Renin-angiotensin system inhibitors prescriptions in Chinese hospitalized chronic kidney disease patients

BACKGROUND Many guidelines have recommended renin-angiotensin system inhibitors(RASI)as the first-line treatment for patients with chronic kidney disease(CKD).We studied RASI prescription trends from 2010 to 2019,and analyzed the characteristics associated with RASI prescription in Chinese hospitalized CKD patients.AIM To study the prescription of renin angiotensin system inhibitors in hospitalized patients with CKD in China.METHODS It was retrospectively,cross-sectional reviewed RASI prescriptions in hospitalized CKD patients in China from 2010 to 2019.RASI prescribing trends were analyzed from 2010 to 2019,and bivariate and multivariate logistic regression analyses were conducted to identify characteristics associated with RASI prescription.RESULTS A total of 35090 CKD patients were included,with 10043(28.6%)RASI prescriptions.Among these patients,18919(53.9%)met the criteria for RASI treatments based on the 2012 kidney disease:Improving global outcomes guidelines.Of these,7246(38.3%)patients received RASI prescriptions.RASI prescriptions showed an initial rapid increase from 2011 to 2012,reached its peak around 2015 and 2016,and then exhibited a subsequent slight decreasing trend.Both bivariate and multivariate analyses showed that several characteristics,including the male gender,age less than 60-year-old,nephrology department admission,lower CKD stage,history of hypertension or diabetes,proteinuria,glomerulonephritis as the CKD etiology,and non-acute kidney injury were associated with RASI prescriptions.CONCLUSION The frequency of RASI prescriptions showed an initial increase but a slight decreasing trend in more recent years.CKD patients with certain characteristics such as elderly age,advanced disease stage,surgery department admission,or acute kidney injury were less likely to receive RASI prescriptions.In the application of RASI in hospitalized CKD patients is insufficient.The actual clinical practice needs to be improved.The development of related research is helpful to guide the correct choice of clini

Developing functional additive resistant to space atomic oxygen(AO)irradiation through simple molecular design and chemical synthesis to enhance the lubricating performance of multialkylated cyclopentanes(MACs)oil is a significant challenge.Herein,sulfur-containing polyhedral oligomere silsesquioxane(POSS)were synthesize via a click-chemistry reaction of octavinyl polyhedral oligomeric with alkyl sulfide.The reduce-friction(RF),anti-wear(AW)properties and anti-AO irradiation of POSS-S-R as MACs base oil additives in atmospheric and simulated space environments were systematically investigated for the first time.Results demonstrate that POSS-S-R not only possesses outstanding anti-AO irradiation capacity but also effectively improves the RF and AW of MACs in atmospheric or simulated space surroundings.This improvement is due to the excellent anti-AO irradiation properties of the POSS structure itself and the high load-carrying ability of silicon-containing and sulfur-containing compounds generated by tribo-chemical reactions,which effectively separates the direct contact of the friction interface.We believe that this synthesized POSS-S-R is a promising additive for space lubricants.

An observed nocturnal ozone transport event in the Sichuan Basin,Southwestern China

The ozone(O_(3))pollution in China drew lots of attention in recent years,and the Sichuan Basin(SCB)was one of the regions confronting worsening O_(3)pollution problem.Many previous studies have shown that regional transport is an important contributor to O_(3)pollution.However,very few features of the O_(3)profile during transport have been reported,especially in the border regions between different administrative divisions.In this study,we conducted tethered balloon soundings in SCB during the summer of 2020 and captured a nocturnal O_(3)transport event during the campaign.Vertically,the O_(3)transport occurred in the bottom of the residual layer,between 200 and 500 m above ground level.Horizontally,the transport pathway was directed from southeast to northwest based on the analysis of the wind field and air mass trajectories.The effect of transport in the residual layer on the surface O_(3)concentration was related to the spatial distribution of O_(3).For cities with high O_(3)concentrations in the upwind region,the transport process would bring clean air masses and abate pollution.For downwind lightly polluted cities,the transport process would slow down the decreasing or even increase the surface O_(3)concentration during the night.We provided observational facts on the profile features of a transboundary O_(3)transport event between two provincial administrative divisions,which implicated the importance of joint prevention and control measures.However,the sounding parameters were limited and the quantitative analysis was preliminary,more integrated,and thorough studies of this topic were called for in the future.

