Ectopic peritoneal paragonimiasis mimicking tuberculous peritonitis:A care report

BACKGROUND The most common site of paragonimiasis is in the lungs.The migratory route passes through the duodenal wall,peritoneum,and diaphragm to the lungs;thus,the thoracic cavity and central nervous system,as well as the liver,intestine,and abdominal cavity may be involved.Here,we present a case of intraperitoneal paragonimiasis without other organ involvement,mimicking tuberculous peritonitis.CASE SUMMARY A 57-year-old man presented with recurrent abdominal pain for 4 wk.Physical examination revealed tenderness in the right lower quadrant.Laboratory findings showed complete blood counts within the normal range without eosinophilia.Multiple reactive lymph nodes and diffuse peritoneal infiltration were noted on abdominal computed tomography(CT).There were no abnormalities on chest CT or colonoscopy.Intraoperative findings of diagnostic laparoscopy for the differential diagnosis of tuberculous peritonitis and peritoneal carcinomatosis included multiple small whitish nodules and an abscess in the peritoneum.Pathological reports confirmed the presence of numerous eggs of Paragonimus westermani(P.westermani).A postoperative serum enzyme-linked immunosorbent assay revealed P.westermani positivity.Persistent and repetitive history-taking led him to retrospectively recall the consumption of freshwater crab.After 3 d of treatment with praziquantel(1800 mg;25 mg/kg),he recovered from all symptoms.CONCLUSION In patients who require diagnostic laparoscopy for the differential diagnosis of tuberculous peritonitis and peritoneal carcinomatosis,repetitive history-taking and preoperative serologic antibody tests against Paragonimus may be helpful in diagnosing intraperitoneal paragonimiasis without other organ involvement.

Objective:While a rushed operation can omit essential procedures,prolonged operative time results in higher morbidity.Nevertheless,the optimal operative time range remains uncertain.This study aimed to estimate the ideal operative time range and evaluate its applicability in laparoscopic cancer surgery.Methods:A prospectively collected multicenter database of 397 patients who underwent laparoscopic distal gastrectomy were retrospectively reviewed.The ideal operative time range was statistically calculated by separately analyzing the operative time of uneventful surgeries.Finally,intraoperative and postoperative outcomes were compared among the shorter,ideal,and longer operative time groups.Results:The statistically calculated ideal operative time was 135.4-165.4 min.The longer operative time(LOT)group had a lower rate of uneventful,perfect surgery than the ideal or shorter operative time(IOT/SOT)group(2.8%vs.8.8%and 2.2%vs.13.4%,all Pleeding,postoperative morbidities,and delayed diet and discharge(all Pmin.Regardless of ideal time range,SOT group achieved the highest percentage of uneventful surgery(13.4%),which was possible by surgeon's ability to retrieve a higher number of lymph nodes and perform≥150 gastrectomies annually.Conclusions:Operative time longer than the ideal time range(especially≥240 min)should be avoided.If the essential operative procedure were faithfully conducted without compromising oncological safety,an operative time shorter than the ideal range leaded to a better prognosis.Efforts to minimize operative time should be attempted with sufficient surgical experience.

Fabrication of two-layer flexible copper clad laminate by electroless-Cu plating on surface modified polyimide

A flexible copper clad laminate(FCCL) was fabricated using electroless-and electro-Cu plating processes and the effects of pre-treatment time on the adhesion strength of the FCCL were evaluated based on interfacial morphology.The neutralization and catalyst time were varied in the range of 0-20 min and 0.1-10 min,respectively,and the interfacial condition of the FCCL was characterized by atomic force microscopy(AFM) and X-ray photoelectron spectroscopy(XPS).It is observed that the peel strength increases significantly as the neutralization and catalyst time increase.Peel strength as high as 7.2-7.3 N/cm is obtained as the neutralization and catalyst time increase up to 20 min and 10 min,respectively,which is comparable to the strength achieved by the conventional laminating and sputtering processes.These improvements are probably due to an increase in the surface roughness of polyimide(PI),the activated surface condition,and the adsorption of palladium ions/atoms(Pd) on the PI surface which act as nucleation sites for Cu.

