Impact of conditioning regimen on peripheral blood hematopoietic cell transplant

AIM To investigate infused hematopoietic cell doses and their interaction with conditioning regimen intensity +/-total body irradiation(TbI) on outcomes after peripheral blood hematopoietic cell transplant(PbHCT).METHODS Our retrospective cohort included 247 patients receiving a first, T-replete, human leukocyte antigen-matched allogeneic PbHCT and treated between 2001 and2012. Correlations were calculated using the Pearson product-moment correlation coefficient. Overall survival and progression free survival curves were generated using the Kaplan-Meier method and compared using the log-rank *** Neutrophil engraftment was significantly faster after reduced intensity TbI based conditioning [reduced intensity conditioning(RIC) + TbI] and > 4 × 10~6 CD34+cells/kg infused. A higher total nucleated cell dose led to a higher incidence of grade II-IV acute graft-versus-host disease in the myeloablative + TbI regimen group(P = 0.03), but no significant difference in grade III-IV graft-versus-host disease. A higher total nucleated cell dose was also associated with increased incidence of moderate/severe chronic graft-versus-host disease, regardless ofconditioning regimen. Overall and progression-free survival were significantly better in patients with a RIC + TbI regimen and total nucleated cell dose > 8 ×10~8/kg(3 years, overall survival: 70% vs 38%, P = 0.02, 3 years, progression free survival: 64% vs 38%, P = 0.02).CONCLUSION TbI and conditioning intensity may alter the relationship between infused cell doses and outcomes after PbHCT. Immune cell subsets may predict improved survival after unmanipulated PbHCT.

Absolute Lymphocyte/Monocyte Ratio at Diagnosis and Interim Positron-Emission Tomography Predict Survival in Classical Hodgkin Lymphoma

Interim Positron-Emission Tomography (int-PET) and the peripheral blood absolute lymphocyte/monocyte ratio at di- agnosis (ALC/AMC-DX) have been shown to be predictors for progression-free survival (PFS) and time to progression (TTP) in classical Hodgkin lymphoma (cHL). Therefore, we studied if the combination of ALC/AMC-DX and the (int-PET) can further stratified PFS and TTP in cHL patients. Patients were required to be diagnosed, treated, and followed with int-PET at Mayo Clinic, Rochester, Minnesota. From 2000 until 2008, 111 cHL patients qualified for the study. The median follow-up was 2.8 years (range: 0.3 - 10.4 years). Patients with a negative int-PET (N = 98) pre- sented with a higher ALC/AMC-DX (median of 2.32, range: 0.26 - 37.5) compared with patients with a positive int-PET (N = 13) (median of 0.9, range: 0.29 - 3.10), p 1.1. Group 1 experienced superior PFS and TTP in comparison with the other groups. In conclusion, the combination of ALC/AMC-DX and the int-PET provides a simple model to assess clinical outcomes in cHL.

This review considers available evidence for mechanisms of conferred adaptive advantages in the face of specific infectious *** short,we explore a number of genetic conditions,which carry some benefits in adverse circumstances including exposure to infectious *** examples discussed are conditions known to result in resistance to a specific infectious disease,or have been proposed as being associated with resistance to various infectious *** infectious diseasedgenetic disorder pairings include malaria and hemoglobinopathies,cholera and cystic fibrosis,tuberculosis and Tay-Sachs disease,mycotic abortions and phenylketonuria,infection by enveloped viruses and disorders of glycosylation,infection by filoviruses and NiemannePick C1 disease,as well as rabies and myasthenia *** also discuss two genetic conditions that lead to infectious disease hypersusceptibility,although we did not cover the large number of immunologic defects leading to infectious disease *** of the resistance-associated pairings(malaria/hemogloginopathies,cholera/cystic fibrosis,tuberculosis/Tay-Sachs,and mycotic abortions/phenylketonuria)appear to be a result of selection pressures in geographic regions in which the specific infectious agent is *** other pairings do not appear to be based on selection pressure and instead may be ***,research investigating these relationships may lead to treatment options for the aforementioned diseases by exploiting established mechanisms between genetically affected cells and infectious *** may prove invaluable as a starting point for research in the case of diseases that currently have no reliably curative treatments,e.g.,HIV,rabies,and Ebola.

