Population Levels of Climate Change Fear in the United States

There is increasing evidence that climate change, like other natural disasters has the potential for significant human health impacts, including mental health. Fear as a psychological construct concerning climate change is not well understood. An online cross-sectional survey was conducted, targeting a demographically representative sample of Americans (n = 546) in terms of ethnicity, age, and gender. Survey questions included demographic information and global questions regarding self-rated anxiety and fear of climate change. Ordinal logistic models were created to determine which demographic factors were most predictive of climate change fear in the US population. Over half of the study sample (50.9%) indicated being moderately or very afraid of climate change. In the end, only three factors remained significant (m>pm> model;self-reported level of anxiety, political affiliation, and identifying and Hispanic/Latino. Climate change fear is still not understood, especially in terms of its impact on the mental health of the population in general, though prolonged fear can be an antecedent to other mental health disorders. This study had demonstrated that fear of climate change impacts over half of the U.S population. Level of fear differs significantly by demographic. This study has provided evidence that climate change fear impacts a significant proportion of the US population, prompting a need to investigate the potential acute and long-term impacts of this fear on the human psyche. The harms and benefits of the fear response to climate change should be explored as well as potential responses to fear due to climate change.

Sepsis and septic shock are life threatening condition associated with high mortality rate in critically-ill patients. This high mortality is mainly related to the inadequacy between oxygen delivery and cellular demand leading to the onset of multiorgan dysfunction. Whether this multiorgan failure affect the pancreas is not fully investigated. In fact, pancreatic injury may occur because of ischemia, overwhelming inflammatory response, oxidative stress, cellular apoptosis and/or metabolic derangement. Increased serum amylase and/or lipase levels are common in patients with septic shock. However, imaging test rarely reveal significant pancreatic damage. Whether pancreatic dysfunction does affect the prognosis of patients with septic shock or not is still a matter of debate. In fact, only few studies with limited sample size assessed the clinical relevance of the pancreatic injury in this group of patients. In this review, we aimed to describe the epidemiology and the physiopathology of pancreatic injury in septic shock patients, to clarify whether it requires specific management and to assess its prognostic value. Our main finding is that pancreatic injury does not significantly affect the outcome in septic shock patients. Hence, increased serum pancreatic enzymes without clinical features of acute pancreatitis do not require further imaging investigations and specific therapeutic intervention.

The use of Live, Virtual and Constructive (LVC) simulations are increasingly being examined for potential analytical use particularly in test and evaluation. In addition to system-focused tests, LVC simulations provide a mechanism for conducting joint mission testing and system of systems testing when fiscal and resource limitations prevent the accumulation of the necessary density and diversity of assets required for these complex and comprehensive tests. LVC simulations consist of a set of entities that interact with each other within a situated environment (i.e., world) each of which is represented by a mixture of computer-based models, real people and real physical assets. The physical assets often consist of geographically dispersed test assets which are interconnected by persistent networks and augmented by virtual and constructive entities to create the joint test environment under evaluation. LVC experiments are generally not statistically designed, but really should be. Experimental design methods are discussed followed by additional design considerations when planning experiments for LVC. Some useful experimental designs are proposed and a case study is presented to illustrate the benefits of using statistical experimental design methods for LVC experiments. The case study only covers the planning portion of experimental design. The results will be presented in a subsequent paper.

Contaminated or infected patients present a risk of cross-contamination for emergency responders, attending medical personnel and medical facilities as they enter a treatment facility. The controlled conditions of an aerosol test chamber are required to examine factors of contamination, decontamination, and cross-contamination. This study presents the design, construction, and a method for characterizing an aerosol test chamber for a full-sized manikin on a standard North Atlantic Treaty Organization litter. The methodology combined air velocity measurements, aerosol particle counts and size distributions, and computational fluid dynamics modeling to describe the chamber’s performance in three dimensions. This detailed characterization facilitates future experimental design by predicting chamber performance for a variety of patient-focused research.

Sahelian countries are confronted with a lack of reliable data on water and climate allowing them to understand the effects of climate variability. To address this situation, with the support of Water Aid, we have collected rainfall data and groundwater level in wells from 2012 to 2018 with help of local populations. Their contribution made it possible to cover a wider geographical area and to obtain the data necessary to analyze the climate variability on a small and large scale in the sub-basin of the Nouhao. The data collected are well correlated with those collected from the rain gauges of the national meteorological agency in the region of Fada N’Gourma. From 2012 to 2018, August appears to be the rainiest month. It recorded, alone, 1/3 of the average annual rainfall. The depletion of surface water tables is faster after the rainy season. The static level of the water table in the crystalline subsoil also depletes and replenishes at night after the peak water collection time, which is between noon and 8 p.m. These few years of measuring rainfall and groundwater fluctuations have shown that the correlation between rainfall and groundwater level is clearly established. Nevertheless this needs to be more investigated during a longer period to confirm the robustness of the method. On the other hand, the approach to securing water resources based on community monitoring of water resources gives good results in accordance with the National meteorological Agency and the Directorate of water resources, however, caution recommends continuing measurements over a few decades to confirm its robustness for this aspect too.

