BACKGROUND Hepatic arterial infusion chemotherapy and camrelizumab plus apatinib(TRIPLET protocol)is promising for advanced hepatocellular carcinoma(Ad-HCC).However,the usefulness of microwave ablation(MWA)after TRIPLET is still *** To compare the efficacy and safety of TRIPLET alone(T-A)vs TRIPLET-MWA(TM)for *** From January 2018 to March 2022,217 Ad-HCC patients were retrospectively *** them,122 were included in the T-A group,and 95 were included in the T-M group.A propensity score matching(PSM)was applied to balance *** survival(OS)was compared using the Kaplan-Meier curve with the log-rank *** overall objective response rate(ORR)and major complications were also *** After PSM,82 patients were included both the T-A group and the T-M *** ORR(85.4%)in the T-M group was significantly higher than that(65.9%)in the T-A group(P1).The cumulative 1-,2-,and 3-year OS rates were 98.7%,93.4%,and 82.0%in the T-M group and 85.1%,63.1%,and 55.0%in the T-A group(hazard ratio=0.22;95%confidence interval:0.10-0.49;P1).The incidence of major complications was 4.9%(6/122)in the T-A group and 5.3%(5/95)in the T-M group,which were not significantly different(P=1.000).CONCLUSION T-M can provide better survival outcomes and comparable safety for Ad-HCC than T-A.

Mornaphthoate E(MPE)is a prenylated naphthoic acid methyl ester isolated from the roots of a famous Chinese medicinal plant Morinda officinalis and shows remarkable cytotoxicity against several human tumor cell *** the current project,the first total synthesis of(±)-MPE was achieved in seven steps and 5.6%overall *** the in vitro anti-tumor activity of MPE was first assessed for both enantiomers in two breast cancer cells,with the levoisomer exerting slightly better *** in vivo anti-tumor effect was further verified by applying the racemate in an orthotopic autograft mouse ***,MPE exerted promising anti-metastasis activity both in vitro and in vivo and showed no obvious toxicity on mice at the therapeutic *** investigations demonstrated that MPE acted as a tubulin polymerization stabilizer and disturbed the dynamic equilibrium of microtubules via regulating PI3K/Akt *** conclusion,our work has provided a new chemical template for the future design and development of next-generation tubulin-targeting chemotherapies.

Management of traumatic hemothorax by closed thoracic drainage using a central venous catheter

Objective:To evaluate the efficacy and safety of the treatment of traumatic hemothorax by closed pleural drainage using a central venous catheter(CVC),compared with using a conventional chest ***:A prospective controlled study with the Ethics Committee approval was undertaken.A total of 407 patients with traumatic hemothorax were involved and they were randomly assigned to undergo closed pleural drainage with CVCs(n=214) or conventional chest tubes(n=193).The Seldinger technique was used for drainage by CVC,and the conventional technique for drainage by chest *** the residual volume of the hemothorax was less than 200 ml after the daily volume of drainage decreased to below 100 ml for two consecutive days,the treatment was considered *** correlative data of efficacy and safety between the two groups were analyzed using t or chi-squared tests with SPSS 13.0.A P value of less than 0.05 was taken as indicating statistical ***:Compared with the chest tube group,the operation time,fraction of analgesic treatment,time of surgical wound healing,and infection rate of surgical wounds were significantly decreased(Plications(P>0.05),or in the mean catheter/tube indwelling time and mean medical costs of patients treated successfully(P>0.05).Conclusions:Management of medium or large traumatic hemothoraxes by closed thoracic drainage using CVC is minimally invasive and as effective as using a conventional large-bore chest *** complications can be prevented and it has the potential to replace the large-bore chest tube.

Ion exchange is a promising synthetic method for alleviating severe cation mixing in traditional layered oxide materials for lithium-ion batteries,leading to enhanced structural ***,the underlying mechanisms of ion exchange are still not fully *** a fundamental study of the ion-exchange mechanism is needed for achieving the controllable synthesis of layered oxides with a stable ***,we thoroughly unearth the underlying mechanism that triggers the ion exchange of Ni-rich materials in aqueous solutions by examining time-resolved structural evolution combined with theoretical *** results reveal that the reaction pathway of ion exchange can be divided into two steps:protonation and *** proton is the key to achieving charge balance in the ion exchange process,as revealed by X-ray adsorption spectroscopy and inductive coupled plasma *** addition,the intermediate product shows high lattice distortion during ion exchange,but it ends up with a most stable product with high lattice *** apparent discrepancies in lattice energy between materials before and after ion exchange emphasize the importance of synthetic design in structural *** work provides new insights into the ion-exchange synthesis of Ni-rich oxide materials,which advances the development of cathode materials for high-performance lithium-ion batteries.

