The objective of this study was to evaluate the expression of estrogen receptors （ER（（α ） and ER（β）） and androgen receptors （ARs） as prognostic factors for biochemical recurrence, disease progression and survival in patients with pT3NOMO prostate cancer （PCa） in an urban Greek population. A total of 100 consecutive patients with pT3NOMO PCa treated with radical prostatectomy participated in the study. The mean age and follow-up were 64.2 and 6 years, respectively. The HSCORE was used for semi-quantitative analysis of the immunoreactivity of the receptors. The prognostic value of the ER（（α） and ER（β） and AR was assessed in terms of recurrence, progression, and survival. AR expression was not associated with any of the above parameters; however, both ERs correlated with the prognosis. A univariate Cox regression analysis showed that ER（α） positive staining was significantly associated with a greater hazard for all outcomes. Increased ER（β） staining was significantly associated with a lower hazard for all outcomes in the univariate analysis. When both ER HSCORES were used for the analysis, it was found that patients with high ER（α） or low ER（β） HSCORES compared with patients with negatively stained ER（α） and 〉1.7 hSCORE ER（β） had 6.03, 10.93, and 10.53 times greater hazard for biochemical disease recurrence, progression of disease and death, respectively. Multiple Cox proportional hazard analyses showed that the age, preoperative prostate specific antigen, Gleason score and ERs were independent predictors of all outcomes. ER expression is an important prognosticator after radical prostatectomy in patients with pT3NOMO PCa. By contrast, AR expression has limited prognostic value.

Right trisectionectomy for posterior liver tumors engaging the right and middle hepatic veins may lead to post-hepatectomy liver failure if the anticipated liver remnant is small. In such patients we developed a parenchymasparing one-step approach, that includes extrahepatic right portal vein ligation accompanied by en bloc resection only of segments 7, 8 and 4a and resection of the right and middle hepatic veins. The technique was applied in 3 patients with normal liver function, where according to the preoperative computed tomography the volume of segments 1, 2 and 3 ranged between 17% and 20% of the total liver volume. In all patients liver biochemistry improved rapidly postoperatively and a doubling of volume of segments 1, 2 and 3 was achieved by the third postoperative week, as extrahepatic right portal vein ligation ameliorated reperfusion injury of the remaining segments 5 and 6 and induced hypertrophy of segments 1, 2, 3 and 4b. There was no mortality or long-term *** are alive and free of disease 74, 50 and 17 months after the operation, respectively. We propose that the term ＂extended upper right sectionectomy＂ may be considered for the en bloc resection of segments 7, 8 and 4a, in future revisions ofthe Brisbane 2000 terminology of hepatic anatomy and resections.

Colonic diverticulosis is one of the most common causes of lower gastrointestinal bleeding. Endoscopy is not only a useful diagnostic tool for localizing the bleeding site, but also a therapeutic modality for its management. To date, haemostatic methods have included adrenaline injection, mechanical clipping, thermal and electrical coagulation or combinations of them. The results of all published data are herein reviewed.

The rock mass in nature is in most cases anisotropic,while the existing classifications are mostly developed with the assumption of isotropic conditions that not always meet the engineering *** this study,an anisotropic system based on China National Standard of BQ,named as A-BQ,is developed to address the classification of anisotropic rock mass incorporating the anisotropy degree as well as the quality of rock *** series of basic rating factors are incorporated including inherent anisotropy and structure *** anisotropy degree of rock mass is characterized by the ratio of maximum to minimum quality score and adjusted by the confining *** quality score of rock mass is determined by the key factors of anisotropic structure occurrence and the correction factors of stress state and groundwater *** quality of rock mass is characterized by a quality score and classified in five *** assessment of stability status and probable failure modes are also suggested for tunnel and slope engineering for different quality ***,two cases of tunnel and slope are presented to illustrate the application of the developed classification system into the rock masses under varied stress state.
