Risk factors for hepatic steatosis in adults with cystic fibrosis: Similarities to non-alcoholic fatty liver disease

AIM To investigate the clinical, biochemical and imaging characteristics of adult cystic fibrosis(CF) patients with hepatic steatosis as compared to normal CF controls.METHODS We performed a retrospective review of adult CF patients in an academic outpatient setting during 2016. Baseline characteristics, genetic mutation analysis as well as laboratory values were collected. Abdominal imaging(ultrasound, computed tomography, magnetic resonance) was used to determine presence of hepatic steatosis. We compare patients with hepatic steatosis to normal controls.RESULTS Data was collected on 114 patients meeting inclusion criteria. Seventeen patients(14.9%) were found to have hepatic steatosis on imaging. Being overweight(BMI > 25)(P = 0.019) and having a higher pp FEV1(75 vs 53, P = 0.037) were significantly associated with hepatic steatosis. Patients with hepatic steatosis had a significantly higher median alanine aminotransferase level(27 vs 19, P = 0.048). None of the hepatic steatosis patients had frank CF liver disease, cirrhosis or portal hypertension. We found no significant association with pancreatic insufficiency or CF related diabetes.CONCLUSION Hepatic steatosis appears to be a clinically and phenotypically distinct entity from CF liver disease. The lack of association with malnourishment and the significant association with higher BMI and higher pp FEV1 demonstrate similarities with non-alcoholic fatty liver disease. Long term prospective studies are needed to ascertain whether CF hepatic steatosis progresses to fibrosis and cirrhosis.

采用简单的溶胶-凝胶法在TiO_(2)中复合加入铜离子或铁离子和硫酸离子,并研究其甲酸光催化转化效果。通过X射线衍射(XRD)、N_(2)物理吸附(BET比表面积测量)、扫描和透射电子显微镜(SEM和TEM)、UV-Vis漫反射光谱(DRS)和X射线光电子能谱(XPS)对不同光催化剂的结构和形貌进行表征,以确定材料的理化性能与其对甲酸氧化的光催化效率之间的关系。结果显示,添加不同金属离子将导致催化剂之间的重要差异。硫酸化TiO_(2)/Cu(1%Cu)具有最佳的光催化效果,仅在5 min内就可获得100%的甲酸转化率。通过复合添加铜离子和硫酸可使催化剂获得良好的物理化学特性,使其在光催化反应中表现出优越的性能。

Peri-implant femoral fractures:Challenges,outcomes,and proposal of a treatment algorithm

Purpose:Non-prosthetic peri-implant fractures are challenging injuries.Multiple factors must be care-fully evaluated for an adequate therapeutic strategy,such as the state of bone healing,the type of implant,the time and performed personnel of previous surgery,and the stability of fixation.The aim of this study is to propose a rationale for the treatment.Methods:The peri-implant femoral fractures(PIFFs)system,a therapeutic algorithm was developed for the management of all patients presenting a subtype A PIFF,based on the type of the original implant(extra-vs.intra-medulllary),implant length and fracture location.The adequacy and reliability of the proposed algorithm and the fracture healing process were assessed at the last clinical follow-up using the Parker mobility score and radiological assessment,respectively.In addition,all complications were noticed.Continuous variables were expressed as mean and standard deviation,or median and range according to their distribution.Categorical variables were expressed as frequency and percentages.Results:This is a retrospective case series of 33 PIFFs,and the mean post-operative Parker mobility score was(5.60±2.54)points.Five patients(15.1%)achieved complete mobility without aids(9 points)and 1(3.0%)patient was not able to walk.Two other patients(6.1%)were non-ambulatory prior to PPIF.The mean follow-up was(21.51±9.12)months(range 6-48 months).There were 7(21.2%)complications equally distributed between patients managed either with nailing or plating.There were no cases of nonunion or mechanical failure of the original implant.Conclusion:The proposed treatment algorithm shows adequate,reliable and straightforward to assist the orthopaedic trauma surgeon on the difficult decision-making process regarding the management of PIFF occurring in previously healed fractures.In addition,it may become a useful tool to optimize the use of the classification,thus potentially improving the outcomes and minimizing complications.

