BACKGROUND: Patients with decompensated hepatitis B vires (HBV)-related cirrhosis tend to have low or undetectable HBV replication. However, some patients continue to have high levels of HBV replication and effective suppression of HBV replication with antiviral agents may potentially decrease hepatic necroinflammation and improve or stabilize liver function. This review was to under stand the efficacy and safety of lamivudine in the treatment of decompensated HBV cirrhosis. DATA SOURCES: An English-language literature search (MEDLINE January 1988-July 2005) was performed, and a total of 52 articles/abstracts relevant to the issue were selected. After review of the selected papers, the meaningful results and conclusions were extracted using scientific crite ria. The papers reviewed pertained mainly to the efficacy and safety profiles of lamivudine treatment for decompensated HBV cirrhosis. RESULTS: The ultimate treatment of decompensated HBV cirrhosis is liver transplantation, but lamivudine treatment may lead to rapid suppression of viral replication and improvement of biochemical and clinical parameters, reduced morbidity and hospitalization for complications of liver disease, increased pre-transplant survival as well as reduced need for transplantation. However, viral resistance can develop after prolonged treatment with lamivudine, and breakthrough hepatitis may be fatal in few patients. Adefovir is effective for lamivudine-resistant HBV mutants. CONCLUSIONS: Antiviral therapy with lamivudine for decompensated HBV cirrhosis can be effective. However, some patients may experience a hepatitis flare with the emergence of YMDD mutants resulting in progressive worsening of liver disease, and should be referred for 'rescue' therapy with other nucleoside/nucleotide analogues such as adefovir dipivoxil.

BACKGROUND: Interferon-alfa has been used in the treat-ment of chronic hepatitis B for more than 20 years and hasits own advantages including a definite course of therapy,no production of drug-resistant variants, and sustained effi-cacy. This review was to understand the role of interferon-alfa therapy in chronic hepatitis *** RESOURCES: An English-language literature searchusing Medscape and MEDLINE was performed and a totalof 48 articles on the treatment of chronic hepatitis with in-terferon-alfa or pegylated interferon-alfa were ***: Interferon-alfa therapy was associated with ahigher HBV DNA inhibition rate and HBeAg loss rate com-pared with controls, and it may have long-term beneficialeffects in terms of HBV clearance, reduction of hepatocel-lular carcinoma, and prolongation of survival. Pegylatedinterferon-alfa was more effective than conventional inter-feron-alfa in the treatment of chronic hepatitis B as well aschronic hepatitis C, and was also associated with greater ef-ficacy than conventional interferon in difficult-to-treat ***: Interferon-alfa is still regarded as one ofthe first-line drugs for the treatment of chronic hepatitis *** interferon is a more promising therapy than con-ventional interferon-alfa, especially in patients with refrac-tory chronic hepatitis B.

Ms-T curves and hysteresis loops were investigated for amorphous Fe78Si9B13, (FeNi)78(CrSiB)22, their lap-wound-cores, and their composite ribbons made by two-chamber-crucible technique. The properties of the lap-wound cores of the two kinds of ribbons are similar. For the composite ribbons, the intrinsic properties are the average of the two alloys. Their technological properties, i.e., hysteresis loops, however, are no longer the average of the two alloys. Instead, they show some dramatic changes compared to the lap-wound-cores. Especially, the shape of the hysteresis loop of the composite ribbon cores is largely different from that of lap-wound-cores. The reason for the difference is supposed to be internal stress induced from cooling after annealing.

