Recurrence after endoscopic piecemeal mucosal resection for large sessile colorectal polyps

AIM: To evaluate the safety and outcomes of endoscopic piecemeal mucosal resection (EPMR) for large sessile colorectal polyps. METHODS: The patients enrolled in this study were 47 patients with 50 large sessile polyps (diameter, 2 cm or greater) who underwent EPMR using a submucosal saline injection technique between December 2002 and October 2005. All medical records, including characteristics of the patients and polyps, complications, and recurrences, were retrospectively reviewed. The first follow-up endoscopic examination was performed at 3-6 mo after initial endoscopic resection, and the second at 12 mo postEPMR. Subsequent surveillance colonoscopic examinations were individualized, taking risk factors into account. RESULTS: The patients were 23 men and 24 women,with a mean age of 60 years. Mean polyp size was 30.1 mm. Of 50 polyps identified, 34 (68%) were benign and 16 (32%) were malignant. There were 6 (12%) cases with EPMR-related bleeding: 5 intra-procedural and 1 early post-procedural bleeding. All bleeding episodes were managed by endoscopic clipping or argon beam coagulation. There were no perforations. Recurrence was identified in 5 cases (12.2%): 4 local recurrences detected at 3 mo post-EPMR and 1 local recurrence detected at 14 mo post-EPMR. The recurrence rate after EPMR was 3.1% for benign polyps and 33.3% for malignant polyps (P < 0.05). Median follow-up time was 37 mo. CONCLUSION: EPMR is safe, but should be applied carefully in malignant polyps. Close follow-up endoscopic examinations are necessary for early detection of recurrence.

Predictors for difficult cecal insertion in colonoscopy: The impact of obesity indices

AIM To identify the factors influencing cecal insertion time(CIT) and to evaluate the effect of obesity indices on CIT. METHODS We retrospectively reviewed the data for participants who received both colonoscopy and abdominal computed tomography(CT) from February 2008 to May 2008 as part of a comprehensive health screening program. Age, gender, obesity indices [body mass index(BMI), waist-to-hip circumference ratio(WHR), waist circumference(WC), visceral adipose tissue(VAT)volume and subcutaneous adipose tissue(SAT) volume on abdominal CT], history of prior abdominal surgery, constipation, experience of the colonoscopist, quality of bowel preparation, diverticulosis and time required to reach the cecum were analyzed. CIT was categorized as longer than 10 min(prolonged CIT) and shorter than or equal to 10 min, and then the factors that required a CIT longer than 10 min were *** A total of 1678 participants were enrolled. The mean age was 50.42 ± 9.931 years and 60.3% were men. The mean BMI, WHR, WC, VAT volume and SAT volume were 23.92 ± 2.964 kg/m2, 0.90 ± 0.076, 86.95 ± 8.030 cm, 905.29 ± 475.220 cm3 and 1707.72 ± 576.550 cm3, respectively. The number of patients who underwent abdominal surgery was 268(16.0%). Colonoscopy was performed by an attending physician alone in 61.9% of cases and with the involvement of a fellow in 38.1% of cases. The median CIT was 7 min(range 2-56 min, IQR 5-10 min), and mean CIT was 8.58 ± 5.291 min. Being female, BMI, VAT volume and involvement of fellow were significantly associated with a prolonged CIT in univariable analysis. In multivariable analysis, being female(OR = 1.29, P = 0.047), lower BMI(< 23 kg/m2)(OR = 1.62, P = 0.004) or higher BMI(≥ 25 kg/m2)(OR = 1.80, P < 0.001), low VAT volume(< 500 cm3)(OR = 1.50, P = 0.013) and fellow involvement(OR = 1.73, P < 0.001) were significant predictors of prolonged CIT. AIM:There is some evidence of functional superiority of colonic J-pouch over straight coloanal anastomosis (CM) in ultralow anterior resection (ULAR) or intersphincteric resection. On the assumption that colonic J-pouch anal anastomosis is superior to straight CM in ULAR with upper sphincter excision (USE: excision of the upper part of the internal sphincter) for low-lying rectal cancer, we compare functional outcome of colonic J-pouch vsthe straight CM. METHODS: Fifty patients of one hundred and thirty-three rectal cancer patients in whom lower margin of the tumors were located between 3 and 5 cm from the anal verge received ULAR including USE from September 1998 to January 2002. Patients were randomized for reconstruction using either a straight (n = 26) or a colonic J-pouch anastomosis (n = 24) with a temporary diverting-loop ileostomy. All patients were followed-up prospectively by a standardized questionnaire [Fecal Inco-ntinence Severity Index (FISI) scores and Fecal Incontinence Quality of Life (FIQL) scales]. RESULTS: We found that, compared to straight anastomosis patients, the frequency of defecation was significantly lower in J-pouch anastomosis patients for 10 mo after ileostomy takedown. The FISI scores and FIQL scales were significantly better in J-pouch patients than in straight patients at both 3 and 12 mo after ileostomy takedown. Furthermore, we found that FISI scores highly correlated with FIQL scales. CONCLUSION: This study indicates that colonic J-pouch anal anastomosis decreases the severity of fecal incontinence and improves the quality of life for 10 mo after ileostomy takedown in patients undergoing ULAR with USE for low-lying rectal cancer.

