AIM： TO inves.igate in vivo, whether CCK2 receptors.（CCK2R） regulate proteins.known to play a crucial role in cell proliferation and cancer development and analys. in vitro the molecular mechanis.s.that lead to s.c activation; in particular, to identify the domains.within the CCK2R s.quence that are implicated in this.activation. METHODs. The expres.ion and activation of s.c and ERK were s.udied in vivo us.ng immunofluores.ence and wes.ern-blot techniques. We us.d pancreatic tis.ues.derived from wild type or Elas.CCK2 mice that expres.ed the CCK2R in pancreatic acini, dis.layed an increas.d pancreatic growth and developed preneoplas.ic les.ons. The pancreatic tumor cell line AR4-2J expres.ing the endogenous.CCK2R or COs.7 cells.trans.ently trans.ected with wild type or mutant CCK2R were us.d as.in vitro models.to s.udy the mechanis. of s.c activation. s.c activation was.meas.red by in vitro kinas. as.ays. ERK activation by wes.ern blot us.ng antiphos.ho-ERK antibodies.and the involvement of s.c in gas.rin-induced cell proliferation by MTT tes.. REs.LTs. We s.owed in vivo that the targeted CCK2R expres.ion in the pancreas.of Elas.CCK2 mice, led to the activation of s.c and the ERK pathway. s.c was.activated ups.ream of the ERK pathway by the CCK2R in pancreatic tumoral cells.and contributed to the proliferative effects.mediated by this.receptor. In vitro res.lts.demons.rated that activation of the s.c/ERK pathway by the CCK2R required the NPXXY motif, located within the CCK2R s.quence at the end of the 7^th trans.embrane domain, and s.gges.ed the putative role of Gq in this.mechanis.. CONCLUs.ON： Deregulation of the s.c/ERK pathway by the CCK2R might repres.nt an early s.ep that contributes.to cell proliferation, formation of preneoplas.ic les.ons.and pancreatic tumor development.

减轻组织的机械性压力在治愈糖尿病相关的足溃疡所需的多种干预措施中最为重要。本文是2023年国际糖尿病足工作组(IWGDF)发布的减压治疗促进糖尿病相关的足溃疡愈合的循证指南,也是对2019年IWGDF指南的更新。我们遵循推荐、评估、发展和评价分级(GRADE)方法学,以患者-干预-比较-结局(patient-intervention-control-outcome,PICO)模式设计临床问题和重要结果,进行系统评价和Meta分析,制定评判表汇总,为每个问题编写推荐和理由。每项推荐都是基于系统综述中发现的证据,在无证据情况下则采用专家意见,以及对判断项目的GRADE等级总结进行仔细权衡,包括可预期和不可预期的影响、证据的确定性、患者价值、所需资源、成本效益、公平性、可行性和可接受性。为了治愈糖尿病患者神经性前足底或中足底溃疡,要使用不可拆卸的齐膝高减压装置作为首选减压干预措施。如果患者存在对不可拆卸减压装置的禁忌证或不能耐受,考虑使用可拆卸齐膝(或踝)高减压装置作为减压干预措施的第二选择。如果无可用的减压装置,考虑使用合适的鞋具结合毡制泡沫作为减压干预措施的第三选择。如果这种非手术减压治疗不能治愈前足底溃疡,可以考虑跟腱延长术、跖骨头切除术、关节置换术或跖骨截骨术。为了治疗继发于屈趾畸形的神经性足底或小趾远端溃疡,可行趾屈肌腱切开术。为了促进后足、非足底或并发感染或缺血的溃疡的愈合,提出了进一步的推荐。所有推荐都已在减压临床路径中进行了总结,以帮助促进本指南在临床实践中的实施。这些减压推荐将帮助医务人员为糖尿病相关的足溃疡患者提供最佳的治疗和预后,降低患者感染、住院和截肢的风险。

Although tes.ing was.ewater for public health s.rveillance has.recently received s.gnificant attention in the United s.ates.res.archers.and public health profes.ionals.have long realized the tremendous.promis. of environmental s.rveillance(Es.,defined as.tracking pathogens.of interes. in fecally contaminated s.rface waters.s.il,or fomites.in low-res.urce s.ttings.for public health action.1,2 The power of Es.in lowres.urce s.ttings.is.mos. clearly illus.rated with the over 30 years.of well-es.ablis.ed and s.andardized polio *** part of the Global Polio Eradication Initiative,the World Health Organization(WHO)has.accredited 146 laboratories.within the Global Polio Lab Network(GPLN).Thes. facilities.acros. 92 countries.proces. over 8,000 was.ewater or fecally contaminated s.rface water s.mples.per year,in addition to over 220,000 s.ool s.mples.annually.3 The data generated from the GPLN are clos.ly linked to clear public health action related to targeted vaccination *** inves.ment and adoption of Es.for polio eradication efforts.s.rves.as.a us.-cas. for other targets.of *** explore the current s.ate of the field,the National s.ience Foundation-funded Res.arch Coordination Network(RCN)on Was.ewater Bas.d Epidemiology for s.Rs.CoV-2 and Emerging Public Health Threats.held a works.op in December 2023 to dis.us. the opportunities.and challenges.for Es.in low-res.urce s.ttings.Figure 1).Dis.us.ions.during the works.op highlighted knowledge gaps.bes.-practices.target s.lection,academic/government/private partners.ips.and overall public health action and s.s.ainability of thes. programs.
