Clinicopathological significance and immunotherapeutic outcome of claudin 18.2 expression in advanced gastric cancer:A retrospective study

Objective: Immunotherapeutic outcomes and clinical characteristics of claudin 18 isoform 2 positive(CLDN18.2-positive) gastric cancer(GC) vary in different clinical studies, making it difficult to optimize antiCLDN18.2 therapy. We conducted a retrospective analysis to explore the association of CLDN18.2 expression with clinicopathological characteristics and immunotherapeutic outcomes in ***: A total of 536 advanced GC patients from 2019 to 2021 in the CT041-CG4006 and CT041-ST-01clinical trials were included in the analysis. CLDN18.2 expression on ≥40% of tumor cells(2+, 40%) and CLDN18.2 expression on ≥70% of tumor cells(2+, 70%) were considered the two levels of positively expressed GC. The clinicopathological characteristics and immunotherapy outcomes of GC patients were analyzed according to CLDN18.2 expression ***: CLDN18.2 was expressed in 57.6%(cut-off: 2+, 40%) and 48.9%(cut-off: 2+, 70%) of *** death-ligand 1(PD-L1) and CLDN18.2 were co-expressed in 19.8% [combined positive score(CPS)≥1, CLDN18.2(cut-off: 2+, 40%)] and 17.2% [CPS≥5, CLDN18.2(cut-off: 2+, 70%)] of ***18.2 expression positively correlated with younger age, female sex, non-gastroesophageal junction(nonGEJ), and diffuse phenotype(P<0.001). HER2 and PD-L1 expression were significantly lower in CLDN18.2-positive GC(both P<0.05). Uterine adnexa metastasis(P<0.001) was more frequent and liver metastasis(P<0.001)was less common in CLDN18.2-positive GC. Overall survival and immunotherapy-related progression-free survival(ir PFS) were inferior in the CLDN18.2-positive ***: CLDN18.2-positive GC is associated with poor prognosis and worse immunotherapeutic outcomes. The combination of anti-CLDN18.2 therapy, anti-PD-L1/PD-1 therapy, and chemotherapy for GC requires further investigation.

The existing indoor fusion positioning methods based on Pedestrian Dead Reckoning(PDR)and geomagnetic technology have the problems of large initial position error,low sensor accuracy,and geomagnetic *** this study,a novel indoor fusion positioning approach based on the improved particle filter algorithm by geomagnetic iterative matching is proposed,where Wi-Fi,PDR,and geomagnetic signals are integrated to improve indoor positioning *** important contribution is that geomagnetic iterative matching is firstly proposed based on the particle filter *** the positioning process,an iterative window and a constraint window are introduced to limit the particle generation range and the geomagnetic matching range *** position is corrected several times based on geomagnetic iterative matching in the location correction stage when the pedestrian movement is detected,which made up for the shortage of only one time of geomagnetic correction in the existing particle filter *** addition,this study also proposes a real-time step detection algorithm based on multi-threshold constraints to judge whether pedestrians are moving,which satisfies the real-time requirement of our fusion positioning *** experimental verification,the average positioning accuracy of the proposed approach reaches 1.59 m,which improves 33.2%compared with the existing particle filter fusion positioning algorithms.

