Symptoms of long-term exposure to moxa smoke in acupuncturists： a correlation analysis

OBJECTIVE: To investigate the effects of long-term exposure to moxa smoke on acupuncturists and to consider the association between physical symptoms and gender, age, secondhand smoke(SHS) exposure, and the duration of ***: A self-report, web-based questionnaire was used to evaluate the effects of moxa smoke onacupuncturists. Logistic regression was used to analyze the relationship between physical symptoms and possible correlative ***: A total of 858 questionnaires were analyzed. The data showed that 42.42% of acupuncturists had been exposed to moxa smoke for 5-10 years. The most frequent symptom was tearing(32.98%) and the least frequent symptom was asthma(5.24%). Logistic regression analysis showed that female acupuncturists were slightly more susceptible to cough and tearing than males(cough:OR: 1.583, 95% CI: 1.079, 2.321, P = 0.019; tearing:OR: 1.519, 95% CI: 1.094, 2.108, P = 0.013). Acupuncturists aged over 25 years reported a slightly lower incidence of eye itchiness than those aged under25 years(26-40 years: OR: 0.604, 95% CI: 0.379,0.965, P = 0.035; over 40 years: OR: 0.330, 95% CI:0.114, 0.958, P = 0.042). Ophthalmodynia occurred less in acupuncturists aged 26-40 years(OR: 0.591,95% CI: 0.359, 0.970, P = 0.038) than in those younger than 25 years. The only association between prevalence of symptoms and tobacco use and SHS exposure was that smokers had a lower occurrence of phlegm in the throat(OR: 0.579, 95% CI: 0.392,0.856, P = 0.006). Shortness of breath was less frequent in participants exposed to moxa smoke for more than 5 years(5-10 years: OR: 0.400, 95% CI:0.204, 0.785, P = 0.008; 11-20 years: OR: 0.392, 95%CI: 0.178, 0.864, P = 0.02), but a slightly higher incidence of eye itchiness was found in those with over20 years of moxa exposure(OR: 4.200, 95% CI:1.344, 3.128, P = 0.014).CONCLUSION: The most frequent symptom of moxa smoke exposure in acupuncturists was tear-ing. Reduction of the ages at diagnosis and operation of biliary atresia in Taiwan: A 15-year population-based cohort study

AIM: To describe the ages at diagnosis and operation of biliary atresia(BA) and its incidence over a 15-year period in ***: This was a population-based cohort study. BA cases were identified from the Taiwan National Health Insurance Research Database based on the International Classification of Diseases,Ninth Revision(ICD-9) code of BA 751.61 plus Kasai operation(ICD-9 procedure code 51.37) or liver transplantation(LT,ICD-9 procedure code 50.5). The patients' characteristics including sex,age at diagnosis,age at receiving Kasai operation and age at receiving LT were compared among three birth cohorts:(1) 1997 to 2001;(2) 2002 to 2006; and(3) 2007 to ***: There were a total of 540 BA cases(275 females) with an incidence of 1.62 per 10000 live births. No seasonality of BA was noted. The mean ages at diagnosis of three cohorts were 57.9,55.6 and 52.6 d.A linear regression model demonstrated a decreasing trend of the mean age at diagnosis(1.27 d per year). The proportion of BA cases that received the Kasai operation within 60 d of age increased from 76% to 81%. A total of 189(35%) BA patients underwent LT. The mean age at LT was reduced from 3-year-old to 1-year-old. The rates of LT were 25.6% and 32.3% in patients who received the Kasai operation within 60 d or after 60 d of age,respectively. All patients who did not undergo a Kasai operation eventually required ***: The ages at diagnosis and operation in BA cases have decreased over time. Kasai operation performed at younger age reduces the need for LT. The incidence of BA in Taiwan fluctuates,but without certain trend.

Is the expression of γ-glutamyl transpeptidase messenger RNA an indicator of biological behavior in recurrent hepatocellular carcinoma?

AIM: To investigate the correlation between gammaglutamyl transpeptidase (γ-GTP) expression in the primary HCC and post-resection recurrence and its biological ***: Forty consecutive patients having curative resection for HCC were included in this study. The primers for reverse-transcription polymerase chain reaction (RTPCR) were corresponding to the 5'-noncoding human γ-GTP mRNA of fetal liver (type A), HepG2 cells (type B),and placenta (type C). Both the cancer and non-cancerous tissues of the resected liver were analyzed. The correlations between the expression of γ-GTP and the clinicopathological variables and outcomes (recurrence and survival) were ***: Those with type B γ-GTP mRNA in cancer had significant higher recurrence rate than those without it (63.6 % vs 14.3 %). Both those with type B in cancer and in non-cancer died significantly more than those without it (45.5 % vs 0 % and 53.6 % vs 0 %, respectively). By multivariate analysis, the significant predictors of recurrence included high serum AFP (P=0.0108), vascular permeation (P=0.0084), and type B γ-GTP mRNA in non-cancerous liver (P=0.0107). The significant predictors of postrecurrence death included high serum AFP (P=0.0141),vascular permeation (P=0.0130), and daughter nodules (P=0.0053). As to the manifestations (recurrent number ≥2, recurrent extent≥2 segments, extra-hepatic metastasis, and death) in recurrent patients, there were no statistical significant differences between those with type B in the primary tumor and those without it. The difference between those with type B in non-cancerous liver and those without it also was not ***: Patients of HCC with type B γ-GTP mRNA both in cancer and in non-cancerous tissue had a worse outcome, earlier recurrence, and more post-recurrence death.

