Objective. Current prognostic models in primary biliary cirrhosis (PBC) have low precision, partly due to the restricted inclusion criteria of some cohorts used for modelling but also because of the prolonged natural course of the disease. It is hypothesized that better precision could be achieved with a staged model, using ascites or peripheral oedema as a new startingpoint for prediction. Material and methods. The study was based on an established database of 289 consecutive patients, followed between 1977 and 1998. Stepwise Cox regression was used to construct a staged model based on 143 patients who first developed ascites (n = 111) or peripheral oedema (n = 32) at entry or during subsequent follow-up. The model was compared with published models using graphical methods and receiver operating characteristics (rOCs). results. Mean time from clinical diagnosis of ascites or peripheral oedema to death was 3.1 years. The following variables had independent prognostic significance: log10(bilirubin) (p ry of encephalopathy (p rvival probabilities predicted by the Ascites Stage Model fitted well with the observed data. The Ascites Stage Model (rOC 0.8324 (SE 0.0348)) was a better predictor of survival than the Mayo long-term model (rOC 0.7833 (SE 0.0397)), the Mayo repeated Patient Visits Model (rOC 0.7779 (SE 0.0399)) and the royal Free PBC Prognostic Model (rOC 0.7785 (SE 0.0396)). Conclusions. The Ascites Stage Model gives a better survival estimate for PBC patients once they have developed ascites or peripheral oedema compared with the current models, and demonstrates an advantage of staged models in diseases with a prolonged natural history.

Karst aquifers supply a significant fraction of the world's drinking water. These types of aquifers are alsohighly susceptible to pollution from the surface with recharge usually occurring through fractures and solution openings at the bedrock surface. Thickness of the protective soil cover, macropores and openings within the soil cover, and the nature of the weathered bedrock surface all influence infiltration. recharge openings at the bedrock surface, however, are often covered by unconsolidated sediments, resulting in the inadvertent placement of landfills, unregulated dump sites, tailing piles, waste lagoons and septic systems over recharge zones. In these settings surface geophysical surveys, calibrated by a few soil cores, could be employed to identify these recharge openings, and qualitatively assess the protection afforded by the soil cover. In a test of this hypothesis, geophysical measurements accurately predicted the thickness of unconsolidated deposits overlying karstic dolomite at a site about 100 km south of Chicago, Illinois. Zones of elevated electrical conductivity and high ground-penetrating radar (GPr) attenuation within the sediments coincided with subcropping solutionally-enlarged hydraulically active bedrock fractures. These fractures extend to over 12-m depth, as shown by 2-D inverted resistivity sections and soil coring. Anomalous electromagnetic (EM) conductivity and GPr response may be due to higher soil moisture above these enlarged fractures. An epikarstal conduit at 2.5-m depth was directly identified through a GPr survey. These results suggest that surface geophysical surveys are a viable tool for assessing the susceptibility of shallow karst aquifers to contamination.

Although the Neotropical territorially dominant arboreal ant Azteca chartifex Forel is very aggressive towards any intruder,its populous colonies tolerate the close presence of the fierce polistine wasp Polybia rejecta(F.).In French Guiana,83.33%of the 48 *** nests recorded were found side by side with those of *** nesting association results in mutual protection from predators(i.e.,the wasps protected from army ants;the ants protected from birds).We conducted field studies,laboratorybased behavioral experiments and chemical analyses to elucidate the mechanisms allowing the persistence of this *** to differences in the cuticular profiles of the two species,we eliminated the possibility of chemical ***,analyses of the carton nests did not reveal traces of marking on the *** ant forager flows were not perturbed by extracts from the wasps’Dufour’s and venom glands,we rejected any hypothetical action of repulsive ***,we noted that the wasps"scraped"the surface of the upper part of their nest envelope using their mandibles,likely removing the ants'scent trails,and an experiment showed that ant foragers were perturbed by the removal of their scent *** leads us to use the term"erasure hypothesis."Thus,this nesting association persists thanks to a relative tolerance by the ants towards wasp presence and the behavior of the wasps that allows them to"contain"their associated ants through the elimination of their scent trails,direct attacks,"wing-buzzing"behavior and ejecting the ants.

Apparent Formation Factor for Leachate-Saturated Waste and Sediments: Examples from the USA and China

The formation factor relates bulk resistivity to pore fluid resistivity in porous materials. Understanding the formation factor is essential in using electrical and electromagnetic methods to monitor ieachate accumulations and movements both within and around landfills. Specifically, the formation factor allows leachate resistivity, the degree of saturation, and, possibly, even the hydraulic conductivity of the waste to be estimated from non-invasive surface measurements. In this study, apparent formation factors are computed for three landfills with different types of waste as well as sediments contaminated by landfill leachate. resistivity soundings at the closed Mallard North landfill in suburban Chicago （Illinois, USA） mapped leachate surfaces that were confirmed by monitoring wells. The resistivity of leachate-saturated waste from resistivity sounding inversions was then divided by the leachate resistivity values measured in-situ to compute apparent formation factors （Fa） ranging from 1.6 to 4.9. A global Fa of 3.0±1.9 was computed for the entire monitored portion of this landfill. At a nearby mixed laboratory waste landfill, a 2D inverted resistivity section was used to compute an Fa of 2.9. Finally, a distinctly different Fa value of 10.6±2.8 was computed for leachate-saturated retorted oil shale wastes north of Maoming （茂名）, Guangdong （广东） Province, China. Shallow aquifers in the Laohuling （老虎岭） Formation near this landfill are polluted by acidic leachate containing heavy metals and organic compounds. The Fa for aquifers containing contaminated groundwater fall in the same range as aquifers with normal groundwater, 1.7-3.9. However, models from inverted sounding curves over these contaminated areas exhibit unusually low resistivity layers, which may be diagnostic of contamination.

