Automated Telephone Calls in the Follow-Up of Self-Care in Outpatients with Type 2 Diabetes: A Feasibility Study

Aims and objectives: To test an automated telephone service as a follow-up and support strategy for self-care in a population of adult Mexican patients with type 2 diabetes. Methods: The design was a two-group comparative design, with pre and post-intervention measurements. Patients were assigned into intervention (n = 31), or comparison (n = 33) groups. Over 12 weeks, each participant in the intervention group received automated phone calls with instructions to facilitate accomplishment of the treatment, and self-care activities. Two focus groups helped to appreciate patients’ follow-up experience. Randomized grouping was performed by selecting numbers from an urn;the final sample comprised 31 (48%) patients in the intervention group, and 33 (52%) in the comparison group;these procedures were performed by the case nurse management. The study was approved by the Ethics Committee of the Autonomous University of nuevo Leon and the University of Michigan;all of the participants signed informed consent. Results: Of the 372 programmed phone calls made to the participants, 234 were completed, representing a 62.9% response rate. Phone calls may be associated to a 0.82% decrease in the HbA1c values and to lower depression scores. HbA1c measurements decreased 0.82% (M = 7.41 - 6.69;t = 25;4.11;p = ?0.001) from the baseline values over the 12-week study in the intervention group and 0.49% (M = 7.24 - 6.75;t = 26;2.11;p = 0.044) in the comparison group. Variables related to medication intake did not exhibit differences between the baseline and second measurements, except with respect to the scale of depression.

Molecular advances support the existence of an alternative pathway of colorectal carcinogenesis that is based on the hypermethylation of specific DnA regions that silences tumor suppressor genes. This alternative pathway has been called the serrated pathway due to the serrated appearance of tumors in histological analysis. new classifications for colorectal cancer(CRC) were proposed recently based on genetic profiles that show four types of molecular alterations: BRAF gene mutations, KRAS gene mutations, microsatellite instability, and hypermethylation of Cp G islands. This review summarizes what is known about the serrated pathway of CRC, including CRC molecular and clinical features, prognosis, and response to chemotherapy.

在文化遗产旅游活动中,博物馆等这些有着收集人类文明经历的机构为旅游者提供了一个发现和重现不同于他们自己的人类日常生活的事物的机会。正如理查德所暗指的,现代旅游者对于利用博物馆来测试他们的观念,行为和好奇心这种行为很反感,这一观点可以通过类似马拉维的卡龙加博物馆和位于阿姆斯特丹的大麻馆等这样的专业化的博物馆的发展情况来验证。发达国家的越来越多的研究都表明博物馆旅游交通有所发展,然而对于马拉维来说,尽管有不同种类的博物馆分布于全国各地,但是总体来说马拉维的博物馆交通还是欠发达。这项研究致力于找出阻碍马拉维由博物馆参观地成为休闲中心的因素。其中有五个博物馆参与其中,即Chamare,Chichiri,卡龙加,曼陀罗和姆祖祖。该研究探讨了收集和博物馆展品和通信的消息是否对带领旅游者参观博物馆有拉动效应。同时本次研究是通过使用丹恩(1977)推拉理论和(克莱蒙斯和伍德拉夫,1992年)的期望失验理论完成的。本次调查使用混合研究方法和使用问卷调查、面对面的访谈、以及观察法来收集解决研究问题的数据。对其中的200人立意取样回答问卷的形式,对5位管理人员采用深度访谈的方式调查。研究结果表明,大多数马拉维人因为没有被妥善的营销而不知道博物馆的存在,所以不会去参观博物馆。另一个原因是,展出的展品大多是和博物馆本身一样古老,自己因此人们不愿意去看他们年轻时就已经看过的展品。而最后,还有一些人表示,他们不愿意交门票费,因此他们也不会去参观博物馆。本研究强烈建议文化部加速开展营销和公民教育活动,以确保马拉维人对博物馆有正确的认识。与此同时,研究建议陈列展品定期更换,以确保游客源源不断进入博物馆。