Experimental and first-principles calculation of TiN growth mechanism on CeAlO_(3) surface in steel

Regular TiN is harmful to the toughness of steel,and its shape and size need to be *** the behavior of TiN precipitation on CeAlO_(3) surfaces is critical for controlling the morphology and formation process of CeAlO_(3)–TiN composite inclusions in the *** results showed that TiN had a square morphology on the CeAlO3 surface,and electron backscatter diffraction phase identification results revealed the orientation relationship between CeAlO_(3) and TiN as follows:(001)_(CeAlO_(3))//(110)_(TiN),(100)_(CeAlO_(3))/(001)_(TiN),and[010]_(CeAlO_(3))/[110]_(TiN).The CeAlO3 crystal structure was studied using the first-principles calculation method,and the adsorption and growth process of TiN on the CeAlO_(3) surface were investigated from the atomic *** calculation results indicate that there is no metallic bonding present in the CeAlO_(3) *** the low-index crystal planes of CeAlO_(3),the(110)planes terminated with O and CeAlO have the highest and lowest surface energies,respectively,with values of 0.373 and 0.051 eV/Å^(2).On the high surface energy plane of CeAlO_(3),the TiN atomic permutation structure is consistent with the arrangement of Ti and N atoms in TiN(100)or(110).For the low surface energy plane of CeAlO_(3),the Ti and N atoms are arranged in a ring-like structure.

Study of the reaction mechanism for preparing powdered activated coke with SO_(2)adsorption capability via one-step rapid activation method under flue gas atmosphere

In this study,the impact of different reaction times on the preparation of powdered activated carbon(PAC)using a one-step rapid activation method under flue gas atmosphere is investigated,and the underlying reaction mechanism is *** indicate that the reaction process of this method can be divided into three stages:stage I is the rapid release of volatiles and the rapid consumption of O_(2),primarily occurring within a reaction time range of 0-0.5 s;stage II is mainly the continuous release and diffusion of volatiles,which is the carbonization and activation coupling reaction stage,and the carbonization process is the main in this *** stage mainly occurs at the reaction time range of 0.5 -2.0 s when SL-coal is used as material,and that is 0.5-3.0 s when JJ-coal is used as material;stage III is mainly the activation stage,during which activated components diffuse to both the surface and interior of *** stage mainly involves the reaction stage of CO_(2)and H2O(g)activation,and it mainly occurs at the reaction time range of 2.0-4.0 s when SL-coal is used as material,and that is 3.0-4.0 s when JJ-coal is used as ***,the main function of the first two stages is to provide more diffusion channels and contact surfaces/activation sites for the diffusion and activation of the activated components in the third *** the reaction mechanism would serve as a crucial reference and foundation for designing the structure,size of the reactor,and optimal positioning of the activator nozzle in PAC preparation.

Integration of metal-organic layers with quantum dots for artificial photosynthesis

Metal-organic layers（MOLs）, a type of new-emerging two-dimensional ultrathin metal-organic framework materials with large surface areas and highly exposed active sites, have shown promising applications in photocatalytic CO2reduction. However, due to a lack of photosensitivity and photooxidation capability, photosensitizers and sacrificial reductants are usually necessary for MOLs-based photocatalytic CO2reduction systems. In this article, by integration of MOLs and quantum dots（QDs）, we constructed MOLs-based catalysts with multi-functions of photosensitivity, photoreduction and photooxidation, which thus can serve as photocatalysts for CO2reduction with H2O as an electron donor. Specifically, by an electrostatic self-assembly approach,nickel（Ⅱ）-based MOLs（Ni-MOLs） and CsPbBr3QDs have been assembled, constructing valid Ⅱ-Scheme Ni-MOLs/CsPbBr3heterojunctions with close Ni-MOLs/CsPbBr3heterointerface. Such a close heterointerface shortens the charge transfer distance,thus effectively boosting the charge separation and transfer. As a result, upon illumination by visible light(λ ≥ 400 nm,100 m W cm-2), the optimized photocatalyst shows high efficiency and stability in photochemical CO2reduction in the absence of any photosensitizers and sacrificial reductants. The CO yield reaches as high as 124 μmol g-1in 4 h, over 6 times higher than that achieved by CsPbBr3. Additionally, the selectivity reaches 100%. This work provides a new way to construct MOL-based catalysts for artificial photosynthesis.
