The Rate of Caesarean Sections in Burkina Faso’s Regional and University Hospitals According to the Classification System of Robson’s Ten Groups

Background: Quality assurance in labor and delivery is needed. The method must be simple and consistent, and be of universal value. The 10-Group Classification System is a simple method providing a common starting point for further detailed analysis within which all perinatal events and outcomes can be measured and compared. Objective: The purpose of this study was to analyze cesarean section (CS) rates using the classification system of Robson’s ten groups and to identify the main contributors to the overall CS rate in Burkina Faso’s regional and university hospitals. Materials and Method: A cross-sectional study with retrospective collection was carried out. All women who gave birth between July 1, 2017 and June 30, 2018, in the Regional Hospital Centres (RHC) and University Hospital Centres (UHC) of Burkina Faso were classified according to the Robson ten-group method. The overall CS rates and in each Robson group were calculated, as well as the contribution of each group to the overall CS rate. Results: The CS rate was 26.5% (8543 out of 32,240 deliveries) during the study period. nulliparous women with single term pregnancy in cephalic presentation during spontaneous labour (group 1), multiparous women with single pregnancy in cephalic presentation, gestational age ≥37SA, spontaneous labour (group 3) and multiparous women with previous CS (group 5) were the main contributors (67.7%) to the overall CS rate. Cancer is one of the deadliest diseases in developing countries. In recent years, natural plant-based compounds have been used in the search for drugs to combat numerous diseases, including cancer. In this study, we evaluate the cytotoxic properties of paanfo tiben 1 and paanfo tiben 2, two traditional herbal formulations from Burkina Faso used in the treatment of cancer in Burkina Faso. To this end, the recipes were infused and freeze-dried. The dry extracts obtained were used to determine total phenolics and flavonoids content, assess antioxidant activity using the DPPH, ABTS and FRAP methods, evaluate anti-inflammatory properties by inhibiting 15-LOX, COX 1 and 2, and assess cytotoxic activity on HeLa cervical cancer and HePG2 liver cancer cell lines using the MTT test. The paanfo tiben 1 recipe showed the highest levels of total phenolics and flavonoids, as well as the best antioxidant activities, with IC50 values of 21.020 ± 0.6 µg/ml and 22.94 ± 0.57 µg/ml for DPPH and ABTS, and 165.15 mM EAA/mg dry extract for FRAP. It also exhibited the best cytotoxic activity with IC50 values of 112.02 ± 0.025 µg/ml on HeLa cells and 80.67 ± 6.08 µg/ml on HepG2 cells. On the other hand, paanfo tiben 2 exhibited the best anti-inflammatory activities through inhibition of 15-LOX and COX 1, with inhibition percentages at 100 µg/ml of 32.523% and 24.717 % respectively. These results could justify the traditional use of these two recipes by traditional health practitioners in the treatment of cancer sufferers in Burkina Faso.

Introduction: Cervical cancer is a public health concern and is mainly caused by Human papillomaviruses (HPV). In many parts of the world, studies are being carried out to understand the different genotypes to better tackle this issue. We conducted a study to determine the prevalence of HPV genotypes in women with chronic hepatitis B or C infection, co-infected or not with HIV, treated at the Hôpital Saint Camille in ouagadougou (Burkina Faso). Methods: This study was conducted from April to July 2023, including 100 women in gastroenterology at Hôpital Saint Camille. A questionnaire on their socio-demographic and life style was administrated;and endocervical samples were collected using sterile swabs and then sent to Centre of Biomolecular Research Pietro Annigoni (CERBA). HPV molecular detection and genotyping were performed by PCR and hybridization using the HPV Direct Flow Chips kit. Data were analysis using chi square test or Fischer’s exact test with a significance threshold for p Results: The prevalence of HPV infection was 28% (28/100) on the sample of women tested. The most frequent genotypes were HPV 52 (8.33%), followed by HPV 18 and 68 (6.25% each) for high-risk HPVs, and HPV 6, 44/55 and 62/81 (8.33% each) for low-risk HPVs. Conclusion: This study, the first of its kind in Burkina Faso on this group of the population, reveals that the most frequent genotypes found in this study are not included in the vaccine available in Burkina Faso (Gardasil®4).

