BACKGROUND Non-invasive assessment of non-alcoholic steatohepatitis(NASH)is increasing in desirability due to the invasive nature and costs associated with the current form of assessment;liver *** multiparametric magnetic resonance imaging(mpMRI)to measure liver fat(proton density fat fraction)and fibroinflammatory disease[iron-corrected T1(cT1)],as well as elastography techniques[vibration-controlled transient elastography(VCTE)liver stiffness measure],magnetic resonance elastography(MRE)and 2D Shear-Wave elastography(SWE)to measure stiffness and fat(controlled attenuated parameter,CAP)are emerging alternatives which could be utilised as safe surrogates to liver *** To evaluate the agreement of non-invasive imaging modalities with liver biopsy,and their subsequent diagnostic accuracy for identifying NASH *** From January 2019 to February 2020,Japanese patients suspected of NASH were recruited onto a prospective,observational study and were screened using noninvasive imaging techniques;mpMRI with LiverMultiScan®,VCTE,MRE and *** were subsequently biopsied,and samples were scored by three independent *** diagnostic performances of the non-invasive imaging modalities were assessed using area under receiver operating characteristic curve(AUC)with the median of the histology scores as the gold standard *** between all three independent pathologists was further explored using Krippendorff’s alpha(a)from weighted kappa *** N=145 patients with mean age of 60(SD:13 years.),39%females,and 40%with body mass index≥30 kg/m2 were included in the *** identifying patients with NASH,MR liver fat and cT1 were the strongest performing individual measures(AUC:0.80 and 0.75 respectively),and the mpMRI metrics combined(cT1 and MR liver fat)were the overall best non-invasive test(AUC:0.83).For identifying fibrosis≥1,MRE performed best(AUC:0.97),compared to VCTE-liver stiffness measure(AU

Overview of recent advances in metastatic triple negative breast cancer

Antibiotic use in early childhood and risk of obesity: longitudinal analysis of a national cohort

Color Doppler ultrasound imaging in varicoceles: Is the difference in venous diameter encountered during Valsalva predictive of palpable varicocele grade?

Objective:The clinical grading system for varicoceles is subjective and dependent on clinician *** Doppler ultrasound(US)has not been standardized in the diagnosis of *** aimed to determine if US measurement of varicocele could be predictive of World Health Organization(WHO)varicocele ***:Men who presented for either scrotal pain or infertility to a tertiary men’s health clinic underwent physical examination,and varicoceles were graded following WHO criteria(0Zsubclinical,1,2,3).US was used to measure largest venous diameter in the pampiniform plexus bilaterally at rest and during Valsalva *** operator characteristic curve analysis was used to determine if resting diameter,diameter during Valsalva,or change in diameter between at rest and during Valsalva provided the highest sensitivity and specificity for determining clinical *** values for diameter were determined from these receiver operator characteristic ***:A total of 102 men(50 with clinical varicocele and 52 with subclinical varicocele)were *** at rest was the best ultrasonographic discriminator between subclinical and clinical varicoceles(area under the curve[AUC]Z0.67)with a diameter threshold of 3.0 mm(sensitivity 79%,specificity 42%).Diameter during Valsalva had the greatest AUC for determining clinical Grades 1 versus 2(AUCZ0.57)with diameter threshold of 5.7 mm(sensitivity 71%,specificity 33%).For differentiating between Grades 2 and 3,diameter at rest had the greatest AUC of 0.65 with a threshold of 3.6 mm(sensitivity 71%,specificity 58%).Conclusion:Our results corroborate other studies that have shown a weak correlation between US and clinical *** use of diameter during Valsalva was less predictive than diameter at rest and was only clinically significant in differentiating between Grade 1 and 2 varicocele.A standardized method for determining clinically relevant varicoceles on US would allow for improved pat

Background:There is currently considerable international debate around school closures/openings and the role of children in the transmission of coronavirus disease 2019(COVID-19).Whilst evidence suggests that children are not impacted by COVID-19 as severely as adults,little is still known about their transmission potential,and with a lot of asymptomatic cases they may be silent transmitters(*** without showing clinical signs of disease),albeit at a lower level than *** relation to this,it is somewhat concerning that in many countries children are cared for,or are often in close contact with,older individuals such as grandparents—the age group most at risk of acquiring serious respiratory complications resulting in *** text:We emphasise that in the absence of a vaccine or an effective therapeutic drug,preventive measures such as good hygiene practices—hand washing,cough etiquette,disinfection of surfaces and social distancing represent the major(in fact only)weapons that we have against ***,we stress that there is a pressing need to develop specific COVID-19 prevention messages for ***:An entertainment education intervention for schoolchildren systematically implemented in schools would be highly effective and fill this *** such measures in place there would be greater confidence around the opening of schools.

Systematic literature review of the antitumor effect of octreotide in neuroendocrine tumors

AIM To provide a comprehensive examination of the existing evidence of the antitumor effect of long-acting octreotide in neuroendocrine tumors(NETs).METHODS A systematic literature review of clinical trials and observational studies was conducted in PubM ed, EMBASE, and Cochrane through January 18, 2017. Conference abstracts for 2015 and 2016 from 5 scientific meetings were also *** Of 41 articles/abstracts identified, 13 unique studies compared octreotide with active or no treatment. Two of the 13 studies were clinical trials; the remaining were observational studies. The phase 3 Placebo-Controlled, Double-Blind, Prospective, Randomized Study of the Effect of Octreotide long-acting repeatable(LAR) in the Control of Tumor Growth in Patients with MetastaticNeuroendocrine Midgut Tumors clinical trial showed that long-acting octreotide significantly prolonged time to tumor progression compared with placebo in patients with functionally active and inactive metastatic midgut NETs; no statistically significant difference in overall survival(OS) was observed, possibly due to the crossover of placebo patients to octreotide. Retrospective observational studies found that long-acting octreotide use was associated with significantly longer OS than no octreotide use for patients with distant metastases although not for those with local/regional disease. CONCLUSION The clinical trial and observational studies with informative evidence support long-acting octreotide's antitumor effect on time to tumor progression and OS. This review showed the rarity of existing studies assessing octreotide's antitumor effect and recommends that future research is warranted.

Using an interprofessional team to provide refugee healthcare in an academic medical centre

Introduction The International Family Medicine Clinic(IFMC)was established in 2002 by the University of Virginia Department of Family Medicine to provide comprehensive,timely,culturally sensitive and high-quality healthcare to the growing refugee and special immigrant population in Central Virginia,*** The purpose of this paper is to describe the IFMC,with a specific focus on interprofessional roles,interprofessional collaboration,community partnerships and the services and resources available to IFMC *** The clinic has served over 3100 refugees from 60 countries in its 16-year *** 2019,the clinic staff now includes 4 attending physicians,2 nurse practitioners and 14 residents who have dedicated clinic time to see refugees;a registered nurse care coordinator and a social worker dedicated to the IFMC refugee population;2 clinical psychologists and doctoral students in clinical psychology;and a clinical *** IFMC also provides onsite psychiatric care.A process flow map depicts the interconnectivity of interprofessional team members working together with other specialty care providers within the medical centre and with community partners on behalf of refugee patients through the resettlement *** Individuals who arrive in the USA as refugees are a particularly vulnerable patient group and often require an interprofessional team *** IFMC may serve as a model for other institutions interested in starting a similar interprofessional,refugee-centred medical home.