目的探讨瑞派替尼加量治疗晚期胃肠道间质瘤(GIST)的临床效果。方法采用回顾性描述性研究方法。收集2020年1月至2022年10月中山大学附属第一医院等全国12家医学中心收治的14例晚期GIST患者的临床病理资料;男9例,女5例;年龄为63(40~73)岁。患者均行瑞派替尼加量治疗。观察指标:(1)患者临床特征情况。(2)瑞派替尼治疗效果。偏态分布的计量资料以M(范围)表示。计数资料以绝对数表示。采用Kaplan‑Meier法绘制生存曲线并计算生存时间。结果(1)患者临床特征情况。14例患者中,肿瘤原发部位在小肠、胃、其他分别为8、4、2例,肿瘤转移部位在肝脏、腹膜、骨、肺分别为13、9、4、2例(每例患者可发生>1种转移),转移肿瘤数目1~5个、6~10个、>10个分别为2、8、4例,有手术史或转移灶局部消融治疗11例,既往治疗线数≤2线、≥3线分别为3、11例,既往治疗药物采用伊马替尼、舒尼替尼、瑞戈非尼、阿伐替尼分别为14、13、9、2例(每例患者治疗药物可>1种)。14例患者均进行基因检测,KIT外显子11合并外显子13基因突变5例,KIT外显子11合并外显子17基因突变4例,KIT外显子11合并外显子13和17基因突变2例,KIT外显子9合并外显子17基因突变、KIT外显子11突变和野生型各1例;其中继发基因突变12例。(2)瑞派替尼治疗效果。14例患者均获得随访,随访时间为17.3(4.4~30.0)个月。①瑞派替尼标准四线治疗:14例患者均行瑞派替尼标准四线治疗,治疗持续时间为6.0(2.0~17.0)个月,完全缓解、部分缓解、疾病稳定分别为1、2、11例,客观缓解占比为3/14,疾病控制占比为14/14;中位无进展生存时间为6.0个月。14例患者中,11例发生1~2级不良事件,均未发生3~4级不良事件。②瑞派替尼加量治疗:14例患者行瑞派替尼标准四线治疗后均发生疾病进展,均行瑞派替尼加量治疗,其中12例行150 mg 2次/d治疗,2例行200 mg 1次/d治疗。治疗持续时间为5.2(1.5~12.0)个月。12例患者可进行疗效评估,部分缓解、疾病稳定、疾病进展分别为1、10、1例,客观缓解占比为1/12,疾病控制占比为11/12;中位无进展生存时间为11.0个月,中位总生存时间为23.0个月。12例患者中,10例发生1~2级不良事件,1例发生3级不良事件为贫血。③瑞派替尼联合治疗:14例患者行瑞派替尼加量治疗后,7例发生疾病进展,其中3例行瑞派替尼联合其他药物治疗,分别为瑞派替尼100 mg 1次/d联合伊马替尼200 mg 1次/d、瑞派替尼标准四线治疗联合舒尼替尼25 mg 1次/d、瑞派替尼标准四线治疗联合阿帕替尼250 mg 1次/d,无进展生存时间分别为7个月、5个月、尚未评估。结论瑞派替尼加量治疗可为晚期GIST患者带来生存获益,安全性良好。

Owing to the outstanding properties provided by nontrivial band topology,topological phases of matter are considered as a promising platform towards low-dissipation electronics,efficient spin-charge conversion,and topological quantum *** ferroelectricity in topological materials enables the non-volatile control of the quantum states,which could greatly facilitate topological electronic ***,ferroelectricity is generally incompatible with systems featuring metallicity due to the screening effect of free *** this study,we report the observation of memristive switching based on the ferroelectric surface state of a topological semimetal(TaSe_(4))*** find that the surface state of(TaSe_(4))2I presents out-of-plane ferroelectric polarization due to surface *** the combination of ferroelectric surface and charge-density-wave-gapped bulk states,an electric-switchable barrier height can be achieved in(TaSe_(4))2I-metal *** employing a multi-terminal-grounding design,we manage to construct a prototype ferroelectric memristor based on(TaSe_(4))2I with on/off ratio up to 103,endurance over 103 cycles,and good retention *** origin of the ferroelectric surface state is further investigated by first-principles calculations,which reveal an interplay between ferroelectricity and band *** emergence of ferroelectricity in(TaSe_(4))2I not only demonstrates it as a rare but essential case of ferroelectric topological materials,but also opens new routes towards the implementation of topological materials in functional electronic devices.

鄂尔多斯盆地北缘太原组砂岩中广泛发育缝合线构造,缝合线成因机理复杂,且会对砂岩储层的物性产生重要影响。基于岩芯观察,并通过能谱、扫描电镜、X衍射及铸体薄片等综合分析,揭示了太原组砂岩缝合线的几何形态、矿物组成及其成因机理。研究表明,太原组砂岩缝合线振幅大小差异明显,可划分出波浪型、尖峰型、长方柱型、复合型及网状交织型5种类型。缝合线物质组成主要包括有机碳、石英、伊利石、高岭石、黄铁矿、金红石和锆石,黏土矿物中伊利石的含量远高于围岩,而高岭石含量远低于围岩。缝合线内有机质与黏土矿物具有典型的沉积成因标志,它们可以作为硅质迁移与沉淀的催化剂,导致缝合线内及两侧砂岩孔隙中的硅质沉淀和碎屑石英颗粒的增生。这些缝合线构造是埋藏过程中沉积有机质压溶的产物,是沉积黏土矿物与有机质成岩作用的综合响应。缝合线相关的硅质沉淀与石英加大是研究区太原组石英砂岩致密的主要因素,但沿缝合线易发育裂缝,起到改善岩石渗透性的作用。