Management of Vulnating Esophageal Foreign Bodies in Burkina Faso

Introduction: Esophageal foreign bodies (EFB) are a diagnostic and therapeutic emergency because of the serious complications they can cause. Aim: This paper aimed to study the vulnating esophageal foreign bodies in the ENT and Head and Neck Surgery departments of the Yalgado ouedraogo and Bogodogo University Hospital. Methodology: This was an analytic cross-sectional study with retrospective data collection over 10 years (2012-2021). Results: We collected 91 cases of vulnating esophageal foreign bodies, i.e. 9.1 cases/year (4.7%). The mean age of the patients was 14 ± 19 years. The sex ratio was 1.6. The circumstances of occurrence were dominated by accidental ingestion of vulnating esophageal foreign bodies (98.9%). The average time to consultation was 7.5 hours. Dysphagia was the dominant symptom (64.8%). Cervico-thoracic radiography found dual contour radiopaque images in 71.4%. Esophagoscopy with rigid tube was performed in 97.8%. The average time for extraction of the vulnating esophageal foreign bodies was 8 hours. Vulnerating esophageal foreign bodies were non-organic in 84.6%. The button cell represented 64.8%. Their location was cervical in 61.5% intraoperatively. The lesion assessment found ulcerative lesions in 42.9% (p Conclusion: Vulnating esophageal foreign bodies are relatively frequent in our ENT practice. Although their diagnosis is often easy, their treatment is still difficult and requires multidisciplinary management. Thus, for us, prevention remains the first effective weapon.

Introduction: Human papillomavirus (HPV) infection is the most widespread sexually transmitted infection in the world. Today, there is growing evidence that HPV can be transmitted early in life, and one potential route is mother-to-child transmission. Data on this route of HPV transmission are scarce in Africa and particularly in Burkina Faso, where no data on the subject are yet available. The aim of our study was to estimate the rate of mother-to-child transmission of HPV infection and to identify circulating genotypes. Methodology: Cervico-uterine samples were collected from 100 full-term pregnant women and, buccal samples were obtained from their newborns at Hopital Saint Camille de ouagadougou (HOSCO) by the specialist physician. HPV DNA amplification and genotyping were performed by PCR followed by hybridization using the HPV Direct Flow Chips kit, detecting 36 genotypes including 18 high-risk and 18 low-risk. Results: The prevalence of HPV in newborns was 8% (8/100). Six (6) HPV-positive neonates had HPV-positive mothers, while 2 HPV-positive neonates had HPV-negative mothers. The vertical transmission rate was 26.09% (6/23). Mother-newborn genotypes were concordant. However, the genotype profile of the newborns was more restricted than that of the mothers. Conclusion: HPV DNA was found in 8% of newborns in our study. The genotype profile of the mother-newborn pair was concordant. Asymptomatic HPV infection in a pregnant woman could constitute a risk factor for vertical transmission.

Lannea microcrapa Engl. & K. Krause (Anacardiaceae) is a fruit and medicinal plant widely used in Burkina Faso. This plant is traditionally used in the treatment of hypertension. The aim of the present study was to evaluate the vasorelaxant effects of the hydroethanolic extract from Lannea microcarpa trunk barks (HE_ELM) on the aorta isolated from NMRI mice. Phytochemical screening by HPTLC, assay of phenolic and flavonoid compounds, assessment of antioxidant activity (DPPH, ABTS, FRAP, and LPO), and myography of HE_ELM (1 - 2000 μg/mL) on mice thoracic aortas in the presence and absence of endothelium were carried out. Endothelium-dependent and endothelium-independant vasodilation were assessed by cumulative addition of Ach (1 nM - 10 μM) on aortic rings precontracted with the thromboxane analogue A2 agonist, 9,11-dideoxy9α,11α-methanoepoxy PGF2α (U46619). L-NAME was used to verify the involvement of NO production in the relaxation mechanism of the extract. Acute oral toxicity of HE_ELM was also evaluated. A phytochemical study revealed the presence of tannins, flavonoids, sterols and triterpenes, saponosides, and high levels of total phenolics and flavonoids. These compounds are thought to be responsible for the extract’s antioxidant and vasorelaxant properties. HE_ELM demonstrated significant antioxidant potential and induced aortic relaxation. Indeed, pharmacological parameters gave EC50 values ranging from 596.45 ± 95.82 μg/mL to 749.48 ± 133.40 μg/mL and Emax values from 85.51% ± 9.59% to 96.81% ± 8.60% for the three conditions of vasodilation of the extract (p > 0.05). A complete antagonism of the contractile effect of U46619 was noted with 1 mg/mL HE_ELM. These results suggest that HE_ELM induces aortic relaxation through a concentration-dependent, endothelium-independent mechanism, possibly involving intracellular calcium mobilization of vascular cells. Introduction: Surgery can cause anxiety and worry, such as waking up in pain, being physically harmed or dying. This study aimed to assess the experience of patients undergoing planned gynaecological surgery in the Obstetrics Gynaecology and Reproductive Medicine department of the University Hospital Centre of Bogodogo. Patients and methods: This was a cross-sectional study with prospective data collection during the period from June 1 to August 31, 2018. The study population consisted of patients who received scheduled gynaecological surgery during the period. Results: Seventy-six (76) patients underwent scheduled surgery and 62 were included. During discussion with the gynaecologist, 58 patients (93.55%) were satisfied. When the surgery was announced, 41 patients (66.13%) were anxious. At the consultation with the anaesthetist 59 patients (95.16%) were satisfied with the discussions. Fifty-five patients (88.7%) were anxious on admission to the operating theatre. The quality of care was satisfactory for 61 patients (98.39%) and all patients were satisfied with the care team. Conclusion: Periodic evaluation of patients’ experiences should therefore be incorporated into care evaluation policies in health facilities in order to contribute to continuous improvement in the quality of medical and surgical care.

