A novel rheumatoid arthritis(rA)synovial fluid protein,Syntenin-1,and its receptor,Syndecan-1(SDC-1),are colocalized on rA synovial tissue endothelial cells and fibroblast-like synoviocytes(FLS).Syntenin-1 exacerbates the inflammatory landscape of endothelial cells and rA FLS by upregulating transcription of IrF1/5/7/9,IL-1β,IL-6,and CCL2 through SDC-1 ligation and HIF1α,or mTOr ***,Syntenin-1 orchestrates rA FLS and endothelial cell invasion via SDC-1 and/or mTOr *** Syntenin-1 reprogrammed endothelial cells,the dynamic expression of metabolic intermediates coincides with escalated glycolysis along with unchanged oxidative factors,AMPK,PGC-1α,citrate,and inactive oxidative ***,rA FLS rewired by Syntenin-1 displayed a modest glycolytic-ATP accompanied by a robust mitochondrial-ATP *** enriched mitochondrial-ATP detected in Syntenin-1 reprogrammed rA FLS was coupled with mitochondrial fusion and fission recapitulated by escalated Mitofusin-2 and DrP1 *** found that VEGFr1/2 and Notch1 networks are responsible for the crosstalk between Syntenin-1 rewired endothelial cells and rA FLS,which are also represented in rA *** to rA explants,morphological and transcriptome studies authenticated the importance of VEGFr1/2,Notch1,rAPTOr,and HIF1αpathways in Syntenin-1 arthritic mice and their obstruction in SDC-1 deficient ***,dysregulation of SDC-1,mTOr,and HIF1αnegated Syntenin-1 inflammatory phenotype in rA explants,while inhibition of HIF1αimpaired synovial angiogenic imprint amplified by *** conclusion,since the current therapies are ineffective on Syntenin-1 and SDC-1 expression in rA synovial tissue and blood,targeting this pathway and its interconnected metabolic intermediates may provide a novel therapeutic strategy.

Mechanisms of pulmonary endothelial barrier dysfunction in acute lung injury and acute respiratory distress syndrome

Endothelial cells(ECs)form a semi-permeable barrier between the interior space of blood vessels and the un-derlying *** endothelial barrier integrity is maintained through coordinated cellular processes involving receptors,signaling molecules,junctional complexes,and protein-regulated cytoskeletal *** acute lung injury(ALI)or its more severe form acute respiratory distress syndrome(ArDS),the loss of endothelial barrier integrity secondary to endothelial dysfunction caused by severe pulmonary inflamma-tion and/or infection leads to pulmonary edema and ***-inflammatory agonists such as histamine,thrombin,bradykinin,interleukin 1𝛽,tumor necrosis factor𝛼,vascular endothelial growth factor,angiopoietin-2,and platelet-activating factor,as well as bacterial toxins and reactive oxygen species,cause dynamic changes in cytoskeletal structure,adherens junction disorganization,and detachment of vascular endothelial cadherin(VE-cadherin)from the actin cytoskeleton,leading to an increase in endothelial *** interactions with leukocytes,platelets,and coagulation enhance the inflammatory ***,inflammatory infil-tration and the associated generation of pro-inflammatory cytokines during infection cause EC death,resulting in further compromise of the structural integrity of lung endothelial *** the use of potent antibiotics and aggressive intensive care support,the mortality of ALI is still high,because the mechanisms of pulmonary EC barrier disruption are not fully *** this review,we summarized recent advances in the studies of endothelial cytoskeletal reorganization,inter-endothelial junctions,endothelial inflammation,EC death,and endothelial repair in ALI and ArDS,intending to shed some light on the potential diagnostic and therapeutic targets in the clinical management of the disease.

Acetabular Development Following Salter’s Innominate Osteotomy for the Treatment of Developmental Dysplasia of the Hip: Long Term Follow-Up

Aim: To investigate the long term results of the patients followed till the skeletal maturity and treated with Salter innominate osteotomy. Patients and Methods: 85 hips of 63 patients whom were operated by the same surgeon between the years 1985 and 1991 were evaluated retrospectively. 34 hips of 25 patients who did not have enough follow-up or did not reach skeletal maturity at their last radiographic control were excluded from the study. So, 51 hips of 38 patients were included into our study. Mean age of the patients at the time of operation was 3 years 2 months (1.5 - 11 years), mean follow-up time was 16 years 6 months (10 - 23 years) and mean age at the last control was 19 years 7 months (15 - 27 years). results: The overall clinical results were favorable (excellent or good) in 48 hips (94.1%) according to the modified McKay clinical evaluation scale. Avascular necrosis was evaluated with the Kalamchi-McEven classification at the last follow up and 45 hips (88.2%) had no avascular necrosis. The radiographic results were assessed by the Severin classification system and 10 hips (19.6%) were stage 1a;10 hips (19.6%) were stage 1b;and 30 hips (58.8%) were stage 2. Upon the analysis of our radiographic results according to Stulberg classification system, we found out that only 2 (3.9%) of our patients were Stulberg stage 5, whereas 36 cases (70.6%) were Stulberg stage 1 and 2 (spherical congruency). According to Croft’s classification of hips for degenerative changes, 36 (70.6%) patients were stage 0;6 (11.8%) patients were stage 1;5 (9.8%) patients were stage 2 and 4 patients (8.7%) were stage 3. Conclusion: When appropriate soft tissue balance is provided, the complications of Salter Innominate Osteotomy are decreased and thus, it provides an excellent functional and radiological result.

Pediatric autoimmune neuropsychiatric disorders associated with or without streptococcal and other bacterial infections (PANDAS/CANS) are emerging as a featured pediatric disorder. Although there is some controversy regarding treatment approaches, especially related to the behavioral sequelae, we have hypothesized in other published work that it is characterized by the rapid onset of reward Deficiency Syndrome (rDS) in children. We propose utilizing a multi-systems biological approach involving the coupling of genetic addiction risk testing and pro-dopamine regulation (KB220/POLYGEN®) to help induce “dopamine homeostasis” in patients with PANDAS, especially those with known DNA-induced hypodopaminergia. This case study examines a 12-year-old Caucasian male with no prior psychiatric issues who presented with a sudden onset of severe anxiety, depression, emotional liability, and suicidal ideation. The patient underwent genotyping and the genetic addiction risk score (GArS) testing, which revealed risk polymorphisms in the dopamine D2 (-DrD2/ANKK (Taq1A), OPrM1 (A/G), DrD3 (C/T), and MAOA (4r) genes. These polymorphisms have been linked to hypodopaminergia. The patient was subsequently placed on research ID-KB220ZPBMPOLY (POLYGEN®), and albeit the possibility of bias, based upon self and parental assessment, a marked rapid improvement in psychiatric symptoms was observed. In the second phase of treatment (102 days utilizing KB220), the patient received standard antibody testing, which was positive for Lyme. Antibacterial therapy started immediately, and KB220z was discontinued to provide a wash-out period. A monotonic trend analysis was performed on each outcome measure, and a consistently decreasing trend was observed utilizing antibacterial therapy. Our recommendation, albeit only one case, is to utilize and further research a combined therapeutic approach, involving precision-guided DNA testing and pro-dopamine regulation along with antibacterial therapy, as well as