In this paper,we investigate the impact of coronary artery dynamics on the wall shear stress(WSS)vector field topology by comparing fluid–structure interaction(FSI)and computational fluid dynamics(CFD)*** one of the most common causes of death globally,coronary artery disease(CAD)is a significant economic burden;however,novel approaches are still needed to improve our ability to predict its *** can include the unique dynamical factors present in the coronary *** investigate the impact of these dynamical factors,we study an idealized artery model with sequential *** transient simulations made use of the hyperelastic artery and lipid constitutive equations,non‐Newtonian blood viscosity,and the characteristic out‐of‐phase pressure and velocity distribution of the left anterior descending coronary *** compare changes to established metrics of time‐averaged WSS(TAWSS)and the oscillatory shear index(OSI)to changes in the emerging WSS divergence,calculated here in a modified version to handle the deforming mesh of FSI *** suggest that the motion of the artery can impact downstream patterns in both divergence and *** magnitude is also decreased by up to 57%due to motion in some *** divergence patterns varied most significantly between simulations over the systolic period,the time of the largest *** investigation highlights that coronary dynamics could impact markers of potential CAD progression and warrants further detailed investigations in more diverse geometries and patient cases.

Predictors of clinical outcomes after non-operative management of symptomatic full-thickness rotator cuff tears

Development of Low Carbon Microalloyed High Strength Ultra-Thin Cast Strip Products Produced by the Castrip~ Process With Excellent Strength-Ductility Properties

The CASTrIP* process produces a range of high strength Ultra-thin Cast Strip (UCS) products (380-550 MPa) in thicknesses between 0.9 mm and 1.5 mm, which is very challenging to produce via conventional hot and cold rolled processing routes. The twin roll CASTrIP process fully exploits the hardenability and strengthening potential of Nb in a low C-Mn-Nb-V microalloyed steel type. Significant microstructural strengthening from solute Nb was obtained, even at low microalloying levels, as well as modest Mn additions, through enhancing the hardenability and further strengthening was obtained in coated products by exploiting age hardening during processing on a continuous hot dip galvanising line. Atom probe tomography and TEM determined that Nb was retained in solid solution and subsequent age hardening resulted from the formation of Nb and V rich nanosized particles. Age hardening was achieved without loss of ductility producing galvanised strip with an excellent strength-ductility combination (Y.S. >600 MPa, T.E. >10%).

This paper addresses the issue of checking consistency in information models. A method based on constraint programming is proposed for identifying inconsistency or proving consistency in information models. The system described here checks information models written in the ISO standard information modelling language EXPrESS. EXPrESS is part of the ISO STEP standard used in the manufacturing and process industries. This paper describes the checking procedure, including EXPrESS model formalization, constraint satisfaction problem (CSP) derivation from the formalized model and satisfaction checking of the derived CSPs. This paper shows a new domain in which constraint programming can be exploited as model verification and validation.

Disagreement in middle ear volume estimation between tympanometry and three-dimensional volume reconstruction in the context of tympanic membrane perforation

Introduction: Middle ear volume(MEV) is a clinically relevant parameter across middle ear diseases. MEV values between these techniques have never before been tested for agreement in ears with perforated tympanic membranes(TMs).Methods: Middle ears were identified from 36 patients ranging 18-89 years of age with TM perforations who underwent tympanometry and temporal bone computed tomography(CT) between 2005 and 2015. MEVs calculated by both tympanometry and three-dimensional volume reconstruction(3DVr) were analyzed for agreement using Bland Altman plots. The differences between tympanometric and 3DVr MEV values for each given middle ear were characterized across MEV quartiles(1= smallest; 4= largest) and across increasing states of middle ear disease using Kruskale Wallis and Wilcoxon testing with Bonferroni ***: Bland Altman plots demonstrated significant disagreement between MEV measurement techniques. Differences between tympanometric(T) and 3DVr MEV values were significantly greater with increasing average(i.e.(Tt3DVr)/2)) MEV per linear regression(p rated between fourth and first average MEV quartiles(p= 0.0024), fourth and second quartiles(p= 0.0024), third and first quartiles(p= 0.0048), and third and second quartiles(p= 0.048). Absolute MEV difference was not significantly different across varying states of middle ear disease(p= 0.44).Conclusion: Statistically and clinically significant disagreement was demonstrated between tympanometric and 3DVr MEV values. Studies that vary in MEV estimation techniques may be expected to demonstrate significantly different results. These preliminary results suggest that clinicians should endeavor to seek further confirmation when interpreting high tympanometric MEV values.
