The role of vascularization in nerve regeneration of nerve graft

Vascularization is an important factor in nerve graft survival and function. The specific molecular regulations and patterns of angiogenesis following peripheral nerve injury are in a broad complex of pathways. This review aims to summarize current knowledge on the role of vascularization in nerve regeneration, including the key regulation molecules, and mechanisms and patterns of revascularization after nerve injury. Angiogenesis, the maturation of pre-existing vessels into new areas, is stimulated through angiogenic factors such as vascular endothelial growth factor and precedes the repair of damaged nerves. Vascular endothelial growth factor administration to nerves has demonstrated to increase revascularization after injury in basic science research. In the clinical setting, vascularized nerve grafts could be used in the reconstruction of large segmental peripheral nerve injuries. Vascularized nerve grafts are postulated to accelerate revascularization and enhance nerve regeneration by providing an optimal nutritional environment, especially in scarred beds, and decrease fibroblast infiltration. This could improve functional recovery after nerve grafting, however, conclusive evidence of the superiority of vascularized nerve grafts is lacking in human studies. A well-designed randomized controlled trial comparing vascularized nerve grafts to non-vascularized nerve grafts involving patients with similar injuries, nerve graft repair and follow-up times is necessary to demonstrate the efficacy of vascularized nerve grafts. Due to technical challenges, composite transfer of a nerve graft along with its adipose tissue has been proposed to provide a healthy tissue bed. Basic science research has shown that a vascularized fascial flap containing adipose tissue and a vascular bundle improves revascularization through excreted angiogenic factors, provided by the stem cells in the adipose tissue as well as by the blood supply and environmental support. Optogenetic inhibition of medial prefrontal cortex projections to the nucleus accumbens core and methyl supplementation via L-methionine attenuates cocaine-primed reinstatement

A matched-pair analysis of laparoscopic versus open pancreaticoduodenectomy: oncological outcomes using Leeds Pathology Protocol

BACKGROUND： Laparoscopic pancreaticoduodenectomy（LPD）is a safe procedure. Oncological safety of LPD is still a matter for debate. This study aimed to compare the oncological outcomes,in terms of adequacy of resection and recurrence rate following LPD and open pancreaticoduodenectomy（OPD）.mETHODS： Between November 2005 and April 2009, 12LPDs（9 ampullary and 3 distal common bile duct tumors）were performed. A cohort of 12 OPDs were matched for age,gender, body mass index（BmI） and American Society of Anesthesiologists（ASA） score and tumor ***： mean tumor size LPD vs OPD（19.8 vs 19.2 mm,P=0.870）. R0 resection was achieved in 9 LPD vs 8 OPD（P=1.000）. The mean number of metastatic lymph nodes and total number resected for LPD vs OPD were 1.1 vs 2.1（P=0.140）and 20.7 vs 18.5（P=0.534） respectively. Clavien complications grade I/II（5 vs 8）, III/IV（2 vs 6） and pancreatic leak（2 vs 1）were statistically not significant（LPD vs OPD）. The mean high dependency unit（HDU） stay was longer in OPD（3.7 vs 1.4 days,P〈0.001）. There were 2 recurrences each in LPD and OPD（logrank,P=0.983）. Overall mortality for LPD vs OPD was 3 vs 6（log-rank, P=0.283） and recurrence-related mortality was 2 vs *** was one death within 30 days in the OPD group secondary to severe sepsis and none in the LPD ***： Compared to open procedure, LPD achieved a similar rate of R0 resection, lymph node harvest and longterm recurrence for tumors less than 2 cm. Though technically challenging, LPD is safe and does not compromise oncological outcome.

Tracing the origin of large respiratory droplets by their deposition characteristics inside the respiratory tract during speech

Origin of differently sized respiratory droplets is fundamental for clarifying their viral loads and the sequential transmission mechanism of SARS-CoV-2 in indoor *** talking activities characterized by low(0.2 L/s),medium(0.9 L/s),and high(1.6 L/s)airflow rates of monosyllabic and successive syllabic vocalizations were investigated by computational fluid dynamics(CFD)simulations based on a real human airway *** k-ωmodel was chosen to predict the airflow field,and the discrete phase model(DPm)was used to calculate the trajectories of droplets within the respiratory *** results showed that flow field in the respiratory tract during speech is characterized by a significant laryngeal jet,and bronchi,larynx,and pharynx-larynx junction were main deposition sites for droplets released from the lower respiratory tract or around the vocal cords,and among which,over 90%of droplets over 5μm released from vocal cords deposited at the larynx and pharynx-larynx ***,droplets’deposition fraction increased with their size,and the maximum size of droplets that were able to escape into external environment decreased with the airflow *** threshold size for droplets released from the vocal folds was 10-20μm,while that for droplets released from the bronchi was 5-20μm under various airflow ***,successive syllables pronounced at low airflow rates promoted the escape of small droplets,but do not significantly affect the droplet threshold *** study indicates that droplets larger than 20μm may entirely originate from the oral cavity,where viral loads are lower;it provides a reference for evaluating the relative importance of large-droplet spray and airborne transmission route of COVID-19 and other respiratory infections.

Shaped Offset Quadrature Phase Shift Keying Based Waveform for Fifth Generation Communication

Fifth generation(5G)wireless networks must meet the needs of emerging technologies like the Internet of Things(IoT),Vehicle-to-everything(V2X),Video on Demand(VoD)services,Device to Device communication(D2D)and many other bandwidth-hungry multimedia applications that connect a huge number of devices.5G wireless networks demand better bandwidth efﬁciency,high data rates,low latency,and reduced spectral *** meet these requirements,a suitable 5G waveform must be *** this work,a waveform namely Shaped Offset Quadrature Phase Shift Keying based Orthogonal Frequency Division multiplexing(SOQPSK-OFDm)is proposed for 5G to provide bandwidth efﬁciency,reduced spectral leakage,and Bit Error Rate(BER).The proposed work is evaluated using a real-time Software Deﬁned Radio(SDR)testbed-Wireless open Access Research Platform(WARP).Experimental and simulation results show that the proposed 5G waveform exhibits better BER performance and reduced Out of Band(OOB)radia-tion when compared with other waveforms like Offset Quadrature Phase Shift Key-ing(OQPSK)and Quadrature Phase Shift Keying(QPSK)based OFDm and a 5G waveform candidate Generalized Frequency Division multiplexing(GFDm).BER analysis shows that the proposed SOQPSK-OFDm waveform attains a Signal to Noise Ratio(SNR)gain of 7.2 dB at a BER of 10�3,when compared with GFDm in a real-time indoor *** SNR gain of 8 and 6 dB is achieved by the proposed work for a BER of 10�4 when compared with QPSK-OFDm and OQPSK-OFDm signals,respectively.A signiﬁcant reduction in OOB of nearly 15 dB is achieved by the proposed work SOQPSK-OFDm when compared to 16 Quadrature Amplitude modulation(QAm)mapped OFDm.