Classically, hepatic artery pseudoaneurysms(HAPs) arise secondary to trauma or iatrogenic causes. With an increasing prevalence of laparoscopic procedures of the hepatobiliary system the risk of inadvertent injury to arterial vessels is increased. Pseudoaneurysm formation post injury can lead to serious consequences of rupture and subsequent hemorrhage, therefore intervention in all identified visceral pseudoaneurysms has been advocated. A variety of interventional methods have been proposed, with surgical management becoming the last step intervention when minimally invasive therapies have failed. The authors present a case of a HAP in a 56-year-old female presenting with jaundice and pruritis suggestive of a Klatskin's tumor. This presentation of HAP in a patient without any significant past medical or surgical intervention is atypical when considering that the majority of HAP cases present secondary to iatrogenic causes or trauma. multiple minimally invasive approaches were employed in an attempt to alleviate the symptomology which included jaundice and associated inflammatory changes. Ultimately, a right hepatic trisegmentectomy was required to adequately relieve the mass effect on biliary outflow obstruction and definitively address the HAP. The presentation of a HAP masquerading as a malignancy with jaundice and pruritis, rather than the classic symptoms of abdominalpain, anemia, and melena, is unique. This presentation is only further complicated by the absent history of either trauma or instrumentation. It is important to be aware of HAPs as a potential cause of jaundice in addition to the more commonly thought of etiologies. Furthermore, given the morbidity and mortality associated with pseudoaneurysm rupture, intervention in identifiable cases, either by minimally invasive or surgical interventions, is recommended.

为研究超低温双螺旋速冻(Ultra-low Temperature Double Spiral Quick Freezing,UTDSQF)对原料猪肉加工品质的影响,该研究以宰后24 h且pH值在正常范围内的猪背最长肌为研究对象,分别采用常规空气冻结(Air-blast Freezing,AF)和超低温双螺旋速冻两种方式对猪背最长肌进行冻结,随后在-20℃冷库中冻藏90 d后于4℃解冻,加工制成乳化香肠,测定两种冻结方式对其理化、质构和感官品质的影响。研究表明:原料肉不同冻结方式对乳化肠pH值没有显著影响(P>0.05);与AF组相比,UTDSQF组的TBARS值和蒸煮损失分别降低26.67%、31.25%(P0.05);同时,UTDSQF处理原料肉显著提升了乳化肠的感官评分(P<0.05)。因此超低温双螺旋速冻处理能够整体提升原料肉的加工品质。

Background Clinical outcomes of cardiac resynchronization therapy （CRT） in patients over the age of 80 have not been well *** retrospectively identified 96 consecutive patients≥ 80 years old who underwent an initial implant or an upgrade to CRT, with or without defibrillator （CRT-Dvs. CRT-P）, at our institution between January 2003 and July 2008. The control cohort consisted of 177 randomly selected patients 〈 80 years old undergoing CRT implant during the same time period. The primary efficacy endpoint was all-cause mortality at 36 months, assessed by Kaplan-meier time to first event *** In the octogenarian cohort, mean age at CRT implant was 83.1 ± 2.9 yearsvs. 60.1 ± 8.8 years among controls （P 〈 0.001）. Across both groups, 70% were male, mean left ventricular ejection fraction （LVEF） was 24.8% ± 14.1% and QRS duration was 154 ± 24.8 ms, without significant differences between groups. Octo-genarians were more likely to have ischemic cardiomyopathy （74%vs. 37%,P 〈 0.001） and more likely to undergo upgrade to CRT instead of an initial implant （42%vs. 19%,P 〈 0.001）. The rate of appropriate defibrillator shocks was lower among octogenarians （14%vs. 27%,P = 0.02） whereas the rate of inappropriate shocks was similar （3%vs. 6%,P = 0.55）. At 36 months, there was no significant difference in the rate of all-cause mortality between octogenarians （11%） and controls （8%,P = 0.381）.ConclusionAppropriately selected octogenarians who are candidates for CRT have similar intermediate-term mortality compared to younger patients receiving CRT.