Characterization of flexible copper laminates fabricated by Cu electro-plating process

Flexible copper clad laminates(FCCLs) were fabricated using the electro-plating process and the combined effect of the current density and plating time on their surface morphology,texture,hardness,electrical resistivity and folding behavior was evaluated.To achieve Cu layers with similar thicknesses,the current density was varied in the range of 0.2-3 A/dm2 and the plating time was controlled in the range of 0.5-7.5 h to compensate for the variation of the current density.The surface morphology,hardness,and folding behavior were characterized by atomic force microscopy,nanoindentation technique and Massachusett Institute of Technology folding endurance test,respectively.The X-ray diffraction patterns indicated that the Cu phase was formed without any secondary phases;however,the preferred orientation changed from(220) to(111) as the current density increased over 1 A/dm2.In addition,it was observed that the root-mean-square and hardness values decreased when the current density increased and the plating time decreased simultaneously.The electrical resistivity was as low as approximately 21 nΩ·m and the number of cycles without failure in the folding test was over 15 000,which were comparable to those of commercial FCCLs.

minimally invasive surgery has become common in the surgical resection of gastrointestinal submucosal tumors(SMTs). The purpose of this article is to review recent trends in minimally invasive surgery for gastric SMTs. Although laparoscopic resection has been main stream of minimally invasive surgery for gastrointestinal SMTs, recent advances in endoscopic procedures now provide various treatment modalities for gastric SMTs. Moreover, investigators have developed several hybrid techniques that include the advantages of both laparoscopic and endoscopic procedure. In addition, several types of reduced port surgeries, modification of conventional laparoscopic procedures, have been recently applied to the surgical resection of SMTs. Meanwhile, robotic surgery for SMTs requires further evidence and improvement.

Roles of humidity and heat treatment temperatures on YBa_2Cu_3O_(7-x) coated conductors by trifluoroacetic acid-metal organic deposition process

YBa2Cu3O7-x(YBCO) films were fabricated on an LAO substrate using the trifluoroacetic acid-metal organic deposition(TFA-MOD) method and the effects of the humidity and heat treatment temperatures on the microstructure,degree of texture and critical properties of the films were evaluated.In order to understand the combined effects of the humidity and the calcining and firing temperatures on critical properties,heat-treatment was performed at various temperatures with the other processing variables fixed.The films were calcined at 400-430 ℃ and fired at 750-800 ℃ in a 0-12.1% humidified Ar-O2 atmosphere.The texture was determined by pole-figure analysis.The amount of the BaF2 phase was effectively reduced and a sharp and strong biaxial texture was formed under a humidified atmosphere,which led to increased critical properties.In addition,the microstructure varied significantly with firing temperature but changed little with calcining temperature.The highest IC of 40 A/cm-width,which corresponds to JC value of 1.8 MA/cm2,was obtained for the films fired at 775 ℃(in 12.1% humidity) after calcining at 400-430 ℃.It is likely that the highest IC value is due to the formation of a more pure YBCO phase,c-axis grains,and a denser microstructure.

Studies on morbid obesity have shown remarkable improvement of diabetes in patients who have undergone bariatric operations.It was subsequently shown that these operations induce diabetes remission independent of the resultant weight loss;as a result,surgeons began to investigate whether operations for gastric cancer(GC)could have the same beneficial effect on diabetes as bariatric operations.It was then shown in multiple reports that followed that certain operations for GC were able to improve or even cure type 2 diabetes mellitus(T2 DM)in GC patients.This finding gave rise to the concept of"oncometabolic surgery",in which a patient diagnosed with both GC and T2 DM undergo a single operation with the purpose of treating both diseases.With the increasing incidence of T2 DM,oncometabolic surgery has the potential to improve the quality of life and even extend survival of many GC patients.However,because the GC patient population and the bariatric patient population are wildly different and because different GC operations have different properties,the effect of oncometabolic surgery must be carefully assessed and engineered in order to maximize benefit and avoid harm.This manuscript aims to summarize the findings made so far in the field of oncometabolic surgery and to provide an outlook regarding the possibility of oncometabolic surgery being incorporated into standard clinical practice.

