Hepatocellular carcinoma in patients with non-alcoholic fatty liver disease

Nonalcoholic fatty liver disease(NAFLD) is the most common chronic liver disease in the United States and represents an increasingly important etiology of hepatocellular carcinoma(HCC) with annual cumulative incidence rates ranging from 2% to 12% in cohorts of NAFLD cirrhosis. While the risk of progression of NAFLD to HCC remains higher among patients with fibrosis or cirrhosis, an increasing amount of literature describes NAFLD-HCC as a disease that can occur in the absence of cirrhosis. Efforts to characterize the pathogenesis of NAFLD-HCC have suggested mechanisms that strongly associate with states of hyperinsulinemia and chronic inflammation, cellular mechanisms including adaptive immune responses and hepatic progenitor cell populations, and genetic polymorphisms including mutations of PNPLA3. Current literature describes NAFLD-HCC mostly as a disease of late presentation with lower rates of receipt of curative therapy and worse prognosis. However, a growing body of evidence has reported comparable and potentially more favorable disease-free and overall survival rates among patients with NAFLD-HCC after receipt of curative treatment. This review summarizes current evidence of epidemiology, pathophysiology, disease presentation, demand and receipt of curative therapy, post-treatment outcomes, and overall survival of NAFLD-associated HCC.

Hepatocellular carcinoma(HCC)has one of the fastestgrowing rates of cancer-related incidence and mortalityin the United States(U.S.)(1-3).From 2009 to 2013,theaverage annual percent change(AAPC)for liver cancerincidence became the highest among other reported cancersfor both males(+2.9%)and females(+3.8%)(1).In addition,cancer-related mortality rates for liver cancer are the highestamong solid malignancies from 2010 to 2014 for males(+2.6%)and females(+3.0%)(1).

[目的]探讨全国各地中医类医院的收入结构特点和差异,为政府制定适宜的中医药政策提供决策参考。[方法]根据《2017年全国中医药统计摘编》的数据,对各地区中医类医院的收入结构进行描述性统计和聚类分析。[结果]2017年我国中医综合医院医疗收入占总收入比重为86.6%,低于综合医院;而药品收入占医疗收入的比重为42.2%,其中基本药物占药品收入的比重为32.9%,均高于综合医院。全国各地中医类医院收入结构可分为四类地区,其中门诊收入占医疗总收入比重位于前三位的是北京(68.7%)、上海(59.7%)和天津(55.0%)。[结论]中医类医院与综合医院相比创收能力不足,药品收入比重较高,基本药物的使用情况更好,此外各地中医类医院业务开展存在明显的差异性。

Views and Experience on Patient Engagement in Healthcare Professionals and Patients—How Are They Different?

A patient-centered approach is used to build a therapeutic alliance between patients and the healthcare professionals in care process which should be supported by a good engagement of both parties. The study aimed to explore the gap between healthcare professionals and patients on patient engagement in hospital. It was a cross-sectional survey. 2774 doctors and nurses from Department of Medicine of public hospitals completed the self-administered questionnaire and 1042 patients discharged from corresponding wards completed the telephone interviews. Participants were interviewed using structural questionnaires. The Mann-Whitney test or Pearson’s chi-square test was used to analyze the agreement between health-care professionals and patients on the views and experiences of patient engagement. A difference was considered to be statistically significant when the p-value was roups valued the importance of patient engagement, there was a discrepancy on understanding, views and experiences. More healthcare professionals particularly in nursing were concerned about the possible negative impact of the engagement. The majority of healthcare professionals reported that they engaged well with patients, and perceived more difficulties than patients did. The findings highlighted the mutual understanding of patient engagement, involvement and challenges encountered by both groups in Department of Medicine, which was crucial in efforts to provide meaningful patient engagement in regards to jurisdictions, health system, specialty, discipline and background of patients. It provided insight that a collaborative strategy involving both healthcare professional and patients might be an alternative approach to improving patient engagement.