Constrained Delaunay Triangular Irregular Networks (CD-TIN),a kind of special data structure,have many practical applications in Geoinformatics,especially in the representation of linear constrained triangulation for DTM and DSM,such as in digital city and digital mine. Past researches on D-TIN mainly focused on point in-sertion and deletion without consideration of constraint,and that on CD-TIN usu-ally paid more attention to the insertion algorithms for points and edges,but little to the deletion algorithms. The presented algorithms are far insufficient for the dy-namic updating of CD-TIN. In this paper,the constraint edge in CD-TIN is consid-ered to be any set of broken lines,i.e.,polygon edges,broken lines and simple segments. The constraint edge may be composed of one or more constraint seg-ments,and it is allowed to be in any form: Open or close,intersection or self-in-tersection. By improving to present insertion and deletion algorithms for D-TIN,two new algorithms for CD-TIN updating are presented. According to the polymorphism of the constraints in CD-TIN,virtual point is adopted to represent the crossing node between constraint edges when a constraint edge is inserted in CD-TIN. Two new algorithms named as Integral Ear Elimination (IEE) and Influence Domain Retrian-gulation for Virtual Point (IDRVP) are presented,the former is for constraint point deletion,while the later is for the insertion and deletion of constraint edge. The principle of IDRVP is that to divide the influence domain of a virtual point into some parts by the constraint-keeping edges,and to retriangulate each part of the influ-ence domain individually referring to the constraint visible property and constraint empty circle (CEC) criterion. Finally,a prototype system is developed with VC++,one case on the integration of 3D terrain and buildings is demonstrated to test the correctness of new algorithms. It shows that the new algorithms are effective for the updating of CD-TIN.

X-ray phase-contrast imaging (PCI) is a new emerging imaging technique that generates a high spatial resolution and high contrast of biological soft tissues compared to conventional radiography. Herein a biomedical application of diffraction enhanced imaging (DEI) is presented. As one of the PCI methods, DEI derives contrast from many different kinds of sample information, such as the sample’s X-ray absorption, refraction gradient and ultra-small-angle X-ray scattering (USAXS) properties, and the sample information is expressed by three parametric images. Combinedwith computed tomography (CT), DEI-CT can produce 3D volumetric images of the sample and can be used for investigating micro-structures of biomedical samples. Our DEI experiments for liver samples were implemented at the topography station of Beijing Synchrotron Radiation Facility (BSRF). The results show that by using our provided information extraction method and DEI-CT reconstruction approach, the obtained parametric images clearly display the inner structures of liver tissues and the morphology of blood vessels. Furthermore, the reconstructed 3D view of the liver blood vessels exhibits the micro blood vessels whose minimum diameter is on the order of about tens of microns, much better than its conventional CT reconstruction at a millimeter resolution. In conclusion, both the information extraction method and DEI-CT have the potential for use in biomedical micro-structures analysis.

BACKGROUND: Primary biliary cirrhosis (PBC) is cha- racterized by frequent presence of antimitochondrial anti- bodies (AMAs). The sensitivity and specificity of AMA for PBC are both greater than 90%-95%, so the presence of AMA in serum is the major hallmark in PBC. However, it has long been recognized that in 5%-10% of patients the clinical, biochemical and histological features are diagnos- tic for PBC, but their sera are consistently tested negative for AMA/AMA-M2. This study aimed to evaluate whether the presence of AMA alters the clinical, serological and his- tological features of the disease. METHODS: Clinical data of 70 patients clinically and/or histologically diagnosed with PBC were reviewed. AMA- negative and AMA-positive patients were compared in terms of clinical, biochemical, immunological and histo- logical features. RESULTS: At presentation, 11 patients were serum AMA/ AMA-M2 negative. At the initial visit, AMA-negative and AMA-positive patients were similar in terms of age, sex, clinical manifestations, liver biochemistries and histological findings. The mean level of serum immunoglobulin M (IgM) was significantly lower in AMA-negative PBC pa- tients than in AMA-positive PBC patients (2851±1418 mg/L vs 6361 ±4928 mg/L, P =0.033). Serum antinuclear anti- bodies ( ANA) and/or smooth muscle antibodies ( SMA) were more frequently positive in the AMA-negative PBC patients than in the AMA-positive patients (81.8% vs 40.7%, P =0.031). CONCLUSION: AMA-negative PBC patients are characte- rized by relatively lower levels of serum IgM and a higher prevalence of serum ANA/SMA and are not associated with substantial differences in the clinical, biochemical and histo- logical spectrum of the disease.
