Effectiveness of breathing exercises during the second stage of labor on labor pain and duration： a randomized controlled trial

BACKGROUND： Some research exists on the effect of non-pharmacological approaches for labor pain relief However, there is limited information about effectiveness of breathing exercises in pregnant women to reduce maternal pain during labor. OBJECTIVE： To determine whether breathing exercises for pregnant women during the second stage of labor have beneficial effects on maternal pain, duration of labor, and the first-minute Appearance, Pulse, Grimace, Activity and Respiration （APGAR） scores. DESIGN, SETTING, PARTICIPANTS AND INTERVENTIONS： This randomized clinical trial involved 250 pregnant women, who were randomly divided into two groups： intervention group （IG; n = 125） and control group （CG; n = 125）. IG received one session breathing exercise training and performed breathing exercises during the second stage of labor versus the CG that did not receive any breathing exercise training. MAIN OUTCOME MEASURES： The effects of breathing exercises on maternal pain were determined by Visual Analogue Scale （VAS）, duration of the second stage of labor, and the first-minute APGAR scores. RESULTS： The mean age of the participants was （23.2 _＋ 4.2）（range： 18 to 42） years. Both IG and CG had similar baseline characteristics in terms of age, education level, occupation, and smoking （P 〉 0.05）. The mean VAS scores of IG and CG were （88.2 ＋_ 6.3） and （90.5 ＋_ 7.0）, respectively （P 〈 0.001）. The duration of the second stage of labor was （369.6 ＋ 92.0） s for IG and （440.7 ＋ 142.5） s for CG （P 〈 0.001 ）. The mean first-minute APGAR scores were （8.84 ＋ 0.50） for IG and （8.73 ＋ 0.89） for CG （P 〉 0.05）. CONCLUSION： Based on this study, breathing exercises with deep inhalation and exhalation in pregnant women are effective in reducing the perception of labor pain and shortening the duration of the second stage of delivery. Therefore, we recommend breathing exercises as an effective modality for labor pain management and shortening the duration of labor. TRIAL REGISTRATION： This study i

Background Noise,the second common environmental cause after air pollution,is associated with the non-auditory disorders such as increased blood *** the risk due to the noise exposure might help to prevent further development of *** We investigated an association between occupational noise exposure and blood pressure in different hypertension stages in models stratified with ***,we examined the noise-BMI,noise-gender,and noise-residence interactions on hypertension risk in separate *** We utilized 124,286 young adults(18-40 years) from project the Environmental and LifEstyle Factors iN metabolic health throughout life-course Trajectories(ELEFANT).We determined the noise exposure as yes/no and measured the blood pressure as mm *** applied logistic regression models adjusted with covariates to assess the associations and *** We found that noise exposure was responsible to increase 0.23 mm Hg(95%CI:0.10,0.38,p=0.0009) in elevated,0.15 mm Hg(95%CI:0.06,0.22,p=0.0008) in stage 1,and 0.41 mm Hg(95%CI:0.27,0.55) in stage ***,noise exposure-BMI interaction was consistently positively associated with 0.71 mm Hg(95%CI:0.44,0.990,p<0.0001) in elevated,0.78 mm Hg(95%CI:0.60,0.96,p<0.0001) in stage 1,and 2.06 mm Hg(95%CI:1.73,2.38,p<0.0001) in stage ***,we found that noise exposure-male interaction showed higher risk for hypertension compared with the noise exposure-female interaction in stage 1(OR=1.67,95%CI:1.47,1.86) and stage 2(OR=1.35,95%CI:1.35,2.05).Finally,we found that noise exposure-urban interaction was consistently associated with 0.32 mm Hg(95%CI:0.17,0.48,p<0.0001) in elevated and 0.44 mm Hg(95%CI:0.27,0.59) in stage 2 *** In young,noise exposure was associated with increased blood pressure in elevated,stage 1,and stage ***,noise exposure was interacted with the BMI,gender,and residence which induced increase the blood pressure.