Is p53 gene mutation an indicatior of the biological behaviors of recurrence of hepatocellular carcinoma?

AIM: To evaluate mutant p53 gene in primary hepatocellular carcinoma and to investigate the correlation between it and the recurrence of hepatocellular ***: Mutations of p53 gene were examined using antihuman p53 monoclonal antibody and immunohistochemical staining in 79 resected hepatocellular carcinomas. The correlations among variables of p53 positivity and invasiveness, disease free interval and survival were *** addition, in those who developed recurrence, the correlation among p53 positivity, clinical features and postrecurrence survival were also ***: Of these 79 cases, 64 (81%) had p53 *** patients with mutant p53 positivityhad significantly more tumor recurrence (76.6 % vs 40.0 %, P=0.0107).However, the COX proportional hazards model showed that p53 overexpression had only weak correlations with recurrence free interval and survival time (P=0.088 and 0.081), which was probably related to the short duration of follow-up. The invasiveness variables may be predictors of HCC recurrence. On univariate analysis, more patients with mutant p53 positivity had vascular permeation [78.1vs 40.0 %, P=0.0088, O.R. (odds ratio) =5.3], grade Ⅱ-ⅣV differentiation (98.4 vs 80.0 %, P=0.0203, O.R. =15.7), no complete capsule (82.8 vs 53.3 %, P=0.0346, O.R. =4.2)and daughter nodules (60.9 vs. 33.3 %, P=0.0527, O.R.=3.1) than patients with negative p53 staining. Onmultivariate analysis, only vascular permeation and grade of differentiation remained significant (P=0.042 and 0.012).There was no statistically significant correlation betweenthe status of p53 in the primary lesion and the clinical features of recurrent hepatocellular carcinomas examined,including extrahepatic metastasis (P=0.1103) and the number of recurrent tumors (P= 1.000) except for diseaseover more than one segment in the extent of recurrent tumors (P=0.0043). AIM: To investigate the prognostic value of the expression of connexin (Cx) 26, 32 and 43 messenger RNA (mRNA) in hepatocellular carcinoma (HCC) tissues. METHODS: Using a reverse-transcriptase polymerase chain reaction (RT-PCR), Cx 26, Cx 32 and Cx 43 mRNAs were determined in the liver tissues of 15 controls and in HCC tissues of 25 patients undergoing curative hepatic resection. The patients were followed up ***: Cx 26 and Cx 32 mRNAs were significantly lower in HCC tissues compared with controls (both P<0.01). By multivariate analysis, a lower level of Cx 26 and Cx 32 mRNA correlated significantly with a risk of HCC recurrence (P = 0.033) and recurrence-related mortality (P = 0.031,P= 0.031). Cx 43 mRNA was higher in HCC tissues compared with controls but did not correlate with postoperative recurrence or recurrence-related mortality. Other significant predictors of HCC recurrence included cellular dedifferentiation (P = 0.033), less encapsulation (P = 0.050), vascular permeation (P= 0.046), and daughter nodules (P= 0.046). Significant variables related to recurrence-related mortality consisted of cell dedifferentiation (P = 0.031), vascular permeation (P = 0.048), and daughter nodules (P = 0.048).The levels of Cx 26 and Cx 32 mRNAs correlated significantly with cell differentiation (P= 0.031). CONCLUSION: A low expression of Cx 26 and Cx 32 mRNAs in HCC tissues is predictive of postoperative recurrence of HCCs.

Adhesive small bowel obstruction: How long can patients tolerate conservative treatment?

AIM: To evaluate how long patients with small bowel obstruction caused by postoperative adhesions can tolerate conservative ***: The records of patients with small bowel obstruction due to postoperative adhesions were retrospectively reviewed. Data collected included the number of admissions, type of management for each admission,duration of conservative treatment, number of repeat laparotomies, and operative ***: One hundred fifty-five patients with this condition from January 1999 to December 2001, for a total of 293 admissions were enrolled in this study. Medical treatment alone was given in 220 admissions, and repeat laparotomy was performed in 73 admissions. The period of observation in patients managed medically ranged from 2 to 12 days (average: 6.9 days), while for those who underwent surgery,the range was 1 to 14 days (average 5.4 days). At surgery,adhesions were the only finding in 46 cases, while there were intestinal complications in 27, or 9.2 % of all 293admissions. Fever and leukocytosis greater than 15 000/mm3were prediction of intestinal ***: With closely monitoring, most patients with small bowel obstruction due to postoperative adhesions could tolerate supportive treatment and recover well averagely within 1 week, although some patients require more than 10 days of observation.
