Over the past one year and three months, 150 out patients with COVID 19 have been treated with early initiation of Oseltamevir 75 mg × 2 [150 mg] twice daily, with azithromycin, cephalosporin [to give atypical bacterial cover, gram positive bacterial cover, respectively] enabling cure. Corona virus pandemic was enabled by global contraception/abortion [non evidence based non medical practice] with its blood/necrotic tissue environmental pollution-documented by rising environmental oestrogen/alpha feto protein/beta human chorionic gonadotropin. Global contraception abortion with its blood, necrotic tissue environmental pollution provides a rich environmental media for Corona virus-a fastidious organism to become pandemic, with recurrent mutations. Guaranteed elimination of this fastidious organism-corona virus [and other microbes like Ebola, Nipah…] by implementing many childbirths per mother [earth bus fare/ticket] as God ordained abolition of global contraception, abortion, one child policy/small family norms [jumping off cliff to fly-marred science] with its blood/necrotic tissue pollution, mandatory reversal of contraception. Steroids are deleterious in any viral infection, being immunosuppressants-secondary mucormycosis, other fungal [less virulent] infections can follow;corona virus itself can mutate, gain virulence, drug resistance;but prior contraception-destroying germ cells, prior abortion [destruction of tissue] in the victim host result in auto-immunity of corresponding gradations including fatal HLH [Hemophagocytic lymphohistiocytosis], cytokine storm, requiring immunomodulation with plasmapheresis, steroids, hydroxychloroquine without much avail. 15 patients after the middle of 2018, during the initial onset of the epidemic of identified Nipah viral infection (fruits borne virus) in adjacent state, presented with fever, acute coma, delirium, movement disorders, required referral to district headquarters hospital, for virology confirmation, and management. 135 mostly afebrile patients, who refused referral to government hospital, for virology confirmation, presented with symptoms of mainly subacute pan encephalitis, over the past 14 months;90% presented with conglomeration of vertigo, staring, personality changes, imbalance in gait, difficulty to walk, bilateral drooling of saliva, tremors, confusion, altered sensorium, nausea, vomiting;83% presented with dysarthria, 28% presented with symptoms of raised intracranial pressure-mimicking accelerated, malignant hypertensive emergencies, bradycardia, vomiting;decreased vision in 4.6%, myoclonic jerks in 58%, choreoathetosis in 8%. All of them could not afford INR 10000 for virology confirmation. Fruits borne viral subacute pan encephalitis, can present as subacute neurovascular syndrome, simulating ischemic stroke, mediated by Hapten triggered autoimmune angiopathy (blood borne) with resultant bilateral, asymmetrical multifocal, infarcts, cerebritis with consequent edema, requiring recognition, successful treatment with acyclovir, antiedema, antiepileptic measures and not antiplatelets, since the etiology is inflammatory and not degenerative. Promoting awareness to steam cook, pressure cook, cook with coconut oil (pie) fruits, steaming fresh fruit juices, before consumption will prevent incidence of fruits borne, tobacco leaves borne, and endemic viral infection. Case 1: In 1990, a 23-year-old woman, married for two years, with primary infertility, was brought by her husband, with ultrasonography of abdomen, pelvis report stating multiple tiny cysts in both ovaries, infantile uterus;so husband claimed he was cheated to marry a woman with an infantile uterus, he wanted to divorce her on medical grounds. Analysing the problem revealed the woman had irregular menstruation before marriage due to polycystic ovaries;the husband took a prescription of oral contraceptive pills from a clinician, for one cycle to regularise menstruation of his wife;which he continued to administer for 2 years, with a desire to enable conception of his wife not understanding oral contraceptive pills with their exogenous oestrogen, suppress endogenous oestrogen preventing ovulation to conceive, produce withdrawal bleeding, due to suppressed endogenous oestrogen-suppressed uterine development resulting in infantile uterus. Case 2: In 1996, a 25-year-old woman underwent lower segment Cesarian section, 10 days prior to her EDC [expected date of child birth], as per the request of her husband who desired to see the baby before boarding his flight overseas;lower segment Cesarian section was performed by a urologist, general surgeon, but the mother expired on the theatre table, probably because the woman’s expected date of childbirth range would have fallen into the 15 days after expected date of childbirth norm, when her oestrogen would not have dipped, oxytocin would not have been released, to prevent postpartum haemorrhage. Case 3: In 1998, a 27-year-old woman presented with postpartum haemorrhage of one hour duration, following vaginal delivery of foeto placental unit;with haemoglobin of 3 gm%;immediately hysterectomy of the soft uterus was performed, mobilising 10 units of blood;once bleeding uterus was severed, all the 10 units of blood were transfused immediately, she survived. Background: Universal concern is an increased prevalence of morbidity, mortality;with improper comprehension, the globe has implemented, non evidence-based medical practice, without therapeutic indication, therapeutic protocols, policies-contraception, abortion, annihilating human race by therapy, destroying germ cells with consecutive systemic inflammatory response syndrome, associated with decreased androgen oestrogen resulting in metabolic syndrome, increased neoplasms—due to lacuna of this comprehension—cholesterol was blamed, resulting in essential fatty acids deprived diet, promoting sunflower seed oil, refined oil, lipid-lowering agents prescription like water, for the past >40 years;the tragedy of this false concept of refined oil, essential fatty acids deprived diet, [against God-ordained marvelous cell metabolism, from cholesterol] resulting in degenerative diseases, defaulted genomic repertoire, as cause and effect phenomenon in the two following morbid, fatal case presentations;contraception, abortion is the culprit and not cholesterol which is the basic life moiety of cell membrane, cell metabolism governing steroid hormones-androgen oestrogen derived from cholesterol. Case 1: 76-year-old male, vegetarian, presented with lethargy, hypotension, unexplained hyponatremia, on fludrocortisone for 2 years. MRI detected empty sella;hence hypopituitarism with cortisol deficiency, resulting in hyponatremia was considered and with hydrocortisone replacement, patient improved. Low levels of LDL, HDL, suggested chronic [>40 years] deprivation of diet containing essential fatty acids had resulted in degenerative—empty sella status with hypopituitarism. An Appraisal of Sexual Behaviors, STI/HIV Prevalence, and HIV Prevention Programs among Truckers in India: A Critical Literature Review

