Objective:Parents may use various information sources to obtain information about sport-related concussions(SRC).This study examined SRCrelated information sources used by parents of United States middle school children(age:10-15 years).Methods:A panel of 1083 randomly selected ***,aged≥18 years and identifying as parents of middle school children,completed an online questionnaire capturing parental and child characteristics,and utilization and perceived trustworthiness of various sources of SRC-related *** logistic regression models identified factors associated with utilizing each *** odds ratios(OR)with 95%confidence intervals(95%CIs)excluding 1.00 were deemed ***:Doctors/healthcare providers(49.9%)and other healthcare-related resources(e.g.,Centers for Disease Control and Prevention,WebMD)(37.8%)were common SRC-related information sources;64.0%of parents utilized≥1 of these *** sources were considered“very”or“extremely”trustworthy for SRC-related information among parents using these sources(doctors/healthcare providers:89.8%;other healthcare-related resources:70.9%).A 10-year increase in parental age was associated with higher odds of utilizing doctors/healthcare providers(adjusted odd ratio(OR_(adjusted))=1.09,95%CI:1.02-1.16)and other healthcare-related resources(OR_(adjusted)=1.11,95%CI:1.03-1.19).The odds of utilizing doctors/healthcare providers(OR_(adjusted)=0.58,95%CI:0.40-0.84)and other healthcare-related resources(OR_(adjusted)=0.64,95%CI:0.44-0.93)were lower among parents whose middle school children had concussion histories versus the parents of children who did not have concussion ***:One-third of parents did not report using doctors/healthcare providers or other healthcare-related resources for SRC-related *** associated with underutilization of these sources may be targets for future *** education for healthcare providers and e

The Influence of Emotions and Behavioral Theories behind Hand Hygiene in COVID-19 Pandemic

Background: Hand hygiene (HH) is essential in preventing healthcare-associated infections, especially during the COVID-19 pandemic, where SARS-CoV2 has shown the ability to survive on surfaces for days. In this study, we explore HH compliance rate and the factors associated with its increase during COVID-19 pandemic. Methods: HH compliance was assessed amongst doctors in Anesthesia and Surgical Intensive Care Unit by direct observation between 2018 and 2021. An anonymous survey was then sent to the doctors to understand factors influencing the observed improvement in HH compliance during the pandemic. Results: Compliance towards the five moments of HH has shown a statistically significant rising trend from a median HH compliance of 60% in 2018, to 70% in 2019 to 94% in 2020. However, HH compliance subsequently declined in the first quarter of 2021 to median of 87%. The follow-up survey had a response rate of 96% (n = 53). 90% of survey participants responded that their HH frequency had increased during the COVID-19 pandemic with 47% stating they were performing HH 11 - 20 times/day. 64% responded that this increased frequency had affected their skin condition. 62% responded that this increased frequency was sustainable even after the pandemic ends. Participants ranked considerations influencing HH compliance. “Prevent transmitting the infection to vulnerable patients” was ranked highest followed by “High infection rate”, “Fear of contracting COVID-19” and lastly, “Public health guidelines”. Conclusions: Despite the absence of new campaigns, HH compliance reached an all-time high. The pandemic has provided opportunity for behavioral change through “reflective” drivers of behavior such as emotions (“Fear”) and knowledge (“High infection rate”). Large Ameloblastoma of Mandible: Our Experience with Intermaxillary Fixation

Ameloblastomas are benign dental tumors, constituting 15% of oral neoplasms, most frequent odontogenic tumor of the jaw. Patient presenting at a late stage usually presents with large swelling with facial deformity. We present a case of 50 years old male who presented with recurrent ameloblastoma on the left side of the mandible causing facial disfigurement and functional impairment. Adequate removal of the tumor required hemimandibulectomy and intermaxillary fixation was done without any bony reconstruction. We would like to focus on the excellent postoperative result with regard to facial symmetry and functional improvement obtained in this case, though the available management protocols differ and are more advanced in current scenario. This case report gives hope and guidance to the surgeons who face difficulty in managing the deforming benign bony tumors due to the unavailability of facility and funds to reconstruct.

