Comparing 11 early warning scores and three shock indices in early sepsis prediction in the emergency department

BACKGROUND:This study aimed to evaluate the discriminatory performance of 11 vital sign-based early warning scores(EWSs)and three shock indices in early sepsis prediction in the emergency department(ED).METHODS:We performed a retrospective study on consecutive adult patients with an infection over 3 months in a public ED in Hong *** primary outcome was sepsis(Sepsis-3 definition)within 48 h of ED *** c-statistics and the DeLong test,we compared 11 EWSs,including the National Early Warning Score 2(NEWS2),Modified Early Warning Score,and Worthing Physiological Scoring System(WPS),etc.,and three shock indices(the shock index[SI],modified shock index[MSI],and diastolic shock index[DSI]),with Systemic Inflammatory Response Syndrome(SIRS)and quick Sequential Organ Failure Assessment(qSOFA)in predicting the primary outcome,intensive care unit admission,and mortality at different time ***:We analyzed 601 patients,of whom 166(27.6%)developed ***2 had the highest point estimate(area under the receiver operating characteristic curve[AUROC]0.75,95%CI 0.70-0.79)and was significantly better than SIRS,qSOFA,other EWSs and shock indices,except WPS,at predicting the primary ***,the pooled sensitivity and specificity of NEWS2≥5 for the prediction of sepsis were 0.45(95%CI 0.37-0.52)and 0.88(95%CI 0.85-0.91),*** discriminatory performance of all EWSs and shock indices declined when used to predict mortality at a more remote time ***:NEWS2 compared favorably with other EWSs and shock indices in early sepsis prediction but its low sensitivity at the usual cut-off point requires further modification for sepsis screening.

We aim to evaluate prostate health index as an additional risk-stratification tool in patients with Prostate Imaging Reporting and Data System score 3 lesions on multiparametric magnetic resonance *** with biochemical or clinical suspicion of having prostate cancer who underwent multiparametric magnetic resonance imaging in two tertiary centers(Queen Mary Hospital and Princess Margaret Hospital,Hong Kong,China)between January 2017 and June 2022 were ***-magnetic resonance imaging fusion biopsies were performed after prostate health index *** who only had Prostate Imaging Reporting and Data System score 3 lesions were further stratified into four prostate health index risk groups and the cancer detection rates were *** of the 747 patients,47.3%had Prostate Imaging Reporting and Data System score 3 lesions *** detection rate of clinically significant prostate cancer in this group was 15.0%.The cancer detection rates of clinically significant prostate cancer had statistically significant differences:5.3%in prostate health indexting and Data System score 3 *** could be avoided in patients with low prostate health index,with a small risk of missing clinically significant prostate cancer.

There are only eight approved small molecule antiviral drugs for treating *** them,four are nucleotide analogues(remdesivir,JT001,molnupiravir,and azvudine),while the other four are protease inhibitors(nirmatrelvir,ensitrelvir,leritrelvir,and simnotrelvir-ritonavir).Antiviral resistance,unfavourable drug‒drug interaction,and toxicity have been reported in previous *** there is a dearth of new treatment options for *** this work,a three-tier cell-based screening was employed to identify novel compounds with anti-SARS-CoV-2 *** compound,designated 172,demonstrated broad-spectrum antiviral activity against multiple human pathogenic coronaviruses and different SARS-CoV-2 variants of *** studies validated by reverse genetics showed that compound 172 inhibits the 3-chymotrypsin-like protease(3CLpro)by binding to an allosteric site and reduces 3CLpro dimerization.A drug synergistic checkerboard assay demonstrated that compound 172 can achieve drug synergy with nirmatrelvir in *** vivo studies confirmed the antiviral activity of compound 172 in both Golden Syrian Hamsters and K18 humanized ACE2 ***,this study identified an alternative druggable site on the SARS-CoV-23CLpro,proposed a potential combination therapy with nirmatrelvir to reduce the risk of antiviral resistance and shed light on the development of allosteric protease inhibitors for treating a range of coronavirus diseases.

Endocrine Disrupting Chemicals(EDCs)are a group of molecules that can influence hormonal balance,causing disturbance of the reproductive system and other health *** the efforts to eliminate EDC in the environment,all current approaches are inefficient and *** previous research,studies revealed that laccase-producing microorganisms may be a potential candidate for EDC degradation,as laccases have been found to be able to degrade many kinds of EDCs effectively and ***,we created two recombinant laccases,each fused with secretion peptide,Novel Signal Peptide 4(NSP4),and expressed them in Escherichia coli(***,BL21),together with one laccase without secretion *** first optimized the culture condition of expressing these ***,we test the activity of the recombinant laccases of decolorizing of a synthetic dye,indigo ***,we confirmed the secreted can degrade one of the EDCs,β-estradiol,showing the potential of using the laccase secretion system to degrade toxic compounds.

背景对已确诊的严重急性呼吸综合征的病人临床特点进行总结，为以后疑似病例的治疗和护理提供参考价值。方法对香港从2003年2月22日到2003年3月22日被确诊的10名患者(5男5女，38～72岁)进行分析。结果：潜伏期从2天至ll天。所有的患者都表现有发热(体温>38℃，超过24小时)，大多数的患者有寒战、干咳、呼吸困难、周身不适、头痛、缺氧症状。物理检查听诊可有肺泡音、叩诊浊音。9名患者出现淋巴细胞降低。大多数患者出现转氨酶升高。但血清肌酐值维持正常水平。连续进行胸部X线检查，提示进行性充气区减少。两名患者死于进行性呼吸功能衰竭。组织活检表明肺组织内广泛性肺泡损伤。但未找到肺衣原体、肺炎支原体和军团菌感染的迹象。所有的患者都接受了皮质激素和病毒唑治疗。有8位患者接受了β-内酰胺类与大环内酯类药物的治疗，临床症状及x线检查都无明显改善。结论 SARS为传染性疾病。发热及继发的进行性呼吸困难是其主要的症状。病原体有待进一步确定。2002年秋冬季，中国广东省报道了305例高传染性的严重呼吸道疾患，表现为在医护人员中的局部流行。2003年2月，美国CDC将其命名为SARS，确定了它的临床表现。2003年3月13日，由于SARS由中国向世界范围内扩散，世界卫生组织发出SARS爆发公告，在全世界范围内进行调查。
