目的:分析二次宫颈锥形切除或广泛宫颈切除术后患者的妊娠结局。方法:选取2012年9月至2022年12月于郑州大学第一附属医院接受二次宫颈锥形切除或广泛宫颈切除术后并行宫颈环扎的单胎妊娠孕妇26例,分析其妊娠结局。结果:26例手术过程均顺利,无一例中转开腹。11例妊娠,其中6例足月剖宫产,2例早产,3例流产。非妊娠组与妊娠组之间年龄、出血量、宫颈切除体积及宫颈切除高度差异均无统计学意义(P>0.05)。宫颈切除体积>4 cm^(3)者中,足月产率(1/3)低于≤4 cm^(3)者(5/8)。术后至妊娠的间隔时间≤12个月者中,足月产率(1/4)小于>12个月者(5/7)。结论:减少宫颈切除体积和手术后延迟妊娠可以改善二次宫颈锥形切除或广泛宫颈切除术后的妊娠结局。

Amorphous powder cores based on spherical （Fe0.76Si0.09B0.1P0.05）99Nb1 amorphous powder and their SiO2 layer prepared by in situ coating insulation process were investigated in detAll. These cores were characterized by scanning electron microscopy and X-ray diffraction analyses, and the results revealed that the surface layer of the amorphous powder was composed of SiO2 with uniform surface coverage. The thickness of the SiO2 insulating layer could be controlled by adjusting the tetraethyl orthosilicate （TEOS） content. By cold-pressing with epoxy resin under a pressure of 1800 MPa, a ring powder core with an outer diameter of 20.3 ram, inner diameter of 12.7 mm, and height of 5.3 mm was prepared. The FeSiBPNb composite core showed its best properties when the TEOS content was 2 mL/g （the volume of TEOS for each gram of （Fe0.76Si0.09B0.1P0.05）99Nb1 amorphous powder, mL/g）, which showed good relative permeability in the high-frequency range of up to 10 MHz and a low core loss of 320 W/kg under the maximum magnetic flux density of 0.1 T and frequency of 100 kHz.

AIM： To compare the efficacy and toxicity of a three-step combination therapy with post-operative radiation alone for locally advanced esophageal cancer.METHODS： Patients with T3-4 and N0-1 esophageal carcinoma from a number of institutions were non-randomly, prospectively enrolled in the study. All patients underwent single-stage curative en bloc esophagectomy. The patients were then assigned into one of two treatment groups based on treatment consisting of either post-operative concurrent chemoradiotherapy （CCRT） with weekly cisplatin 30 mg/m^2 followed by systemic adjuvant chemotherapy （four monthly cycles of cisplatin 20 mg/m^2 and 5-fluorouracil 1 000 mg/m^2 for five consecutive days）, or, post-operative radiation alone. The radiotherapy dose was 55-60 Gy for all patients. Primary end-point of this study was to assess the per-protocol patients＇ improvement of overall survival benefit. Secondary end-point was designed to evaluate both the per-protocol and intent-totreat patients＇ outcome of survival. RESULTS： A total of 60 patients （n=30 per group） were enrolled in this study. The two groups were generally comparable for demographic characteristics and hematological and non-hematological toxicities. The CCRT with weekly cisplatin was well tolerated, with significantly better overall survival （30.9 mo vs 20.7 mo; 95% CI, 27.5-36.4 vs 15.2-26.1） and 3-year survival （70.0% vs 33.7%; P=0.003）. Low histological grade of tumor （P〈0.001） was associated with favorable survival in these locally advanced patients. CONCluSION： For locally advanced esophageal cancer, the combination of esophagectomy, post-operative CCRT with weekly cisplatin and systemic adjuvant chemotherapy is well tolerated and effective. A large-scale, prospective randomized trial of this regimen is in progress.

