Staging of portal hypertension and portosystemic shunts using dynamic nuclear medicine investigations

AIM： To explore portal hypertension and portosystemic shunts and to stage chronic liver disease （CLd） based on the pathophysiology of portal hemodynamics. METHOdS： Per-rectal portal scintigraphy （PRPS） was performed on 312 patients with CLd and liver angioscintigraphy （LAS） on 231 of them. The control group included 25 healthy subjects. We developed a new model of PRPS interpretation by introducing two new parameters, the liver transit time （LTT） and the circu-lation time between right heart and liver （RHLT）. LTT for each lobe was used to evaluate the early portal hypertension. RHLT is useful in cirrhosis to detect liver areas missing portal inflow. We calculated the classical per-rectal portal shunt index （PRSI） at PRPS and the hepatic perfusion index （HPI） at LAS. RESULTS： The normal LTT value was 24 ± 1 s. Abnormal LTT had PPV = 100% for CLd. Twenty-seven noncirrhotic patients had LTT increased up to 35 s （median 27 s）. RHLT （42 ± 1 s） was not related to liver disease. Cirrhosis could be excluded in all patients with PRSI 〈 5% （P 〈 0.01）. PRSI 〉 30% had PPV = 100% for cirrhosis. Based on PRPS and LAS we propose the classification of CLd in 5 hemodynamic stages. Stage 0 is normal （LTT = 24 s, PRSI 〈 5%）. In stage 1, LTT is increased, while PRSI remains normal. In stage 2, LTT is decreased between 16 s and 23 s, whereas PRSI is increased between 5% and 10%. In stage 3, PRSI is increased to 10%-30%, and LTT becomes undetectable by PRPS due to the portosystemic shunts. Stage 4 includes the patients with PRSI 〉 30%. RHLT and HPI were used to subtype stage 4. In our study stage 0 had NPV = 100% for CLd, stage 1 had PPV = 100% for non-cirrhotic CLd, stages 2 and 3 represented the transition from chronic hepatitis to cirrhosis, stage 4 had PPV = 100% for cirrhosis. CONCLUSION： LTT allows the detection of early portal hypertension and of opening of transhepatic shunts. PRSI is useful in CLd with extrahepatic portosystemic shunts. The construction industry is recognised to pose serious environmental threats due to its anthropogenic *** confront this challenge,green building has been famed as an auspicious strategy to alleviate energy-related carbon emissions alongside climate change ***,it has birthed diverse empirical studies and holistic reviews by the research ***,these previous studies have been carried out from a global and continental viewpoint regardless of the green building development specifically in emerging *** present study seeks to fill the sparse research upshots on the synthesis of green building research at the country level with core focus on South *** this light,the current study provides a scoping overview of the intellectual exploration on green building research in the South African *** study’s findings highlight the varying growth regarding the subject matter,the drivers,drawbacks,and socio-economic issues confronting in developing green building projects in the *** optimize the advancement of green building in the region,this study recommends the implementation of financing schemes,utilization of advanced digital technologies,and promotion of green and sustainable building curricula across the institutions as well as post-occupancy-related studies within the green building spectrum towards a safe and salutogenic built-environment in the country.

Background In ambrosia and bark beetles–fungi interaction, volatile organic compounds(VOCs) play a central role in mediating various aspects of community dynamics of beetles and/or fungi. These functions include facilitating beetle habitat location, mate identification, and fungal partner differentiation. However, the understanding on this context remains limited, especially in the globally distributed subfamily Platypodinae, which comprises predomi-nantly ambrosia beetles. There is a lack of chemical data on ambrosia fungi from native South American species. This study addresses this gap by characterizing VOCs from twelve fungal species associated with Megaplatypus mutatus and assessing species-specific behavioral responses during *** Fungal VOCs were collected by gas chromatography–mass spectrometry combined with solid-phase microextraction and Y-olfactometry assays of males and females were performed at dispersal stage. Statistical analyses involved: non-metric multidimensional scaling multivariate plot and PERMANOVA test, a cluster analysis through unweighted pair group method with Jaccard index, and finally, a chi-square goodness-of-fit test for beetle behavioral *** We identified 72 VOCs from the fungal species isolated from M. mutatus galleries, exocuticle, and gut. The olfactory behavior of M. mutatus demonstrated its capacity to discriminate between volatile profiles, showing a pref-erence for either the fungus or the control source. Our results also enhance the understanding in a chemotaxonomic context and in the behavioral responses of M. mutatus revealing the beetle's remarkable low temperature toler-ance and its capability to maintain mobility and orientation toward volatile sources even after zero-degree Celsius *** This study presents a comprehensive insight into fungal VOC profiles, emphasizing the sources of isola-tion within pest associated fungi, as well as its symbiotic species from the Raffaelea genus. Effect of pediatric Tuina on children's recurrent acute respiratory tract infections: a retrospective cohort study in Southern China

OBJECTIVE: To determine the effectiveness of pediatric Tuina(PT) in preventing recurrent acute respiratory tract infections(ARTIs) in children. METHOdS: This is a retrospective cohort study based on the electronic medical records of children with recurrent ARTIs in 2016. Children were divided into a PT group or a non-PT group, according to whether they had received PT or not in 2016. The primary outcome was the number of ARTI episodes in 2017 and 2018. The secondary outcomes were the number of ARTIs leading to outpatient department visits and outpatient antibiotic prescriptions due to ARTIs in the same time period. Negative binomial regressions were used to detect the association between PT and the outcomes. RESULTS: A total of 2303 children were included in the analysis, including 94 in the PT group and 2209 in the non-PT group. Children who received PT six or more times in 2016 had fewer episodes of ARTIs in 2017 [incidence rate ratio(IRR): 0.59, 95% confidence interval(CI)(0.42-0.84)] and 2018 [IRR: 0.58, 95% CI(0.36-0.94)] and fewer outpatient department visits due to ARTIs in 2017 [IRR: 0.56, 95% CI(0.38-0.83)] than children who had not received PT in 2016. There was no significant difference in the number of outpatient antibiotic prescriptions between the two groups. CONCLUSIONS: Receiving PT six or more times within one year is associated with a decrease in recurrent ARTIs in children in the following two years. Randomized controlled trials are needed for effect evaluation prior to establishing PT as a method for preventing recurrent ARTIs among children.

Nuclear medicine dynamic investigations in the diagnosis of Budd-Chiari syndrome

Interaction network between frugivorous birds and zoochorous plants in cloud forest riparian strips immersed in anthropic landscapes