Stroke gets in your eyes: stroke-induced retinal ischemia and the potential of stem cell therapy

Stroke persists as a global health and economic crisis,yet only two interventions to reduce stroke-induced brain injury exist.In the clinic,many patients who experience an ischemic stroke often further suffer from retinal ischemia,which can inhibit their ability to make a functional recovery and may diminish their overall quality of life.Despite this,no treatments for retinal ischemia have been developed.In both cases,ischemia-induced mitochondrial dysfunction initiates a cell loss cascade and inhibits endogenous brain repair.Stem cells have the ability to transfer healthy and functional mitochondria not only ischemic neurons,but also to similarly endangered retinal cells,replacing their defective mitochondria and thereby reducing cell death.In this review,we encapsulate and assess the relationship between cerebral and retinal ischemia,recent preclinical advancements made using in vitro and in vivo retinal ischemia models,the role of mitochondrial dysfunction in retinal ischemia pathology,and the therapeutic potential of stem cell-mediated mitochondrial transfer.Furthermore,we discuss the pitfalls in classic rodent functional assessments and the potential advantages of laser Doppler as a metric of stroke progression.The studies evaluated in this review highlight stem cell-derived mitochondrial transfer as a novel therapeutic approach to both retinal ischemia and stroke.Furthermore,we posit the immense correlation between cerebral and retinal ischemia as an underserved area of study,warranting exploration with the aim of these treating injuries together.

无容量设施选址(UFL)问题是经典的优化问题,属于NP难题,易于描述却难于求解.首先,介绍了UFL问题的数学模型,并对UFL问题的特点进行深入分析,得到其最优解所具有的基本特征;其次,针对UFL问题的最优解所具有的基本特征,设计了两种局部搜索策略,并将其与基本蚁群算法相结合,提出了一种用于求解UFL问题的混合蚁群搜索算法;最后,为了测试该算法的性能,分别利用混合蚁群算法和基本蚁群算法求解UFL问题基准问题库中的16个测试算例.计算结果表明,混合蚁群算法有效改进了基本蚁群算法求解UFL问题时易陷入局部最优、收敛速度慢等不足,该算法对求解UFL问题具有明显的可行性和有效性.

Epidemics on networks: Reducing disease transmission using health emergency declarations and peer communication

Understanding individual decisions in a world where communications and information move instantly via cell phones and the internet,contributes to the development and implementation of policies aimed at stopping or ameliorating the spread of diseases.In this manuscript,the role of official social network perturbations generated by public health officials to slow down or stop a disease outbreak are studied over distinct classes of static social networks.The dynamics are stochastic in nature with individuals(nodes)being assigned fixed levels of education or wealth.Nodes may change their epidemiological status from susceptible,to infected and to recovered.Most importantly,it is assumed that when the prevalence reaches a pre-determined threshold level,P*,information,called awareness in our framework,starts to spread,a process triggered by public health authorities.Information is assumed to spread over the same static network and whether or not one becomes a temporary informer,is a function of his/her level of education or wealth and epidemiological status.Stochastic simulations show that threshold selection Pand the value of the average basic reproduction number impact the final epidemic size differentially.For the ErdÖos-Rényi and Small-world networks,an optimal choice for Pthat minimize the final epidemic size can be identified under some conditions while for Scalefree networks this is not case.

