the initial grandlure contents of lures used in boll weevil eradication programs.

Physicochemical, Proximate and Sensory Properties of Pineapple (<i>Ananas</i>sp.) Syrup developed from Its Organic Side-Stream

A major economical industrial challenge from pineapple (Ananas sp.) processing contributing to environmental pollution is the organic side-streams of pineapple. The physicochemical, proximate and sensory properties of organic sidestream pineapple syrup (OSPS) developed from Smooth cayenne, Sugar loaf and Md2 pineapple varieties were evaluated. Organic side-stream pineapple syrup developed from Md2 recorded the highest moisture content with a corresponding water activity. The colour change in OSPS was significant among the three varieties and Sugar loaf variety deviated from the standard yellow colour more than Smooth cayenne and Md2 varieties. This was buttress by the high Total Soluble Solids in 10% and 20% dilutions of Sugar loaf. The OSPS was acidic. In bread, incorporating 5% OSPS (w:w) of Sugar loaf recorded the highest percentage acceptability among the pineapple varieties. Interestingly, in the production of cakes with 15%, 20% and 30% OSPS, Md2 recorded the highest percentage overall acceptance. For bread and cake, there was varied significance (P 2 and 5% Sugar loaf was more acceptable.