A bifunctional POM-based Cu-viologen complex with mixed octamolybdate clusters for rapid oxidation desulfurization and effective photogeneration of hydrogen

The design and synthesis of catalysts for the oxidation desulfurization and production of hydrogen are extremely important for solving environmental pollution and energy shortage.Herein,a novel bifunctional [α-Mo_(8)O_(26)]^(4-)/[β-Mo_(8)O_(26)]^(4-)-based Cu-viologen complex H_(4)[Cu_(2)~ⅠCl_(2)(Hbcbpy)_(4)] [α-Mo_(8)O_(26)] [β-Mo8O_(26)]·H_(2)O(BHU-2,Hbcbpy=1-(4-carboxybenzyl)-4,4'-bipyridinium) was synthesized and characterized by single-crystal X-ray diffraction(XRD),infrared radiation spectra,powder X-ray diffraction(PXRD) and X-ray photoelectron spectroscopy(XPS) spectra.The structural characteristic of BHU-2 is the presence of two types of octamolybdate clusters [α-Mo_(8)O_(26)]^(4-)/[β-Mo_(8)O_(26)]^(4-)and a new binuclear Cu~Ⅰ-Hbcbpy complex linked by Cl-bridges[Cu_(2)ⅠCl_(2)(Hbcbpy)_(4)]^(4+).BHU-2 as a heterogeneous catalyst exhibits excellent activities to the oxidation desulfurization and photocatalytic hydrogen production.At room temperature,BHU-2 can catalyze 96% conversion of methyl phenyl sulfide with 98% selectivity,and the process obeys the pseudo-first-order reaction kinetic with the half-life of9.6 min.The notorious 2-chloroethyl ethyl sulfide can achieve 99% conversion with 98% selectivity within only1 min at the presence of BHU-2,and the turnover frequency(TOF) is up to 7400 h^(-1).BHU-2 also exhibits high catalytic activity for the oxidation of other aromatic and aliphatic thioethers within short time at room temperature.Furthermore,BHU-2 shows a high catalytic activity for visible-light-driven hydrogen evolution with an H_(2) evolution rate of 1677.85 μmol·g^(-1)·h^(-1) within 10 h.Moreover,the catalytic activities do not decrease evidently after three cycles,revealing the prominent structural stability and recyclability.

Additive manufacturing ofβ-type titanium alloy is expected to replace Ti-6Al-4V alloy in the field of orthopedic implantation because of their low elastic modulus,excellent corrosion resistance,and biocompatibility.After briefly introducing the laser powder bed fusion(LPBF)process and physical phenomena,this paper reviews the recent progresses in LPBF-edβ-type Ti alloys.The strategies to strengthening and tougheningβ-type Ti alloys are critically reviewed.This is followed by the processing routes employed to achieve to low modulus for orthopedic applications,especially a new methodology for tailoring crystallographic orientation called multi-track coupled directional solidification.The effect of processing and compositions on performance metrics ofβ-type Ti alloys included corrosion behavior,and biocompatibility is reviewed.In the end,challenges in additive manufacturing ofβ-type Ti alloys in future are highlighted,with the aim to ensue clinical application of LPBF-edβ-type Ti alloys.

Cell replacement therapy using neural progenitor cells(NPCs)has been shown to be an effective treatment for ischemic stroke.However,the therapeutic effect is unsatisfactory due to the imbalanced homeostasis of the local microenvironment after ischemia.Microenvironmental acidosis is a common imbalanced homeostasis in the penumbra and could activate acid-sensing ion channels 1a(ASIC1a),a subunit of proton-gated cation channels following ischemic stroke.However,the role of ASIC1a in NPCs post-ischemia remains elusive.Here,our results indicated that ASIC1a was expressed in NPCs with channel functionality,which could be activated by extracellular acidification.Further evidence revealed that ASIC1a activation inhibited NPC migration and neurogenesis through RhoA signaling-mediated reorganization of filopodia formation,which could be primarily reversed by pharmacological or genetic disruption of ASIC1a.In vivo data showed that the knockout of the ASIC1a gene facilitated NPC migration and neurogenesis in the penumbra to improve behavioral recovery after stroke.

