BACKGROUND‘Splenosis’is defined as the autotransplantation of splenic tissue following trauma or surgery,usually in the form of intraperitoneal *** proliferation of imaging techniques has resulted in increased unexpected discoveries of splenosis nodules,and achieving a differential diagnosis can be *** medicine studies have been playing an increasingly important role in this process,but the clinical significance of asymptomatic nodules remains *** SUMMARY We present a case of pelvic splenosis in a 73-year-old man diagnosed 56 years after a splenectomy during a computed tomography(CT)follow-up for B-cell lymphoma,presenting intense contrast enhancement of an 18 mm nodule in the right recto-vesical space.18F-fluorodeoxyglucose demonstrated weak metabolic *** histological diagnosis was deemed necessary,the nodule was easily removed with robotically assisted laparoscopy,together with another 6 mm left a paracolic *** latter was previously undiagnosed but retrospectively visible on the CT *** In a patient requiring differential diagnosis of splenosis nodules from lymphomarecurrence, the robotic approach provided a safe en bloc removal with shorthospitalization. The Da Vinci Xi robot was particularly helpful because its opticscan be introduced from all ports, facilitating visualization and lysis of multipleintra-abdominal adhesions.

Antihypertensive Effect of Sodium-Glucose Cotransporter 2 Inhibitors and Glucagon-like Peptide 1 Receptor Agonists

An increasing body of evidence shows that new antidiabetic drugs—particularly sodium-glucose cotransporter 2(SGLT2)inhibitors and glucagon-like peptide 1(GLP-1)receptor agonists—have a beneficial effect on cardiovascular *** majority of these studies have been performed in patients with heart failure and the results have shown first positive effect on blood pressure(BP)*** effects are more pronounced with SGLT2 inhibitors than with GLP-1 receptor ***,the reasons and mechanisms of action inducing BP reduction are still not sufficiently *** mechanisms of SGLT2 inhibitors involve the natriuretic effect,modification of the renin-angiotensin-aldosterone system,and/or the reduction in the sympathetic nervous ***-1 receptor agonists have several mechanisms that are related to glycemic,weight,and BP *** data show that SGLT2 inhibitors have a stronger antihypertensive effect than GLP-1 receptor agonists,which is mainly related to their renal ***,SGLT2 inhibitors increase the response to diuretics and decrease the meal-related antinatriuretic pressure by lowering post-prandial hyperglycemia and hyperinsulinemia and prevent proximal sodium ***2 inhibitors can be used as second-line therapy in patients with diabetes mellitus or heart disease and concomitant *** article aims to summarize current knowledge regarding the antihypertensive effect of SGLT2 inhibitors and GLP-1 receptor agonists.

Motorized spiral enteroscopy(MSE)is the latest advance in device-assisted *** events related to MSE were discussed in a recent large systematic review and meta-analysis and were directly compared with those of balloon enteroscopy in a case-matched study and a randomized controlled *** the real-life application of MSE,an unexpected safety issue emerged regarding esophageal injury and the technique has been withdrawn from the global market,despite encouraging results in terms of diagnostic and therapeutic *** conducted an Italian multicenter real-life prospective study,which was prematurely terminated after the withdrawal of MSE from the *** primary goals were the evaluation of MSE performance(both diagnostic and therapeutic)and its safety in routine endoscopic practice,particularly in the early phase of introduction in the endoscopic unit.A subanalysis,which involved patients who underwent MSE after unsuccessful balloon enteroscopy,demonstrated,for the first time,the promising performance of MSE as a rescue *** its remarkable performance in clinical practice and its potential role as a backup technique following a previously failed enteroscopy,it may be more appropriate to refine and enhance MSE in the future rather than completely abandoning it.

DICE:An open-source MATLAB application for quantification and parametrization of digital outcrop model-based fracture datasets

An open-source MATLAB application(app)named Discontinuity Intensity Calculator and Estimator(DICE)was developed in order to quantitatively characterize the fractures,or in more general,discontinuities within a rocky outcrop in three-dimensional(3D)digital data,such as digital outcrop model(DOM).The workflow proposed for the parametrization of the discontinuities consists of the following steps:(1)Analysis and mapping of the fractures detected within the 3D DOMs;(2)Calculation of the orientation,position and dimensions of discontinuities that are represented by best-fit circular planes;(3)Determining the discontinuity parameters(dimension,distribution,spacing and intensity)by the DICE algorithm using different 3D oriented sampling techniques(3D oriented scanline,3D oriented circular scan window and spherical scan volume).Different sampling methods were bench tested with a synthetic,as well as a natural case study,and compared in order to understand the advantages and limitations of each *** 3D oriented circular scan window appears to be the most effective method for fracture intensity estimation with high accuracy(error 0.4%)and stability with variations in scan radius.