Transcatheter Closure of Multiple Defects of the Atrial Septum: Technique and Follow-Up

Introduction: Although multiple defects of the atrial septum are not uncommon, there remain limited data regarding the use of multiple devices in these patients. A variety of approaches to transcatheter closure have been used, and in this paper we describe the experience from two operators in a single centre. Methods: From September 2002 to September 2012, 673 transcatheter atrial septal defects (ASD) and patent foramen ovale (PFO) closure procedures were performed and retrospectively examined in a registry analysis. Results: Of these, 22 patients had multiple discrete defects, and four different approaches to closure were used. In 4 patients (18.2%) one device was used percutaneously to close multiple defects. Eleven patients (50%) had two devices inserted during the same procedure while two patients (9%) had two devices inserted as staged procedures. One patient (4.5%) had three devices inserted over two procedures. Four patients (18.2%) were found not to be suitable for percutaneous closure during the procedure and were referred for surgical closure. Conclusion: Our experience with the implant procedures, and clinical follow up of patients, shows that patients with multiple defects can be effectively treated with transcatheter device techniques including single device closure, multiple devices in one procedure and multiple devices in staged procedures and also with surgical repair.

Earthworms: Charles Darwin’s ‘Unheralded Soldiers of Mankind’: Protective &Productive for Man &Environment

Earthworms promises to provide cheaper solutions to several social, economic and environmental problems plaguing the human society. Earthworms can safely manage all municipal and industrial organic wastes including sewage sludge and divert them from ending up in the landfills. Their body work as a ‘biofilter’ and they can ‘purify’ and also ‘disinfect’ and ‘detoxify’ municipal and several industrial wastewater. They reduce the BOD & COD loads and the TDSS of wastewater significantly. They can even remove the EDCs (endocrine disrupting chemicals) from sewage which is not removed by the conventional sewage treatments plants. Earthworms can bio-accumulate and bio-transform many chemical contaminants including heavy metals and organic pollutants in soil and clean-up the contaminated lands for re-development. Earthworms restore & improve soil fertility by their secretions (growth hormones) and excreta (vermicast with beneficial soil microbes) & boost ‘crop productivity’. They have potential to replace the environmentally destructive chemical fertilizers from farm production. The ‘protein rich’ earthworm biomass is being used for production of ‘nutritive feed materials’ for fishery, dairy & poultry industries. They are also being used as ‘raw materials’ for rubber, lubricant and detergent industries. The bioactive compounds isolated from earthworms are finding new uses in production of ‘life saving medicines’ for cardiovascular diseases and cancer cure.

Baroreflex dysfunction in chronic kidney disease

Chronic kidney disease （CKD） patients have high cardiovascular mortality and morbidity. The presence of traditional and CKD related risk factors results in exaggerated vascular calcification in these patients. Vascular calcification is associated with reduced large arterial compliance and thus impaired barorefex sensi-tivity （BRS） resulting in augmented blood pressure （BP） variability and hampered BP regulation. Barorefex plays a vital role in short term regulation of BP. This review discusses the normal barorefex physiology, methods to assess baroreflex function, its determinants along with the prognostic significance of assessing BRS in CKD patients, available literature on BRS in CKD patients and the probable patho-physiology of barorefex dysfunction in CKD.

Immunohistochemical Analysis of Citrulline-Nitric Oxide Cycle Enzymes and Glutamine Synthetase in Different Regions of Brain in Epilepsy Rat Model

The aim of this study was to determine the immunoreactivity of neuronal and inducible nitric oxide synthetase, argininosuccinate synthetase, argininosuccinate lyase, glutamine synthetase in different regions of brain in rats of kainic acid mediated epilepsy. Male Sprague-Dawley rats were used in this study. The acute group animals were sacrificed after 2 hours and the chronic group animals were sacrificed after 5 days of a single subcutaneous injection of kainic acid (15 mg/kg body weight). The cerebral cortex, cerebellum and brain stem slices were fixed and immunohistostained for the above enzymes. Images were captured and analyzed. In acute group, argininosuccinate synthetase and inducible nitric oxide synthetase were increased in cerebral cortex and cerebellum, neuronal nitric oxide synthetase increased in cerebral cortex and brain stem, and there was no change in argininosuccinate lyase immunoreactivity compared to control group. In chronic group, glutamine synthetase was decreased and all other enzymes immunoreactivity was increased in all the brain regions tested. This study demonstrated the up-regulation of citrul-line-nitric oxide cycle enzymes and may contribute to enhancing recycling of citrulline to arginine to support the increased production of nitric oxide in epilepsy. The decreased glutamine synthetase may increase glutamate in chronic epilepsy and may lead to neurodegeneration.