A novel predictive braking energy recovery strategy for electric vehicles considering motor thermal protection

Braking energy recovery(BER)aims to recover the vehicle's kinetic energy by coordinating the motor and mechanical braking torque to extend the driving range of the electric vehicle(EV).To achieve this goal,the motor/generator mode requires frequent switching and prolonged operation during driving.In this case,the motor temperature will unavoidably rise,potentially triggering motor thermal protection(MTP).Activating MTP increases the risk of motor component failure,and the EV typically disables the BER function.Thus,maximizing BER while reducing the risk of motor overheating is a challenging problem.To address this issue,this article proposes a predictive BER strategy with MTP using the non-smooth Pontryagin Minimum Principle(NSPMP)for EVs.Firstly,a Markov long short-term memory(MLSTM)model is designed to obtain future velocity information.Secondly,the BER problem with MTP in the studied EV is embedded in a model predictive control(MPC)framework.Then,under the MPC framework,the NSPMP strategy is proposed to resolve the problem of MTP.Finally,the performance of the proposed strategy is verified through simulation and a hardware-in-loop test.The results show that in two real-world driving cycles,compared to the rule-based strategy,the proposed strategy reduced power consumption by 1.24%and0.96%,respectively,and effectively limited motor temperature.Additionally,under global cycle conditions,this strategy demonstrated better MTP control performance compared to other benchmark strategies.

Securing multi-chain consensus against diverse miner behavior attacks in blockchain networks

As cross-chain technologies enable interactions among different blockchains(hereinafter“chains”),multi-chain consensus is becoming increasingly important in blockchain networks.However,more attention has been paid to single-chain consensus schemes.Multi-chain consensus schemes with trusted miner participation have not been considered,thus offering opportunities for malicious users to launch diverse miner behavior(DMB)attacks on different chains.DMB attackers can be friendly in the consensus process on some chains,called mask chains,to enhance their trust value,while on others,called kill chains,they engage in destructive behaviors on the network.In this paper,we propose a multi-chain consensus scheme named Proof-of-DiscTrust(PoDT)to defend against DMB attacks.The idea of distinctive trust(DiscTrust)is introduced to evaluate the trust value of each user across different chains.The trustworthiness of a user is split into local and global trust values.A dynamic behavior prediction scheme is designed to enforce DiscTrust to prevent an intensive DMB attacker from maintaining strong trust by alternately creating true or false blocks on the kill chain.Three trusted miner selection algorithms for multi-chain environments can be implemented to select network miners,chain miners,and chain miner leaders,separately.Simulation results show that PoDT is secure against DMB attacks and more effective than traditional consensus schemes in multi-chain environments.

D-scheduler:A scheduler in time-triggered distributed system through decoupling dependencies between tasks and messages

Time-triggered architecture,as a mainstream design of the distributed real-time system,has been successfully applied in the aerospace,automotive and mechanical industries.However,time-triggered scheduling is a challenging NP-hard problem.There are few studies that could quickly solve the scheduling problem of large distributed time-triggered systems.To solve this problem,a communication affinity parameter is defined in this paper to describe the degree of bias of the shaper task towards sending or receiving messages.Based on this,an innovative task-message decoupling model named D-scheduler is built to reduce the computation complexity of the scheduling problem in large-scale systems.Additionally,we provide mathematical proof that our model is a convex optimization that is easy to solve with existing computational tools.Our experiments substantiate the efficacy of the D-scheduler.It dramatically reduces the scheduling complexity of large-scale real-time systems with a small loss of solving space compared to the federal scheduler.

Despite transosseous rotator cuff tear repair using sutures is widely accepted for tendon-bone fixation,the fibrocartilaginous enthesis regeneration is still hardly achieved with the traditional sutures.In the present work,degradable magnesium(Mg)alloy wire was applied to suture supraspinatus tendon in a rat acute rotator cuff tear model with Vicryl Plus 4±0 absorbable suture as control.The shoulder joint humerus-supraspinatus tendon complex specimens were retrieved at 4,8,and 12 weeks after operation.The Mg alloy suture groups showed better biomechanical properties in terms of ultimate load to failure.Gross observation showed that hyperplastic response of the scar tissue at the tendon-bone interface is progressively alleviated over time in the both Mg alloy suture and Vicryl suture groups.In the histological analysis,for Mg alloy suture groups,chondrocytes appear to proliferate at 4 weeks postoperatively,and the tendon-bone interface showed an orderly structural transition zone at 8 weeks postoperatively.The collagenous fiber tended to be aligned and the tendon-bone interlocking structures apparently formed,where transitional structure from unmineralized fibrocartilage to mineralized fibrocartilage was closer to the native fibrocartilaginous enthesis.In vivo degradation of the magnesium alloy wire was completed within 12 weeks.The results indicated that Mg alloy wire was promising as degradable suture with the potential to promotes fibrocartilaginous interface regeneration in rotator cuff repair.