Water depth,salinity,current,and suspended sediment concentration(SSC) were measured along with the grain size distribution of bed sediment along an estuarine longitudinal *** floc size increased with increase in the percentage of clay and silt,while decreased with increase in the percentage of sand content of bed *** turbulent shear,G,had a direct effect on floc size with its value increasing with increase in G up to a G value of 15 s,while an inverse relation existed between floc size and G at higher G(C> 15 s).Further,higher turbulence enabled sand to get resuspended and cause additional shear leading to the break-up of *** attempt was made to modify G to account for the combined effect of water turbulence(C) and shear imparted by sand(C) and the impact of the modification of G on the predictability of floc size was evaluated.A new model was developed which explains floc size in terms of sediment concentration(C),salinity gradient(S),and G for different scenarios based on the value of *** analysis was done for observed floc size(FS) and predicted floc size using four approaches:(Ⅰ) FS β C~χ;(Ⅱ) FS α C~χS;(Ⅲ) FS α C~χSG~z for G nd FS α C~χSGfor G> 15 s; and(Ⅳ) FS α C~χSGfor G> 15 sand G=G+G,where x,y,and z are determined by *** was observed that the predictability of the floc size improved when the turbulence was modified to account for shear imparted by sand so that the coefficient of determination was increased from 0.78 for model Ⅲ to 0.89 for model Ⅳ.Further,the settling velocity was expressed as a function of suspended sediment concentration,turbulent shear,and salinity *** predictability of settling velocity was improved(R~2 increased from 0.77 to 0.86) when the additional turbulence created by sand was incorporated in the non-dimensional empirical *** study highlights the influence of sand in causing the break-up of flocs and suggests that for turbulence shear values high enough to resuspend

This paper presented the microstructure, mechanical properties and oxidation behavior of silicon nitride obtained by both conventional sintering and sintering followed by hot isostatic pressing （HIP）. Silicon nitride with additives such as 5 wt.% Al2O3 and 5 wt.% Ln203 （Ln= La, La concentrate, Gd or La＋Gd） were studied. The results revealed that Gd203 additions increased the formation of elongated grains of β-Si3n4, the fracture toughness and oxidation resistance. La2O3 additions led to higher densification and hardness values, while addition of La2O3 concentrate promoted the formation of materials with intermediate properties, compared to the other studied compositions. Hot isostatic pressing increased the hardness but decreased the fracture toughness of the material, mainly because it allowed residual pores to close and also reduced the average aspect ratio of β-Si3n4 grains.

Aims We tested whether—in addition to weather conditions—the concentrations of nitrogen and phosphorus in the substrate have an effect on the radial stem increment of nothofagus dombeyi trees in old-growth forest stands on volcanic soil at the western slopes of the Andes in South-Central *** We took soil samples and tree increment cores from five proximate sites(1000-1300 m a.s.l.)that are located in the volcanic region of the Conguillío national Park and differ in the age of the substrate(Miocene—3500 years B.P.)and in its concentrations of nitrogen(n)and phosphorus(P).The soil samples were also analysed on their concentrations of other plant mineral nutrients,carbon(C)and nitrogen isotope ratios(δ^(15)n).Tree-ring widths and the stem basal area increment(BAI)were related to climate *** selected tree rings,the stable isotope ratios of carbon(δ^(13)C)and oxygen(δ^(18)O)were determined and related to growth and climate *** FindingsConsistent with theory,the soils on the oldest substrate showed the highest(least negative)δ^(15)n values,but mineral n was the only nutrient whose concentration exhibited a straightforward(increasing)relationship with increasing substrate *** BAI was largest on the soil with the highest concentration of plant-available *** contrast to BAI,tree-ring chronologies did not differ among the study ***,tree-ring chronologies and BAI exhibited significantly positive correlations with summer precipitation,and negative correlations,with summer(December)temperature.A negative correlation was found betweenδ^(13)C and precipitation anomalies in the growing season(november-March).We interpret the negative correlations between growth and temperature,and betweenδ^(13)C andδ^(18)O in the tree rings,as an impairment of net carbon assimilation by anomalously warm weather conditions during the growing *** conclude that the growth of *** is mainly affected by high temperature and low precipitation