Due to the competition and high cost associated with die casting defects, it is urgent to adopt a rapid and effective method for defect analysis. In this research, a novel expert network approach was proposed to avoid some disadvantages of rule-based expert system. The main objective of the system is to assist die casting engineer in identifying defect, determining the probable causes of defect and proposing remedies to eliminate the defect. 14 common die casting defects could be identified quickly by expert system on the basis of their characteristics. BP neural network in combination with expert system was applied to map the complex relationship between causes and defects, and further explained the cause determination process. Cause determination gives due consideration to practical process conditions. Finally, corrective measures were recommended to eliminate the defect and implemented in the sequence of difficulty.

针对柴油发动机在冷启动状态下存在机油建压慢问题,利用Pumplinx流体动力学分析软件对机油进入压力传感器常温和低温下的时间进行仿真计算,同时测试分析实际发动机常温和低温下传感器充油时间,确定机油粘度和传感器内部截流孔孔径对机油进入主油道和传感器内部有重要影响。随着截流孔孔径增大,机油压力脉冲对传感器感应芯片冲击的作用减弱,影响建压时间;低温状态下,机油粘度大,导致机油建压慢。为解决机油建压慢问题,基于低温下机油粘度大,在无加热措施情况下,建议优化冷启动故障诊断策略。

Higher efficacy of oral etoposide for mobilization of peripheral blood stem cells in patients with multiple myeloma

This study compares the efficacy,toxicity,hematopoietic recovery,and cost of stem-cell mobilization using intermediate-dose cyclophosphamide(IDCy)plus granulocyte colony-stimulating factor(G-CSF)compared with etoposide(VP-16)plus pegylated granulocyte colony-stimulating factor(PEG-rhG-CSF)in multiple myeloma(MM)*** hundred forty-four consecutive patients undergoing mobilization with IDCy(3-3.5g/m^(2))plus G-CSF(n=155)were compared with patients receiving VP-16 plus PEG-rhG-CSF(n=89),including oral etoposide(n=65)and intravenous etoposide(n=24).Compared with IDCy,VP-16 use was associated with significantly higher median peak peripheral blood CD34+cell count(8.20[range:1.84-84]×10^(6)/kg vs 4.58[range:0.1-27.9]×10^(6)/kg,P=.000),and ideal CD34+cell yield of more than 6×10^(6)/kg(56.8%vs 35.1%,P=.001),notably with a higher efficacy in oral VP-16 use compared with IDCy use(CD 34+cell counts:median peak peripheral blood 5.87 vs 4.58?106/kg and≥6×10^(6)/kg[48.4%vs 35.1%]).The median number of apheresis courses was reduced from two in the IDCy group to one in the VP-16 group(P=.000).IDCy use was associated with significantly more frequent episodes of neutropenia(70.2%vs 35.2%;P=.000),intravenous antibiotic use(13.2%vs 11.4%;P=.672),and hospitalization(P=.000).The recoveries of neutrophils and platelets after autologous stem-cell transplantation were significantly faster in the VP-16 group compared with the IDCy group(P=.000).Our data indicate robust stem-cell mobilization in MM patients with VP-16 delivered either orally or *** compared with intravenous VP-16,oral VP-16 mobilization was associated with significantly more convenient,lower average total costs,and especially decreased the risk of hospital visits and exposure.

Dendritic branching patterns at variable cross-sections in Ni-based single crystal(SX) castings of different generations were investigated using optical microscope(OM), electro probe microanalyzer(EPMA),differential scanning calorimeter(DSC), Thermo-Cal software and Pro-CAST software. Results show that the dendritic branching patterns are similar in outward platform in SXs of different generations. That is, the primary dendrites(PDs) are introduced into the platform by developing a series of secondary dendrites(SDs) to occupy the bottom of the platform, and the ternary dendrites(TDs) originating from these SDs grow upward to fill up the platform. With the SX generation increasing, the undercooling of melts in the inward platform increases significantly due to the increasing alloying elements and the segregation in the directional solidification(DS)process, and the growth velocity of the dendrite tip increases according to the dynamic model of dendrite growth,which is beneficial for the high-order dendrite development. The stronger dendritic branching ability is shown in the inward platform of the higher generation Ni-based SX.
