Context: Prosthetic treatments for partial edentulism aim to restore or improve oral functions such as mastication, phonation and aesthetics. They have an impact on nutritional status, general health and quality of life. Objectives: To determine the frequency and determinants of wearing dental prostheses in partial edentulous Congolese adults as well as the reasons of prosthetic non-rehabilitation. Methods: Partial edentulous patients attending five medical-dental centers in Kinshasa were included in this cross-sectional study. Each patient completed a questionnaire and underwent a clinical examination. Chi-square test was used for searching associations between variables and logistic regression for determinants. The significance level was 0.05. Results: Three hundred partial edentulous patients with an average age of 40.3 ± 15.9 years participated in this study. 11% (CI: 7.5% and 14.5%) of partial edentulous Congolese adults wore a dental prosthesis. Age was the independent determinant of denture wearing. Young adults (under 30) were 5 times more likely to not wear dentures than older people. The absence of discomfort was the major reason for not prosthetic rehabilitation (32%) followed by finances (30%). Conclusion: Very few partially edentulous Congolese adults were prosthetics wearer. Age was the only determinant.

We examine an energy harvesting system of two magnetopiezoelastic oscillators coupled by electric circuit and driven by harmonic excitation. We focus on the effects of synchronization and escape from a single potential well. In the system with relative mistuning in the stiffness of the har- vesting oscillators, we show the dependence of the voltage output for different excitation frequencies.

Traumatic Penile Amputation Post Circumcision: A Series of 3 Cases

Chlorine-hydroxyl diffusion was measured in pargasitic amphibole from Yunnan province,China at 1.0 GPa,625 to 800℃.Experiments were performed by immersing unoriented crystals in waterbearing NaCl in a piston cylinder for durations from 100 to 454 *** profiles were on the order of greater than tens of micrometers in length,and electron microprobe analysis allow us to extract semi-quantitative diffusivities from these *** preliminary diffusion coefficients for chlorine in amphibole in the water-bearing experiments are 2.6×10m/s at 625℃,4.9×10m/s at 650℃,7.6×10m/s at 700℃,1.8×10m/s at 750℃,2.8×10m/s at 800℃.For temperatures between 625 and 800℃,the Arrhenius relation for chlorine-hydroxyl diffusion has an activation energy of 106.6±7.8 kJ/K mol and a D of 4.53(+7.3,-2.8)×10m/*** measurements do not show evidence of anisotropy in the diffusion of Cl-OH into amphibole,but future experiments need to better investigate this possibility.

Circumcision remains a frequently performed surgical procedure and could be associated with various complications, ranging from mild to catastrophic. Penile amputation is a rare and severe complication usually complex and challenging to manage. We describe three cases of penile amputation injuries following circumcision referred within a week at the urological service of the Yaoundé Central Hospital. The first case was a 5-year-old who had complete penile amputation during circumcision by a nurse assistant at a rural health center. The second was a 7-year-old boy who sustained total penile glans amputation while undergoing circumcision by a nurse under local anesthesia at a rural health facility. The third involved a 6-year-old who had total penile amputation with loss of the amputated stump during circumcision by a traditional practitioner at home. Non-microsurgical penile re-implantations were done with diverse outcomes. The preservation of the amputated stump, the ischemic time and the severity of injury are factors affecting surgical outcome. The aim of this study is to evaluate our management experience and outcome of penile amputation injuries in resource-limited settings. Microsurgical replantation remains the gold standard in the management of penile amputation injuries. However, in resource-limited settings macroscopic replantation could be used as an alternative remedy to salvage the amputated penis.

Introduction-Objectives: Through the presentation of epidemiological, anatomo-clinical and surgical aspects, we report our experience in the management of traumatic axillary lesions. Materials and Methods: A descriptive retrospective study was based on the medical records of patients who suffered vascular axillary and/or brachial plexus trauma and who underwent surgical repair at the Abidjan Cardiology Institute from January 2008 to June 2022. Epidemiological, anatomo-clinical and surgical data were studied. Results: Thirty-four medical files belonging to 33 men and one woman, aged 32 on average, were collected. The circumstances of occurrence were dominated by the stab wound (n = 22). The combinations of injuries were as follows: associated involvement of the axillary artery and vein (n = 4);isolated involvement of axillary artery (n = 3);isolated involvement of the axillary vein (n = 2);associated involvement of the axillary artery and brachial plexus (n = 17);associated involvement of the axillary artery and vein and brachial plexus (n = 08). Anatomic lesions included acute arterial lesions (n = 29) and arteriovenous fistula (n = 1) and false aneurysms (n = 4). All patients were operated on under general anesthesia;vascular repair included direct suturing (n = 16), arterial and venous bypass using a long saphenous graft (n = 9), prosthetic arterial bypass (n = 5) and prosthetic flattening-graft (n = 4). Brachial plexus surgery consisted of an end-to-end anastomosis of each transected bundle in all cases (n = 25). The medium-term postoperative course was marked by success without functional sequelae in 88.24% of cases (n = 30) and by the persistence of distal paralysis of the thoracic limb after 6 months in 05.88% (n = 2) of all patients, i.e., 8% of patients who presented with brachial plexus injury. Introduction: A penile fracture is the traumatic rupture of the turnica albuginea of one of both cavernous bodies of an erect penis. It is a urologic emergency in young adults. The objective of our study is to determine the epidemiologic, clinical, and therapeutic aspects and evaluate the complications at the Yaounde Central Hospital (YCH). Methodology: It was a cross-sectional retrospective and descriptive study, carried out for a period of five years, from 2015 to 2020, followed by a prospective phase for a period of 1 year, from December 2020 to 2021 at the Urology Unit of Yaounde Central Hospital. Results: Our study involved 23 patients. The mean age was 34 ± 4 years with extremes of 23 - 65 years. Married men were the most involved, with 16 cases (69.6%). The majority of patients presented for consultation within the first 6 hours (60.9%). The mean delay time before the consultation was 5 [3 - 24] hours, with extreme values of 1 - 72 hours. Sitting position during sexual intercourse was mainly found (56.5% of cases), while alcohol consumption was the main environmental risk factor found (47.8% of cases). Penile pain (100.0%) and deformation of the penile shaft (91.3%) were the most encountered symptom during admission. The frequency of cavernosa involement was 87.0%. The injury was partial in the majority of cases (95.0%), involving both corpus cavernosa (55.0%) and mostly on the right (60.0%). The mean management delay was 5 hours standard deviation [5 - 7], with extremes of 2 - 48 hours. The intra-cavernous hematoma was the essential per-operative finding in all the operated cases (100.0%). Most patients resumed sexual intercourse 2 - 3 months following management (65.3% of cases). After one year of treatment, 78.3% of patients declared satisfactory sexual activity. Conclusion: Penile fracture is an anthological emergency in young adults. The management is essentially surgical. Functional prognosis depends on the promptitude of surgical intervention.