Multitemporal landslide inventory analysis of an intertropical mountain in west-central Mexico——Basis for hazard management

Landslides in intertropical mountainous areas of Mexico are a natural hazard that could potentially generate a disaster. Despite this, many areas lack landslide inventories, making it difficult to monitor and efficiently manage the associated risks for local populations. For this reason, the aim of this research was to undertake a multitemporal landslide inventory and analyze its conditioning factors in the Pico de Tancítaro stratovolcano, which, in recent decades, has presented several high-risk geomorphological events with disastrous consequences for nearby localities. The spatial distribution of landslides and its conditioning factors were mapped at a 1:50,000 scale for the 1995-2020 period, through visual interpretation of orthophotos and satellite images(Landsat, SPOT, and SENTINEL). The conditioning factors that link the most to landslides were Quaternary andesite and basaltic andesite rocks;volcanic cones, mountain river valleys, and very steep to steep slopes(30° to >45°), oriented to the South, North, and East;Andosols and Leptosols;and the oyamel fir and mixed forests. 505 landslide events classified into slides and flows that occurred in 1995, 2004, 2010, and 2015 were identified. New scars generated by the reactivation of landslide processes were also observed. The landslides in the study area were triggered by torrential rains caused by hurricanes and tropical storms;such as hurricanes Alex and Patricia. The inventory and analysis of conditioning factors highlighted the distribution and dynamics of slides and flows in the study area. In addition, the most important factors for studies of landslide susceptibility, hazards, and risks in the Pico de Tancítaro stratovolcano were identified, contributing to the management of geomorphological risks in intertropical mountain areas.

Growing evidence suggests pollution and other environmental factors have a role in the development of tuberculosis(TB),however,such studies have never been conducted in Peru.Considering the association between air pollution and specific geographic areas,our objective was to determine the spatial distribution and clustering of TB incident cases in Lima and their co-occurrence with clusters of fine particulate matter(PM2.5)and poverty.We found co-occurrences of clusters of elevated concentrations of air pollutants such as PM2.5,high poverty indexes,and high TB incidence in Lima.These findings suggest an interplay of socio-economic and environmental in driving TB incidence.

Foodborne outbreaks in Brazil associated with fruits and vegetables:2008 through 2014

Objectives:Foodborne disease outbreaks linked to fruits and vegetables have been increasing in occurrence worldwide;therefore,the aim of this study was to identify the reported foodborne outbreaks associated with fruit and vegetable consumption in Brazil from 2008 to 2014.Results And Limitations:Thirty produce related outbreaks resulted in 2926 illnesses,347 hospitalizations,and no deaths.Only bacterial pathogens were identified as etiological agents.Among these,Salmonella was the most frequent(30 per cent of outbreaks)followed by Staphylococcus aureus(23.3 per cent),Escherichia coli(10 per cent),Bacillus cereus(6.6 per cent),and thermotolerant coliforms(3.3 per cent),whereas etiological agents could not be determined for 26.6 per cent of outbreaks.The most common food vehicles implicated in outbreaks were generically named as fruits and vegetables(46.6 per cent of outbreaks).The term salad was used generically and specifically like salads(two outbreaks),raw/cooked salads(four outbreaks),vegetable salad,tropical salad,Caesar salad,and raw salad of cabbage and tomato.Only one outbreak was related exclusively to fruit(fruit pulp),whereas other outbreaks were related to cooked carrot,lettuce,cucumber,watermelon/cabbage,and chard/beet.Contamination sources and issues related to the future control of produce-related foodborne disease outbreaks are discussed.

将投资限制引入经典约束p-中位问题,提出带投资的约束p-中位问题,该问题更适用于交通、物流等领域的设施选址。在深入分析带投资约束p-中位问题的数学模型的基础上,首先提出了适用于该问题求解的局部搜索策略;其次,将局部搜索策略与拉格朗日启发式算法和蚁群算法相结合,设计了求解该问题的拉格朗日混合蚁群算法。实验结果表明,带投资的约束p-中位问题能够根据投资金额规划不同的投资方案;且提出的混合蚁群算法较大程度上提高了蚁群算法和拉格朗日启发式算法的求解精度,具有较好的收敛性。