Length of negative resection margin does not affect local recurrence and survival in the patients with gastric cancer

AIM:To investigate the influence of the resection margin on local recurrence and survival in gastric cancer patients.METHODS:We reviewed the medical records of 1788patients who had undergone gastrectomy for gastric cancer at the Seoul National University Bundang Hospital,South Korea,between May 2003 and July 2009.The patients were divided into early and advanced gastric cancer groups.In each group,we analyzed the relationship between clinicopathologic factors and survival outcomes,and compared the hazard rates of event occurrence between patients with resection margins above and below the cut-off value,using a Cox proportional hazard model.RESULTS:The early and advanced gastric cancer groups included 1001 and 787 patients,respectively.The hazard rates of event occurrence did not significantly differ between the patients with resection margins above the cut-off value and those with resection margins below the cut-off value(P>0.05,in all comparisons).Based on the multivariable analyses,the proximal and distal resection margins were not significantly associated with survival outcomes and local recurrence(P>0.05,in all analyses).CONCLUSION:The proximal or distal resection margins did not affect the prognosis of patients with gastric cancer if the margins were pathologically negative.

Use of artificial intelligence in the field of pain medicine

In this editorial we comment on the article“Potential and limitations of ChatGPT and generative artificial intelligence in medial safety education”published in the recent issue of the World Journal of Clinical Cases.This article described the usefulness of artificial intelligence(AI)in medial safety education.Herein,we focus specifically on the use of AI in the field of pain medicine.AI technology has emerged as a powerful tool,and is expected to play an important role in the healthcare sector and significantly contribute to pain medicine as further developments are made.AI may have several applications in pain medicine.First,AI can assist in selecting testing methods to identify causes of pain and improve diagnostic accuracy.Entry of a patient’s symptoms into the algorithm can prompt it to suggest necessary tests and possible diagnoses.Based on the latest medical information and recent research results,AI can support doctors in making accurate diagnoses and setting up an effective treatment plan.Second,AI assists in interpreting medical images.For neural and musculoskeletal disorders,imaging tests are of vital importance.AI can analyze a variety of imaging data,including that from radiography,computed tomography,and magnetic resonance imaging,to identify specific patterns,allowing quick and accurate image interpretation.Third,AI can predict the outcomes of pain treatments,contributing to setting up the optimal treatment plan.By predicting individual patient responses to treatment,AI algorithms can assist doctors in establishing a treatment plan tailored to each patient,further enhancing treatment effectiveness.For efficient utilization of AI in the pain medicine field,it is crucial to enhance the accuracy of AI decision-making by using more medical data,while issues related to the protection of patient personal information and responsibility for AI decisions will have to be addressed.In the future,AI technology is expected to be innovatively applied in the field of pain medicine.The advancement of AI is anticipa

Long-term outcome of stem cell transplantation with and without anti-tumor necrotic factor therapy in perianal fistula with Crohn’s disease

BACKGROUND Stem cell transplantation is a promising therapeutic option for curing perianal fistula in Crohn’s disease(CD).Anti-tumor necrotic factor(TNF)therapy combined with drainage procedure is effective as well.However,previous studies are limited to proving whether the combination treatment of biologics and stem cell transplantation improves the effect of fistula closure.AIM This study aimed to evaluate the long-term outcomes of stem cell transplantation and compare Crohn’s perianal fistula(CPF)closure rates after stem cell transplantation with and without anti-TNF therapy,and to identify the factors affecting CPF closure and recurrence.METHODS The patients with CD who underwent stem cell transplantation for treating perianal fistula in our institution between Jun 2014 and December 2022 were enrolled.Clinical data were compared according to anti-TNF therapy and CPF closure.RESULTS A total of 65 patients were included.The median age of females was 26 years(range:21-31)and that of males was 29(44.6%).The mean follow-up duration was 65.88±32.65 months,and complete closure was observed in 50(76.9%)patients.The closure rates were similar after stem cell transplantation with and without anti-TNF therapy(66.7%vs 81.6%at 3 year,P=0.098).The patients with fistula closure had short fistulous tract and infrequent proctitis and anorectal stricture(P=0.027,0.002,and 0.008,respectively).Clinical factors such as complexity,number of fistulas,presence of concurrent abscess,and medication were not significant for closure.The cumulative 1-,2-,and 3-year closure rates were 66.2%,73.8%,and 75.4%,respectively.CONCLUSION Anti-TNF therapy does not increase CPF closure rates in patients with stem cell transplantation.However,both refractory and non-refractory CPF have similar closure rates after additional anti-TNF therapy.Fistulous tract length,proctitis,and anal stricture are risk factors for non-closure in patients with CPF after stem cell transplantation.