Background: A systematic review portraying the changing pattern of sexual behaviors, STI and HIV prevalence and key strategies curb HIV and STI among truckers in India is lacking. This paper therefore aims to present a chronological review of literature regarding sexual behaviors, STI/HIV prevalence, and various HIV prevention programs implemented among truckers in India. Methodology: Published and unpublished studies (1990-2011) were identified through electronic databases, and hand searching. Results: Most studies on sexual behaviors and STI/HIV prevalence among truckers focused only on drivers and ignored their helpers. Evidences suggest that consistent condom use by truckers with both paid and non-paid female partners has increased during the past decade. Many recent studies suggest that the HIV prevalence among truckers is about 2% and it has been declining slowly during the past decade. The HIV prevention programs among truckers which started with the aim of raising awareness about HIV during early 1990s, have grown multi-folds to encompass not only the standard strategies like mid-media events, interpersonal counselling and STI care for HIV prevention, but also innovations such as the use of business franchisee models, and integrated research and evidence based planning in the program. The possibilities of using new approaches such as male circumcision and the use of pre-exposure prophylaxes are underway. Conclusion: More studies are needed to explore helpers’ vulnerabilities to HIV. Very few studies have attempted to examine the impact of large-scale prevention programs among truckers. Efforts are required to examine the impact of different components of such programs to guide the HIV prevention efforts among truckers in India.

Study of Active Screening to Detect Un-Registered Open Cases of Tuberculosis in Rural Areas of Capital District of rajasthan, India

Tuberculosis is one among top 10 causes of deaths worldwide. Access to effective diagnosis by every TB patient is one among five missions of Global Plan. Active case finding among vulnerable groups has been the key focus area of National Strategic Plan to control TB in India 2017-25. This cross-sectional study was carried out to find out the additional open cases of TB through active screening of general & rural population in selected villages of Jaipur, the capital district in Rajasthan state of India. An active screening team of trained staff visited each house-hold in selected study villages for interviewing the head of house-hold or other adult member available at the time of visit. Family members of age ≥ 15 years were screened for complain of chronic cough of duration 2 weeks or more with study questionnaire. Presumptive cases found at screening were referred for seeking medical consultations from nearest designated microscopy centre. The referred patients were followed up on telephone up to five times for results of medical consultancies. Laboratory technicians of catchment DMCs were also followed up for verification of results. Total 19,306 individuals were covered. Study included 52.8% male, 69.3% ≥ 15 years age, and 99.3% Hindu participants. There were total 153 presumptive TB patients. Out of these, total 10 cases were confirmed as TB. The yield of detection was 0.51 cases/1000 population screened.