随着短期海洋气象预报技术的逐渐成熟和极端天气造成的灾害性后果不断增大,对天气系统的中期(55天)尺度的预报技术的探索逐渐成为欧美大气科学界的前沿研究课题。对亚洲季风地区天气活动的统计分析结果表明,其中的强对流降雨事件有很强的季节内振荡特征。应用欧洲中期天气预报中心的数值预报产品,开发针对南亚季风河流流量的实时概率预报模式。该模式在过去6年里多次成功地提前70天预警了发生在恒河和雅鲁藏布江下游的超警戒水位事件,取得很大社会效益。在此基础上开发了针对亚洲热带气旋活动的预报模式,虽然热带气旋的水平范围较季风降雨事件小,但对于多个超级台风的预报结果分析表明此程序能够提前50天预测出气旋中心的大致移动路径以及最低气压值和最大风速,其中对移动路径的预报较好。

Identification of HCV Inhibitors from a Cell-based Sub-Genomic Replicon Screen

A high throughput screen of the Pfizer compound collection was carried out using a hepatitis C virus (HCV) genotype 1b subgenomic replicon cell line. Those confirmed hits that demonstrated broad spectrum activity without overt cytotoxicity were further evaluated, leading to the identification of a series of pyrrolopyridines with excellent antiviral activity in a fully infectious HCV cell-based assay and pharmacokinetic properties.

Aim:To develop a comprehensive item library of patient-reported,immunotherapy-related adverse events(irAEs)that draws from and expands on the Functional Assessment of Chronic Illness Therapy(FACIT)Measurement ***:Literature review and iterative expert *** on a literature review of irAEs,we developed a framework of immunotherapy classes and their associated *** experts then reviewed iterations of symptom summaries and item maps linked to the immunotherapy *** provided content review and feedback was shared across experts until consensus was *** iterative process facilitated creation of a Primary Symptom List associated with immune checkpoint modulators(ICMs),drawn from the larger set of *** FACIT items were mapped to the symptom list,and new items were written as needed to create the item ***:The full item library of irAEs is comprised of 239 items,covering 142 unique symptoms across 75 inflammatory reactions/immune conditions.A subset of 66 items comprises a Primary Symptom List considered most common/relevant to ICM *** includes gastrointestinal,skin,pulmonary,neurologic,musculoskeletal,and multiple miscellaneous and constitutional ***:The FACIT Immunotherapy Item Library is a compilation of 239 self-report items that capture the wide range of AEs experienced by people receiving immune treatments.A subset of 66 items comprises a Primary Symptom List meant for ICM *** of items selected from this library is encouraged in clinical research and clinical practice evaluation.

As climatic changes and human uses intensify,resource managers and other decision makers are taking actions to either avoid or respond to ecosystem tipping points,or dramatic shifts in structure and function that are often costly and hard to *** indicates that explicitly addressing tipping points leads to improved management *** on theory and examples from marine systems,we distill a set of seven principles to guide effective management in ecosystems with tipping points,derived from the best available *** principles are based on observations that tipping points(1)are possible everywhere,(2)are associated with intense and/or multifaceted human use,(3)may be preceded by changes in earlywarning indicators,(4)may redistribute benefits among stakeholders,(5)affect the relative costs of action and inaction,(6)suggest biologically informed management targets,and(7)often require an adaptive response to *** suggest that early action to preserve system resilience is likely more practical,affordable,and effective than late action to halt or reverse a tipping *** articulate a conceptual approach to management focused on linking management targets to thresholds,tracking early-warning signals of ecosystem instability,and stepping up investment in monitoring and mitigation as the likelihood of dramatic ecosystem change *** approach can simplify and economize management by allowing decision makers to capitalize on the increasing value of precise information about threshold relationships when a system is closer to tipping or by ensuring that restoration effort is sufficient to tip a system into the desired regime.
