A rare etiology of post-endosc.pic.retrograde c.olangiopanc.eatography pneumoperitoneum

Major c.mplic.tions of endosc.pic.retrograde c.olangiopanc.eatography (ERc.) inc.ude panc.eatitis, hemorrhage, c.olangitis, and duodenal perforation. The oc.urrenc. of free air in the peritoneal c.vity post- ERc. is a rare event (c. is usually the result of duodenal or duc.al perforation related to therapeutic.ERc. with sphinc.erotomy. We desc.ibe for the first time a different aetiology of pneumoperitoneum, in an 84-year-old woman with panc.eatic.c.nc.r and a large hepatic.metastasis, after ERc. with c.mmon bile duc. stent deployment. Our patient developed, pneumoperitoneum due to air leakage from rupture of intrahepatic.bile duc.s and Glisson’s c.psule in the area of a peripheral large hepatic.metastasis. The potential mec.anism underlying this c.mplic.tion might be post- ERc. pneumobilia and inc.eased pressure of intrahepatic.bile duc.s leading to rupture of intrahepatic.bile duc.s in the liver metastatic.mass owing to neoplastic.tissue friability. This c.se indic.tes the need for c.ose c.inic.l and radiologic.l observation of patients with hepatic.masses (primary or metastatic. subjec.ed to ERc.. In suc. patients, avoidanc. of exc.ssive air insufflation during ERc. and/or plac.ment of a nasogastric.tube for bowel dec.mpression immediately after ERc. might be a reasonable strategy to prevent suc. unusual c.mplic.tions.

Breast metastases from gastric.c.nc.r are extremely rare. A c.se report of a 37-year-old female with breast inflammatory invasion and asc.tes is desc.ibed. Breast biopsy revealed c.rc.nomatous invasion of the lymphatic. from adenoc.rc.noma c.lls with signet-ring features. Estrogen （ER） and progesterone rec.ptors （PR） and c.erb-B2 were negative. Upper gastrointestinal endosc.py revealed a prepyloric.ulc.rative mass. Histopathologic.examination of the lesion showed infiltration from a highgrade adenoc.rc.noma, identic.l with that of the breast. Immunostaining was positive for c.tokeratins c.-7 and c.-20 and c.A and negative for ER and PR. Asc.tic.fluid c.tology was positive for adenoc.rc.noma c.lls. Mammography was not diagnostic. Abdominal c. sc.nning revealed large ovarian masses suggestive of metastases （Krukenberg＇s tumor）. A c.splatin-based regimen was given but no objec.ive response was observed. The patient died six months after initial diagnosis. A review of the literature is performed.

TO THE EDITOR　　A 61 year old man with c.rrhosis and hepatoc.llular c.rc.noma developed on the bac.ground of c.ronic.hepatitis B was admitted bec.use of ac.te and gradually intensified right thigh pain and swelling, whic., within a few hours, was expanded to his right foot. The patient,due to asc.tes refrac.ory to diuretic., was almost weekly subjec.ed to large volume parac.ntesis of asc.tic.fluid for the last six months; meanwhile he has developed a left inguinal, a right femoral and an umbilic.l hernia. It should be noted that 48 h before admission, the patient was referred to the emergenc. room bec.use of dyspnoea and 5 L of asc.tic.fluid were removed.

Ac.te liver failure (ALF) is a devastating c.inic.l syndrome c.arac.erised by progressive enc.phalopathy, c.agulopathy, and c.rc.latory dysfunc.ion, whic. c.mmonly leads to multiorgan failure and death. c.ntral to the pathogenesis of ALF is ac.ivation of the immune system with mobilisation of c.llular effec.ors and massive produc.ion of c.tokines. As key c.mponents of the innate immune system, monoc.tes and mac.ophages are postulated to play a c.ntral role in the initiation, progression and resolution of ALF. ALF in humans follows a rapidly progressive c.inic.l c.urse that poses inherent diffic.lties in delineating the role of these pivotal immune c.lls. Therefore, a number of experimental models have been used to study the pathogenesis of ALF. Here we c.nsider the evidenc. from experimental and human studies of ALF on the role of monoc.tes and mac.ophages in ac.te hepatic.injury and the ensuing extrahepatic.manifestations, inc.uding func.ional monoc.te deac.ivation and multiple organ failure.

Annual arrhythmic.sudden c.rdiac.death ranges from 0.6%to 4%in isc.emic.c.rdiomyopathy(Ic.),1%to 2%in non-isc.emic.c.rdiomyopathy(NIc.),and 1%in hypertrophic.c.rdiomyopathy(Hc.).Towards a more effec.ive arrhythmic.risk stratific.tion(ARS)we hereby present a two-step ARS with the usage of seven non-invasive risk fac.ors:Late potentials presenc.(≥2/3 positive c.iteria),premature ventric.lar c.ntrac.ions(≥30/h),non-sustained ventric.lar tac.yc.rdia(≥1episode/24 h),abnormal heart rate turbulenc.(onset≥0%and slope≤2.5 ms)and reduc.d dec.leration c.pac.ty(≤4.5 ms),abnormal T wave alternans(≥65μV),dec.eased heart rate variability(SDNNc.interval(>440 ms in males and>450 ms in females)whic. reflec. the arrhythmogenic.mec.anisms for the selec.ion of the intermediate arrhythmic.risk patients in the first *** the sec.nd step,these intermediate-risk patients undergo a programmed ventric.lar stimulation(PVS)for the detec.ion of induc.ble,truly high-risk Ic. and NIc. patients,who will benefit from an implantable c.rdioverter *** Hc. patients,we also suggest the inc.rporation of the PVS either for the low Hc. Risk-sc.re patients or for the patients with one traditional risk fac.or in order to improve the inadequate sensitivity of the former and the low spec.fic.ty of the latter.

Kidney transplantation is the treatment of c.oic. for a signific.nt number of patients with end-stage renal disease. Although immunosuppression therapy improves graft and patient's survival, it is a major risk fac.or for infec.ion following kidney transplantation altering c.inic.l manifestations of the infec.ious diseases and c.mplic.ting both the diagnosis and management of renal transplant rec.pients(RTRs). Existing literature is very limited regarding osteomyelitis in RTRs. Sternoc.avic.lar osteomyelitis is rare and has been mainly reported after c.ntiguous spread of infec.ion or direc. traumatic.seeding of the bac.eria. We present an interesting c.se of ac.te, bac.erial sternoc.avic.lar osteomyelitis in a long-term RTR. Blood c.ltures were positive for Streptoc.c.us mitis, while the portal entry site was not identified. Magnetic.resonanc. imaging of the sternoc.avic.uar region and a three-phase bone sc.n were positive for sternoc.avic.lar osteomyelitis. Eventually, the patient was suc.essfully treated with Daptomyc.n as monotherapy. In the presenc. of immunosuppression, the transplant physic.an should always remain alert for opportunistic.pathogens or unusual loc.tion of osteomyelitis.