Objective: This study focused on the antibacterial and anti-proliferative activity of extracts from Carica papaya and Cocos nucifera roots. Methodology: The minimum inhibitory concentration and the minimum bactericidal concentration of the extracts on Escherichia coli, Pseudomonas aeruginosa, Streptococcus mutans, and Staphylococcus aureus were deduced by the microdilution method. The anti-biofilm activity was determined on all four strains and anti-quorum sensing activity by inhibition of violacein production in Chromobacterium violaceum. Anti-proliferative activity on prostate cultured cancer cells was evaluated by MTT assay. Sterols and triterpenes were also assayed in this study. Results: The methanolic extract of Carica papaya showed the best anti-biofilm effect with a percentage inhibition of 66.10 ± 1.79. The methanolic extract of Cocos nucifera had the strongest inhibition on the production of quorum sensing (61.42 ± 0.28). In addition, the methanolic extract of Cocos nucifera roots showed the best cytotoxic effect on prostate cancer LNCaP cell lines (IC50 = 26.98 ± 2.6 μg/mL) and Carica papaya on the PC-3 lines (IC50 = 127.20 ± 5.99 μg/mL). The extracts were also rich in triterpenes and sterols. Conclusion: This study provides support for the ethnomedical use of Carica papaya and Cocos nucifera roots as an antimicrobial and anticancer.

In Burkina Faso, the seeds of Balanites aegyptiaca have been considered to be potentially high lipid, protein and mineral sources but seem to be neglected and sometimes unknown by communities. This study aims to complete data on seed oil and detail on amino acids and minerals composition of the seed of B. aegyptiaca for better use. Physicochemical parameters such as moisture and ash content, saponification index, iodine, acid value, peroxide value and the melting point were determined. Seed oil triglycerides, fatty acids and amino acids have been estimated throughout this study. The moisture and ash content were 3.70% ± 0.1% and 2.90% ± 0.2%, respectively. The saponification and iodine values were respectively 181.96 ± 0.4 mg KOH/g of oil and 104.86 ± 0.6 g of iodine/100 g of oil. Polyunsaturated fatty acid content (50.94%) was the most important. Triglyceride (LLO, 22.4%) was the major triglyceride. 9 essential amino acids and 9 non-essential amino acids were identified. Phenylalanine (11697.82 ± 0.00 mg/kg) was the most important essential amino acid. The content of 21 minerals was determined and the most important was potassium (9323.13 ± 0.01 mg/kg). Ca/P, Ca/Mg, Ca/Mg and Na/K ratios were 0.34;1.18;0.04 and 0.19 respectively. B. aegyptiaca seed oil is a source of multiple nutritional values and can be used by the population for multipurpose.

Objective: To do a report on the quality of the treatment of malaria in pregnant women at University Teaching Hospital Yalgado ouedraogo (UTH-YO) in ouagadougou. Materials and Methods: This was a descriptive cross-sectional study conducted from 29 July to 02 October 2016. The targets were pregnant women and gynecology and obstetrics service care providers. The study used an individual questionnaire administered and clinical parasitological research by the Rapid Diagnostic Test (RDT). Some information was verified in the patient’s prenatal consultation logbook. A total of 351 women and 44 providers participated in the study. The data was captured on a microcomputer and analyzed using Epi info version 7 software. Results: Regarding preventive measures, patients’ responses were correct for long-lasting impregnated mosquito net (LLIN) in 98.8% of cases and for intermittent preventive treatment (IPT) in 96.5% of cases. About providers, they had given a good answer in all cases for using LLIN and taking IPT. Regarding care providers’ knowledge of the clinical signs of malaria, fever was cited in 95.5% of cases for simple malaria and Frankish jaundice in 97.7% of cases for severe malaria. With regard to curative treatment, 77.3% of the care providers surveyed stated that quinine was the reference molecule for the treatment of simple malaria in pregnant women. In terms of the availability of inputs used for malaria management, our survey found no RDTs. Also, the only molecule available was quinine in its compressed and injectable forms. Conclusion: Care Providers’ and patients’ knowledge of malaria in the obstetrics and genecology department is quite satisfactory. However, some shortcomings were noted in the provision of screening and curative and preventive treatment. The elimination of malaria in our country will require joint efforts at all levels.