本研究目标是开发一种机器人插管系统，并对应用于气管内插管进行可行性实验研究。开普勒插管系统由一个远程控制中心（操纵杆和插管主控界面）以及通过机器人手臂连接到的一个标准化的视频喉镜组成。研究中90例插管都由同一个操作者在提供气道训练的模拟人，采用开普勒插管系统完成的。第一组的30例气管插管，操作者采用直视模型的方式（直视组）；第二组的30例为操作者无法直接看到模型的情况下操作（非直视组）；第三组的30例使机器人按照之前记录的步骤进行半自动化插管（半自动化组）。记录首次尝试插管的成功率和插管时间，并使用线性回9-5进行分析。所得数据用均数（标准差，SD）表示，所有插管在第一次尝试均成功。直视组、非直视组和半自动化组的平均插管时间分别为46（18）秒、51（19）秒和41（1）秒。直视组和非直视组的斜率均为负值，表示每一次连续实验需要的时间减少。半自动化组的斜率为0并且SD值低至1秒，表明自动化插管的再现性高。我们认为一种可以在40～60秒内完成插管的远程遥控的机械插管系统已经成熟。

Introduction: Surgical management of adrenal tumors has greatly improved over the past years, with laparoscopic adrenalectomy being the gold standard. However, Open adrenalectomy is indicated in large adrenal tumors, malignant tumors and large phaeochromocytomas. We report surgical outcomes of 18 cases of functional adrenal tumors from 2007 to 2022. Methods: We conducted a retrospective cross-sectional and descriptive study in three tertiary hospitals in Yaounde, Cameroon. We reviewed files of patients who underwent adrenalectomy over a period of 15 years from July 2007 to July 2022. Clinical and diagnostic components of adrenal tumors, indications and surgical outcomes were analyzed. Results: A total of 18 patients were included in our study. The average age of patients was 38.33 years, with a female-to-male sex ratio of 2:1. Weight gain (72.2%) was the most represented clinical sign. The secretory nature of tumor and malignancy represented 55.5% and 33.3% of the operative indications and all 18 (100%) of the patients had open adrenalectomy. Vascular injury was the most common intraoperative complication with 5.63%, while acute adrenal insufficiency (16.7%) was the most common post-operative complication. The average tumor size was 6.22 cm and the mean duration of hospitalization was 11.61 days. Adenoma 7 (38.9%) and adrenocortical carcinoma 5 (27.8%) were the frequent histological types. One patient died two months post-surgery from anemia-related complications. Conclusion: The success of adrenal surgery is linked to multidisciplinary patient care and the experience of the surgeon. Conventional surgery still has indications with satisfactory short- and medium-term results in our context.

Introduction: Twisting of the spermatic cord is part of urological emergencies. It can occur at any age with a predilection for children and adolescents. There is no literature on spermatic cord torsion in octogenarians, probably due to its rarity. The majority of recent studies show spermatic cord torsion in patients under 55 years of age. This is why we present a case of spermatic cord torsion in an 80 years old subject. Case Presentation: We present a case of torsion of the spermatic cord in an 80-year-old subject. The diagnosis was made on anamnestic and clinical grounds. The patient presented with scrotal pain, unilateral, of sudden onset, violent intensity without urinary signs with an ascended testicle. The urine dipstick was unremarkable. Management was done surgically and as an emergency. The postoperative course was satisfactory. Conclusion: Spermatic cord torsion in octogenarians is exceptional and should be suspected in the first instance in the presence of any sudden onset, severe testicular pain without urinary signs with an ascended testicle and a negative urine dipstick.