As the brightest gamma-ray burst ever observed,GRB 221009A provided a precious opportunity to explore spectral line *** this article,we performed a comprehensive spectroscopy analysis of GRB 221009A jointly with GECAM-C and Fermi/GBM data to search for emission and absorption *** the first time we investigated the line feature throughout this GRB including the most bright part where many instruments suffered problems,and identified prominent emission lines in multiple time *** central energy of the Gaussian emission line evolves from about 37 to 6 MeV,with a nearly constant ratio(about 10%)between the line width and central ***,we find that both the central energy and the energy flux of the emission line evolve with time as a power law decay with power law index of–1 and–2,*** suggest that the observed emission lines most likely origin from the blue-shifted electron positron pair annihilation 511 ke V *** find that a standard high latitude emission scenario cannot fully interpret the observation,thus we propose that the emission line comes from some dense clumps with electron positron pairs traveling together with the *** this scenario,we can use the emission line to directly,for the first time,measure the bulk Lorentz factor of the jet(Γ)and reveal its time evolution(i.e.,Γ~t^(-1))during the prompt ***,we find that the flux of the annihilation line in the co-moving frame keeps *** discoveries of the spectral line features shed new and important lights on the physics of GRB and relativistic jet.

Adrenocorticotropic hormone combined with magnesium sulfate therapy for infantile epileptic spasms syndrome:a real-world study

Background Infantile epileptic spasms syndrome(IESS)is a serious disease in infants,and it usually evolves to other epilepsy types or syndromes,especially refractory or super-refractory focal *** adrenocorticotropic hormone(ACTH)is one of the first-line and effective treatment plans for IESS,it has serious side effects and is not sufficiently *** A retrospective study of the clinical outcomes of ACTH combined with magnesium sulfate(MgSO4)therapy for IESS in two hospital centers was *** major outcome of the single and combined treatment was evaluated by changes in seizure frequency and improvements in hypsarrhythmia electroencephalography(EEG).To reduce the confounding bias between the two groups,we used SPSS for the propensity score matching(PSM)*** We initially recruited 1205 IESS patients from two Chinese hospitals and treated them with ACTH combined with MgSO4 and ACTH *** 1005 patients were enrolled in the treatment(ACTH combined with MgSO4:744,ACTH:261),and both treatment plans had a more than 55%response ***,compared to patients treated with ACTH alone,those patients treated with ACTH combined with MgSO4 had better performance in terms of the seizure frequency and hypsarrhythmia *** PSM,the two groups also showed significant differences in responder rate[70.8%(95%confidence interval,CI)=66.7%–74.8%)vs.53.8%(95%CI=47.4%–60.2%),Plinical *** with less than 3 months of lead time responded to the treatment much better than those with>3 months(P<0.05).In addition,the overall incidence of adverse reactions in the ACTH combined with MgSO4 group was much lower than that in the ACTH group(31.4%vs.63.1%,P<0.001).During the treatment,only

The Gravitational wave high-energy Electromagnetic Counterpart All-sky Monitor(GECAM)mission is designed to monitor the Gamma-Ray Bursts(GRBs)associated with gravitational waves and other high-energy transient *** mission consists of two microsatellites which are planned to operate at the opposite sides of the *** GECAM satellite could detect and localize GRBs in about 8 keV-5 MeV with its 25 Gamma-Ray Detectors(GRDs).In this work,we report the in-flight energy calibration of GRDs using the characteristic gamma-ray lines in the background spectra,and show their performance evolution during the commissioning ***,a preliminary cross-calibration of energy response with Fermi GBM data is also presented,validating the energy response of GRDs.

Neuropeptides are crucial in regulation of a rich variety of developmental,physiological,and behavioral functions throughout the life cycle of *** an integrated approach of multiomics,we identified neuropeptide precursors in the greater wax moth Galleria mellonella,which is a harmful pest of honeybee hives with a worldwide ***,a total of 63 and 67 neuropeptide precursors were predicted and annotated in the *** genome and transcriptome,in which 40 neuropeptide precursors were confirmed in the *** ***,we identified 12 neuropeptide precursor genes present in *** but absent in honeybees,which may be potential novel pesticide target *** hives were contaminated with heavy metals such as lead,enabling its bioaccumulation in *** bodies through the food chain,we performed transcriptome sequencing to analyze the effects of Pb stress on the mRNA expression level of *** neuropeptide *** treatment by Pb,the expression of neuropeptide F1 was found to be significantly downregulated,implying that this neuropeptide might be associated with responding to the heavy metal stress in *** study comprehensively identified neuropeptide precursors in ***,and discussed the effects of heavy metals on insect neuropeptides,with the example of *** results are valuable for future elucidation of how neuropeptides regulate physiological functions in *** and contribute to our understanding of the insect's environmental plasticity and identify potential new biomarkers to assess heavy metal toxicity in insects.
